1渤海湾盆地大歧口凹陷新生代构造演化与盆地原型显示文摘大歧口凹陷位于渤海湾盆地黄骅坳陷中北部,是中新生代叠合盆地中新生界沉积最厚的凹陷。本文基于大量二维、三维地震剖面和钻井资料,从凹陷主控断裂活动性、沉积沉降中心分布和构造格架等方面,侧重对大歧口凹陷新生代构造演化过程和原型盆地类型进行研究。认为:1)大歧口凹陷构造单元有4级,凹陷具有'东西分带、南北分块'的平面构造格局和北断南超箕状断陷的剖面结构。2)凹陷内断裂有4级,主要断裂的规模、活动期次和断裂活动具有演化的阶段性和空间的迁移性,典型构造带活动性具有由北往南迁移的特点。3)新生代期间整个大歧口凹陷的沉积沉降中心均在东部海域的歧口主凹内,但整体具有往南迁移的趋势。4)凹陷经历了拓展裂谷、拉分断陷、箕状断陷和碟状坳陷4个构造演化阶段,断陷阶段在整个构造演化时期意义重大,且具有走滑拉分特点。故认为大歧口凹陷原型盆地是一种'板内拉分盆地'。索艳慧 李三忠 许立青 戴黎明 刘鑫 郭玲莉 曹花花 Дмитриенко Людмила Валерьевна Roya Nadiri 王鹏程 周立宏 楼达 2015地质科学2015,50,2:17
2口服消炎痛和布洛芬治疗早产儿动脉导管未闭的比较显示文摘目的静脉注射消炎痛是早产儿动脉导管未闭的常规治疗方法,但治疗过程中常出现一些副作用,如少尿、消化道出血、脑血流灌注减少。近年来,静脉注射布洛芬已用于治疗早产儿动脉导管未闭。布洛芬治疗不会减少脑血流灌注,也不会影响胃肠道和肾脏的血流动力学。伊朗目前尚无消炎痛和布洛芬的静脉制剂供应。该研究旨在比较这两种药的口服制剂治疗早产儿动脉导管未闭的疗效和安全性。方法36例胎龄小于34周经超声心动图确诊患有动脉导管未闭的早产儿被随机分为两组,每组18人。一组给予消炎痛口服,每次0.2mg/kg,24h给药1次,共3次。另一组给予布洛芬口服,共3次,间隔时间为24h,首剂为10mg/kg,随后两次各5mg/kg。用药后观察导管闭合率、副作用、并发症及临床过程。结果用药后布洛芬组18例患儿动脉导管都闭合(100%),而消炎痛组18例中有15例患儿动脉导管闭合(83.3%)(P〉0.05)。两组疗效差异统计学无显著性意义。治疗前后两组的血清尿素氮和肌酐含量差异也无显著性意义。消炎痛组发生了3例(16.6%)坏死性小肠结肠炎,布洛芬组则无,差异有显著性意义(P〈0.05)。治疗1个月后两组成活率均为94%(17/18)。消炎痛组1例死于坏死性小肠结肠炎,布洛芬组1例死于败血症。结论口服布洛芬治疗早产儿动脉导管未闭和口服消炎痛治疗一样有效,而且坏死性小肠结肠炎的发生率较口服消炎痛治疗低。Saed Hossein FAKHRAEE Zohreh BADIEE Saied MOJTAHEDZADEH Mohammad KAZEMIAN Roya KELISHADI Shahid Beheshti 2007中国当代儿科杂志2007,9,5:11
3表面活性物质替代治疗新生儿呼吸窘迫综合征的疗效评估(英文)显示文摘目的呼吸窘迫综合征(RDS)是早产儿发病和死亡的重要原因。RDS主要由肺表面活性物质缺乏所致。该研究调查了表面活性物质治疗在减少早产儿死亡率中的作用,并评估了表面活性物质疗效与一些RDS相关危险因素的关系。方法通过简单抽样抽取符合入选要求的89例RDS早产儿,他们均来自于ShaheedBeheshti医院新生儿重症监护室。生后48 h内给予表面活性物质治疗(100 mg/kg)。结果89例得到表面活性物质治疗的RDS早产儿中,34例(38.2%)幸存,55例死亡。其中胎龄大于32周,表面活性物质治疗开始于生后24 h内,1,5分钟Apgar评分大于7/10分,或出生体重大于1 500 g者治疗效果较好,存活率分别为47.5%,43.3%,48.1%,52.5%。表面活性物质治疗后母亲孕前经过激素治疗的RDS早产儿的存活率(41.7%)显著高于那些母亲孕前未经激素治疗者(34.2%)(P<0.05)。结论表面活性物质替代治疗新生儿RDS应尽可能早地进行,能减少38.2%的死亡率。新生儿胎龄、出生体重、Apgar评分、表面活性物质治疗开始时间及母亲孕期是否经激素治疗等均可影响表面活性物质替代治疗的疗效。Sedigheh GHAEMI Maryam MOHAMADYMASODI Roya KELISHADI 2009中国当代儿科杂志2009,11,3:9
4伊朗伊斯法罕新生儿重症监护中心新生儿气胸的发病因素、发生率和死亡率调查(英文)显示文摘目的该研究旨在调查伊朗伊斯法罕新生儿重症监护中心(NICU)住院新生儿气胸的发病因素、发生率和死亡率。方法 738例入住NICU的新生儿中,43例发生了气胸。回顾性分析气胸患儿的临床资料,包括胎龄、出生体重、Apgar评分、出生方式、母亲年龄、产次、围产期窒息、出生时复苏、气胸发生部位、机械通气情况、肺表面活性物质治疗、肺部疾病等。结果气胸患儿平均胎龄为31周,出生体重为1596g。12例(28%)患儿胎龄小于28周。28例(65%)出生体重低于1500g。共43例(5.8%)新生儿发生了气胸。97%的气胸为单侧(n=63),双侧气胸仅占3%(n=2)。呼吸窘迫综合征(40/43,93%)和机械通气(37/43,86%)是导致气胸发生的常见原因。共28例(65%)患儿死亡。死亡患儿与幸存患儿出生体重、胎龄及胸管留置时间差异有统计学意义。需要肺表面活性物质治疗的气胸患儿死亡率显著增加,与无需表面活性物质治疗的气胸患儿比较差异有统计学意义。结论该研究中气胸的发生率与死亡率高于其他报道,其原因可能是该研究中新生儿出生体重和胎龄都较低。呼吸窘迫综合征和机械通气是导致新生儿气胸发生的常见原因。患儿胎龄越小,体重越低,肺部疾病越严重,死亡率则越高。Fakhri NAVAEI Banafshe ALIABADI Masoud MOGHTADERI Roya KELISHADI 2010中国当代儿科杂志2010,12,6:6

6Propane catalytic cracking on pretreated La-ZSM-5 zeolite during calcination for light olefins production显示文摘The purpose of this study was to increase light olefins yield and selectivity in catalytic cracking of propane. Pretreatment of ZSM-5 zeolite was performed during calcination. First, lanthanum was loaded on the catalyst by wet impregnation method and then the effect of parameters, such as temperature(400–800 oC), time(120–480 min) and type of stream, on La-ZSM-5 zeolite was examined during calcination using central composite design(CCD) method. Based on the proposed models, the optimized condition for maximizing selectivity of light olefins for air stream was 680 oC and 310 min and for the nitrogen stream was 735 oC and 173 min that under these conditions, selectivity amount increased 24% for nitrogen stream and 19% for air stream as compared with H-ZSM-5 catalyst. The synthesized catalysts were characterized by X-ray diffraction(XRD) and temperature-programmed desorption(TPD) techniques. According to the results of NH_3-TPD, as the temperature increased, the number of strong acid sites(specially Bronsted acid sites) reduced and resulted in increased production of light olefins as well as catalyst stability. 19地氟烷与七氟烷在门诊手术全身麻醉维持中的对比研究:对苏醒早晚期及围手术期咳嗽的影响显示文摘背景目前地氧垸已经用于门诊全身麻醉手术的维持阶段,其相对于七氖烷在围手术期的优略性尚存争议。尽管各研究一致指出,地氟烷比七氟烷苏醒更快,但这种差异对苏醒晚期的影响却未有明确报道。并且,地氟烷与七氟烷相比,对咳嗽发生率的影响也存在争议。方法本研究中纳入了130例在全身麻醉下行浅表门诊手术的患者,随机分为2组。所有患者都静脉注射丙泊酚2mg/kg诱导,置入喉罩,采用1%-3%的七氟烷或3%-8%的地氟炕维持麻醉,吸入混合空气或氧气。调节吸入麻醉药的浓度,维持血流动力学稳定,并维持脑电双频谱指数(BIS)于50—60。并用局部浸润和静脉注射酮洛酸30mg镇痛。在手术结束时给予预防性止吐药物,包括昂丹司琼4mg、地塞米松4mg及甲氧氨普胺10mg。评估标准包括睁眼恢复时间,对指令做出反应的时间,定向力恢复的时间,14分快通道评分,首次进食时间,能坐立,可站立,独立行走及出院时间。手术后早期及离院后24小时内,记录患者对于麻醉的满意度,手术后第1天恢复正常活动的能力,不良反应的发生(如咳嗽、意向性运动、氧饱和度〈90%、咽痛及手术后恶心、呕吐),手术后镇痛药及止吐药的使用情况。结果2组患者的流行病学资料相近。尽管围手术期地氯烷组咳嗽的总体发生率高于七氟烷组(分别为60%和32%,P〈0.05),然而在真正应用吸入麻醉药期间(即麻醉维持中),咳嗽发生率的组间差异无显著性。地氧烷组患者苏醒较快,但两组所有患者在离开手术室之前都达到了快通道恢复标准(快通道评分≥12)。此外,离院时间(七氟炕组90±31分钟,地氟炕组98±5分钟)及手术后第1天能恢复正常活动的患者比率(七氟烷48%,地氟炕60%),在两组间的差异无显著性。结论用地氧烷进行维持全身麻醉具有苏醒迅速、咳嗽发生率高的特点。尽管与七氟烷相比,地氟炕在麻醉早期恢复较快,但两组晚期恢复的差异无显著性。两种吸入麻醉药都适用于门诊手术的麻醉。Paul F.
