| 1 | Efficacy of neoadjuvant therapy and surgical rescue for locally advanced hepatoblastomas:10 year single-center experience and literature review显示文摘AIM:To report our experience with long-term outcomes after multimodal management therapy.METHODS:An observational retrospective study was performed containing seven patients with hepatoblastoma(Hbl)treated in our institution,a tertiary referral center,from 2003 to 2011.Demographic,preoperative,surgical,and outcome variables were collected.A survival analysis and a review of the current literature related to combination neoadjuvant chemotherapy and surgical resection on Hbl were performed.RESULTS:The median age at surgery was 14.4 mo,with a male to female ratio of 4:3.Pretext staging at diagnosis was as follows:stageⅠ,4 cases;stageⅡ,2 patients;and stageⅢ,1 case.Mean pretreatment tumor volume was 735 cm3.Five out of seven patients received neoadjuvant chemotherapy according to SIOPEL-3 or SIOPEL-6 protocols.Tumor volume and alphafetoprotein levels significantly dropped after neoadjuvant therapy.Surgical procedures performed included hemihepatectomies,segmentectomies and atypical resection.All patients received chemotherapy after surgery.Median postoperative hospital stay was 8 d.All patients were alive and disease-free after a median follow-up period of 23 mo.With regards to the literature review,seventeen articles were found that were related to our search.CONCLUSION:Our series shows how multimodal management of Hbl,exhaustive control and a meticulous surgical approach leads to almost 100%complete resection with optimal postoperative results. | Dolores Ayllon Teran Oscar Gómez Beltran Rubén Ciria Bru Elena Mateos González María José Pea Rosa Antonio Luque Molina Pedro López Cillero Javier Briceo Delgado | 2014 | World Journal of Gastroenterology2014,20,29: | 12 |
| 2 | Gut microbiota-related complications in cirrhosis显示文摘Gut microbiota plays an important role in cirrhosis. The liver is constantly challenged with commensal bacteria and their products arriving through the portal vein in the so-called gut-liver axis. Bacterial translocation from the intestinal lumen through the intestinal wall and to mesenteric lymph nodes is facilitated by intestinal bacterial overgrowth, impairment in the permeability of the intestinal mucosal barrier, and deficiencies in local host immune defences. Deranged clearance of endogenous bacteria from portal and systemic circulation turns the gut into the major source of bacterialrelated complications. Liver function may therefore be affected by alterations in the composition of the intestinal microbiota and a role for commensal flora has been evidenced in the pathogenesis of several complications arising in end-stage liver disease such as hepatic encephalopathy, splanchnic arterial vasodilatation and spontaneous bacterial peritonitis. | 3 | Effect of phacoemulsification on intraocular pressure in patients with primary open angle glaucoma and pseudoexfoliation glaucoma显示文摘AIM: To compare the effect of phacoemulsification on intraocular pressure(IOP) in patients with primary open angle glaucoma(POAG) and pseudoexfoliation glaucoma(PXG). METHODS: A retrospective comparative case series conducted at the Glaucoma Department at the Association to Prevent Blindness in Mexico. The study enrolled consecutive patients having phacoemulsification with intraocular lens(IOL) implantation and a diagnosis of POAG or PXG. Data about IOP values and number of glaucoma medications used was collected at baseline, 1, 3, 6 and 12 mo postoperatively. RESULTS: The study enrolled 88 patients(88 eyes). After phacoemulsification, there was a statistically significant reduction in IOP values and glaucoma medications use compared to baseline in both POAG and PXG patients(P<0.001). In the POAG group, a 20% decrease in IOP values was evidenced, and a 56.5% reduction in the number of medications used at the one-year follow-up. The PXG group showed a 20.39%, and a 34.46% decrease in IOP and number of medications used, respectively. A significant difference in the mean ΔIOP(postoperative changes in IOP) was evidenced between groups(P=0.005). The reduction of the postsurgical IOP mean values in both groups, the POAG group showed a greater reduction in IOP values compared to the PXG group.CONCLUSION: In both types of glaucoma, phacoemulsification cataract surgery can result in a significant IOP reduction(20%) over a 12 mo follow-up period. | 6 | Electrocardiographic markers of cardiac resynchronization therapy response:delayed time to intrinsicoid deflection onset in lateral leads显示文摘Cardiac resynchronization therapy(CRT)has emerged as an important intervention for patients with heart failure(HF)with reduced ejection fraction and delayed ventricular activation.In these patients,CRT has demonstrated to improve quality of life,promote reverse left ventricular(LV)remodeling,reduce HF hospitalizations,and extend survival.However,despite advancements in our understanding of CRT,a significant number of patients do not respond to this therapy.Several invasive and non-invasive parameters have been assessed to predict response to CRT,but the electrocardiogram(ECG)has remained as the prevailing screening method albeit with limitations.Ideally,an accurate,simple,and reproducible ECG marker or set of markers would dramatically overcome the current limitations.We describe the clinical utility of an old ECG parameter that can estimate ventricular activation delay:the onset to intrinsicoid deflection(ID).Based on the concept of direct measurement of ventricular activation time(intrinsic deflection onset),time to ID onset measures on the surface ECG the time that the electrical activation time takes to reach the area subtended by the corresponding surface ECG lead.Based on this principle,the time to ID on the lateral leads can estimate the delay activation to the lateral LV wall and can be used as a predictor for CRT response,particularly in patients with non-specific intraventricular conduction delay or in patients with left bundle branch block and QRS<150 ms.The aim of this review is to present the current evidence and potential use of this ECG parameter to estimate LV activation and predict CRT response. | Rubén KA Tapia-Orihuela S Michael Gharacholou Samuel J Asirvatham Freddy Del-Carpio Munoz | 2022 | Journal of Geriatric Cardiology2022,19,1: | 3 |

| 8 | Immunomodulating effects of antibiotics used in the prophylaxis of bacterial infections in advanced cirrhosis显示文摘The use of norfloxacin either as primary or secondary prophylaxis of bacterial infections in advanced cirrhosis has improved patient's survival. This may be explained not only due to a significant decrease in the number of infections, but also because of a direct immunomodulatory effect. Selective intestinal decontamination with norfloxacin reduces translocation of either viable bacteria or bacteria-driven products from the intestinal lumen. In addition, norfloxacin directly modulates the systemic inflammatory response. The proinflammatory cytokine profile secreted by neutrophils from these patients shows a close, significant, and inverse correlation with serum norfloxacin levels. Similar effects have been described with other quinolones in different clinical conditions. Although the underlying mechanisms are not well defined for most of the antibiotics, the pathways triggered for norfloxacin to induce such immunomodulatory effects involve the down-regulation of pro-inflammatory inducible nitric oxide synthase, cyclooxygenase-2, and NF-κB and the up-regulation of heme-oxygenase 1 and IL-10 expression. | 9 | Specific and cross-reactive immune response against Mycobacterium tuberculosis antigens in mice immunized with proteoliposomes from Mycobacterium bovis BCG显示文摘Objective: To characterize the immunogenicity and the induction of cross-reactive responses against Mycobacterium tuberculosis(M. tuberculosis) of a proteoliposome(PL)from Mycobacterium bovis Bacillus Calmette–Guerin(BCG) with and without alum hydroxide(AL) as adjuvant(PLBCG-AL and PLBCG, respectively) in BALB/c mice.Methods: BALB/c mice were inoculated with phosphate buffer solution, BCG, PLBCG and PLBCG-AL. The humoral immunogenicity was determined by ELISA [immunoglobulin G(Ig G), Ig G1 and Ig G2a] and the cellular immunogenicity was evaluated in vivo by delayed type hypersensitivity. The humoral cross-reactive response against M. tuberculosis was determined by Western blot.Results: Sera from animals immunized with PLBCG-AL and PLBCG showed significant increase in specific total Ig G and Ig G1 antibodies and the presence of cross-reactive antibodies against M. tuberculosis antigens, which were more intense with the use of alum as adjuvant. | 12 | Resource-sharing strategies in ecotypes of the invasive clonal plant Carpobrotus edulis: specialization for abundance or scarcity of resources显示文摘Aims Carpobrotus edulis(L.)N.E.Br.is known to invade many coastal ecosystems around the world,and it has been considered as one of the most severe threats to numerous terrestrial plant communi-ties.Therefore,the study of the relationships between life-history traits that may favour its invasiveness and the invasibility of the environments is necessary to improve our knowledge about inva-sion success.In this research,we specifically tested the effects of physiological integration in genotypes from contrasting habitats,where the importance of integration is expected to differ.Thus,the main objective of this work was to detect the presence of adaptive plasticity in the capacity for clonal integration in this aggressive invader.Methods In a greenhouse experiment,we compared the performance,in terms of growth and photochemical efficiency,of two C.edulis ecotypes.Connected and severed ramet pairs from coastal sand dunes and rocky coast habitats were grown in substrates of different quality and heterogeneity.Important Findings Our study clearly indicates that clonal integration improves growth and photosynthetic efficiency in the aggressive invader C.edulis.Two differ-ent aspects of clonal integration determine site-specific strategies in this species in order to optimize its successful propagation in a particular habitat.We demonstrated that the adaptation of C.edulis to local envi-ronments has led to a differential selection of two complex clonal traits associated with the capacity for clonal integration.In patchy sand dunes,C.edulis has evolved ecotypes with ramets highly plastic in patterns of biomass allocation,which allows ramets to specialize in the acquisi-tion of the resource that is locally most abundant,i.e.,a spatial division of labour among ramets.However,in the harsher rocky coast habitats,local adaptation produced highly integrated ecotypes,where resource sharing is not combined with a specialization of resource acquisition. | Josefina G.Campoy Rubén Retuerto Sergio R.Roiloa | 2017 | Journal of Plant Ecology2017,10,4: | 3 |

| 14 | Wavelet-based deconvolution of ultrasonic signals in nondestructive evaluation显示文摘In this paper, the inverse problem of reconstructing reflectivity function of a medium is examined within a blind deconvolution framework. The ultrasound pulse is estimated using higher-order statistics, and Wiener filter is used to obtain the ultrasonic reflectivity function through wavelet-based models. A new approach to the parameter estimation of the inverse filtering step is proposed in the nondestructive evaluation field, which is based on the theory of Fourier-Wavelet regularized deconvolution (ForWaRD). This new approach can be viewed as a solution to the open problem of adaptation of the ForWaRD framework to perform the convolution kernel estimation and deconvolution interdependently. The results indicate stable solutions of the esti- mated pulse and an improvement in the radio-frequency (RF) signal taking into account its signal-to-noise ratio (SNR) and axial resolution. | 20 | Late hepatic artery pseudoaneurysm:A rare complication after resection of hilar cholangiocarcinoma显示文摘We report an unusual pathological entity of a pseudoaneurysm of the right hepatic artery, which developed two years after the resection of a type Ⅱ hilar cholangiocarcinoma and secondary to an excessive skeletonization for regional lymphadenectomy and neoadjuvant external-beam radiotherapy. After a sudden and massive hematemesis, a multidetector computed tomographic angiography (MDCTA) showed a hepatic artery pseudoaneurysm. Angiography with embolization of the pseudoaneurysm was attempted using microcoils with adequate patency of the hepatic artery and the occlusion of the pseudoaneurysm. A new episode of hematemesis 3 wk later revealed a partial revascularization of the pseudoaneurysm.
