Scrub typhus:pathophysiology, clinical manifestations and prognosis显示文摘Scrub typhus is a zoonosis caused by the pathogen Orientia tsutsugamushi(O.tsvtsugamushi). The disease has significant prevalence in eastern and Southeast Asia.Usually presenting as an acute febrile illness, the diagnosis is often missed because of similarities with oilier tropical febrile infections. Many unusual manifestations are present, and these are described in this review, together with an outline of current knowledge of pathophysiology.Awareness of these unusual clinical manifestations will help the clinician to arrive at an early diagnosis, resulting in early administration of appropriate antibiotics.Prognostic indicators for severe disease have not yet been clearly established. | Senaka Rajapakse Chaturaka Rodrigo Deepika Fernando | 2012 | Asian Pacific Journal of Tropical Medicine2012,5,4: | 12
In vitro investigation of the effect of dairy propionibacteria on rumen pH,lactic acid and volatile fatty acids显示文摘Ruminal acidosis is a prevalent disorder in ruminants such as dairy cows and feedlot beef cattle, caused primarily by the inclusion of a high percentage of readily fermentable concentrates in the diet. The disorder presents as an accumulation of lactic acid, a decrease of p H in the rumen and a subsequent imbalance of the rumen fermentation process with detrimental impacts on the animal's health and productivity. Dairy propionibacteria, a group of bacteria characterised by utilization of lactic acid as the favoured carbon source, with propionic acid produced as a by-product, were evaluated in this study as potential direct-fed microbials for use in controlling ruminal acidosis. Acidosis was simulated by introduction of high concentrations of lactic acid into rumen fluid samples and a multi-strain in vitro analysis was conducted, whereby changes in p H and lactic acid metabolism were compared in identical acidified rumen samples, following inoculation with various propionibacteria. This was followed by a study to evaluate the effect of bacterial inoculation dosage on acid metabolism. The results indicated that lactic acid levels in the rumen fluid were significantly reduced, and propionic acid and acetic acid concentrations both significantly increased, following addition of propionibacteria. Significant ‘between strains' differences were observed, with Propionibacterium acidopropionici 341, Propionibacterium freudenreichii CSCC 2207, Propionibacterium jensenii NCFB 572 and P. jensenii 702 each producing more rapid reduction of lactic acid concentration than P. freudenreichii CSCC 2206, P. acidopropionici ATCC 25562 and Propionibacterium thoenii ATCC 4874. Furthermore, the efficacy of this application was dosage related, with the rates of reduction in lactic acid levels and production of propionic acid, both significantly greater for the higher(1010 cfu mL^(-1)) compared with lower(105 cfu mL^(-1)) dosage inoculation. The results confirmed that the introduction of propionibacteria could promote more rapid reduction of lactic acid levels than would occur without their addition, demonstrating their potential in controlling ruminal acidosis. | Jianbiao Luo Chaminda Senaka Ranadheera Stuart King Craig Evans Surinder Baines | 2017 | Journal of Integrative Agriculture2017,16,7: | 2

Exertional heat stroke in a young military trainee:is it preventable?显示文摘Background: Heat stroke is a life-threatening condition with exertional heat stroke occurring frequently among soldiers and athletes. Because of its common occurrence, many military trainees practice preventive measures prior to any activity requiring severe exertion. Although it is said to be common in practice, different presentations of heat stroke are scarcely described in literature.Case Presentation: We describe a case of an exertional heat stroke in a 23-year-old male Sinhalese soldier who developed early changes of renal failure, liver failure and rhabdomyolysis. The patient initially presented with convulsions, delirium and loss of consciousness to an outside health care facility before being transferred to our institution. Conclusion: It is clear that heat stroke does occur in military trainees while preventive strategies are being practiced. It is important for those who provide healthcare to soldiers to provide proper advice on how to identify impending heat stroke prior to any exercises resulting in severe physical exertion. Further, treating physicians should educate all military trainees about preventive strategies. | Buddhika T.B.Wijerathne Senaka D.Pilapitiya Vadivel Vijitharan Mohammed M.F.Farah Yashodhara V.M.Wimalasooriya Sisira H Siribaddana | 2016 | Journal of Medical Colleges of PLA(China)2016,31,2: | 1

Production of short chain fatty acids and vitamin B_(12) during the in-vitro digestion and fermentation of probiotic chocolate显示文摘The prebiotic functions of chocolate to improve the function and health benefit of the chocolate when enriched with probiotics has not been fully investigated. This study investigated the role of chocolates enriched with probiotics in the production of SCFAs and vitamin B_(12) during the in-vitro colonic fermentation.Seven probiotic bacteria were encapsulated using a mixture of cocoa powder(10%),FOS(2%)and Na-alginate(1%)before added to chocolates. The results revealed that encapsulated Lactobacillus plantarum and Bifidobacterium animalis spp.lactis BB12 produced significantly higher amounts(1876.5±105.16 and 1348.51±77.37 mmol, respectively)of acetic acid after 48h of colonic fermentation.Chocolates with encapsulated BB12,La5,Lactobacillus sanfranciscensis and Streptococcus thermophilus yielded higher amounts of propionic, isobutyric, butyric and isovaleric acid.Probiotic-chocolates with 70%cocoa produced significantly more acetic, propionic, and butyric acids than that of 45%cocoa.However,Probiotic-chocolates with 45%cocoa produced greater amounts of vitamin B_(12) than 70%cocoa.These data demonstrate that probiotic-chocolate could be a functional snack with additional health benefits. | Md Nur Hossain Chaminda Senaka Ranadheera Zhongxiang Fang Said Ajlouni | 2022 | Food Bioscience2022,47,3: | 1
Serum nitrite levels in Sri Lankan patients with leptospirosis显示文摘Objective:To determine whether blood nitrite levels are elevated in patients with leptospirosis. Methods:Male patients Fulfilling clinical and epidemiological criteria for a diagnosis of leptospirosis were recruited. Those with MAT title of≥400 together with those seroconverting to a titer of≥200 were included in the analysis.Serum nitrite levels were measured in these patients and age.sex matched healthy controls. Results:Patients from 3 hospitals(n=75) were screened during a 3 month period from 28th June to 3rd September 2009,of whom 20 were eligible for the study.Serum nilrile levels were found to be significantly higher in patients with acute leptospirosis[n=20.(0.359±0.229)μM]compared to controls[(n= 13.(0.216±0.051)μM] (P=0.014).A significant correlation was also observed between the MAT titre and the day of illness (r = 0.547;P<0.0001).Conclusions:Serum nitrite levels are higher in patients with acute leptospirosis compared to age and sex matched controls. No correlation could be assessed with severity of illness,as sample size was inadequate to determine this. | Rohini I Gunaratna Shiroma M Handunnetti MRC Bulathsinghalage Pranitha Somaratne Ananda Jayanaga HJ de Silva Senaka Rajapakse | 2012 | Asian Pacific Journal of Tropical Medicine2012,5,1: | 0
Chemoprophylaxis in malaria:drugs, evidence of efficacy and costs显示文摘This review concentrates on different aspects of malaria chemoprophylaxis, namely drug combinations, resistance, impact of malaria prevention in pregnancy and cost effectiveness. A MEDLINE search was performed for all articles with the key word ’Malaria’ in the title field and ’Prophylaxis’ in any field. The search was restricted to articles published in English within the last decade(1999-2009).Data sources included review articles published in core clinical journals, cohort studies, interventional studies, case control studies and cross sectional analyses. The mechanism of action, trial evidence of efficacy, side effects and geographical distribution of resistance is discussed for each prophylactic drug regimen.Impact of prophylaxis in pregnancy and the cost considerations are discussed under two separate sub topics. | Sumadhya Deepika Fernando Chaturaka Rodrigo Senaka Rajapakse | 2011 | Asian Pacific Journal of Tropical Medicine2011,4,4: | 0
