| 3 | Increased hepcidin expression in colorectal carcinogenesis显示文摘AIM:To investigate whether the iron stores regulator hepcidin is implicated in colon cancer-associated anae- mia and whether it might have a role in colorectal car- cinogenesis. METHODS: Mass spectrometry (MALDI-TOF MS and SELDI-TOF MS) was employed to measure hepcidin in urine collected from 56 patients with colorectal cancer. Quantitative Real Time RT-PCR was utilised to determine hepcidin mRNA expression in colorectal cancer tissue. Hepcidin cellular localisation was determined using im- munohistochemistry. RESULTS: We demonstrate that whilst urinary hepcidin expression was not correlated with anaemia it was posi- tively associated with increasing T-stage of colorectal cancer (P < 0.05). Furthermore, we report that hepcidin mRNA is expressed in 34% of colorectal cancer tissue specimens and was correlated with ferroportin repres- sion. This was supported by hepcidin immunoreactivity in colorectal cancer tissue. CONCLUSION: We demonstrate that systemic hepcidin expression is unlikely to be the cause of the systemic anaemia associated with colorectal cancer. | 5 | Chemoprotective effects of curcumin in esophageal epithelial cells exposed to bile acids显示文摘AIM:To investigate the ability of curcumin to counteract the impact of bile acids on gene expression of esophageal epithelial cells.METHODS:An esophageal epithelial cell line(HET1A)was treated with curcumin in the presence of deoxycholic acid.Cell proliferation and viability assays were used to establish an appropriate dose range for curcumin.The combined and individual effects of curcumin and bile acid on cyclooxygenase-2(COX-2)and superoxide dismutase(SOD-1 and SOD-2)gene expression were also assessed.RESULTS:Curcumin in a dose range of 10-100μmol/L displayed minimal inhibition of HET-1A cell viability.Deoxycholic acid at a concentration of 200μmol/L caused a 2.4-fold increase in COX-2 gene expression compared to vehicle control.The increased expression of COX-2 induced by deoxycholic acid was partially reversed by the addition of curcumin,and curcumin reduced COX-2 expression 3.3-to 1.3-fold.HET-1A cells exposed to bile acid yielded reduced expression of SOD-1 and SOD-2 genes with the exception that high dose deoxycholic acid at 200μmol/L led to a 3-fold increase in SOD-2 expression.The addition of curcumin treatment partially reversed the bile acid-induced reduction in SOD-1 expression at all concentrations of curcumin tested.CONCLUSION:Curcumin reverses bile acid suppression of gene expression of SOD-1.Curcumin is also able to inhibit bile acid induction of COX-2 gene expression. | Matthew R Bower Harini S Aiyer Robert CG Martin Yan Li | 2010 | World Journal of Gastroenterology2010,16,33: | 10 |
| 6 | High-flow nasal oxygen availability for sedation decreases the use of general anesthesia during endoscopic retrograde cholangiopancreatography and endoscopic ultrasound显示文摘AIM To examine whether high-flow nasal oxygen(HFNO) availability influences the use of general anesthesia(GA) in patients undergoing endoscopic retrograde cholangiopancreatography(ERCP) and endoscopic ultrasound(EUS) and associated outcomes.METHODS In this retrospective study, patients were stratified into 3 eras between October 1, 2013 and June 30, 2014 based on HFNO availability for deep sedation at the time of their endoscopy. During the first and last 3-mo eras(era 1 and 3), no HFNO was available, whereas it was an option during the second 3-mo era(era 2). The primary outcome was the percent utilization of GA vs deep sedation in each period. Secondary outcomes included oxygen saturation nadir during sedation between periods, as well as procedure duration, and anesthesia-only time between periods and for GA vs sedation cases respectively.RESULTS During the study period 238 ERCP or EUS cases were identified for analysis. Statistical testing was employed and a P < 0.050 was significant unless the Bonferroni correction for multiple comparisons was used. General anesthesia use was significantly lower in era 2 compared to era 1 with the same trend between era 2 and 3(P = 0.012 and 0.045 respectively). The oxygen saturation nadir during sedation was significantly higher in era 2 compared to era 3(P < 0.001) but not between eras 1 and 2(P = 0.028) or 1 and 3(P = 0.069). | 8 | Radiofrequency ablation for early oesophageal squamous neoplasia:Outcomes form United Kingdom registry显示文摘AIM:To report outcomes on patients undergoing radiofrequency ablation(RFA)for early oesophageal squamous neoplasia from a National Registry.METHODS:A Prospective cohort study from 8 tertiary referral centres in the United Kingdom.Patients with squamous high grade dysplasia(HGD)and early squamous cell carcinoma(ESCC)confined to the mucosa were treated.Visible lesions were removed by endoscopic mucosal resection(EMR)before RFA.Following initial RFA treatment,patients were followed up 3monthly.Residual flat dysplasia was treated with RFA until complete reversal dysplasia(CR-D)was achieved or progression to invasive Squamous cell cancer defined as infiltration into the submucosa layer or beyond.The main outcome measures were CR-D at 12 mo from start of treatment,long term durability,progression to cancer and adverse events.RESULTS:Twenty patients with squamous HGD/ESCC completed treatment protocol.Five patients(25%)had EMR before starting RFA treatment.CR-D was 50%at12 mo with a median of 1 RFA treatment,mean 1.5(range 1-3).Two further patients achieved CR-D with repeat RFA after this time.Eighty per cent with CR-D remain dysplasia free at latest biopsy,with median follow up 24 mo(IQR 17-54).Six of 20 patients(30%)progressed to invasive cancer at 1 year.Four patients(20%)required endoscopic dilatations for symptomatic structuring after treatment.Two of these patients have required serial dilatations thereafter for symptomatic dysphagia with a median of 4 dilatations per patient.The other 2 patients required only a single dilatation to achieve an adequate symptomatic response.One patient developed cancer during follow up after end of treatment protocol.CONCLUSION:The role of RFA in these patients re-mains unclear.In our series 50%patients responded at12 mo.These figures are lower than limited published data. | Rehan J Haidry Mohammed A Butt Jason Dunn Matthew Banks Abhinav Gupta Howard Smart Pradeep Bhandari Lesley Ann Smith Robert Willert Grant Fullarton Morris John Massimo Di Pietro Ian Penman Marco Novelli Laurence B Lovat | 2013 | World Journal of Gastroenterology2013,19,36: | 7 |
| 9 | MicroRNAs and liver cancer associated with iron overload:Therapeutic targets unravelled显示文摘Primary liver cancer is a global disease that is on the increase.Hepatocellular carcinoma(HCC)accounts for most primary liver cancers and has a notably low survival rate,largely attributable to late diagnosis,resistance to treatment,tumour recurrence and metastasis.MicroRNAs(miRNAs/miRs)are regulatory RNAs that modulate protein synthesis.miRNAs are involved in several biological and pathological processes including the development and progression of HCC.Given the poor outcomes with current HCC treatments,miRNAs represent an important new target for therapeutic intervention.Several studies have demonstrated their role in HCC development and progression.While many risk factors underlie the development of HCC,one process commonly altered is iron homeostasis.Iron overload occurs in several liver diseases associated with the development of HCC including Hepatitis C infection and the importance of miRNAs in iron homeostasis and hepatic iron overload is well characterised.Aberrant miRNA expression in hepatic fibrosis and injury response have been reported,as have dysregulated miRNA expression patterns affecting cell cycle progression,evasion of apoptosis,invasion and metastasis.In2009,miR-26a delivery was shown to prevent HCC progression,highlighting its therapeutic potential.Several studies have since investigated the clinical potential of other miRNAs with one drug,Miravirsen,currently in phaseⅡclinical trials.miRNAs also have potential as biomarkers for the diagnosis of HCC and to evaluate treatment efficacy.Ongoing studies and clinical trials suggest miRNA-based treatments and diagnostic methods will have novel clinical applications for HCC in the coming years,yielding improved HCC survival rates and patient outcomes. | Catherine M Greene Robert B Varley Matthew W Lawless | 2013 | World Journal of Gastroenterology2013,19,32: | 5 |
| 10 | RECENT ADVANCES IN RESEARCH AND FORECASTING OF TROPICAL CYCLONE TRACK, INTENSITY, AND STRUCTURE AT LANDFALL显示文摘This review prepared for the fourth International Workshop on Tropical Cyclone Landfall Processes(IWTCLP-4) summarizes the most recent(2015-2017) theoretical and practical knowledge in the field of tropical cyclone(TC) track, intensity, and structure rapid changes at or near landfall. Although the focus of IWTCLPIV was on landfall, this summary necessarily embraces the characteristics of storms during their course over the ocean prior to and leading up to landfall. In the past few years, extremely valuable observational datasets have been collected for TC forecasting guidance and research studies using both aircraft reconnaissance and new geostationary or low-earth orbiting satellites at high temporal and spatial resolution. Track deflections for systems near complex topography such as that of Taiwan and La Réunion have been further investigated, and advanced numerical models with high spatial resolution necessary to predict the interaction of the TC circulation with steep island topography have been developed. An analog technique has been designed to meet the need for longer range landfall intensity forecast guidance that will provide more time for emergency preparedness. Probabilistic track and intensity forecasts have also been developed to better communicate on forecast uncertainty. Operational practices of several TC forecast centers are described herein and some challenges regarding forecasts and warnings for TCs making landfall are identified. | 13 | Use of the Endocuff during routine colonoscopy examination improves adenoma detection: A meta-analysis显示文摘AIM To perform meta-analysis of the use of Endocuff during average risk screening colonoscopy.METHODS Scopus, Cochrane databases, MEDLINE/Pub Med, and CINAHL were searched in April 2016. Abstracts from Digestive Disease Week, United European Gastroenterology, and the American College of Gastroenterology meeting were also searched from 2004-2015. Studies comparing EC-assisted colonoscopy(EAC) to standard colonoscopy, for any indication, were included in the analysis. The analysis was conducted by using the Mantel-Haenszel or DerS imonian and Laird models with the odds ratio(OR) to assess adenoma detection, cecal intubation rate, and complications performed. RESULTS Nine studies(n = 5624 patients) were included in the analysis. Compared to standard colonoscopy, procedures performed with EC had higher frequencies for adenoma(OR = 1.49, 95%CI: 1.23-1.80; P = 0.03), and sessile serrated adenomas detection(OR = 2.34 95%CI: 1.63-3.36; P < 0.001). There was no significant difference in cecal intubation rates between the EACgroup and standard colonoscopy(OR = 1.26, 95%CI: 0.70-2.27, I2 = 0%; P = 0.44). | 16 | Distal radius volar rim plate: Technical and radiographic considerations显示文摘AIM To determine technical considerations and radiographic outcomes of the Synthes volar rim distal radius plate to treat complex intra-articular fractures. METHODS This review highlights technical considerations learnt using this implant since it was introduced in a major trauma unit in November 2011, including anatomical reduction and whether this was maintained radiographically. RESULTS Twenty-six of the 382 internally fixed distal radial fractures at our unit(6.8%) were deemed to require this plate in order to achieve optimal fracture fixation between November 2011 and May 2014. A further dorsal and/or radial plate was necessary in 35% and variable angle screws were used in 54% of cases. Postoperatively, mean radial height, inclination, volar tilt and ulnar variance restored were 11.7 mm, 21o, 4.3o and-1.2 mm respectively. There were no cases of non-union or flexor/extensor tendon rupture; one case of loss of fracture reduction. Overall incidence of plate removal was 15% with one plate removed for flexor and one for extensor tendon irritation CONCLUSION The use of a rim plate enables control of challenging fardistal fracture patterns. However, additional plates were required to improve and maintain reduction. Variable angle screws were necessary in half the cases to avoid intra-articular screw penetration. | 17 | Efficacy of selective laser trabeculoplasty following incisional glaucoma surgery显示文摘AIM: To evaluate the efficacy of selective laser trabeculoplasty(SLT) in glaucomatous eyes with previous incisional glaucoma surgery. METHODS: A retrospective cohort of eyes that underwent SLT at a single institution from 2013-2015 were followed for 1 y. Reduction in intraocular pressure(IOP) following SLT was evaluated in eyes with prior trabeculectomy with Ex Press mini shunt(Alcon, Ft Worth, TX, USA), Ahmed valve(New World Medical, Cucamonga, CA, USA), or combined phacoemulsification-trabeculectomy. A control group was included with eyes without prior surgery that underwent SLT. Success was defined as >20% drop in IOP from pre-SLT baseline. RESULTS: One-hundred and six eyes were included with 53 in both the prior glaucoma surgery(PGS) and no prior glaucoma surgery(NPGS) groups. Mean pre-SLT IOP was 19.2±4.3 and 20.6±6.0 mm Hg for PGS and NPGS groups, respectively(P=0.17). Both groups produced statistically significant IOP reductions at 1 and 6 mo(P<0.04). At 6 mo, mean IOP reduction reached 7.3% and 10.8% for the PGS and NPGS groups, respectively(P=0.42). Overall, 27.9% and 31.7% of eyes in PGS and NPGS groups met success criteria at 1 y(P=0.70). In the PGS group, eyes with baseline IOP ≥21 mm Hg had IOP reductions of 18.1%(P<0.001), 16.7%(P<0.01), and 8.4%(P=0.31) compared to eyes with baseline IOP <21 mm Hg who had IOP reductions of 2.3%(P=0.39), 3.4%(P=0.19), and 1.1%(P=0.72) at 1, 6 mo, and 1 y, respectively. CONCLUSION: SLT is efficacious in eyes with prior incisional glaucoma surgery and results in similar IOP reductions compared to eyes without PGS. | 18 | Mitigation of atrazine,S-metolachlor,and diazinon using common emergent aquatic vegetation显示文摘By the year 2050, the population of the United States is expected to reach over 418 million,while the global population will reach 9.6 billion. To provide safe food and fiber, agriculture must balance pesticide usage against impacts on natural resources. Challenges arise when storms cause runoff to be transported to aquatic receiving systems. Vegetated systems such as drainage ditches and constructed wetlands have been proposed as management practices to alleviate pesticide runoff. Twelve experimental mesocosms(1.3 × 0.71 × 0.61 m) were filled with sediment and planted with a monoculture of one of three wetland plant species(Typha latifolia, Leersia oryzoides, and Sparganium americanum).Three mesocosms remained unvegetated to serve as controls. All mesocosms were amended with 9.2 ± 0.8 μg/L, 12 ± 0.4 μg/L, and 3.1 ± 0.2 μg/L of atrazine, metolachlor, and diazinon, respectively, over a 4 hr hydraulic retention time to simulate storm runoff.Following the 4 hr amendment, non-amended water was flushed through mesocosms for an additional 4 hr. Outflow water samples were taken hourly from pre-amendment through8 hr, and again at 12, 24, 48, 72, and 168 hr post-amendment. L. oryzoides and T. latifolia had mean atrazine, metolachlor, and diazinon retentions from 51%–55% for the first 4 hr of the experiment. Aside from S. americanum and atrazine(25% retention), unvegetated controls had the lowest pesticide retention(17%–28%) of all compared mesocosms.
