1Cholangiopancreatography troubleshooting:the usefulness of endoscopic retrieval of migrated biliary and pancreatic stents显示文摘BACKGROUND:Stent migration in the hepatopancreatic duct might arise as one of the rare complications associated with biliary or pancreatic stenting.Although there are some procedures to retrieve the migrated stent,including surgical,percutaneous,and endoscopic approaches,endoscopy should be attempted first because it is least invasive.This study set out to evaluate the usefulness of endoscopic retrieval of migrated biliary and pancreatic stents.METHODS:Plastic stents that migrated in the bile duct(35 patients)or pancreatic duct(2)were retrieved with endoscopic retrograde cholangiopancreatography.Devices used were snare forceps,a basket catheter,grasping forceps,biopsy forceps,a balloon catheter,and the Soehendra stent retriever.RESULTS:Endoscopic retrieval of migrated stents was performed successfully in 36(97.0%)of the 37 patients.The devices utilized for successful treatment were basket catheter(13 patients),grasping forceps(10),snare forceps(8),balloon catheter(3),biopsy forceps(1),and the Soehendra stent retriever(1).The unsuccessfully treated patient with chronic pancreatitis underwent surgery since the guide wire did not move forward due to bile duct stenosis,and there was also duodenal stenosis.One patient developed mild pancreatitis after withdrawal of the stent;the pancreatitis was relieved with conservative treatment.CONCLUSIONS:Endoscopic retrieval of migrated biliary and pancreatic stents appears to be useful because of its safety and low invasiveness.However,various forceps should be prepared for the retrieval of a migrated stent.Yuji Sakai Toshio Tsuyuguchi Takeshi Ishihara Harutoshi Sugiyama Reiko Eto Tatsuya Fujimoto Shin Yasui Ryo Tamura Seiko Togo Motohisa Tada Osamu Yokosuka 2009Hepatobiliary & Pancreatic Diseases International2009,8,6:8
2Molecular Functions of Oxygen-Evolving Complex Family Proteins in Photosynthetic Electron Flow显示文摘Oxygen-evolving complex (OEC) protein is the original name for membrane-peripheral subunits of photosystem (PS) II. Recently, multiple isoforms and homologs for OEC proteins have been iden- tified in the chloroplast thylakoid lumen, indicating that functional diversification has occurred in the OEC family. Gene expression profiles suggest that the Arabidopsis OEC proteins are roughly categorized into three groups: the authentic OEC group, the stressresponsive group, and the group including proteins related to the chloroplast NAD(P)H dehydrogenase (NDH) complex involved in cyclic electron transport around PSI. 3Helicobacter heilmannii sensu stricto-related gastric ulcers:A case report显示文摘A spiral bacterium(SH9),morphologically different from Helicobacter pylori(H.pylori),was found in a 62-yearold woman’s gastric mucosa.Gastroscopic examination revealed multiple gastric ulcers near the pyloric ring;mapping gastric biopsy showed mild mononuclear infiltration with large lymphoid follicles in the antrum,without corpus atrophy.Urea breath test and H.pylori culture were negative,but Giemsa staining of biopsies revealed tightly coiled bacteria that immunostained with anti-H.pylori antibody.Sequencing of SH9 16S rRNA and the partial urease A and B subunit genes showed that the former sequence had highest similarity(99%;1302/1315 bp)to Helicobacter heilmannii(H.heilmannii)sensu stricto(H.heilmannii s.s.)BC1 obtained from a bobcat,while the latter sequence confirmed highest similarity(98.3%;1467/1493 bp)to H.heilmannii s.s.HU2 obtained from a human.The patient was diagnosed with multiple gastric ulcers associated with H.heilmannii s.s.infection.After triple therapy(amoxicillin,clarithromycin,and lansoprazole)with regimen for eradicating H.pylori,gastroscopy showed ulcer improvement and no H.heilmannii s.s.upon biopsy.Takehisa Matsumoto Masatomo Kawakubo Taiji Akamatsu Naohiko Koide Naoko Ogiwara Seiko Kubota Mitsutoshi Sugano Yoshiyuki Kawakami Tsutomu Katsuyama Hiroyoshi Ota 2014World Journal of Gastroenterology2014,20,12:4
4Endocytoscopic narrow-band imaging efficiency for evaluation of inflammatory activity in ulcerative colitis显示文摘AIM:To assess the efficacy of endocytoscopic narrowband imaging(EC-NBI)for evaluating the severity of inflammation in ulcerative colitis(UC).METHODS:This retrospective study was conducted at a single tertiary care referral center.We included UC patients who underwent colonoscopy with endocytoscopy from July 2010 to December 2013.ECNBI was performed,and the images were evaluated by assessing visibility,increased vascularization,and the increased calibers of capillaries and were classified as Obscure,Visible or Dilated.Obscure was indicative of inactive disease,while Visible and Dilated were indicative of acute inflammation.This study received Institutional Review Board approval.The primary outcome measures included the diagnostic ability of EC-NBI to distinguish between active and inactive UC on the basis of histological activity.The conventional endoscopic images were classified according to the Mayo endoscopic score.A score of 0 or 1 indicated inactive disease,whereas a score of 2 indicated active disease.RESULTS:Fifty-two patients were enrolled.There was a strong correlation between the EC-NBI findings and the histological assessment(r=0.871,P<0.01).The sensitivity,specificity,positive predictive value,negative predictive value,and accuracy of EC-NBI for diagnosing acute inflammation were 84.0%,100%,87.1%,100%,and 92.3%,respectively,while those for the Mayo endoscopic score were 100%,40.7%,100%,61.0%,and 69.2%,respectively.Compared with conventional endoscopy,EC-NBI was superior in diagnostic specificity,negative predictive value,and accuracy(P<0.001,P=0.001 and P=0.047,respectively).CONCLUSION:The EC-NBI finding of capillaries in the rectal mucosa was strongly correlated with histological inflammation and aided in the differential diagnosis between active and inactive UC.Yasuharu Maeda Kazuo Ohtsuka Shin-ei Kudo Kunihiko Wakamura Yuichi Mori Noriyuki Ogata Yoshiki Wada Masashi Misawa Akihiro Yamauchi Seiko Hayashi Toyoki Kudo Takemasa Hayashi Hideyuki Miyachi Fuyuhiko Yamamura Fumio Ishida Haruhiro Inoue Shigeharu Hamatani 2015World Journal of Gastroenterology2015,21,7:4
