| 1 | Phase 1 human trial of autologous bone marrow-hematopoietic stem cell transplantation in patients with decompensated cirrhosis显示文摘AIM: To evaluate safety and feasibility of autologous bone marrow-enriched CD34+ hematopoietic stem cell Tx through the hepatic artery in patients with decompensated cirrhosis.METHODS: Four patients with decompensated cirrhosis were included. Approximately 200 mL of the bone marrow of the patients was aspirated, and CD34+ stem cells were selected. Between 3 to 10 million CD34+ cells were isolated. The cells were slowly infused through the hepatic artery of the patients.RESULTS: Patient 1 showed marginal improvement in serum albumin and no significant changes in other test results. In patient 2 prothrombin time was decreased; however, her total bilirubin, serum creatinine, and Model of End-Stage Liver Disease (MELD) score worsened at the end of follow up. In patient 3 there was improvement in serum albumin, porthrombin time (PT), and MELD score. Patient 4 developed radiocontrast nephropathy after the procedure, and progressed to type 1 hepatorenal syndrome and died of liver failure a few days later. Because of the major side effects seen in the last patient, the trial was prematurely stopped.CONCLUSION: Infusion of CD34+ stem cells through the hepatic artery is not safe in decompensated cirrhosis. Radiocontrast nephropathy and hepatorenal syndrome could be major side effects. | 2 | Osteoarticular manifestations of human brucellosis:A review显示文摘Brucellosis is a common global zoonotic disease, which is responsible for a range of clinical manifestations. Fever, sweating and musculoskeletal pains are observed in most patients. The most frequent complication of brucellosis is osteoarticular involvement, with 10% to 85% of patients affected. The sacroiliac(up to 80%) and spinal joints(up to 54%) are the most common affected sites.Spondylitis and spondylodiscitis are the most frequent complications of brucellar spinal involvement. Peripheral arthritis, osteomyelitis, discitis, bursitis and tenosynovitis are other osteoarticular manifestations, but with a lower prevalence. Spinal brucellosis has two forms: focal and diffuse. Epidural abscess is a rare complication of spinal brucellosis but can lead to permanent neurological deficits or even death if not treated promptly. Spondylodiscitis is the most severe form of osteoarticular involvement by brucellosis, and can have single-or multifocal involvement. Early and appropriate diagnosis and treatment of the disease is important in order to have a successful management of the patients with osteoarticular brucellosis. Brucellosis should be considered as a differential diagnosis for sciatic and back pain, especially in endemic regions. Patients with septic arthritis living in endemic areas also need to be evaluated in terms of brucellosis. Physical examination, laboratory tests and imaging techniques are needed to diagnose the disease. Radiography, computed tomography, magnetic resonance imaging(MRI) and bone scintigraphy are imaging techniques for the diagnosis of osteoarticular brucellosis. MRI is helpful to differentiate between pyogenic spondylitis and brucellar spondylitis. | 3 | p16 promoter hypermethylation:A useful serum marker for early detection of gastric cancer显示文摘AIM: To determine p16 promoter hypermethylation in gastric tumoral tissue and serum samples, its impact on p16-protein expression, and correlation with clinical and histological features. METHODS: Samples were obtained from 52 histologically confirmed cases of gastric adenocarcinoma. Gastric tissue and serum of 50 age- and sex-matched individuals with normal gastroscopy and biopsy were obtained as control samples. Methylation-specific polymerase chain reaction (MSP) was used to evaluate methylation status of p16 promoter. p16-protein expression was analyzed by immunohistochemical staining on paraffin-embedded sections. RESULTS: Methylation was detected in 44.2% (23/52) of tumoral tissues. 60.9% of them were also methylated in serum, i.e., 26.9% of all patients (14/52). Methylation was not detected in tissue and sera of control samples. p16-protein expression was decreased in 61.5% of cases (32/52), and was significantly associated with promoter hypermethylation (P < 0.001). Methylation was significantly more frequent in higher pathological grades (P < 0.05). Methylation was not associated with other clinicopathological features and environmental factors including H pylori infection and smoking. CONCLUSION: p16 promoter hypermethylation is an important event in gastric carcinogenesis. It is the principle mechanism of p16 gene silencing. It is related to malignant tumor behavior. The synthetic capacity of the mammary gland depends largely on the number and efficiency of functional mammary epithelial cel s. Structural development of the mammary gland occurs during fetal growth, prepubertal and post-pubertal periods, pregnancy, and lactation under the control of various hormones(particularly estrogen, growth hormone, insulin-like growth factor-I, progesterone, placental lactogen, and prolactin) in a species- and stage-dependent manner. Milk is essential for the growth, development, and health of neonates. Amino acids(AA), present in both free and peptide-bound forms, are the most abundant organic nutrients in the milk of farm animals. Uptake of AA from the arterial blood of the lactating dam is the ultimate source of proteins(primarily β-casein and α-lactalbumin) and bioactive nitrogenous metabolites in milk. Results of recent studies indicate extensive catabolism of branched-chain AA(leucine, isoleucine and valine) and arginine to synthesize glutamate,glutamine, alanine, aspartate, asparagine, proline, and polyamines. The formation of polypeptides from AA is regulated not only by hormones(e.g., prolactin, insulin and glucocorticoids) and the rate of blood flow across the lactating mammary gland, but also by concentrations of AA, lipids, glucose, vitamins and minerals in the maternal plasma, as well as the activation of the mechanistic(mammalian) target rapamycin signaling by certain AA(e.g.,arginine, branched-chain AA, and glutamine). Various kinds of stem cells have been used for the tissue engineering and regenerative medicine. Such as, several stromal cells have been employed as treat option for regenerative medicine. For example, human bone marrow-derived stromal cells and adipose-derived stromal cells(ADSCs) are used in cell-based therapy. Data relating to the stem cell therapy and processes associated with ADSC has developed remarkably in the past 10 years. As medical options, both the stromal vascular and ADSC suggests good opportunity as marvelous cell-based therapeutics. The some biological features are the main factors that impact the regenerative activity of ADSCs, including the modulation of the cellular immune system properties and secretion of bioactive proteins such as cytokines, chemokines and growth factors, as well as their intrinsic anti-ulcer and anti-inflammatory potential. A variety of diseases have been treated by ADSCs, and it is not surprising that there has been great interest in the possibility that ADSCs might be used as therapeutic strategy to improve a wider range of diseases. | 6 | Hepatocellular carcinoma in Asia: Prevention strategy and planning显示文摘AIM: To review all of epidemiological and etiological aspects of hepatocellular carcinoma(HCC) and examined the prevention of this disease in Asia.METHODS: We conducted a systematic review according to the PRISMA guidelines. We were chosen articles that published previously, from Pub Med(MEDLINE), the Cochrane database and Scopus. The key words used in this research were as follows: HCC in Asia and the way of prevention of this disease, with no language limitations. We selected those papers published before 2014 that we considered to be most important and appropriate. All relevant articles were accessed in full text and all relevant materials was evaluated and reviewed.RESULTS: More than 70% of all new cases of liver cancer were diagnosed in Asia, a region that 75% of all those chronically infected with hepatitis B virus(HBV) in the world. Chronic HBV infection is the main cause of HCC in Asia, where the virus is endemic and vertical transmission is common. Japan, Saudi Arabia, Egypt and Pakistan are exception because of high prevalence of HCV infection in these regions. The prevalence of this cancer is high in Eastern and South-Eastern Asia, But Middle Eastern countries are characterized as moderate prevalence rate of HCC region and Central Asia and some part of Middle Eastern countries are known as low prevalence rate of HCC. In addition of HBV and HCV the other factors such as aflatoxin, alcohol, obesity, diabetes and non-alcoholic fatty liver disease(NAFLD) might be responsible for a low prevalence of HCC in Asian countries. Currently available HCC therapies, chemotherapy, surgical are inefficient, mainly due to usually late diagnosis and high recurrence rates after surgical resection, and usually end with treatment failure. Liver transplantation also remains as a difficult strategy in patients with HCC. Thus prevention of HCC by treating and prevention HBV and HCV infection, the major causative agents of HCC, and the other riskfactors such as aflatoxin, alcohol, obesity, diabetes and NAFLD is of a great medical importance. | 8 | Biochemical and physiological bases for utilization of dietary amino acids by young Pigs显示文摘Protein is quantitatively the most expensive nutrient in swine diets. Hence it is imperative to understand the physiological roles played by amino acids in growth, development, lactation, reproduction, and health of pigs to improve their protein nutrition and reduce the costs of pork production. Due to incomplete knowledge of amino acid biochemistry and nutrition, it was traditionally assumed that neonatal, post-weaning, growing-finishing, and gestating pigs could synthesize sufficient amounts of all 'nutritionally nonessential amino acids' (NEAA) to support maximum production performance. Therefore, over the past 50 years, much emphasis has been placed on dietary requirements of nutritionally essential amino acids as building blocks for tissue proteins. However, a large body of literature shows that NEAA, particularly glutamine, glutamate, arginine and proline regulate physiological functions via cell signaling pathways, such as mammalian target of rapamycin, AMP-activated protein kinase, extracellular signal-related kinase, Jun kinase, mitogen-activated protein kinase, and NEAA-derived gaseous molecules (e.g., nitric oxide, carbon monoxide, and hydrogen sulfide). Available evidence shows that under current feeding programs, only 70% and 55% of dietary amino acids are deposited as tissue proteins in 14-day-old sow-reared piglets and in 30-day-old pigs weaned at 21 days of age, respectively. Therefore, there is an urgent need to understand the roles and dietary requirements of NEAA in swine nutrition. | 9 | Validation of nutritional risk index method against patientgenerated subjective global assessment in screening malnutrition in colorectal cancer patients显示文摘Objective:To validate malnutrition screening tool of nutrition risk index(NRI)against patient-generated subjective global assessment(PG-SGA)as a gold standard tool in colorectal cancer patients before radiotherapy.Methods:Nutritional status of 52 volunteer colorectal cancer patients with a mean age of 54.1±16.8 years who referred to radiotherapy center were assessed by PG-SGA(gold standard method)and NRI.Serum albumin levels of patients were determined by colorimetric method.A contingency table was used to determine the sensitivity,specificity,and predictive value of the NRI in screening patients at risk of malnutrition,in comparison with the PG-SGA in patients before radiotherapy.Results:The findings of PG-SGA and NRI showed that 52%and 45%of patients in our study were moderately or severely malnourished respectively.The NRI had a sensitivity of 66%and a specificity of 60%against PG-SGA.The positive predictive value was 64%and the negative predicative value was 62%.The agreement between NRI and PG-SGA was statistically insignificant(kappa=0.267;P>0.05).Conclusions:The findings of present study showed that the prevalence of malnutrition was high in patients with colorectal cancer.Moreover,NRI method had low sensitivity and specificity in assessing nutritional status of patients with cancer.It seems that the combination of anthropometric,laboratory parameters and a subjective scoring system may be helpful tools in screening of malnutrition in cancer patients. | Elnaz Faramarzi Reza Mahdavi Mohammad Mohammad-Zadeh Behnam Nasirimotlagh | 2013 | Chinese Journal of Cancer Research2013,25,5: | 23 |

Insulin signaling dysregulation and small vessel disease in the base of diabetes may be important contributing factors in Alzheimer's disease and vascular dementia pathogenesis, respectively. Optimal glycemic control in type 1 diabetes and identification of diabetic risk factors and prophylactic approach in type 2 diabetes are very important in the prevention of cognitive complications. In addition, hypoglycemic attacks in children and elderly should be avoided. | 13 | 柑桔黄龙病的可见-近红外光谱特征显示文摘柑桔黄龙病是一种以木虱为载体的细菌病原,目前还没有行之有效的治疗方法,对世界柑桔产业构成了严重的威胁。探索快速检测柑桔黄龙病的方法,对该病的诊断、评估及进一步的控制都具有重要意义。该研究采用了快速、无损的光谱方法对该病害特征进行初步探索。实验针对健康及染病植株的叶片及冠层,分别在实验室条件及田间环境下测量了其可见-近红外光谱反射率,以分析寻找二者的光谱差异。对原始光谱数据进行了平滑、聚类平均等预处理,并求取了一阶微分以确定其红边位置(red edge position,REP)。为了应对一阶微分在REP处的多个波峰现象,采用了线性外插值法及拉格朗日插值法量化REP。研究结果显示,健康及染病样本的反射率在可见光、近红外具有明显的差异。相比于健康样本,染病样本因其呈现的黄化现象,使其反射率在可见光区较高;又因黄龙病菌会明显阻碍叶片对水分的吸收而使其反射率在近红外较低。REP同样显示了潜在的区分能力,其明显随着染病程度的加深逐渐向红波段移动。在染病程度差异较大的数据集中,REP平均值相差达20nm;而在染病程度差异较小的数据集中,阈值分割法的分类精度也高达90%以上,且线性外插值法的分类精度略高于拉格朗日插值法。本研究成果为利用光谱技术快速无损检测柑桔黄龙病提供了可靠的理论依据。 | 李修华 李民赞 Won Suk Lee Reza Ehsani Ashish Ratn Mishra | 2014 | 光谱学与光谱分析2014,34,6: | 20 |

Diagnosis and therapeutic management of VCs constitute a major challenge in terms of increasing the success rate of liver transplantation. While surgical treatment used to be considered the first choice for management, advances in endovascular intervention have increased to make this a viable therapeutic option. Considering VC as a rare but a major and dreadful issue in OLT history, and in view of the continuing and rapid progress in recent years, an update on these uncommon conditions seemed necessary. | 15 | Global elimination of hepatitis C virus infection:Progresses and the remaining challenges显示文摘Today, with the introduction of interferon-free direct-acting antivirals and outstanding progresses in the prevention, diagnosis and treatment of hepatitis C virus(HCV) infection, the elimination of HCV infection seems more achievable. A further challenge is continued transmission of HCV infection in high-risk population specially injecting drug users(IDUs) as the major reservoir of HCV infection. Considering the fact that most of these infections remain undiagnosed, unidentified HCVinfected IDUs are potential sources for the rapid spread of HCV in the community. The continuous increase in the number of IDUs along with the rising prevalence of HCV infection among young IDUs is harbinger of a forthcoming public health dilemma, presenting a serious challenge to control transmission of HCV infection. Even the changes in HCV genotype distribution attributed to injecting drug use confirm this issue. These circumstances create a strong demand for timely diagnosis and proper treatment of HCV-infected patients through risk-based screening to mitigate the risk of HCV transmission in the IDUs community and, consequently, in the society. Meanwhile, raising general awareness of HCV infection, diagnosis and treatment through public education should be the core activity of any harm reduction intervention, as the root cause of failure in control of HCV infection has been lack of awareness among young drug takers. | 16 | Molecular identification of hepatitis B virus genotypes/subgenotypes:Revised classification hurdles and updated resolutions显示文摘The clinical course of infections with the hepatitis B virus(HBV)substantially varies between individuals,as a consequence of a complex interplay between viral,host,environmental and other factors.Due to the high genetic variability of HBV,the virus can be categorized into different HBV genotypes and subgenotypes,which considerably differ with respect to geographical distribution,transmission routes,disease progression,responses to antiviral therapy or vaccination,and clinical outcome measures such as cirrhosis or hepatocellular carcinoma.However,HBV(sub)genotyping has caused some controversies in the past due to misclassifications and incorrect interpretations of different genotyping methods.Thus,an accurate,holistic and dynamic classification system is essential.In this review article,we aimed at highlighting potential pitfalls in genetic and phylogenetic analyses of HBV and suggest novel terms for HBV classification.Analyzing fulllength genome sequences when classifying genotypes and subgenotypes is the foremost prerequisite of this classification system.Careful attention must be paid to all aspects of phylogenetic analysis,such as bootstrapping values and meeting the necessary thresholds for(sub)genotyping.Quasi-subgenotype refers to subgenotypes that were incorrectly suggested to be novel.As many of these strains were misclassified due to genetic differences resulting from recombination,we propose the term'recombino-subgenotype'.Moreover,immigration is an important confounding facet of global HBV distribution and substantially changes the geographic pattern of HBV(sub)genotypes.We therefore suggest the term'immigro-subgenotype'to distinguish exotic(sub)genotypes from native ones.We are strongly convinced that applying these two proposed terms in HBV classification will help harmonize this rapidly progressing field and allow for improved prophylaxis,diagnosis and treatment. | Mahmoud Reza Pourkarim Samad Amini-Bavil-Olyaee Fuat Kurbanov Marc Van Ranst Frank Tacke | 2014 | World Journal of Gastroenterology2014,20,23: | 20 |

雄村铜(金矿)是新近发现的超大型铜(金)矿,由作者负责勘探工作的实施,迄今完成近7万米钻探工作量.Ⅰ号矿体勘探报告已经通过评审,Ⅰ号矿体于2007年10月完成勘探,正在编写报告.'雄村式'铜金矿主要分布在日喀则弧前盆地的谢通门县-日喀则市一线.该区是西藏特提斯-喜马拉雅构造域南部,属冈底斯-念青唐古拉陆壳地体基础上发育的冈底斯南缘燕山期-早喜马拉雅期陆缘岩浆弧与昂仁-日喀则中-新生代弧前盆地转换部位.地层以一套早侏罗统一始新世的沉积-火山岩建造为主,主要为一套含火山物质的陆内碎屑岩夹碳酸盐岩、火山碎屑岩.由于受早期板块俯冲和后期板块碰撞作用的影响,岩浆活动强烈,早中晚侏罗世、白垩世、第三纪均有岩浆活动,其中与成矿有关的主要为中晚侏罗世和白垩世的岩浆活动,形成与以上岩浆活动有关的铜金矿(如雄村铜金矿、洞嘎金多金属矿、洞嘎普-普钦木铜金矿、则莫多拉铜金矿).…… | 唐菊兴 李志军 张丽 黄勇 邓起 郎兴海 Jim Lang Reza Tafiti | 2007 | 矿物学报2007,,z1: | 19 |
| 18 | FREE VIBRATION ANALYSIS OF SIMPLY SUPPORTED BEAM WITH BREATHING CRACK USING PERTURBATION METHOD显示文摘In this paper,a new analytical method for vibration analysis of a cracked simply supported beam is investigated.By considering a nonlinear model for the fatigue crack,the governing equation of motion of the cracked beam is solved using perturbation method.The solution of the governing equation reveals the superharmonics of the fundamental frequency due to the nonlinear effects in the dynamic response of the cracked beam.Furthermore,considering such a solution,an explicit expression is also derived for the system damping changes due to the changes in the crack parameters,geometric dimensions and mechanical properties of the cracked beam.The results show that an increase in the crack severity and approaching the crack location to the middle of the beam increase the system damping.In order to validate the results,changes in the fundamental frequency ratios against the fatigue crack severities are compared with those of experimental results available in the literature.Also,a comparison is made between the free response of the cracked beam with a given crack depth and location obtained by the proposed analytical solution and that of the numerical method.The results of the proposed method agree with the experimental and numerical results. | Mousa Rezaee Reza Hassannejad | 2010 | Acta Mechanica Solida Sinica2010,23,5: | 19 |

| 20 | Expression of cell adhesion molecule CD44 in gastric adenocarcinoma and its prognostic importance显示文摘AIM: To evaluate the relation of cluster of differentiation 44 (CD44) expression with clinicopathological features of gastric adenocarcinoma, and also its effect on prognosis with an emphasis on the differences between intestinal and diffuse types. METHODS: From 2000 to 2006, 100 patients with gastric adenocarcinoma, who had undergone total or subtotal gastrectomy without any prior treatment, were studied. Haematoxylin & eosin (HE) staining was used for histological evaluation, including the type (Lauren's classifi cation) and grading of the tumor. The expression of CD44 in the gastric adenocarcinoma mucosa and the adjacent mucosa were determined by immunohistochemistry. The survival analysis was obtained using the Kaplan-Meier test. RESULTS: Of 100 patients, 74 (74%) patients were male. The tumors were categorized as intestinal type (78%) or diffuse type (22%). Sixty-five percent of patients were CD44-positive. CD44 expression was not detected in normal gastric mucosa. Rather, CD44 was more commonly expressed in the intestinal subtype (P = 0.002). A signifi cant relation was seen between the grade of tumor and the expression of CD44 (P = 0.014). The survival analysis showed a poor prognosis of patients with CD44-positive tumors (P = 0.008); and this was more prominent in the intestinal (P = 0.001) rather than diffuse type. CONCLUSION: Cell adhesion molecule CD44 is highly expressed in gastric adenocarcinoma. CD44 expression is correlated with a poor prognosis in patients with the intestinal type of gastric adenocarcinoma.
