2Effectiveness of Helicobacter pylori eradication in the treatment of early-stage gastric mucosa-associated lymphoid tissue lymphoma:An up-to-date meta-analysis显示文摘BACKGROUND Gastric mucosa-associated lymphoid tissue(MALT)lymphoma(GML)is usually a low-grade B-cell neoplasia strongly associated with Helicobacter pylori(H.pylori)-induced chronic gastritis.Clinical practice guidelines currently recommend H.pylori eradication as the preferred initial treatment for early-stage GML.To determine the practical effect of bacterial eradication as the sole initial therapy for early-stage GML,an updated analysis and review of available evidence is imperative.AIM To perform a meta-analysis to assess the rate of complete remission(CR)of H.pylori-positive early-stage GML following bacterial eradication.METHODS We performed independent,computer-assisted literature searches using the PubMed/MEDLINE,Embase,and Cochrane Central databases through September 2022.Prospective and retrospective observational studies evaluating the CR of early-stage GML following bacterial eradication in H.pylori-positive patients.The risk of bias was assessed using Joanna Briggs Institute(JBI)Critical Appraisal Tools.The pooled estimate of the complete histopathological remission rate and respective confidence intervals(95%CI)were calculated following the random-effects model.Heterogeneity and inconsistency were assessed using Cochran’s Q test and I2 statistic,and heterogeneity was defined as P<0.01 and I²>50%,respectively.Subgroup and meta-regression analyses were conducted to explore potential sources of heterogeneity.RESULTS The titles and abstracts of 1576 studies were screened;96 articles were retrieved and selected for full-text reading.Finally,61 studies were included in the proportional meta-analysis(P-MA).Forty-six were prospective and fifteen were retrospective uncontrolled,single-arm,observational studies.The overall risk of bias was low to moderate in all but a single report,with an average critical appraisal score across all studies of 79.02%.A total of 2936 H.pylori-positive early-stage GML patients,in whom H.pylori was successfully eradicated,were included in the analysis.The pooled CR of H.pylori-positive early-stage GML after bacterial eradication was 75.18%(95%CI:70.45%-79.91%).P-MA indicated the substantial heterogeneity in CR reported across studies(I2=92%;P<0.01).Meta-regression analysis identified statistically significant effect modifiers,including the proportion of patients with t(11;18)(q21;q21)-positive GML and the risk of bias in each study.CONCLUSION Comprehensive synthesis of available evidence suggests that H.pylori eradication is effective as the sole initial therapy for early-stage GML.Although the substantial heterogeneity observed across studies limits the interpretation of the pooled overall CR,the present study is a relevant to informing clinical practice.Fabian Fellipe Bueno Lemos Caroline Tianeze de Castro Mariana Santos Calmon Marcel Silva Luz Samuel Luca Rocha Pinheiro Clara Faria Souza Mendes dos Santos Gabriel Lima Correa Santos Hanna Santos Marques Henrique Affonso Delgado Kádima Nayara Teixeira Cláudio Lima Souza Márcio Vasconcelos Oliveira Fabrício Freire de Melo 2023World Journal of Gastroenterology2023,29,14:2

13Depressant effects of Agastache mexicana methanol extract and one of major metabolites tilianin显示文摘Objective:To determine the depressant-like effects and the possible mechanism of action of tilianin isolated from active methanol extract of Agastache mexicana(A.mexicana).Also,to establish the pharmacophoric requirements of tilianin,as a possible ligand of GABA_A/BZD receptor,by the alignment of diazepam.CGS-9896 and diindole,using a previously described pharmacophoric model.Methods:Tilianin(30 to 300 mg/kg.ip.and 300 mg/kg,pa.) and methanol crude extract(10 to 300 mg/kg,ip.and 300 mg/kg po.) from A.mexicana were evaluated for potential sedative and anxiolytic-like response drugs by using open-field,hole-board,cylinder of exploration,plus-maze and sodium pentobarbital-induced hypnosis mice methods.Results:Methanol extract and tilianin showed anxiolytic-like activity from a dosage of 30 mg/kg,ip.or 300 mg/kg,po.and were less potent than diazepam 0.1 mg/kg.a reference anxiolytic drug used.Moreover,depressant activity of both potentiates sodium pentobarbital(SP)-induced sleeping time.The anxiolytic-like effect of 30 mg/kg ip.observed for the extract and tilianin,by using the plus-maze model,was partially prevented in the presence of flumazenil(a GABA_A/BZD antagonist,5 mg/kg ip.) but not in the presence of WAY100635(a selective 5-HT_(1A) receptor antagonist,0.32 mg/kg.ip.).Pharmacophoric modeling alignments of three agonist of GABA_A/BZD allow identify seven chemical features.Tilianin contains six of the seven features previously determined.Conclusions:Results indicate that tilianin is one of the bioactive metabolites in the anxiolytic-like activity of 4.mexicana.reinforcing its central nervous system uses,where GABA_A/BZD,but not 5-HT_(1A),receptors are partially involved.María Eva González-Trujano Hilda Ponce-Muoz Sergio Hidalgo-Figueroa Gabriel Navarrete-Vázquez Samuel Estrada-Soto 2015Asian Pacific Journal of Tropical Medicine2015,8,3:0
14New avenues for the treatment of immunotherapy-resistant pancreatic cancer显示文摘Pancreatic cancer(PC)is characterized by its extremely aggressive nature and ranks 14th in the number of new cancer cases worldwide.However,due to its complexity,it ranks 7th in the list of the most lethal cancers worldwide.The pathogenesis of PC involves several complex processes,including familial genetic factors associated with risk factors such as obesity,diabetes mellitus,chronic pancreatitis,and smoking.Mutations in genes such as KRAS,TP53,and SMAD4 are linked to the appearance of malignant cells that generate pancreatic lesions and,consequently,cancer.In this context,some therapies are used for PC,one of which is immunotherapy,which is extremely promising in various other types of cancer but has shown little response in the treatment of PC due to various resistance mechanisms that contribute to a drop in immunotherapy efficiency.It is therefore clear that the tumor microenvironment(TME)has a huge impact on the resistance process,since cellular and non-cellular elements create an immunosuppressive environment,characterized by a dense desmoplastic stroma with cancerassociated fibroblasts,pancreatic stellate cells,extracellular matrix,and immunosuppressive cells.Linked to this are genetic mutations in TP53 and immunosuppressive factors that act on T cells,resulting in a shortage of CD8+T cells and limited expression of activation markers such as interferon-gamma.In this way,finding new strategies that make it possible to manipulate resistance mechanisms is necessary.Thus,techniques such as the use of TME modulators that block receptors and stromal molecules that generate resistance,the use of genetic manipulation in specific regions,such as microRNAs,the modulation of extrinsic and intrinsic factors associated with T cells,and,above all,therapeutic models that combine these modulation techniques constitute the promising future of PC therapy.Thus,this study aims to elucidate the main mechanisms of resistance to immunotherapy in PC and new ways of manipulating this process,resulting in a more efficient therapy for cancer patients and,consequently,a reduction in the lethality of this aggressive cancer.Luis Guilherme de Oliveira Silva Fabian Fellipe Bueno Lemos Marcel Silva Luz Samuel Luca Rocha Pinheiro Mariana dos Santos Calmon Gabriel Lima Correa Santos Gabriel Reis Rocha Fabrício Freire de Melo 2024World Journal of Gastrointestinal Oncology2024,16,4:0

16Acute heart failure in the elderly:setting related differences in clinical features and management显示文摘BACKGROUND Administrative data show that acute heart failure(HF)patients are older than those enrolled in clinical registries and frequently admitted to non-cardiological settings of care.The purpose of this study was to describe clinical characteristics of old patients hospitalised for acute HF in Cardiology,Internal Medicine or Geriatrics wards.METHODS Data came from ATHENA(AcuTe Heart failurE in advaNced Age)registry which included elderly patients(≥65 years)admitted to the above mentioned settings of care from December 1,2014 to December 1,2015.RESULTS We enrolled 396 patients,15.4%assigned to Cardiology,69.7%to Internal Medicine,and 14.9%to a Geriatrics ward.Mean age was 83.5±7.6 years(51.8%of patients≥85 years)and was higher in patients admitted to Geriatrics(P<0.001);more than half were females.Medical treatments did not differ significantly among settings of care(in a context of a low prescription rate of renin-angiotensin-aldosterone system inhibitors)whereas significant differences were observed in comorbidity patterns and management guidelines recommendation adherence for decongestion evaluation with comparison of weight and N-terminal pro-B-type natriuretic peptide levels on admission and at discharge(both P=0.035 and P<0.001),echocardiographic evaluation(P<0.001)and follow-up visits planning(P<0.001),all higher in Cardiology.Mean in-hospital length of stay was 9±5.9 days,significantly higher in Geriatrics(13.7±6.5 days)and Cardiology(9.9±6.7 days)compared to Internal Medicine(8±5.2 days),P<0.001.In-hospital mortality was 9.3%,resulting higher in Geriatrics(18.6%)and Cardiology(16.4%)than Internal Medicine(5.8%),P=0.001.CONCLUSIONS In elderly patients hospitalised for acute HF,clinical characteristics and management differ significantly according to the setting of admission.Francesco Orso Alessandra Pratesi Andrea Herbst Anna Chiara Baroncini Francesca Bacci Gabriele Ciuti Andrea Berni Camilla Tozzetti Carlo Nozzoli Alberto Moggi Pignone Loredana Poggesi Luciano Gabbani Mauro Di Bari Francesco Fattirolli Massimo Milli Andrea Ungar NiccolòMarchionni Samuele Baldasseroni 2021Journal of Geriatric Cardiology2021,18,6:0

18Chemical Composition and Bioactivity of Lippia adoensis Hochst ex.Walp(Verneneaceae) Leaf Essential Oil Against Callosobruchus maculatus Fabricius(Coleoptera: Chrysomelidae)显示文摘Essential oil(EO) of Nigeria-grown Lippia adoensis leaf was analyzed using gas chromatography mass spectrometry(GCMS) and its fumigant and repellent properties against Callosobruchus maculatus were evaluated. Sixteen compounds predominated by monoterpenes were identified. The major compounds were Eucalyptol(28.36%), α-Terpineol(25.99%), γ-Terpinene(15.24%), α-Pinene(5.08%), 1H-Cyclopropa[a]naphthalene(4.25%) and 1, 3, 6, 10-Dodecatetraene(3.74%). Percentage mortality due to fumigant toxicity was dose- and exposure period-dependent. One hour after treatment(HAT), application of L. adoensis leaf EO at 107 μL· L^(-1) air caused significantly(p?0.05) higher mortality(50.00%) than 0.00% mortality observed at 0-53 μL· L^(-1) air, but not significantly(p?0.05) different from 22.50% observed in 80 μL· L^(-1) air. At 3 HAT, application of L. adoensis EO at 80 μL· L^(-1)air caused significantly higher mortality(90.00 %) than mortality observed at 0 μL· L^(-1) air. At 6 HAT, application of L. adoensis EO at 53-107 μL· L^(-1) air caused significantly higher mortality(100.00 %) than that was observed in the control. The same trend was observed at 12 HAT where 100 % mortality observed in 27-107 μL· L^(-1) air was significantly greater than 13.33 % observed in the control. At 3 HAT, percentage repellence was significantly(p<0.05) affected by doses.
