1Pathogenesis and clinical management of Helicobacter pylori gastric infection显示文摘Helicobacter pylori(H.pylori)is a gram-negative bacterium that infects approximately 4.4 billion individuals worldwide.However,its prevalence varies among different geographic areas,and is influenced by several factors.The infection can be acquired by means of oral-oral or fecal-oral transmission,and the pathogen possesses various mechanisms that improve its capacity of mobility,adherence and manipulation of the gastric microenvironment,making possible the colonization of an organ with a highly acidic lumen.In addition,H.pylori presents a large variety of virulence factors that improve its pathogenicity,of which we highlight cytotoxin associated antigen A,vacuolating cytotoxin,duodenal ulcer promoting gene A protein,outer inflammatory protein and gamma-glutamyl transpeptidase.The host immune system,mainly by means of a Th1-polarized response,also plays a crucial role in the infection course.Although most H.pylori-positive individuals remain asymptomatic,the infection predisposes the development of various clinical conditions as peptic ulcers,gastric adenocarcinomas and mucosa-associated lymphoid tissue lymphomas.Invasive and non-invasive diagnostic methods,each of them with their related advantages and limitations,have been applied in H.pylori detection.Moreover,bacterial resistance to antimicrobial therapy is a major challenge in the treatment of this infection,and new therapy alternatives are being tested to improve H.pylori eradication.Last but not least,the development of effective vaccines against H.pylori infection have been the aim of several research studies.Breno Bittencourt de Brito Filipe Ant?nio Fran?a da Silva Aline Silva Soares Vinícius Afonso Pereira Maria Luísa Cordeiro Santos Mariana Miranda Sampaio Pedro Henrique Moreira Neves Fabrício Freire de Melo 2019World Journal of Gastroenterology2019,25,37:84
2Exercise training in hypertension:Role of microRNAs显示文摘Hypertension is a complex disease that constitutes an important public health problem and demands many studies in order to understand the molecular mechanisms involving his pathophysiology. Therefore, an increasing number of studies have been conducted and new therapies are continually being discovered. In this context, exercise training has emerged as an important non-pharmacological therapy to treat hypertensive patients, minimizing the side effects of pharmacological therapies and frequently contributing to allow pharmacotherapy to be suspended. Several mechanisms have been associated with the pathogenesis of hypertension, such as hyperactivity of the sympathetic nervous system and renin-angiotensin aldosterone system,impaired endothelial nitric oxide production, increased oxygen-reactive species, vascular thickening and stiffening, cardiac hypertrophy, impaired angiogenesis, and sometimes genetic predisposition. With the advent of microRNAs(miRNAs), new insights have been added to the perspectives for the treatment of this disease, and exercise training has been shown to be able to modulate the miRNAs associated with it. Elucidation of the relationship between exercise training and miRNAs in the pathogenesis of hypertension is fundamental in order to understand how exercise modulates the cardiovascular system at genetic level. This can be promising even for the development of new drugs. 3Clinical and pathological characterization of epstein-barr virus-associated gastric carcinomas in portugal显示文摘AIM To determine the prevalence of epstein-barr virus(eb V)-associated gastric carcinomas in the North Region of Portugal and to study its clinicopathological characteristics. METHODS We have performed a retrospective study including a total of 179 consecutive patients with gastric cancer(GC) submitted to gastrectomy during 2011 at the Portuguese Oncology Institute of Porto. Clinical and pathological data was collected from individual clinical records and inserted on a database with unique codification. Tumour tissues were collected from the institutional tumour bank. eb V was detected by in situ hybridization for the detection of eb V-encoded small RNAs(ebe Rs) and eb V latent proteins(LMP1 and LMP2 A) were detected by immunohistochemistry.RESULTS The analysis showed that eb V-associated gastric carcinomas(eb Va GC) represents 8.4%(15/179) of all GC cases, with a significant differential distribution among histological types(P < 0.001): 100%(3/3) of medullary carcinomas, 100%(1/1) of adenosquamous carcinoma, 8.7%(8/92) of tubular adenocarcinomas, 8.0%(2/25) of mixed carcinomas and 2%(1/51) in poorly cohesive carcinomas. The analysis revealed a higher predominance of eb Va GC in the upper third and middle(cardia, fundus and body) of the stomach(P = 0.041), a significant lower number of regional lymph nodes invasion(P = 0.025) and a tendency for better prognosis(P = 0.222). eb V latent protein expression revealed that all eb Va GC cases were LMP1-negative, nevertheless 6 cases(40%) expressed LPM2 A, which reveals that these cases show a distinct eb V-Latency profile(latency II-like).CONCLUSION eb Va GC represents 8.4% of all GC in the North Region of Portugal. 4Comparative stability analyses of traditional and selective room-and-pillar mining techniques for sub-horizontal tungsten veins显示文摘The stability and productivity concerning a modification on the traditional room and pillar for a new selective technique at the Portuguese Panasqueira Mine have been described.The traditional room-and-pillar stoping uses 5.0-m wide rooms with 3.0 m×3.0 m pillars,while the selective room-and-pillar mining technique consists in stoping with rooms of 4.0 m wide and pillars of 4 m×4 m with a subsequent selective cutting of the quartz veins at the mid pillar of approximately 0.5 m high,to obtain a pillar section with an area of 3.0 m×3.0 m.The stability and productivity analyses indicate that the selective technique obtains smaller average pillar safety factor,more rock mass displacement,more extraction and selectivity ratios,and ore grade improvement,compared with the traditional technique.These results show that the selective technique is also more convenient.This proposed selective room-and-pillar mining technique is applicable to any sub-horizontal narrow quartz veins with wolfram,gold,etc.such as the famous La Rinconada gold mine in the Peruvian Andes.Vidal Felix Navarro Torres Carlos Dinis da Gama Matilde Costa e Silva Paula Falcao Neves Qiang Xie 2011International Journal of Minerals,Metallurgy and Materials2011,18,1:7
5Mercury in aquatic fauna contamination:A systematic review on its dynamics and potential health risks显示文摘Mercury is an important pollutant,released into aquatic ecosystems both naturally and by anthropogenic action.This element is transferred to aquatic organisms in different ways,causing potential health risks.In addition,mercury can be accumulated by humans,especially through the consumption of contaminated food.This systematic review aims to present mercury pathways,the major routes through which this element reaches the aquatic environment and its transformations until becoming available to living animals,leading to bioaccumulation and biomagnification phenomena.The key biotic and abiotic factors affecting such processes,the impact of mercury on animal and human health and the issue of seafood consumption as a source of chronic mercury contamination are also addressed.A total of 101 articles were retrieved from a standardized search on three databases(PubMed,Emabse,and Web of Science),in addition to 28 other studies not found on these databases but considered fundamental to this review(totaling 129 articles).Both biotic and abiotic factors display fundamental importance in mediating mercurial dynamics,i.e.,muscle tropism,and salinity,respectively.Consequently,mercurial contamination in aquatic environments affects animal health,especially the risk of extinction species and also on human health,with methylmercury the main mercury species responsible for acute and chronic symptomatology.Paloma de Almeida Rodrigues Rafaela Gomes Ferrari Luciano Neves dos Santos Carlos Adam Conte Junior 2019Journal of Environmental Sciences2019,31,10:6
6WJH 6^(th) Anniversary Special Issues(4): Cirrhosis Role of vaptans in the management of hydroelectrolytic imbalance in liver cirrhosis显示文摘Ascites and hyponatremia are the most common complications in patients with liver cirrhosis and develop as a consequence of a severe impairment of liver function and portal hypertension. Increasing evidences support the central role of renal function alterations in the pathogenesis of hydroelectrolytic imbalances in cirrhotic patients, thus implying a dense cross-talk between liver and kidney in the systemic and splanchnic vascular homeostasis in such subjects. Since Arginin Vasopressin(AVP) hyperincretion occurs at late stage of cirrhosis and plays an important role in the development of refractory ascites, dilutional hyponatremia and finally hepato-renal syndrome, selective antagonists of AVP receptors V2(vaptans) have been recently introduced in the therapeutic algorithm of advanced cirrhotic patients. Despite the promising results of earlier phasetwo studies, randomized controlled trials failed to find significant results in terms of efficacy of such drugs both in refractory ascites and hyponatremia. Moreover, concerns on their safety profile arise, due to the number of potentially severe side effects of vaptans in the clinical setting, such as hypernatremia, dehydration, renal impairment, and osmotic demyelination syndrome. 7Detection of flock movement and behaviour of broiler chickens at different feeders using image analysis显示文摘The behaviour of broiler chickens has been extensively studied as a function of stocking density and environmental conditions,but limited information was reported in the scientific literature about the effect of feeder type on birds’feeding process.The main objective of this study was to assess the effects of three different feeder types in relation to the birds’behaviour in its surroundings.The analysed feeders were:tube-type with partition grid(F1);tube-type without partition grid(F2);and automatic type with partition grid(F3).The considered variables were:occupied area(OA);activity index(AI)(flock movement);total birds presented in the area(TB);and birds effectively eating(EE).OA and AI were calculated by computational image analysis while TB and EE were manually measured.The results indicated that the feeder type could have influenced the birds’behaviour regarding to OA(R2=0.56),TB(R2=0.48),and EE(R2=0.40),but AI(R2=0.01)was not found to be directly influenced by the feeder type.A higher percentage of birds effectively eating were found in F2(86.4%),which was the one with the largest free area to access the feed.Similar average number of total birds was found in F1,but with a lower percentage of individuals effectively eating(63.3%),which means that birds were nearby this feeder performing other behaviours.Since the assessed feeders were in the same house under the same conditions,it can be suggested that not only the free area to access the feed but potentially the design of feeders could have influenced the birds’feeding preference.The real beneficial effect of the adoption of partition grid on feed trays is still uncertain,and it is also unclear whether the financial value of reduced wastage would compensate the possible reduction in feed intake.Diego Pereira Neves Saman Abdanan Mehdizadeh Matthew Tscharke Irenilza de Alencar Nääs Thomas Michael Banhazi 2015Information Processing in Agriculture2015,2,3:4
8Analysis of foundation sliding of an arch dam considering the hydromechanical behavior显示文摘This paper presents the application of a methodology which can be used to assess arch dam foundation stability,using the discrete element method(DEM)and the code 3DEC.A global three-dimensional model of a dam foundation was developed,in which some discontinuities were simulated and both the grout and drainage curtains were represented.The model,calibrated taking into account recorded data,was used to carry out nonlinear mechanical analysis.The same model was employed to perform a hydraulic analysis,based on equivalent continuum concepts,which allowed the water pressure pattern within the foundation to be obtained.These water pressures were applied on discontinuities involved in the possible sliding mechanism along the dam/foundation interface,and the safety of the dam/foundation system was evaluated using a process of reduction of strength characteristics,with the aim of calculating the minimum safety factors that ensure stability.Results were compared with those obtained with the usual bi-linear uplift pressure distribution at the base of the dam,commonly used in concrete dam design.The relevance of carrying out hydraulic analysis in arch dam foundation failure studies is highlighted.Maria Luísa Braga FARINHA JoséVieira de LEMOS Emanuel MARANHA DAS NEVES 2012Frontiers of Structural and Civil Engineering2012,6,1:4
9Impact of enclosure management on soil properties and microbial biomass in a restored semi-arid rangeland, Kenya显示文摘Rangeland degradation is a serious problem throughout sub-Saharan Africa and its restoration is a challenge for the management of arid and semi-arid areas. In Lake Baringo Basin of Kenya, communities and individual farmers are restoring indigenous vegetation inside enclosures in an effort to combat severe land degradation and address their livelihood problems. This study evaluated the impact of enclosure management on soil properties and microbial biomass, being key indicators of soil ecosystem health. Six reseeded communal enclosures using soil embankments as water-harvesting structures and strictly regulated access were selected, varying in age from 13 to 23 years. In six private enclosures, ranging from 3 to 17 years in age, individual farmers emulated the communal enclosure strategy and restored areas for their exclusive use. Significant decreases in bulk density, and increases in the soil organic carbon, total nitrogen and microbial biomass contents and stocks were found in the enclosures as compared with the degraded open rangeland. In the private enclosures, the impact of rehabilitation on the soil quality was variable, and soil quality was in general lower than that obtained under communal management. The significant increase of absolute stocks of carbon, nitrogen and microbial biomass compared to the degraded open rangeland indicates the potential for the restoration of soil quality through range rehabilitation. 11Bitcoin pricing: impact of attractiveness variables显示文摘The research seeks to contribute to Bitcoin pricing analysis based on the dynamics between variables of attractiveness and the value of the digital currency.Using the error correction model,the relationship between the price of the virtual currency,Bitcoin,and the number of Google searches that used the terms bitcoin,bitcoin crash and crisis between December 2012 and February 2018 is analyzed.The study also applied the same analysis to prices of Bitcoin denominated in different sovereign currencies traded during the same period.The Johansen(J Econ Dyn Control 12:231-254,1988)test demonstrates that the price and number of searches on Google for the first two terms are cointegrated.This research indicates that there are strong short-term and long-term dynamics among attractiveness factors,suggesting that an increase in worldwide interest in Bitcoin is usually preceded by a price increase.In contrast,an increase in market mistrust over a collapse of the currency,as measured by the term bitcoin crash,is followed by a fall in price.Intense world economic crisis events appear to have a strong impact on interest in the virtual currency.This study demonstrates that during a worldwide crisis Bitcoin becomes an alternative investment,increasing its price.Based on it,bitcoin may be used as a safe haven by the financial market and its intrinsic characteristics might help the investors and governments to find new mechanisms to deal with monetary transactions.Rodrigo Hakim das Neves 2020Financial Innovation2020,6,1:3

13Parathyroid ultrasonography and bone metabolic profile of patients on dialysis with hyperparathyroidism显示文摘AIM To evaluate the parathyroid ultrasonography and define parameters that can predict poor response to treatment in patients with secondary hyperparathyroidism due to renal failure.METHODS This cohort study evaluated 85 patients with chronic kidney disease stage V with parathyroid hormone levels above 800 pg/mL. All patients underwent ultrasonography of the parathyroids and the following parameters were analyzed: Demographic characteristics(etiology of chronic kidney disease, gender, age, dialysis vintage, vascular access, use of vitamin D), laboratory(calcium, phosphorus, parathyroid hormone, alkaline phosphatase, bone alkaline phosphatase), and the occurrence of bone changes, cardiovascular events and death. The χ~2 test were used to compare proportions or the Fisher exact test for small sample frequencies. Student t-test was used to detect differences between the two groups regarding continuous variables.RESULTS Fifty-three patients(66.4%) had parathyroid nodules with higher levels of parathyroid hormone, calcium and phosphorus. Sixteen patients underwent parathyroidectomy and had higher levels of phosphorus and calcium × phosphorus product(P = 0.03 and P = 0.006, respectively). They also had lower mortality(32% vs 68%, P = 0.01) and lower incidence of cardiovascular or cerebrovascular events(27% vs 73%, P = 0.02). Calcium × phosphorus product above 55 mg^2/dL^2 [RR 1.48(1.06, 2.08), P = 0.03], presence of vascular calcification [1.33(1.01, 1.76), P = 0.015] and previous occurrence of vascular events [RR 2.25(1.27, 3.98), P < 0.001] were risk factors for mortality in this population. However, it may provoke intestinal fibrosis with the development of strictures which require surgery. Therefore, fibrogenesis may be considered as a 'two-faced' process when related to chronic intestinal inflammation. Many types of cells may be converted into the fibrogenic phenotype at different levels of the intestinal wall. A complex interaction of cytokines, adhesion molecules and growth factors is involved in the process. We report an overview of recent advances in molecular mechanisms of stricturizing Crohn’s disease(CD) including the potential role of trasforming growth factor beta, protein kinase C and Ras, Raf and ERK proteins. Fibrotic growth factors such as vascular endothelial growth factor and platelet-derived growth factor, as well as the Endothelial-to-Mesenchymal Transition induced by transforming growth factor-β, are considered. Finally, our experience, focused on tumor necrosis factor α(the main cytokine of inflammatory bowel diseases) and the link between syndecan 1(a heparan sulphate adhesion molecule) and basic fibroblast growth factor(a strong stimulator of collagen synthesis) is described. We hypothesize a possible molecular pattern for mucosal healing as well as how its deregulation could be involved in fibrotic complications of CD. 19Robotic surgery techniques to approach benign prostatic hyperplasia disease:A comprehensive literature review and the state of art显示文摘Objective:The robotic-assisted approach to simple prostatectomy(RASP)was conceived,essentially reproducing the fundaments of open simple prostatectomy.Since the first report,RASP underwent several technical modifications.The study aims to identify and describe the current robotic surgery techniques to approach benign prostatic hyperplasia(BPH).Methods:The paper performed a non-systematic literature review accessing PubMed and Embase databases for all full-text articles published from 2008 to May 2020,assessing robot-assisted surgical techniques for BPH treatment using the terms“robot-assisted simple prostatectomy”OR“robotic simple prostatectomy”OR“RASP”AND“surgical technique”.Results:After careful review of 180 studies in PubMed and 198 in Embase,16 papers reporting different RASP techniques.After the first procedure described by Sotelo et al.[9],several authors contributed to the development of the RASP technique.John et al.[24]proposed the extraperitoneal access,and Yuh et al.[23]first reported the adenoma transcapsular dissection.Some modifications were proposed by Coelho et al.[31]on trigonization,posterior reconstruction,and urethro-vesical anastomosis.Other groups focused on urethral-preserving procedures.Moschovas et al.[28]and Clavijo et al.[32]recently described an intrafascial RASP with the removal of the entire prostatic tissue.Finally,Kaouk et al.[29]reported the feasibility and safety of the da Vinci Single Port approach.Conclusion:In the last eighteen years,the robotic-assisted approach to BPH disease has been evolved,and different techniques have been described.This review details all the technical developments on RASP that distinctive groups have proposed since the multiport robotic platforms until the new da Vinci Single Port.Marcio Covas Moschovas Frederico Timo´teo Leonardo Lins Ose´as de Castro Neves Kulthe Ramesh Seetharam Bhat Vipul R.Patel 2021Asian Journal of Urology2021,8,1:2
