5Cryptogenic multifocal ulcerous stenosing enteritis: Radiologic features and clinical behavior显示文摘AIM To investigate the characteristic radiologic findings of cryptogenic multifocal ulcerous stenosing enteritis(CMUSE) which can be differentiated from other similar bowel disease and to assess their clinical behavior.METHODS Twenty pathologically and clinically confirmed CMUSE patients(males:females = 8:12; mean age: 40.4 years) between March 2002 and August 2015 from seven academic centers in South Korea were retrospectively reviewed. We evaluated small bowel series(SBS; n = 25), computed tomography(CT) enterography(n = 21), magnetic resonance(MR) enterography(n = 2), and abdominopelvic CT(n = 18) images, focusing on enteric and perienteric manifestations. Any change in radiologic features during followup period was recorded. We evaluated clinical data including presenting symptoms, laboratory finding and presence of relapse from electronic medical records. Histopathologic findings were also evaluated. RESULTS The main symptoms were abdominal pain(n = 12) and anemia(n = 10). All patients showed small bowel strictures(n = 52, mean: 2.6 per patient) on initial CT/MR, located in the ileum(n = 47) or jejunum(n = 5). Strictures showed short-length(mean: 10.44 mm) and circumferential bowel wall thickening(mean: 5.56 mm) with layered enhancement(n = 48) that were also noted on initial SBS(n = 36) with shallow ulcers(n = 10). Some ulcerative lesions or wall thickening progressed into strictures on follow-up SBS/CT, and some strictures revealed recurrent ulceration on followup SBS. There were no penetrating disease features like fistula or abscess and no gastrointestinal tract involvement except the small bowel. Nine patients experienced disease recurrence(median relapse-free period: 32 mo) even post-operatively. Histopathologic features of surgically resected specimens were characterized as multiple superficial ulcerations confined to mucosa or submucosa and multiple strictures.CONCLUSION Under characteristic radiologic findings with multiple short-segmental strictures and/or shallow ulcers of the small intestine, CMUSE should be considered when assessing patients with recurrent abdominal pain and anemia.Jiyoung Hwang Jin Sil Kim Ah Young Kim Joon Seok Lim Se Hyung Kim Min Ju Kim Mi Sung Kim Kyoung Doo Song Ji Young Woo 2017World Journal of Gastroenterology2017,23,25:4
680年来埃塞俄比亚北部高原土地利用/覆被变化(英文)显示文摘Despite many studies on land degradation in the Highlands of Northern Ethiopia, quantitative information regarding long-term changes in land use/cover(LUC) is rare. Hence, this study aims to investigate the LUC changes in the Geba catchment(5142 km2), Northern Ethiopia, over 80 years(1935–2014). Aerial photographs(APs) of the 1930 s and Google Earth(GE) images(2014) were used. The point-count technique was utilized by overlaying a grid on APs and GE images. The occurrence of cropland, forest, grassland, shrubland, bare land, built-up areas and water body was counted to compute their fractions. A multivariate adaptive regression spline was applied to identify the explanatory factors of LUC and to create fractional maps of LUC. The results indicate significant changes of most types, except for forest and cropland. In the 1930 s, shrubland(48%) was dominant, followed by cropland(39%). The fraction of cropland in 2014(42%) remained approximately the same as in the 1930 s, while shrubland significantly dropped to 37%. Forests shrank further from a meagre 6.3% in the 1930 s to 2.3% in 2014. 7Muskingum equation based downstream sediment flow simulation models for a river system显示文摘Applications of sediment transport and water flow characteristics based sediment transport simulation models for a river system are presented in this study.An existing water-sediment model and a new sediment-water model are used to formulate the simulation models representing water and sediment movement in a river system.The sediment-water model parameters account for water flow characteristics embodying sediment transport properties of a section.The models are revised formulations of the multiple water inflows model describing water movement through a river system as given by the Muskingum principle.The models are applied to a river system in Mississippi River basin to estimate downstream sediment concentration,sediment discharge,and water discharge.River system and the river section parameters are estimated using a revised and the original multiple water inflows models by applying the genetic algorithm.The models estimate downstream sediment transport rates on the basis of upstream sediment/water flow rates to a system.Model performance is evaluated by using standard statistical criteria;downstream water discharge resulting from the original multiple water inflows model using the estimated river system parameters indicate that the revised models satisfactorily describe water movement through a river system.Results obtained in the study demonstrate the applicability of the sediment transport and water flow characteristics-based simulation models in predicting downstream sediment transport and water flow rates in a river system.Briti Sundar Sil Parthasarathi Choudhury 2016International Journal of Sediment Research2016,31,2:2

19Effect of composite yogurt enriched with acacia fiber and Bifidobacterium lactis显示文摘AIM: To investigate whether composite yogurt with acacia dietary fiber and Bifidobacterium lactis (B. lactis) has additive effects in irritable bowel syndrome (IBS). METHODS: A total of 130 patients were randomly allocated to consume, twice daily for 8 wk, either the composite yogurt or the control product. The composite yogurt contained acacia dietary fiber and high-dose B. lactis together with two classic yogurt starter cultures. Patients were evaluated using the visual analogscale via a structured questionnaire administered at baseline and after treatment. RESULTS: Improvements in bowel habit satisfaction and overall IBS symptoms from baseline were significantly higher in the test group than in the control group (27.16 vs 15.51, P = 0.010, 64.2 ± 17.0 vs 50.4 ± 20.5, P < 0.001; respectively). In constipation-predominant IBS, improvement in overall IBS symptoms was significantly higher in the test group than in the control group (72.4 ± 18.4 vs 50.0 ± 21.8, P < 0.001). In patients with diarrhea-predominant IBS, improvement in bowel habit satisfaction from baseline was significantly higher in the test group than in the control group (32.90 vs 7.81, P = 0.006).
