1Early vs late endoscopic retrograde cholangiopancreatography in patients with acute cholangitis: A nationwide analysis显示文摘AIM To assess the effect of early vs late endoscopic retrograde cholangiopancreatography(ERCP) on mortality and readmissions in acute cholangitis, using a nationally representative sample.METHODS We used the 2014 National Readmissions Database to identify adult patients hospitalized with acute cholangitis who underwent therapeutic ERCP within one week of admission. Early ERCP was defined as ERCP performed on the same day of admission or the next day(days 0 or 1, < 48 h), and late ERCP was performed on days 2 to 7 of admission. Patients with severe cholangitis had any of the following additional diagnoses: Severe sepsis, septic shock, acute renal failure,acute respiratory failure, or thrombocytopenia. Multivariate logistic regression was used to calculate the adjusted odds of association of ERCP timing with inhospital mortality, 30-d mortality, and 30-d readmissions, controlling for age, sex,severe disease and comorbidities.RESULTS Four thousand five hundred and seventy patients satisfied the inclusion criteria;with a mean age of 64.1 years. Of these, 66.6% had early ERCP, while 33.4% had late ERCP. Early ERCP was associated with lower in-hospital mortality [1.2% vs2.4%, adjusted odds ratio(aOR) = 0.50, 95%CI: 0.76-0.83, P = 0.001] and lower 30-d mortality(1.5% vs 3.3%, aOR = 0.48, 95%CI: 0.33-0.69, P < 0.0001) compared to the late ERCP group. Similarly, early ERCP was associated with lower 30-d readmissions(9.7% vs 15.1%, aOR = 0.58, 95%CI: 0.49-0.7, P < 0.0001). When stratified by severity of cholangitis, there was a similar benefit of early ERCP on all outcomes in those with and without severe cholangitis. The mean length of stay was higher in the late ERCP group compared to the early ERCP group(6.9 d vs 4.5 d, P < 0.0001). The mean hospitalization cost was higher in the late ERCP group($21459 vs $16939, P < 0.0001).CONCLUSION Early ERCP is associated with lower in-hospital and 30-d mortality in those with or without severe cholangitis. 2Survival analysis of cholangiocarcinoma:A 10-year experience in Malaysia显示文摘AIM:To investigate the clinical features and survival of patients treated for cholangiocarcinoma in our institution and to analyze the factors affecting their survival.METHODS:This retrospective cohort study assessed patients diagnosed with cholangiocarcinoma between January 1997 and December 2007 at the University Malaya Medical Centre in Malaysia.The clinical data and associated outcomes were collected using a structured proforma.RESULTS:Of the 69 patients diagnosed with cholangiocarcinoma,38 (55%) were male;mean patient age was 61 years.Twelve patients (17%) had intrahepatic,38 (55%) had perihilar and 19 (28%) had distal tumors.Only 12 patients underwent curative surgery,including seven R0 resections.Only one patient died within 30 d after surgery.The overall median survival was 4 mo,whereas the median survival of R0 resected patients was 16 mo.The overall 1-,2-and 3-year cumulative survival rates were 67%,17% and 17%,respectively.Survival rates were significantly associated with curative resection (P=0.002),intrahepatic tumor (P=0.003),negative margin status (P=0.013),early tumor stage (P=0.016),higher tumor differentiation (P=0.032) and absence of jaundice (P=0.038).Multivariate analysis showed that tumor location was a significant independent predictor of patient survival.CONCLUSION:Curative,margin-negative resection of early stage,well-differentiated intrahepatic tumors is associated with improved patient survival.Ahmad Ramzi Yusoff Mohd Muzammil Abdul Razak Yoong Boon Koon R Vijeyasingam Siti Zuraidah Mahmud 2012World Journal of Gastroenterology2012,18,5:7
3Effect of β-blocker therapy in diabetic patients with stable coronary heart disease: a meta-analysis显示文摘Background β-blocker (BB) therapy is a cornerstone for the treatment of coronary heart disease (CHD).The evidence of the benefit from long-term BB therapy in diabetic patients with stable CHD is scarce.This meta-analysis summarizes the evidence relating to the BB therapy in diabetic patients with stable CHD.Methods A meta-analysis was performed according to PRISMA and MOOSE guidelines for reporting of systematic reviews of observational studies.PubMed,Embase,and Cochrane central were searched and two authors independently screened studies for eligibility.The quality of studies was assessed with the Newcastle Ottawa scale.The primary outcome of interest was all-cause mortality,cardiovascular (CV) mortality and major adverse cardiovascular events (MACE) in diabetic patients with and without BB therapy.A generic inverse variance model was used to pool odds ratio or hazards ratio from included studies to calculate the overall effect estimate.The significance threshold was set at P-value < 0.05.Heterogeneity was assessed by I2.Results Four non-randomized studies with 9515 participants were selected for the analyses.Four studies were post-hoc analyses of randomized controlled trials,and one article was an analysis of a nationally representative survey.In a fixed effects model,BB therapy in diabetic patients with stable CHD was found to be associated with increased risk of CV mortality,and MACE (27% and 32% respectively;P-value < 0.05) and was not associated with a reduction in all-cause mortality (HR 1.12;95% CI: 0.94–1.33;P-value = 0.22).Conclusion BB therapy in diabetic patients with stable CHD appears to be linked to higher mortality.Large randomized trials are needed in this population to confirm these findings.Aaqib H Malik Suchith Shetty Kausik Kar Ramzi El Accaoui 2019Journal of Geriatric Cardiology2019,16,3:7

5Experimental evaluation of eco-friendly flocculants prepared from date palm rachis显示文摘Sodium carboxymethylcellulose(CMCNa) is an anionic water soluble polyelectrolyte widely used in many industrial sectors including food,textiles,papers,adhesives,paints,pharmaceuticals,cosmetics and mineral processing.CMCNa was produced by chemical modification of cellulose,and represents many advantages:natural,renewable,non-toxic and biodegradable.In this study,different kinds of CMCNa,prepared from an agricultural waste date palm rachis,were tested as eco-friendly flocculants for drinking water treatment and their performances as flocculants in turbidity removal enhancement were assessed.The prepared materials were characterized by the degree of substitution(DS) and polymerisation(DP) .The study of the effect of some experimental parameters on the coagulation-flocculation performance,using the prepared materials combined with aluminium sulphate(as coagulant) ,showed that the best conditions for turbidity treatment were given for pH 8,coagulant dose 20 mg/L,flocculant concentration of 100 mg/L and stirring velocity(during the flocculation step) of 30 r/min.Under the optimum conditions,the turbidity removal using CMCNa,prepared from raw material,was about 95%.A comparison study between the flocculation performance of a commercial anionic flocculant(A100PWG:polyacrylamide) and that of the prepared CMCNa showed that the performance of the waste-based flocculant with a DS of 1.17 and a DP of 480 was 10%better than that achieved by the commercial one.Ramzi Khiari Sonia Dridi-Dhaouadi Chadlia Aguir Mohamed Farouk Mhenni 2010Journal of Environmental Sciences2010,22,10:3
6Application of Prunus amygdalus By-products in Eco-friendly Dyeing of Textile Fabrics显示文摘Natural dyes have become an interesting subject of study because of their better ecological properties in comparison to their synthetic counterparts.This article concerns the dyeing of wool,silk,and polyamide fabrics with natural dyes extracted from almond shells and stems.The developed method of dyeing by these extracts is interesting and very attractive for several reasons:firstly,the extracts used are the black liquor discharged from the industries of delignification,which is a chemical process for removing lignin from agricultural wastes to produce a cellulosic fiber;(ii)these natural dyes are renewable and available in large quantities;(iii)this method is economical;and(iv)lastly,the dyeing performances of the obtained dyed textiles are very promising.The color of each dyed material was investigated in terms of the CIELAB coordinates and their fastness properties measured by washing,rubbing,and light.Ibtissem Moussa Noureddine Baaka Ramzi Khiari Ali Moussa Gérard Mortha Mohamed Farouk Mhenni 2018Journal of Renewable Materials2018,6,1:3

9Numerical and experimental analysis for simulating fuel reactor in chemical looping combustor system显示文摘The greenhouse problem has a significant effect on our communities such as,health and climate.Carbon dioxide is one of the main gases that cause global warming.Therefore,CO2 capture techniques have been the focus of attention these days.The chemical looping combustion technique adopted the air reactor and fuel reactor to recycle heat energy.This study presents a numerical and experimental investigation on a fuel reactor in chemical looping combustor(CLC)system.The present numerical model is introduced by the kinetic theory of granular flow and coupled with gas–solid flow with chemical reactions to simulate the combustion of solids in the CLC.The k–εturbulent model was used to model the gas phase and the particle phase.The developed model simplify the prediction of flow patterns,particle velocities,gas velocities,and composition profiles of gas products and the distribution of heterogeneous reaction rates under the same operating conditions.The predicted and experimental results were compared according to the basis of determination coefficient(R2).In addition the results showed that there is a good agreement between the predicted and experimental data.The value of(R2)for CO,CO2 and CH4 was 0.959,0.925 and 0.969 respectively.This shows that the present model is a promising simulation for solid particle combustion and gives the power direction for the design and optimization of the CLC systems.Tamer M.Ismail Lu Ding Khaled Ramzy MAbd El-Salam 2020International Journal of Coal Science & Technology2020,7,3:2

14New Biobased Polyurethane Materials from Modified Vegetable Oil显示文摘Bio-based polyurethanes(PUs)have been occurred a large attention nowadays.It was found to be an alternative to the petrochemical based materials to the fact of their weak environmental influence,availability,good price and biodegradability.In addition,the nature shows several bio-derived compounds as raw materials for the synthesis of polyols,including the vegetable oils,polyphenol,terpene,and other bio-renewable sources.With the aim to develop a new family of biobased polyurethanes(PUs)via vegetable oils,the elaboration of new Jojoba-based PUs was performed by catalyst-free polycondensation reaction of a synthesized Jojoba diol with various diisocyanates such us toluene diisocyanate(TDI)and isophorone diisocyanate(IPDI).All the compounds were characterized by FTIR and NMR spectroscopies,and their properties were determined by gel permeation chromatography,differential scanning calorimetry and thermogravimetric analysis.The obtained results show renewable vegetable oils-based PUs materials can be preparing using a new environmentally ways and giving various good properties performances.Chakib Mokhtari Fouad Malek Sami Halila Mohamed Naceur Belgacem Ramzi Khiari 2021Journal of Renewable Materials2021,9,7:2
