| 1 | Single center experience of capsule endoscopy in patients with obscure gastrointestinal bleeding显示文摘AIM:To identify optimum timing to maximize diagnostic yield by capsule endoscopy (CE) in patients with obscure gastrointestinal bleeding (OGIB).METHODS:We identified patients who underwent CE at our institution from August 2003 to December 2009.Patient medical records were reviewed to determine type of OGIB (occult,overt),CE results and complications,and timing of CE with respect to onset of bleeding.RESULTS:Out of 385 patients investigated for OGIB,284 (74%) had some lesion detected by CE.In 222 patients (58%),definite lesions were detected that could unequivocally explain OGIB.Small bowel ulcer/erosions secondary to Crohn's disease,tuberculosis or non-steroidal anti-inflammatory agent use were the commonest lesions detected.Patients with overt GI bleeding for < 48 h before CE had the highest diagnostic yield (87%).This was significantly greater (P < 0.05) compared to that in patients with overt bleeding prior to 48 h (68%),as well as those with occult OGIB (59%).CONCLUSION:We established the importance of early CE in management of OGIB.CE within 48 h of overt bleeding has the greatest potential for lesion detection. | Mahesh Kumar Goenka Shounak Majumder Sanjeev Kumar Pradeepta Kumar Sethy Usha Goenka | 2011 | World Journal of Gastroenterology2011,17,6: | 39 |
| 2 | Initial detections and spread of invasive Spodoptera frugiperda in China and comparisons with other noctuid larvae in cornfields using molecular techniques显示文摘The fall armyworm,Spodoptera frugiperda,is a species native to the Americas and has spread to many countries in Africa and Asia in recent years.Proactive actions for potential invasion of S.frugiperda to China coordinated by government agencies and agricultural extension systems resulted in timely detection in January 2019 in Yunnan province neighboring onto Myanmar.The extensive monitoring in southern provinces of China since February 2019 resulted in dynamic tracking of S.frugiperda spreading to 13 provincial regions in China within 4 months by May 10,2019,which is crucial for timely management actions in the fields.The first detections of S.frugiperda(corn strain)in China were confirmed using cytochrome oxidase subunit 1(COl)and triosephosphate isomerase(Tpi)genes molecular marker method.In addition to S.frugiperda,larvae of three other noctuid species with similar morphological appearance(S.litura,S.exigna and Mythimna separata)can occur simultaneously and cause similar damage in cornfields in southern China.Thus,we can use both morphological and molecular marker methods to compare larval stages of four noctuid species.Further,we discuss the risk of potential spread of invasive S.frugiperda to other regions and impact on corn production in China. | Da-Peng Jing Jing-Fei Guo Yu-Ying Jiang Jian-Zhou Zhao Amit Sethi Kang-Lai He Zhen-Ying Wang | 2020 | Insect Science2020,27,4: | 35 |
| 3 | Targeting the PI3K/Akt signaling pathway in gastric carcinoma: A reality for personalized medicine?显示文摘Frequent activation of phosphatidylinositol-3 kinases(PI3K)/Akt/m TOR signaling pathway in gastric cancer(GC) is gaining immense popularity with identification of mutations and/or amplifications of PIK3 CA gene or loss of function of PTEN,a tumor suppressor protein,to name a few; both playing a crucial role in regulating this pathway. These aberrations result in dysregulation of this pathway eventually leading to gastric oncogenesis,hence,there is a need for targeted therapy for more effective anticancer treatment. Several inhibitors are currently in either preclinical or clinical stages for treatment of solid tumors like GC. With so many inhibitors under development,further studies on predictive biomarkers are needed to measure the specificity of any therapeutic intervention. Herein,we review the common dysregulation of PI3K/Akt/m TOR pathway in GC and the various types of single or dual pathway inhibitors under development that might have a superior role in GC treatment. | 4 | Selective embolization for bleeding visceral artery pseudoaneurysms in patients with pancreatitis显示文摘BACKGROUND:Pancreatitis is associated with arterial complications in 4%-10%of patients,with untreated mortality approaching 90%.Timely intervention at a specialist center can reduce the mortality to 15%.We present a single institution experience of selective embolization as first line management of bleeding pseudoaneurysms in pancreatitis. METHODS:Sixteen patients with pancreatitis and visceral artery pseudoaneurysms were identified from searches of the records of interventional angiography from January 2000 to June 2007.True visceral artery aneurysms and pseudoaneurysms arising as a result of post-operative pancreatic or biliary leak were excluded from the study. RESULTS:In 50%of the patients,bleeding complicated the initial presentation of pancreatitis.Alcohol was the offending agent in 10 patients,gallstones in 3,trauma,drug-induced and idiopathic pancreatitis in one each.All 16 patients had a contrast CT scan and 15 underwent coeliac axis angiography. The pseudoaneurysms ranging from 0.9 to 9.0 cm affected the splenic artery in 7 patients:hepatic in 3,gastroduodenal and right gastric in 2 each,and left gastric and pancreatico-duodenal in 1 each.One patient developed spontaneous thrombosis of the pseudoaneurysm.Fourteen patients had effective coil embolization of the pseudoaneurysm.One patient needed surgical exclusion of the pseudoaneurysm following difficulty in accessing the coeliac axis radiologically.There were no episodes of re-bleeding and no in-hospital mortality. | 5 | Utility of flexible fiberoptic bronchoscopy for critically ill pediatric patients:A systematic review显示文摘AIM: To investigate the diagnostic yield, therapeutic efficacy, and rate of adverse events related to flexible fiberoptic bronchoscopy(FFB) in critically ill children. METHODS: We searched Pub Med, SCOPUS, OVID, and EMBASE databases through July 2014 for English language publications studying FFB performed in the intensive care unit in children < 18 years old. We identified 666 studies, of which 89 full-text studies were screened for further review. Two reviewers independently determined that 27 of these studies met inclusion criteria and extracted data. We examined the diagnostic yield of FFB among upper and lower airway evaluations, as well as the utility of bronchoalveolar lavage(BAL). RESULTS: We found that FFB led to a change in medical management in 28.9%(range 21.9%-69.2%) of critically ill children. The diagnostic yield of FFB was 82%(range 45.2%-100%). Infectious organisms were identified in 25.7%(17.6%-75%) of BALs performed, resulting in a change of antimicrobial management in 19.1%(range: 12.2%-75%). FFB successfully reexpanded atelectasis or removed mucus plugs in 60.3%(range: 23.8%-100%) of patients with atelectasis. Adverse events were reported in 12.9%(range: 0.5%-71.4%) of patients. The most common adverse effects of FFB were transient hypotension, hypoxia and/or bradycardia that resolved with minimal intervention, such as oxygen supplementation or removal of the bronchoscope. Serious adverse events were uncommon; 2.1% of adverse events required intervention such as bag-mask ventilation or intubation and atropine for hypoxia and bradycardia, normal saline boluses for hypotension, or lavage and suctioning for hemorrhage. | 6 | A SURVEY OF STACKELBERG DIFFERENTIAL GAME MODELS IN SUPPLY AND MARKETING CHANNELS显示文摘Stackelberg differential game models have been used to study sequential decision making in noncooperative games in diverse fields.In this paper,we survey recent applications of Stackelberg differential game models to the supply chain management and marketing channels literatures.A common feature of these applications is the specification of the game structure:a decentralized channel composed of a manufacturer and independent retailers,and a sequential decision procedure with demand and supply dynamics and coordination issues.In supply chain management,Stackelberg differential games have been used to investigate inventory issues,wholesale and retail pricing strategies,and outsourcing in dynamic environments.The underlying demand typically has growth dynamics or seasonal variation.In marketing,Stackelberg differential games have been used to model cooperative advertising programs,store brand and national brand advertising strategies,shelf space allocation,and pricing and advertising decisions.The demand dynamics are usually extensions of the classical advertising capital models or sales-advertising response models.We begin by explaining the Stackelberg differential game solution methodology and then provide a description of the models and results reported in the literature. | Xiuli HE Ashutosh PRASAD Suresh P. | 7 | Pseudoaneurysm following laparoscopic cholecystectomy显示文摘BACKGROUND:Laparoscopic cholecystectomy(LC)is the operation of choice for removal of the gallbladder. Unrecognized bile duct injuries present with biliary peritonitis and systemic sepsis.Bile has been shown to cause damage to the vascular wall and therefore delay the healing of injured arteries leading to pseudoaneurysm formation.Failure to deal with bile leak and secondary infection may result in pseudoaneurysm formation. This study was to report the incidence and outcomes of pseudoaneurysm in patients with bile leak following LC referred to our hospital. METHODS:A retrospective analysis of our prospectively maintained liver database using pseudoaneurysm, bile leak and bile duct injury following laparoscopic cholecystectomy from January 2000 to December 2005 was performed. RESULTS:A total of 86 cases were referred with bile duct injury and bile leak following LC and of these,4 patients (4.5%)developed hepatic artery pseudoaneurysm(HAP) presenting with haemobilia in 3 and massive intra- abdominal bleed in 1.Selective visceral angiography confirmed pseudoaneurysm of the right hepatic artery in 2 cases,cystic artery stump in one and an intact but ectatic hepatic artery with surgical clips closely applied to the right hepatic artery at the origin of the cystic artery in the fourth case.Effective hemostasis was achieved in 3 patients with coil embolization and the fourth patient required emergency laparotomy for severe bleeding and hemodynamic instability due to a ruptured right hepatic artery.Of the 3 patients treated with coil embolization, 2 developed late strictures of the common hepatic duct. . (CHD)requiring hepatico-jejunostomy and one developed a stricture of left hepatic duct.All the 4 patients are alive at a median follow up of 17 months(range 1 to 65)with normal liver function tests. | 8 | Extraocular retinoblastoma in Indian children:clinical,imaging and histopathological features显示文摘AIM:To study eyes with extraocular dissemination(EORB),with the following aims:first to establish the mean lag period and to understand various reasons for delayed presentation,second to study their imaging profiles and third to analyze histopathological features of eyes enucleated after neoadjuvant chemotherapy.·METHODS:Prospective study of clinical and imaging features of EORBs(stage Ⅲ and Ⅳ International Retinoblastoma Staging System) presenting to a tertiary eye care centre.Histopathological features of eyes enucleated after receiving neoadjuvant chemotherapy were analyzed.A pictorial illustration of the varied imaging profile of EORB was also presented.·RESULTS:Over a period of one year,97 eyes were diagnosed with retinoblastoma;32 children(36 eyes)(37.1%) had EORB.Mean age 3.6±1.9 years,71.9% males,71.9% unilateral,3.1% with positive family history and40.6% with metastasis.On imaging,there was extrascleral involvement in 22.2%,involvement of orbital part of optic nerve in 33.3%,involvement of central nervous system in 27.8% and orbital wall involvement in2.9% eyes.On histopathological analysis of eyesenucleated after neoadjuvant chemotherapy,25.0% had no residual viable tumour tissue and rest all tumours were poorly differentiated.·CONCLUSION:There are very few human malignancies where definitive treatment is started without any confirmed histopathological diagnosis and imaging plays an important role in diagnosis and appropriate staging of the disease.Chemotherapy has a variable effect on EORB,75.0% of eyes with EORB had residual viable tumour tissue when enucleated after receiving neoadjuvant chemotherapy. | Sumita Sethi Neelam Pushker Seema Kashyap Sanjay Sharma Mridula Mehta Sameer Bakhshi Saurbhi Khurana Supriyo Ghose | 2013 | International Journal of Ophthalmology(English edition)2013,6,4: | 5 |

| 10 | Garcinol sensitizes human head and neck carcinoma to cisplatin in a xenograft mouse model despite downregulation of proliferative biomarkers显示文摘OBJECTIVE Platinum compounds such as cisplatin and carboplatin are frequently used as the first-line chemotherapy for the treatment of the head and neck squamous cell carcinoma(HNSCC).In the present study,we investigated whether garcinol,apolyisoprenylated benzophenone can chemosensitize HNSCC to cisplatin.METHODS The effect of garcinol and cisplatin on HNSCC was assessed by MTT,Western blotting,real time PCR,FACS,immunohistochemistry,DNA binding assay and xenograft mouse model.RESULTS We found that garcinol inhibited the viability of a panel of diverse HNSCC cell lines,enhanced the apoptotic effect of cisplatin,suppressed constitutive as well as cisplatin-induced NF-κB activation,and downregulated the expression of various oncogenic gene products(cyclin D1,Bcl-2,survivin and VEGF).In vivo study showed that administration of garcinol alone(0.5 mg·kg-1,ip five times/week)significantly suppressed the growth of the tumor,and this effect was further increased by cisplatin.Both the markers of proliferation index(Ki-67)and microvessel density(CD31)were downregulated in tumor tissues by the combination of cisplatin and garcinol.The pharmacokinetic results of garcinol indicated that good systemic exposure was achievable after ip administration of garcinol at 0.5and 2mg·kg-1 with mean peak concentration(cmax)of 1825.4 and 6635.7nmol·L-1 in the mouse serum,respectively.CONCLUSION Overall,our results suggest that garcinol can indeed potentiate the effects of cisplatin by negative regulation of various inflammatory and proliferative biomarkers. | Feng Li Muthu K. | 12 | RNF6 promotes myeloma cell proliferation and survival by inducing glucocorticoid receptor polyubiquitination显示文摘RNF6,a RING-type ubiquitin ligase,has been identified as an oncogene in various cancers but its role in multiple myeloma(MM)remains elusive.In the present study we first showed that the expression levels of RNF6 in MM were significantly elevated compared with the bone marrow cells of healthy donors.Overexpression of RNF6 in LP1 and PRMI-8266 MM cell lines promoted cell proliferation,whereas knockdown of RNF6 led to apoptosis of MM cells.Furthermore,we revealed that RNF6,as a ubiquitin ligase,interacted with glucocorticoid receptor(GR)and induced its K63-linked polyubiquitination.Different from current knowledge,RNF6 increased GR stability at both endogenous and exogenous contexts.Such an action greatly promoted GR transcriptional activity,which was confirmed by luciferase assays and by the increased expression levels of prosurvival genes including Bcl-xL and Mcl-1,two typical downstream genes of the GR pathway.Consistent with these findings,ectopic expression of RNF6 in MM cells conferred resistance to dexamethasone,a typical anti-myeloma agent.In conclusion,we demonstrate that RNF6 promotes MM cell proliferation and survival by inducing atypical polyubiquitination to GR,and RNF6 could be a promising therapeutic target for the treatment of MM. | Ying Ren Xin Xu Chen-yu Mao Kun-kun Han Yu-jia Xu Bi-yin Cao Zu-bin Zhang Gautam Sethi Xiao-wen Tang Xin-liang Mao | 2020 | Acta Pharmacologica Sinica2020,41,3: | 3 |
| 13 | Maturity status classification of papaya fruits based on machine learning and transfer learning approach显示文摘Papaya(Carica papaya)is a tropical fruit having commercial importance because of its high nutritive and medicinal value.The packaging of papaya fruit as per its maturity status is an essential task in the fruit industry.The manual grading of papaya fruit based on human visual perception is time-consuming and destructive.The objective of this paper is to suggest a novel non-destructive maturity status classification of papaya fruits.The paper suggested two approaches based on machine learning and transfer learning for classification of papaya maturity status.Also,a comparative analysis is carried out with different methods of machine learning and transfer learning.The experimentation is carried out with 300 papaya fruit sample images which includes 100 of each three maturity stages.The machine learning approach includes three sets of features and three classifiers with their different kernel functions.The features and classifiers used in machine learning approaches are local binary pattern(LBP),histogram of oriented gradients(HOG),Gray Level Cooccurrence Matrix(GLCM)and k-nearest neighbour(KNN),support vector machine(SVM),Naı¨ve Bayes respectively.The transfer learning approach includes seven pretrained models such as ResNet101,ResNet50,ResNet18,VGG19,VGG16,GoogleNet and AlexNet.The weighted KNN with HOG feature outperforms other machine learningbased classification model with 100%of accuracy and 0.0995 s training time.Again,among the transfer learning approach based classification model VGG19 performs better with 100%accuracy and 1 min 52 s training time with consideration of early stop training.The proposed classification method for maturity classification of papaya fruits,i.e.VGG19 based on transfer learning approach achieved 100%accuracy which is 6%more than the existing method. | Santi Kumari Behera Amiya Kumar Rath Prabira Kumar Sethy | 2021 | Information Processing in Agriculture2021,8,2: | 3 |

| 15 | Management of glaucoma in pregnancy:risks or choices,a dilemma?显示文摘·The treatment of glaucoma in and around pregnancy offers the unique challenge of balancing the risk of vision loss to the mother as against the potential harm to the fetus or newborn. Most anti-glaucoma drugs(i.e.beta-blockers, prostaglandin analogues, carbonic anhydrase inhibitors topical and systemic, cholinergics,anticholinesterases, and apraclonidine) are considered category C agents and ophthalmologists are usually limited to treating patients with the category B drugs of brimonidine and dipivefrin. Brimonidine is generally the preferred first-line drug in the first, second and early third trimester. Late in the third trimester, brimonidine should be discontinued because it can induce central nervous system depression in newborns wherein topical carbonic anhydrase inhibitors may be the optimal choice.Glaucoma surgery can be performed with caution in second and third trimester if the patients have a strong indication for the procedure. However, anesthetics,sedative agents, and antimetabolites still have potential risk for the fetus. Argon laser trabeculoplasty(ALT) or selective laser trabeculoplasty(SLT) is an alternative treatment that can be performed in all trimesters.Carbonic anhydrase inhibitors and β-blockers are certified by the American Academy of Pediatrics for use during nursing. However, low doses of these medications should be considered when used in the breast feeding period.
