| 1 | Treatment of laryngopharyngeal reflux disease: A systematic review显示文摘BACKGROUNG For a long time,laryngopharyngeal reflux disease(LPRD)has been treated by proton pump inhibitors(PPIs)with an uncertain success rate.AIM To shed light the current therapeutic strategies used for LPRD in order to analysis the rationale in the LPRD treatment.METHODS Three authors conducted a PubMed search to identify papers published between January 1990 and February 2019 about the treatment of LPRD.Clinical prospective or retrospective studies had to explore the impact of medical treatment(s)on the clinical presentation of suspected or confirmed LPRD.The criteria for considering studies for the review were based on the population,intervention,comparison,and outcome framework.RESULTS The search identified 1355 relevant papers,of which 76 studies met the inclusion criteria,accounting for 6457 patients.A total of 64 studies consisted of empirical therapeutic trials and 12 were studies where authors formally identified LPRD with pH-monitoring or multichannel intraluminal impedance-pH monitoring(MII-pH).The main therapeutic scheme consisted of once or twice daily PPIs for a duration ranged from 4 to 24 wk.The most used PPIs were omeprazole,esomeprazole,rabeprazole,lansoprazole and pantoprazole with a success rate ranging from 18%to 87%.Other composite treatments have been prescribed including PPIs,alginate,prokinetics,and H2 Receptor antagonists.CONCLUSION Regarding the development of MII-pH and the identification of LPRD subtypes(acid,nonacid,mixed),future studies are needed to improve the LPRD treatment considering all subtypes of reflux. | Jerome R Lechien Francois Mouawad Maria R Barillari Andrea Nacci Seyyedeh Maryam Khoddami Necati Enver Sampath Kumar Raghunandhan Christian Calvo-Henriquez Young-Gyu Eun Sven Saussez | 2019 | World Journal of Clinical Cases2019,7,19: | 9 |

However, there are high failure rates associated with tumor ingrowth and epithelial overgrowth as well as internally from biofilm development and subsequent clogging. Within the last decade, the use of prophylactic drug eluting stents as a means to reduce stent failure has been investigated. In this review we provide an overview of the current research on drug eluting biliary stents. | 3 | Asiaticoside, a trisaccaride triterpene induces biochemical and molecular variations in brain of mice with parkinsonism显示文摘Background:Parkinson’s disease characterized by oxidative stress and mitochondrial damage in the pars compacta of substantia nigra remains a challenge to manage with an added disadvantage of side effects of L-levo dopa,the standard drug used for therapy.Thus,an alternative approach of utilizing natural components would be beneficial in the management of the disease.The present study was aimed to investigate the potential role of asiaticoside(As),a trisaccaride triterpene against1–methyl 4–phenyl 1,2,3,6 tetrahydropyridine(MPTP)-induced neurotoxicity in experimental mice.Methods:Mice were divided into 4 groups:Group I received vehicle saline,group II was treated with 20 mg/kg of body weight of MPTP(2 doses with 2 h intervals),group III received MPTP along with 50 mg/kg body weight of As for the 21 consecutive days starting from the day of MPTP intoxication.Group IV received 50 mg/kg body weight of asiaticoside for the same period serving as drug control.Animals were sacrificed at the end of experimental period and the striatum and midbrain samples were analyzed for enzyme assays,transmission electron microscopic(TEM)analysis.Immunofluorescent assay was performed to study the expression of GFAP to detect astrocyte,which are activated due to neuronal damage.Imunohistochemical studies were carried out to quantify the expression of Bax and Bcl2,the molecular signatures that would provide clues of the extent of neurodegeneration.Results:The activities of enzymes were increased on As administration when compared with those of group II animals.Expressions of Bax and Bcl2 along with GFAP did show significant variations(p<0.05)on MPTP treatment when compared to control animals and the changes were found to be reversed significantly(p<0.05)after treatment with asiaticoside.TEM analysis also showed attenuated degenerative architecture on As administration.The mice which received As alone(drug control IV)did not show significant variation from that of the control mice.Conclusion:The observations suggest that asiaticoside may be efficacious in protecting neurons from the oxidative damage caused by the insult of MPTP. | Uvarajan Sampath Vanisree Arambakkam Janardhanam | 2013 | Translational Neurodegeneration2013,2,1: | 4 |

| 5 | Anti-synchronization of Four-wing Chaotic Systems via Sliding Mode Control显示文摘Sliding mode control is an important method used in nonlinear control systems. In robust control systems, the sliding mode control is often adopted due to its inherent advantages of easy realization, fast response and good transient performance as well as its insensitivity to parameter uncertainties and disturbances. In this paper, we derive new results based on the sliding mode control for the anti-synchronization of identical Qi three-dimensional (3D) four-wing chaotic systems (2008) and identical Liu 3D four-wing chaotic systems (2009). The stability results for the anti-synchronization schemes derived in this paper using sliding mode control (SMC) are established using Lyapunov stability theory. Since the Lyapunov exponents are not required for these calculations, the SMC method is very effective and convenient to achieve global chaos anti-synchronization of the identical Qi four-wing chaotic systems and identical Liu four-wing chaotic systems. | 7 | Autonomic functions and gastric motility in children with functional abdominal pain disorders显示文摘BACKGROUND Abdominal pain-predominant functional gastrointestinal disorders(AP-FGIDs)are the most common cause of recurrent abdominal pain in children. Despite its high prevalence, the underlying pathophysiology of this condition is poorly understood.AIM To assess the role of gastric dysmotility and autonomic nervous system dysfunction in the pathophysiology of AP-FGIDs.METHODS One hundred children, fulfilling Rome III criteria for AP-FGIDs, and 50 healthy controls, aged 5 to 12 years, were recruited after obtaining parental consent. All patients were investigated for underlying organic disorders. Gastric motility and cardiovascular autonomic functions were assessed using validated non-invasive techniques.RESULTS The main gastric motility parameters assessed(gastric emptying rate [45.7 vs 59.6 in controls], amplitude [48.7 vs 58.2], frequency of antral contractions [8.3 vs 9.4],and antral motility index [4.1 vs 6.4]) were significantly lower in children with AP-FGIDs(P < 0.05). The post-prandial antral dilatation at 1 min after the test meal significantly correlated with the severity of abdominal pain(P < 0.05).Assessment of autonomic functions in AP-FGID patients showed neither a significant difference compared to the control group, nor a correlation with gastric motility abnormalities(P > 0.05). The duration of pain episodes negatively correlated with the parasympathetic tone(maladaptive parasympathetic tone)(P< 0.05).CONCLUSION Children with AP-FGIDs have abnormal gastric motility but normal cardiovascular autonomic functions. There is no relationship between abnormal gastric motility and autonomic functions. | 8 | A combined technique of Kalman filter, artificial neural network and fuzzy logic for gas turbines and signal fault isolation显示文摘The target of this paper is the performance-based diagnostics of a gas turbine for the automated early detection of components malfunctions. The paper proposes a new combination of multiple methodologies for the performance-based diagnostics of single and multiple failures on a two-spool engine. The aim of this technique is to combine the strength of each methodology and provide a high success rate for single and multiple failures with the presence of measurement malfunctions. A combination of KF(Kalman Filter), ANN(Artificial Neural Network) and FL(Fuzzy Logic) is used in this research in order to improve the success rate, to increase the flexibility and the number of failures detected and to combine the strength of multiple methods to have a more robust solution. The Kalman filter has in his strength the measurement noise treatment, the artificial neural network the simulation and prediction of reference and deteriorated performance profile and the fuzzy logic the categorization flexibility, which is used to quantify and classify the failures. In the area of GT(Gas Turbine) diagnostics, the multiple failures in combination with measurement issues and the utilization of multiple methods for a 2-spool industrial gas turbine engine has not been investigated extensively.This paper reports the key contribution of each component of the methodology and brief the results in the quantification and classification success rate. The methodology is tested for constant deterioration and increasing noise and for random deterioration. For the random deterioration and nominal noise of 0.4%, in particular, the quantification success rate is above 92.0%, while the classification success rate is above 95.1%. | 9 | Ileocecal valve dysfunction in small intestinal bacterial overgrowth: A pilot study显示文摘AIM: To explore whether patients with a defective ileocecal valve (ICV)/cecal distension reflex have small intestinal bacterial overgrowth. METHODS: Using a colonoscope, under conscious sedation, the ICV was intubated and the colonoscope was placed within the terminal ileum (TI). A manometry catheter with 4 pressure channels, spaced 1 cm apart, was passed through the biopsy channel of the colonoscope into the TI. The colonoscope was slowly withdrawn from the TI while the manometry catheter was advanced. The catheter was placed across the ICV so that at least one pressure port was within the TI, ICV and the cecum respectively. Pressures were continuously measured during air insufflation into the cecum, under direct endoscopic visualization, in 19 volunteers. Air was insufflated to a maximum of 40 mmHg to prevent barotrauma. All subjects underwent lactulose breath testing one month after the colonoscopy. The results of the breath tests were compared with the results of the pressures within the ICV during air insufflation. RESULTS: Nineteen subjects underwent colonoscopy with measurements of the ICV pressures after intubation of the ICV with a colonoscope. Initial baseline readings showed no statistical difference in the pressures of the TI and ICV, between subjects with positive lactulose breath tests and normal lactulose breath tests. The average peak ICV pressure during air insufflation into the cecum in subjects with normal lactu-lose breath tests was significantly higher than cecal pressures during air insufflation (49.33 ± 7.99 mmHg vs 16.40 ± 2.14 mmHg, P = 0.0011). The average percentage difference of the area under the pressure curve of the ICV from the cecum during air insufflations in subjects with normal lactulose breath tests was significantly higher (280.72% ± 43.29% vs 100% ± 0%, P = 0.0006). The average peak ICV pressure during air insufflation into the cecum in subjects with positive lactulose breath tests was not significantly different than cecal pressures during air insufflation 21.23 ± 3.52 mmHg vs 16.10 ± 3.39 mmHg. The average percentage difference of the area under the pressure curve of the ICV from the cecum during air insufflation was not significantly different 101.08% ± 7.96% vs 100% ± 0%. The total symptom score for subjects with normal lactulose breath tests and subjects with positive lactu-lose breath tests was not statistically different (13.30 ± 4.09 vs 24.14 ± 6.58). The ICV peak pressures during air insufflations were significantly higher in subjects with normal lactulose breath tests than in subjects with positive lactulose breath tests (P = 0.005). The average percent difference of the area under the pressure curve in the ICV from cecum was significantly higher in subjects with normal lactulose breath tests than in subjects with positive lactulose breath tests (P = 0.0012). Individuals with positive lactulose breath tests demonstrated symptom scores which were significantly higher for the following symptoms: not able to finish normal sized meal, feeling excessively full after meals, loss of appetite and bloating. CONCLUSION: Compared to normal, subjects with a positive lactulose breath test have a defective ICV cecal distension reflex. | 10 | Larvicidal activity of green synthesized silver nanoparticles using bark aqueous extract of Ficus racemosa against Culex quinquefasciatus and Culex gelidus显示文摘Objective:To investigate the larvicidal activity of synthesized silver nanoparticles(Ag NPs) utilizing aqueous bark extract of Ficus racemosa(F.racemosa) was tested against fourth instar larvae of filariasis vector,Culex quinquefasciatus(Cx.quinquefasciatus) and japanese encephalitis vectors,Culex gelidus(Cx.gelidus).Methods:The synthesized Ag NPs was characterized by UV-vis spectrum,X-ray diffraction(XRD),Scanning electron microscopy(SEM) and Fourier transform infrared(FTIR).The larvicidal activities were assessed for 24 h against the larvae of Cx.quinquefasciatus and Cx.gelidus with varying concentrations of aqueous bark extract off.racemosa and synthesized Ag NPs.LC50 and r2 values were calculated.Results:The maximum efficacy was observed in crude aqueous extract of F.racemosa against the larvae of Cx.quinquefasciatus and Cx.gelidus(LC50=67.72 and 63.70 mg/L;r2=0.995 and 0.985) and the synthesized Ag NPs(LC50=12.00 and 11.21 mg/L;r2=0.997 and 0.990).respectively.Synthesized Ag NPs showed the XRD peaks at 2θvalues of 27.61,29.60,35.48,43.48 and 79.68 were identified as (210),(121),(220),(200) and(311) reflections,respectively.The FTIR spectra of Ag NPs exhibited prominent peaks at 3 425,2 878,1 627 and 1 382 in the region 500-3 000 cm-1.The peaks correspond to the presence of a stretching vibration of(NH) C=O group.SEM analysis showed shape in cylindrical,uniform and rod with the average size of 250.60 nm.Conclusions:The biosynthesis of silver nanoparticles using bark aqueous extract of F.racemosa and its larvicidal activity against the larvae of disease spreading vectors.The maximum larvicidal efficacy was observed in the synthesized Ag NPs. | Kanayairam Velayutham Abdul Abdul Rahuman Govindasamy Rajakumar Selvaraj Mohana Roopan Gandhi Elango Chinnaperumal Kamaraj Sampath Marimuthu Thirunavukkarasu Santhoshkumar Moorthy Iyappan Chinnadurai Siva | 2013 | Asian Pacific Journal of Tropical Medicine2013,6,2: | 2 |

| 16 | SARS-COV-2 infection(coronavirus disease 2019)for the gastrointestinal consultant显示文摘The current pandemic due to the severe acute respiratory syndrome coronavirus 2 has caused an extreme burden for health care systems globally,and the number of cases is expected to continue to increase,at least in the immediate future.The virus is estimated to have infected more than 1.5 million individuals.The available reports suggest that gastrointestinal(GI)involvement in coronavirus disease 2019(COVID-19)is common and in some cases the GI symptoms may precede the respiratory symptoms.In addition to direct effects of severe acute respiratory syndrome coronavirus 2,the infected patients remain at risk for the complications commonly managed by gastroenterology and hepatology consultants.The most commonly reported GI manifestation of COVID-19 is diarrhea,which is reported in a third to up to more than half of the patients.Mild to moderate elevation of the liver enzymes are also common,although no case of acute liver failure has been reported so far.Many of the medications used for treatment of COVID-19 can also be associated with GI symptoms or liver injury and can be included in the differential diagnosis in these patients.Although the diagnosis of the infection is currently based on RNA analysis in respiratory samples,the available literature on fecal shedding of this virus suggests that fecal RNA testing might prove to be a useful diagnostic test.It is reasonable to delay all non-urgent endoscopic procedures during the peak of the pandemic and use additional protective equipment such as N95 respirators during endoscopy while most patients can be considered high risk for having been exposed to the virus. | Kaveh Hajifathalian Srihari Mahadev Robert E Schwartz Shawn Shah Kartik Sampath Felice Schnoll-Sussman Robert S Brown Jr David Carr-Locke David E Cohen Reem Z Sharaiha | 2020 | World Journal of Gastroenterology2020,26,14: | 2 |
