1Long-term follow up of endoscopic resection for type 3 gastric NET显示文摘AIM:To clarify the short and long-term results and to prove the usefulness of endoscopic resection in type 3gastric neuroendocrine tumors(NETs).METHODS:Of the 119 type 3 gastric NETs diagnosed from January 1996 to September 2011,50 patients treated with endoscopic resection were enrolled in this study.For endoscopic resection,endoscopic mucosal resection(EMR)or endoscopic submucosal dissection(ESD)was used.Therapeutic efficacy,complications,and follow-up results were evaluated retrospectively.RESULTS:EMR was performed in 41 cases and ESD in 9 cases.Pathologically complete resection was performed in 40 cases(80.0%)and incomplete resection specimens were observed in 10 cases(7 vs 3 patients in the EMR vs ESD group,P=0.249).Upon analysis of the incomplete resection group,lateral or vertical margin invasion was found in six cases(14.6%)in the EMR group and in one case in the ESD group(11.1%).Lymphovascular invasions were observed in two cases(22.2%)in the ESD group and in one case(2.4%)in the EMR group(P=0.080).During the follow-up period(43.73;13-60 mo),there was no evidence of tumor recurrence in either the pathologically complete resection group or the incomplete resection group.No recurrence was reported during follow-up.In addition,no mortality was reported in either the complete resection group or the incomplete resection group for the duration of the follow-up period.CONCLUSION:Less than 2 cm sized confined submucosal layer type 3 gastric NET with no evidence of lymphovascular invasion,endoscopic treatment could be considered at initial treatment.Yong Hwan Kwon Seong Woo Jeon Gwang Ha Kim Jin Il Kim Il-Kwun Chung Sam Ryong Jee Heung Up Kim Geom Seog Seo Gwang Ho Baik Kee Don Choi Jeong Seop Moon 2013World Journal of Gastroenterology2013,19,46:14
2Long-term efficacy and safety of ExPress implantation for treatment of open angle glaucoma显示文摘AIM: To compare the long-term efficacy and safety of Ex Press implantation and standard trabeculectomy in patients with primary open angle glaucoma(POAG).METHODS: In this retrospective study, we compared 17 eyes treated by Ex Press implantation with 23 eyes treated by trabeculectomy. Efficacy was assessed according to the relevant intraocular pressure(IOP) values and success rates during the first year of follow-up. Postoperative corneal endothelial cell loss was also compared. RESULTS: The number of antiglaucoma medications and the IOP reduction were similar between the 2 groups during the follow-up period. Although the mean IOP was similar, the IOP-fluctuation rate during the early postoperative period was significantly lower in the Ex Press group than in the trabeculectomy group(P=0.038). A Kaplan-Meier survival curve analysis showed no significant successrate difference between the groups(P=0.810). The corneal endothelial cell loss rate, moreover, was significantly lower in the Ex Press group(P=0.05). CONCLUSION: Ex Press implantation compared with trabeculectomy showed similar IOP-reduction and success rates along with lower IOP fluctuation and endothelial cell loss rates. 10Integration of Machining and Measuring Processes Using On-Machine Measurement Technology显示文摘This paper presents an integration methodology for ma chining and measuring processes using OMM (On-Machine Measurement) technology b ased on CAD/CAM/CAI integration concept. OMM uses a CNC machining center as a me asuring station by changing the tools into measuring probes such as touch-type, laser and vision. Although the measurement accuracy is not good compared to tha t of the CMM (Coordinate Measuring Machine), there are distinctive advantages us ing OMM in real situation. In this paper, two topics are handled to show the eff ectiveness of the machining and measuring process integration: (1) inspection pl anning strategy for sculptured surface machining and (2) tool path compensation for profile milling process. For the first topic, as a first step, effective mea suring point locations are determined to obtain optimum results for given sampli ng numbers. Two measuring point selection methods are suggested based on the CAD /CAM/CAI integration concept: (1) by the prediction of cutting errors and (2) by considering cutter contact points to avoid the measurement errors caused by cus ps. As a next step, the TSP (Traveling Salesman Problem) algorithm is applied to minimize the probe moving distance. Appropriate simulations and experiments are performed to verify the proposed inspection planning strategy, and the results are analyzed. For the second topic, a methodology for profile milling error comp ensation is presented by using an ANN (Artificial Neural Network) model trained by the inspection database of OMM system. First, geometric and thermal errors of the machining center are compensated using a closed-loop configuration for the improvement of machining and inspection accuracy. The probing errors are also t aken into account. Then, a specimen workpiece is machined and then the machi ning surface error distribution is measured on the machine using touch-type pro be. In order to efficiently analyze the machining errors, two characteristic err or parameters (W err and D err) are defined. Subsequently, these param eters are modeled by applying the RFB (Radial Basis Function) network approach a s an ANN model. Based on the RBF network model, the tool paths are compensated i n order to effectively reduce the errors by employing an iterative algorithm.
