1CA19-9 antigen levels can distinguish between benign and malignant pancreaticobiliary disease显示文摘BACKGROUND:CA19-9 is a carbohydrate tumor-associated antigen which is frequently upregulated in pancreatobiliary neoplasia.However,it may also be elevated in patients with jaundice in the absence of a tumor due to biliary obstruction,and in other non-hepato-pancreatico-biliary conditions.This study aimed to evaluate whether CA19-9 levels could accurately differentiate between benign and malignant pancreatobiliary disease.METHODS:All patients referred to a single surgeon for investigation of pancreaticobiliary disease in 2003 in whom a firm diagnosis had been established were included.For malignant disease,a histological diagnosis was required but for benign disease a firm radiological diagnosis was deemed adequate.The patients were divided into 4 categories:pancreatic adenocarcinoma(PCa);cholangiocarcinoma(CCa);chronic pancreatitis(CP)and biliary calculous disease(Calc).Bilirubin and alkaline phosphatase levels corresponding to the point of assessment of CA19-9 were also noted.RESULTS:Final diagnoses were made of pancreatic adenocarcinoma(PCa,n=73),cholangiocarcinoma(CCa,n=19),ampullary carcinoma(Amp,n=7),neuroendocrine carcinoma(Neu,n=4),duodenal carcinoma(Duo,n=3),chronic pancreatitis(CP,n=115),and biliary calculous disease(Calc,n=27).Median CA19-9 levels(U/ml)were:PCa,653;CCa,408;Duo,403;Calc,27;CP,19;Neu,10.5;Amp,8(reference range:0-37).The CA19-9 levels were significantly greater for malignant than for benign disease,could differentiate PCa from CCa/Duo,and were significantly higher in unresectable than in resectable PCa.The sensitivity,specificity,positive predictive value(PPV)and negative predictive value(NPV)for CA19-9 were 84.9%,69.7%,67.7%and 86.1%,respectively.A ROC analysis provided an area under the curve for CA19-9 of 0.871(0.820-0.922),giving an optimal CA19-9 of 70.5 U/ml for differentiating benign from malignant pathology.Using this cut-off,the sensitivity was 82.1%,while specificity,PPV and NPV improved to 85.9%,81.3%and 86.5%,respectively.When standard radiology was included(US/ CT/MRCP)in the decision process,the results improved to 97.2%,88.7%,86.6%,and 97.7%.For benign disease,the CA19-9 correlated directly with the serum bilirubin,but for malignant disease,CA19-9 levels were elevated independent of the bilirubin level.CONCLUSIONS:CA19-9 is useful in the differentiation of pancreatobiliary disease and when using an optimized cut-off and combining with routine radiology,the diagnostic yield is improved significantly,thus stressing the importance of a multi-disciplinary approach to pancreatobiliary disease.Gareth Morris-Stiff Mary Teli Nicky Jardine Malcolm CA Puntis 2009Hepatobiliary & Pancreatic Diseases International2009,8,6:33

3Simple and customizable method for fabrication of high-aspect ratio microneedle molds using low-cost 3D printing显示文摘We present a simple and customizable microneedle mold fabrication technique using a low-cost desktop SLA 3D printer.As opposed to conventional microneedle fabrication methods,this technique neither requires complex and expensive manufacturing facilities nor expertise in microfabrication.While most low-cost 3D-printed microneedles to date display low aspect ratios and poor tip sharpness,we show that by introducing a two-step“Print&Fill”mold fabrication method,it is possible to obtain high-aspect ratio sharp needles that are capable of penetrating tissue.Studying first the effect of varying design input parameters and print settings,it is shown that printed needles are always shorter than specified.With decreasing input height,needles also begin displaying an increasingly greater than specified needle base diameter.Both factors contribute to low aspect ratio needles when attempting to print submillimeter height needles.By setting input height tall enough,it is possible to print needles with high-aspect ratios and tip radii of 20–40μm.This tip sharpness is smaller than the specified printer resolution.Consequently,high-aspect ratio sharp needle arrays are printed in basins which are backfilled and cured in a second step,leaving sub-millimeter microneedles exposed resulting microneedle arrays which can be used as male masters.Silicone female master molds are then formed from the fabricated microneedle arrays.Using the molds,both carboxymethyl cellulose loaded with rhodamine B as well as polylactic acid microneedle arrays are produced and their quality examined.A skin insertion study is performed to demonstrate the functional capabilities of arrays made from the fabricated molds.This method can be easily adopted by the microneedle research community for in-house master mold fabrication and parametric optimization of microneedle arrays.Kevin J.Krieger Nicky Bertollo Manita Dangol John T.Sheridan Madeleine M.Lowery Eoin D.O’Cearbhaill 2019Microsystems & Nanoengineering2019,5,1:4

5NON-DESTRUCTIVE ANALYSIS OF THE NUCLEI OF TRANSGENIC LIVING CELLS USING LASER TWEEZERS AND NEAR-INFRARED RAMAN SPECTROSCOPIC TECHNIQUE显示文摘Transgenic cell lines of loblolly pine (Pinus taeda L.) were analyzed by a compact laser-tweezers-Raman-spectroscopy (LTRS) system in this investigation. A low power diode laser at 785 nm was used for both laser optical trapping of single transgenic cells and excitation for near-infrared Raman spectroscopy of the nuclei of synchronized cells, which were treated as single organic particles, at the S-phase of the cell cycle. Transgenic living cells with gfp and uidA genes were used as biological samples to test this LTRS technique. As expected, di?erent Raman spectra were observed from the tested biological samples. This technique provides a high sensitivity and enables real-time spectroscopic measurements of transgenic cell lines. It could be a valuable tool for the study of the fundamental cell and molecular biological process by trapping single nucleus and by providing a wealth of molecular information about the nuclei of cells.Wei Tang Ronald J. 17Tuberculosis infection prevention and control:why we need a whole systems approach显示文摘Infection prevention and control(IPC)measures to reduce transmission of drug-resistant and drug-sensitive tuberculosis(TB)in health facilities are well described but poorly implemented.The implementation of TB IPC has been assessed primarily through quantitative and structured approaches that treat administrative,environmental,and personal protective measures as discrete entities.We present an on-going project entitled Umoya omuhle(“good air”),conducted in two provinces of South Africa,that adopts an interdisciplinary,‘whole systems’approach to problem analysis and intervention development for reducing nosocomial transmission of Mycobacterium tuberculosis(Mtb)through improved IPC.We suggest that TB IPC represents a complex intervention that is delivered within a dynamic context shaped by policy guidelines,health facility space,infrastructure,organisation of care,and management culture.Methods drawn from epidemiology,anthropology,and health policy and systems research enable rich contextual analysis of how nosocomial Mtb transmission occurs,as well as opportunities to address the problem holistically.A‘whole systems’approach can identify leverage points within the health facility infrastructure and organisation of care that can inform the design of interventions to reduce the risk of nosocomial Mtb transmission.Karina Kielmann Aaron S.Karat Gimenne Zwama Christopher Colvin Alison Swartz Anna S.Voce Tom A.Yates Hayley MacGregor Nicky McCreesh Idriss Kallon Anna Vassall Indira Govender Janet Seeley Alison D.Grant 2020Infectious Diseases of Poverty2020,9,3:1
