| 3 | Linagliptin alleviates fatty liver disease in diabetic db/db mice显示文摘AIM To study the effects of linagliptin on the structural signs of non-alcoholic fatty liver disease(NAFLD) in db/db mice. METHODS Male diabetic db /db mice(BKS.Cg-Dock7m+/+Leprdb/J) aged 10 wk received the dipeptidyl peptidase 4(DPP4) inhibitor linagliptin(10 mg/kg) or saline as a placebo once per day by gavage for 8 wk. Intact db/db mice served as controls. Structural changes in the liver were analyzed from light and electron microscopic images of sections from intact, placebo-treated and linagliptin-treated animals. We estimated the changes in hepatocytes, sinusoidal cells, liver microvasculature and lymphatic roots. Hepatic staining for lymphatic vessel endothelial hyaluronan receptor-1(LYVE-1) was assessed by immunohistochemistry. RESULTS In 18-wk-old diabetic mice, liver steatosis(predominantly microvesicular and mediovesicular steatosis) was accompanied by dilation of the roots of the lymphatic system, interlobular blood vessels and bile canaliculi. Compared to saline-treated mice, linagliptin-treated mice exhibited a reduction in the mean numeral densities of hepatocytes with lipid droplets(92.4% ± 1.7% vs 64.9% ± 5.8% per field of view, P = 0.0002) and a lower proportion of hepatocytes with a high density of lipid droplets(20.7% ± 3.6% vs 50.4% ± 3.1%, P = 0.0007). We observed heterogeneous hepatocytes and relatively preserved cell structures in the linagliptin group. Dilation of blood and lymphatic vessels, as well as ultrastructural changes in the hepatocyte endoplasmic reticulum and mitochondria, were alleviated by linagliptin treatment. In intact and placebo-treated mice, immunohistochemical staining for LYVE-1 was observed in the endothelial cells of interlobular lymphatic vessels and on the membranes of some endothelial sinusoidal cells. We observed an enlarged LYVE-1 reaction area in linagliptin-treated mice compared to intact and placebo-treated mice. The improvement in the structural parameters of the liver in linagliptin-treated mice was independent to changes in the plasma glucose levels. | 4 | Sex-specific differences in white matter microvascular integrity after ischaemic stroke显示文摘background and purpose Functional outcomes after ischaemic stroke are worse in women,despite adjusting for differences in comorbidities and treatment approaches.White matter microvascular integrity represents one risk factor for poor long-term functional outcomes after ischaemic stroke.The aim of the study is to characterise sex-specific differences in microvascular integrity in individuals with acute ischaemic stroke.Methods A retrospective analysis of subjects with acute ischaemic stroke and brain MRI with diffusion-weighted(DWI)and dynamic-susceptibility contrast-enhanced(DSC)perfusion-weighted imaging obtained within 9 hours of last known well was performed.In the hemisphere contralateral to the acute infarct,normal-appearing white matter(NAWM)microvascular integrity was measured using the K_(2) coefficient and apparent diffusion coefficient(ADC)values.Regression analyses for predictors of K_(2) coefficient,DWI volume and good outcome(90-day modified Rankin scale(mRS)score<2)were performed.results 105 men and 79 women met inclusion criteria for analysis.Despite no difference in age,women had increased NAWM K_(2) coefficient(1027.4 vs 692.7×10^(-6)/s;p=0.006).In women,atrial fibrillation(β=583.6;p=0.04)and increasing NAWM ADC(β=4.4;p=0.02)were associated with increased NAWM K_(2) coefficient.In multivariable regression analysis,the K_(2) coefficient was an independent predictor of DWI volume in women(β=0.007;p=0.01)but not men.Conclusions In women with acute ischaemic stroke,increased NAWM K_(2) coefficient is associated with increased infarct volume and chronic white matter structural integrity.Prospective studies investigating sex-specific differences in white matter microvascular integrity are needed. | Mark R Etherton Ona Wu Pedro Cougo Svetlana Lorenzano Hua Li Lisa Cloonan Mark J R J Bouts Arne Lauer Ken Arai Eng H Lo Steve K Feske Karen L Furie Natalia S Rost | 2019 | Stroke & Vascular Neurology2019，4，4： | 2 |
| 5 | Estimation of visceral fat is useful for the diagnosis of significant fibrosis in patients with non-alcoholic fatty liver disease显示文摘BACKGROUND Obesity is a risk factor for non-alcoholic fatty liver disease(NAFLD),although obese patients with NAFLD do not always develop significant fibrosis.The distribution of body fat could predict the risk of NAFLD progression.AIM To investigate the role of bioelectrical impedance-estimated visceral fat(VF)in assessing NAFLD severity.METHODS In this cross-sectional study,patients with biopsy-proven NAFLD were prospectively included.All patients underwent anthropometric evaluation,blood tests and bioelectrical impedance analysis.RESULTS Between 2017 and 2020,119 patients were included[66.4%male,56 years(SD 10.7),62.2%obese,61.3%with metabolic syndrome].Sixty of them(50.4%)showed significant fibrosis(≥F2)in liver biopsy.Age,VF and metabolic syndrome were associated with significant fibrosis(61 years vs 52 years,16.4 vs 13.1,73.3%vs 49.2%,respectively;P<0.001 for all).In the multivariate analysis,VF and age were independently associated with significant fibrosis(VF,OR:1.11,95%CI:1.02-1.22,P=0.02;age,OR:1.08,95%CI:1.03-1.12,P<0.01).A model including these variables showed and area under the receiver operating characteristic curve(AUROC)of 0.75,which was not inferior to transient elastography or NAFLD fibrosis score AUROCs.We developed a nomogram including age and VF for assessing significant fibrosis in routine practice.CONCLUSION VF is a surrogate marker of liver fibrosis in patients with NAFLD.Bioelectrical impedance analysis is an inexpensive and simple method that can be combined with age to guide patient referral when other resources may be unavailable. | Marta Hernández-Conde Elba Llop Carlos Fernández Carrillo Beatriz Tormo Javier Abad Luis Rodriguez Christie Perelló Marta López Gomez JoséLuis Martínez-Porras Natalia Fernández Puga Maria Trapero-Marugan Enrique Fraga Carlos Ferre Aracil JoséLuis Calleja Panero | 2020 | World Journal of Gastroenterology2020，26，42： | 2 |
