| 3 | Aquaporin-8 expression is reduced in ileum and induced in colon of patients with ulcerative colitis显示文摘AIM: To study susceptibility genes which may play a potential role in the pathogenesis and etiology of inflammatory bowel disease (IBD). METHODS: To identify potential susceptibility genes we performed global gene expression profiling in patients with IBD and control specimens. For determination of an intrinsic gene expression profile in ulcerative colitis (UC) and Crohn’s disease (CD) compared to normal subjects, mucosal biopsies of non-inflamed regions of the colon and the terminal ileum were subjected to DNA microarray analysis. Real-time RT-PCR and immunohistochemistry were used for verification of selected regulated candidate genes and a genetic analysis was performed. RESULTS: We could show that aquaporin-8 (AQP8) mRNA and protein levels were significantly increased in the colon of UC patients compared to controls. Genetic analysis of the six exons and the promoter region of AQP8, however, revealed no mutations or polymorphisms in IBD patients. | 5 | Exogenous sphingomyelinase causes impaired intestinal epithelial barrier function显示文摘AIM: To test the hypothesis that hydrolysis of sphingomyelin to ceramide changes the composition of tight junctions (TJs) with increasing permeability of the intestinal epithelium. METHODS: Monolayers of Caco-2 cells were used as an in vitro model for the intestinal barrier. Permeability was determined by quantifi cation of transepithelialflux and transepithelial resistance. Sphingolipid-rich membrane microdomains were isolated by a discontinuous sucrose gradient and characterized by Western-blot. Lipid content of microdomains was analysed by tandem mass spectrometry. Ceramide was subcellularly localized by immunofluorescent staining.RESULTS: Exogenous sphingomyelinase increased transepithelial permeability and decreased transepithelial resistance at concentrations as low as 0.01 U/mL. Lipid analysis showed rapid accumulation of ceramide in the membrane fractions containing occludin and claudin-4, representing TJs. In these fractions we observed a concomitant decrease of sphingomyelin and cholesterol with increasing concentrations of ceramide. Immunofluorescent staining confirmed clustering of ceramide at the sites of cell-cell contacts. Neutralization of surface ceramide prevented the permeability-increase induced by platelet activating factor. CONCLUSION: Our findings indicate that changes in lipid composition of TJs impair epithelial barrier functions. | 6 | Guanling Biota: A Marker of Triassic Biotic Recovery from the end-Permian Extinction in the Ancient Guizhou Sea显示文摘After a slow recovery from the end-Permian extinction during the Early Triassic and rapid radiation in the Middle Triassic, evolution of organisms reached a new peak phase in the Late Triassic. The Guanling Biota from the Wayao Member (conodont Paragondolella polygnathiformis Zone), Falang Formation, Xinpu, Guanling County, Guizhou Province, southwestern China corresponds to this peak that marks the full recovery from the end-Permian extinction of marine ecosystems. The biota is of high diversity, containing well preserved and completely articulated skeletons of vertebrates comprising marine reptiles, fishes, and invertebrates including crinoids, ammonites, bivalves, and other fossils, and is one of the best examples of marine ecosystem records in life history. The fossil marine reptiles and crinoids are most significant in this biota, especially the marine reptiles, which provide an important link between the Triassic Pacific and Tethys, and between Triassic basal forms and the Jurassic-Cretaceous marine top predators. The most remarkable fossils are the large completely articulated ichthyosaur skeletons up to and more than 10 m, and the first recorded thalattosaurs and placodonts in China. Following our review, of the 17 named reptilian taxa the eight listed here are considered to be valid: three ichthyosaurs (Qianichthyosaurus zhoui Li, 1999; Guizhouichthyosaurus tangae Cao and Luo in Yin et al., 2000, Guanlingsaurus liangae Yin in Yin et al., 2000), three thalattosaurs (Anshunsaurus huangguoshuensis Liu, 1999, Xinpusaurus suni Yin in Yin et al., 2000, Xinpusaurus kohi Jiang et al., 2004), and two placodonts (Sinocyamodus xinpuensis Li, 2000, Psephochelys polyosteoderma Li and Rieppel, 2002). Mixosaurus guanlingensis Cao in Yin et al., 2000 might be a junior synonym of Qianichthyosaurus zhoui Li, 1999, and Cymbospondylus asiaticus Li and You, 2002 and Panjiangsaurus epicharis Chen and Cheng, 2003 might be junior synonyms of Guizhouichthyosaurus tangae Cao and Luo in Yin et al., 2000. | 7 | 104例老年骨质疏松性骨盆骨折的治疗——骨水泥型经髂骨螺钉内固定系统介绍显示文摘目的探讨骨水泥型经髂骨螺钉内固定系统(TIFI)治疗老年骨质疏松性骨盆骨折的方法。方法回顾性分析2004年1月至2014年11月期间收治的104例年龄≥59岁的骨盆骨折患者资料。男11例,女93例;年龄为59~94岁,平均(77.5±7.6)岁。骨折根据Ptommens分型:I型31例,Ⅱ型39例,Ⅲ型19例,Ⅳ型15例。致伤原因:平地摔伤89例,从低于3m高处摔伤8例,无明确外伤史7例。保守治疗65例;手术治疗39例,其中17例患者采用骨水泥型TIFI固定治疗,将宁心钉和SchalIZ钉从髂后卜棘到髂前下棘置入,并固定于髋臼上方。结果65例保守治疗患者出院当天活动度:坐轮椅1例,平地行走26例,可上下楼8例,未记录30例。39例手术治疗患者出院当天活动度:坐轮椅6例,平地行走26例,可上下楼1例,未记录6例。34例Schanz钉固定患者中,有2例固定髂骨时出现螺钉穿透骶髂关节。16例Schanz钉结合骨水泥固定患者中,未观察到骨水泥溢出至骶髂关节。Schanz钉髂骨置钉深度平均为(100±20)mm,连接杆的长度平均为(62±16)mm。17例采用TIFI固定的患者中,2例术后出现血肿,I例出现浅表伤口感染,未出现深层组织感染或内固定感染。结论老午骨质疏松性骨盆骨折可根据Rommens分型来选择治疗方式。骨水泥型TIFI固定是~一种治疗老年骨质疏松性骨笳骨折的微创、可靠方法。 | Paul Schmitz Stephan Luedeek 王博炜 Rupert Schupfner Michael Worlieek Michael Nerlich Michael Bernhard Muller | 2017 | 中华创伤骨科杂志2017,19,5: | 10 |
| 8 | Receptance Coupling for Tool Point Dynamics Prediction on Machine Tools显示文摘Chatter has been a primary obstacle to the successful implementation of high speed machining.The frequency response function(FRF) of the tool point is crucial for identification of chatter free cutting conditions.In order to quickly acquire the FRF of the different components combinations of machine tool,the assembly of machine tool was always decomposed into several parts,where the fluted portion of tool,however,was always treated as a uniform beam,and the associated discrepancy was ignored.This paper presents a new method to predict the dynamic response of the machine-spindle-holder-tool assembly using the receptance coupling substructure analysis technique,where the assembly is divided into three parts:machine-spindle,holder and tool shank,and tool's fluted portion.Impact testing is used to measure the receptance of machine-spindle,the Timoshenko beam model is employed to analyze the dynamics of holder and tool shank,and the finite element method(FEM) is used to calculate the receptance of the tool's fluted portion.The approximation of the fluted portion cross section using an equivalent diameter is also addressed.All the individual receptances are coupled by using substructure method.The predicted assembly receptance is experimentally verified for three different tool overhang lengths.The results also show that the equivalent diameter beam model reaches an acceptable accuracy.The proposed approach is helpful to predict the tool point dynamics rapidly in industry. | ZHANG Jun SCHMITZ Tony ZHAO Wanhua LU Bingheng | 2011 | Chinese Journal of Mechanical Engineering2011,24,3: | 9 |
| 9 | Ephrin-B2 is differentially expressed in the intestinal epithelium in Crohn's disease and contributes to accelerated epithelial wound healing in vitro显示文摘AIM: Eph receptor tyrosine kinases and their membrane bound receptor-like ligands, the ephrins, represent a bi-directional cell-cell contact signaling system that directs epithelial movements in development. The meaning of this system in the adult human gut is unknown. We investigated the Eph/ephrin mRNA expression in the intestinal epithelium of healthy controls and patients with inflammatory bowel disease (IBD).METHODS: mRNA expression profiles of all Eph/ephrin family members in normal small intestine and colon were established by real-time RT-PCR. In addition, differential expression in IBD was investigated by cDNA array technology, and validated by both real-time RT-PCR and immunohistochemistry. Potential effects of enhanced EphB/ephrin-B signaling were analyzed in an in vitro IEC-6 cell scratch wound model.RESULTS: Human adult intestinal mucosa exhibits a complex pattern of Eph receptors and ephrins. Beside the known prominent co-expression of EphA2 and ephrinA1,we found abundantly co-expressed EphB2 and ephrin-B1/2.Interestingly, cDNA array data, validated by real-time PCR and immunohistochemistry, showed upregulation of ephrin-B2 in both perilesional and lesional intestinal epithelial cells of IBD patients, suggesting a role in epithelial homeostasis. Stimulation of ephrin-B signaling in ephrinB1/2 expressing rat IEC-6-cells with recombinant EphB1Fc resulted in a significant dose-dependent acceleration of wound closure. | 10 | AKT and ERK1/2 signaling in intrahepatic cholangiocarcinoma显示文摘Intrahepatic cholangiocarcinomas (ICC) are neoplasms that originate from cholangiocytes and can occur at any level of the biliary tree. Surgical resection is the current therapy of choice for this highly aggressive cancer. However, the 5-year survival still is poor, with high recurrence rates. Due to the intrahepatic growth a signifi cant proportion of patients present with advanced disease and are not candidates for curative surgery or transplantation. The existing palliative options are of limited benefit and there is a great necessity for novel therapeutic options. In this article we review the role of the phosphoinositide 3-kinase(PI3K)/ AKT and extracellular regulated kinase (ERK) signaling pathways in ICC and present new data on the prognostic value of these protein kinases. | 17 | Different cava reconstruction techniques in liver transplantation:piggyback versus cava resection显示文摘BACKGROUND: Originally, cava reconstruction(CR) in liver transplantation meant complete resection and reinsertion of the donor cava. Alternatively, preservation of the recipients inferior vena cava(IVC) with side-to-side anastomosis(known as 'piggyback') can be performed. Here, partial clamping maintains blood flow of the IVC, which may improve cardiovascular stability, reduce blood loss and stabilize kidney function. The aim of this study was to compare both techniques with particular focus on kidney function.METHODS: A series of 414 patients who had had adult liver transplantations(2006-2009) were included. Among them,176(42.5%) patients had piggyback and 238 had classical CR operation, 112(27.1%) of the patients underwent CR accompanied with veno-venous bypass(CR-B) and 126(30.4%)without a bypass. The choice of either technique was based on the surgeons' individual preference. Kidney function [serum creatinine, calculated glomerular filtration rate(GFR), RIFLE stages] was assessed over 14 days.RESULTS: Lab-MELD scores were significantly higher in CR-B(22.5±11.0) than in CR(17.3±9.0) and piggyback(18.8±10.0)(P=0.008). Unexpectedly, the incidences of arterial stenoses(P=0.045) and biliary leaks(P=0.042) were significantly increased in piggyback. Preoperative serum creatinine levels were the highest in CR-B [1.45±1.17 vs 1.25±0.85(piggyback)and 1.13±0.60 mg/dL(CR); P=0.033]. Although a worsening of postoperative kidney function was observed among all groups,this was most pronounced in CR-B [creatinine day 14: 1.67±1.40 vs 1.35±0.96(piggyback) and 1.45±1.03 mg/dL(CR); P=0.102].Accordingly, the proportion of patients displaying RIFLE stages≥2 was the highest in CR/CR-B(26%/19%) when compared to piggyback(18%).CONCLUSIONS: Piggyback revealed a shorter warm ischemic time, a reduced blood loss, and a decreased risk of acute kidney failure. Thus, piggyback is a useful technique, which should be applied in standard procedures. | 19 | Serum bile acid profiling reflects enterohepatic detoxification state and intestinal barrier function in inlammatory bowel disease显示文摘AIM:To determine free and conjugated serum bile acid (BA) levels in in? ammatory bowel disease (IBD) subgroups with defi ned clinical manifestations.METHODS: Comprehensive serum BA profiling was performed in 358 IBD patients and 310 healthy controls by liquid chromatography coupled to electrospray ionization tandem mass spectrometry.RESULTS: Serum levels of hyodeoxycholic acid, the CYP3A4-mediated detoxification product of the secondary BA lithocholic acid (LCA), was increased significantly in Crohn's disease (CD) and ulcerative colitis (UC), while most other serum BA species were decreased significantly. Total BA, total BA conjugate, and total BA glycoconjugate levels were decreased only in CD, whereas total unconjugated BA levels were decreased only in UC. In UC patients with hepatobiliary manifestations, the conjugated primary BAs glycocholic acid, taurocholic acid, and glycochenodeoxycholic acid were as significantly increased as the secondary BAs LCA, ursodeoxycholic acid, and tauroursodeoxycholic acid compared to UC patients without hepatobiliary manifestations. | 20 | Evaluating the management of anaphylaxis in US emergency departments:Guidelines vs.practice显示文摘BACKGROUND:Anaphylaxis is characterized by acute episodes of potentially life-threatening symptoms that are often treated in the emergency setting.Current guidelines recommend:1) quick diagnosis using standard criteria;2) first-line treatment with epinephrine;and 3) discharge with a prescription for an epinephrine auto-injector,written instructions regarding long-term management,and a referral(preferably,allergy) for follow-up.However,studies suggest low concordance with guideline recommendations by emergency medicine(EM) providers.The study aimed to evaluate how emergency departments(EDs) in the United States(US) manage anaphylaxis in relation to guideline recommendations.METHODS:This was an online anonymous survey of a random sample of EM health providers in US EDs.RESULTS:Data analysis included 207 EM providers.For respondent EDs,approximately 9%reported using agreed-upon clinical criteria to diagnose anaphylaxis;42%reported administering epinephrine in the ED for most anaphylaxis episodes;and <50%provided patients with a prescription for an epinephrine auto-injector and/or an allergist referral on discharge.Most provided some written materials,and follow-up with a primary care clinician was recommended.CONCLUSIONS:This is the first cross-sectional survey to provide 'real-world' data showing that practice in US EDs is discordant with current guideline recommendations for the diagnosis,treatment,and follow-up of patients with anaphylaxis.The primary gaps are low(or no) utilization of standard criteria for defining anaphylaxis and inconsistent use of epinephrine.Prospective research is recommended. | W.Scott Russell Judith Rosen Farrar Richard Nowak Daniel P.Hays Natalie Schmitz Joseph Wood Judi Miller | 2013 | World Journal of Emergency Medicine2013,4,2: | 4 |
