1Resolution of non-alcoholic steatohepatitis by rosuvastatin monotherapy in patients with metabolic syndrome显示文摘AIM: To investigate the effect of rosuvastatin monotherapy on non-alcoholic steatohepatitis(NASH). At present there is no effective treatment for non-alcoholic fatty liver disease or its advanced form NASH.METHODS: This prospective study included 20 biopsy proven patients with NASH, metabolic syndrome(Met S) and dyslipidaemia. Biochemical parameters of the blood of the patients and an ultrasonography of the liver were performed at baseline. Then patients receivedlifestyle advice and were treated for a 12 mo period with rosuvastatin(10 mg/d) monotherapy. Patients were re-evaluated during the study at 3 mo intervals, during which biochemical parameters of the blood were measured including liver enzymes. A repeat biopsy and ultrasonography of the liver were performed at the end of the study in all 20 patients. Changes in liver enzymes, fasting plasma glucose, serum creatinine, serum uric acid(SUA), high sensitivity C reactive protein(hs CRP) and lipid profile were assessed every 3 mo. The primary endpoint was the resolution of NASH and the secondary endpoints were the changes in liver enzyme and lipid values.RESULTS: The repeat liver biopsy and ultrasonography showed complete resolution of NASH in 19 patients, while the 20 th, which had no improvement but no deterioration either, developed arterial hypertension and substantial rise in triglyceride levels during the study, probably due to changes in lifestyle including alcohol abuse. Serum alanine transaminase, aspartate transaminase, and γ-glutamyl transpeptidase were normalised by the 3rd treatment month(ANOVA P < 0.001), while alkaline phosphatase activities by the 6th treatment month(ANOVA, P = 0.01). Fasting plasma glucose and glycated haemoglobin were significantly reduced(P < 0.001). Lipid values were normalised by the 3rd treatment month. No patient had Met S by the 9th treatment month. Body mass index and waist circumference remained unchanged during the study. Thus, changes in liver pathology and function should be attributed solely to rosuvastatin treatment. A limitation of the study is the absence of a control group.CONCLUSION: These findings suggest that rosuvastatin monotherapy could ameliorate biopsy proven NASH and resolve Met S within 12 mo. These effects and the reduction of fasting plasma glucose and SUA levels may reduce the risk of vascular and liver morbidity and mortality in NASH patients. 2海南地区强磁暴期间强区域扩展F事件研究显示文摘利用2004年海南DPS-4数字测高仪观测到的强区域扩展F(SSF)数据,分析研究了5个强磁暴(Dst<-100 nT)事件期间海南SSF的响应特征.结果发现,在海南地区,5个强磁暴事件中有3个磁暴Dst最小值位于2200—0200 LT之间,在磁暴主相及恢复相初期均出现了SSF现象,这种触发作用可能源于磁层直接渗透电场的作用,而另两个磁暴Dst最小值均发生在白天,一个SSF现象出现在磁暴的恢复相晚间,另一个SSF现象出现在超强磁暴的初相晚间,后者可能由该超强磁暴的急始造成的直接渗透电场所触发;5个强磁暴期间发生的SSF现象或者仅出现在午夜前,或者先出现在午夜前并持续到午夜后;同时,还就这些观测结果与Dabas等人有关磁暴对ESF影响的结论进行对比和讨论.王国军 史建魁 王霄 刘振兴 Zherebtsov G Olga P 2007空间科学学报2007,27,5:6
3Unmanned aerial vehicles(UAV)for assessment of qualitative classification of Norway spruce in temperate forest stands显示文摘The study investigates the potential of UAV-based remote sensing technique for monitoring of Norway spruce health condition in the affected forest areas.The objectives are:(1)to test the applicability of UAV visible an near-infrared(VNIR)and geometrical data based on Z values of point dense cloud(PDC)raster to separate forest species and dead trees in the study area;(2)to explore the relationship between UAV VNIR data and individual spruce health indicators from field sampling;and(3)to explore the possibility of the qualitative classification of spruce health indicators.Analysis based on NDVI and PDC raster was successfully applied for separation of spruce and silver fir,and for identification of dead tree category.Separation between common beech and fir was distinguished by the object-oriented image analysis.NDVI was able to identify the presence of key indicators of spruce health,such as mechanical damage on stems and stem resin exudation linked to honey fungus infestation,while stem damage by peeling was identified at the significance margin.The results contributed to improving separation of coniferous(spruce and fir)tree species based on VNIR and PDC raster UAV data,and newly demonstrated the potential of NDVI for qualitative classification of spruce trees.The proposed methodology can be applicable for monitoring of spruce health condition in the local forest sites.Olga Brovkina Emil Cienciala Peter Surový Přemysl Janata 2018Geo-Spatial Information Science2018,21,1:6
4Pancreatic adenocarcinoma:Outstanding problems显示文摘Pancreatic adenocarcinoma remains the fourth leading cause of cancer-related death and is one of the most aggressive malignant tumors with an overall 5-year survival rate of less than 4%.Surgical resection remains the only potentially curative treatment but is only possible for 15%-20% of patients with pancreatic adenocarcinoma.About 40% of patients have locally advanced nonresectable disease.In the past,determination of pancreatic cancer resectability was made at surgical exploration.The development of modern imaging techniques has allowed preoperative staging of patients.Institutions disagree about the criteria used to classify patients.Vascular invasion in pancreatic cancers plays a very important role in determining treatment and prognosis.There is no evidence-based consensus on the optimal preoperative imaging assessment of patients with suspected pancreatic cancer and a unified definition ofborderline resectable pancreatic cancer is also lacking.Thus,there is much room for improvement in all aspects of treatment for pancreatic cancer.Multi-detector computed tomography has been widely accepted as the imaging technique of choice for diagnosing and staging pancreatic cancer.With improved surgical techniques and advanced perioperative management,vascular resection and reconstruction are performed more frequently;patients thought once to be unresectable are undergoing radical surgery.However,when attempting heroic surgery,a realistic approach concerning the patient's age and health status,probability of recovery after surgery,perioperative morbidity and mortality and life quality after tumor resection is necessary.Olga P Zakharova Grigory G Karmazanovsky Viacheslav I Egorov 2012World Journal of Gastrointestinal Surgery2012,4,5:4
5Reasons of PEG failure to eliminate gastroesophageal reflux in mechanically ventilated patients显示文摘AIM:To investigate factors predicting failure of percutaneous endoscopic gastrostomy(PEG)to eliminate gastroesophageal reflux(GER). METHODS:Twenty-nine consecutive mechanically ventilated patients were investigated.Patients were evaluated for GER by pH-metry pre-PEG and on the 7th post-PEG day.Endoscopic and histologic evidence of reflux esophagitis was also carried out.A beneficial response to PEG was considered when pH-metry on the 7th post-PEG day showed that GER was below 4%. 7Current trends of liver cirrhosis in Mexico: Similitudes and differences with other world regions显示文摘AIM To investigate the main current etiologies of cirrhosis in Mexico.METHODS We performed a cross-sectional retrospective multicenter study that included eight hospitals in different areas of Mexico. These hospitals provide health care to people of diverse social classes. The inclusion criteria were a histological, clinical, biochemical, endoscopic, or imaging diagnosis of liver cirrhosis. Data were obtained during a 5-year period(January 2012-December 2017). RESULTS A total of 1210 patients were included. The mean age was 62.5 years(SD = 12.1), and the percentages of men and women were similar(52.0% vs 48.0%). The most frequent causes of liver cirrhosis were hepatitis C virus(HCV)(36.2%), alcoholic liver disease(ALD)(31.2%), and nonalcoholic steatohepatitis(23.2%), and the least frequent were hepatitis B virus(1.1%), autoimmune disorders(7.3%), and other conditions(1.0%).CONCLUSION HCV and ALD are the most frequent causes of cirrhosis in Mexico. However, we note that non-alcoholic fatty liver disease(NAFLD) as an etiology of cirrhosis increased by 100% compared with the rate noted previously.
