| 1 | Alternatives for large-scale production of cultured beef: A review显示文摘Cultured beef is a method where stem cells from skeletal muscle of cows are cultured in vitro to gain edible muscle tissue. For large-scale production of cultured beef, the culture technique needs to become more efficient than today's 2-dimensional(2D) standard technique that was used to make the first cultured hamburger. Options for efficient large-scale production of stem cells are to culture cells on microcarriers, either in suspension or in a packed bed bioreactor, or to culture aggregated cells in suspension. We discuss the pros and cons of these systems as well as the possibilities to use the systems for tissue culture. Either of the production systems needs to be optimized to achieve an efficient production of cultured beef. | 3 | Socio-cultural determinants of timely and delayed treatment of Buruli ulcer: implications for disease control显示文摘Introduction:Public health programmes recommend timely medical treatment for Buruli ulcer(BU)infection to prevent pre-ulcer conditions from progressing to ulcers,to minimise surgery,disabilities and the socio-economic impact of BU.Clarifying the role of socio-cultural determinants of timely medical treatment may assist in guiding public health programmes to improve treatment outcomes.This study clarified the role of socio-cultural determinants and health system factors affecting timely medical treatment for BU in an endemic area in Ghana.Methods:A semi-structured explanatory model interview based on the explanatory model interview catalogue(EMIC)was administered to 178 BU-affected persons.Based on research evidence,respondents were classified as timely treatment(use of medical treatment 3 months from awareness of disease)and delayed treatment(medical treatment 3 months after onset of disease and failure to use medical treatment).The outcome variable,timely treatment was analysed with cultural epidemiological variables for categories of distress,perceived causes of BU,outside-help and reasons for medical treatment in logistic regression models.The median time for the onset of symptoms to treatment was computed in days.Qualitative phenomenological analysis of respondents’narratives clarified the meaning,context and dynamic features of the relationship of explanatory variables with timely medical treatment.Results:The median time for initiating treatment was 25 days for pre-ulcers,and 204 days for ulcers.Income loss and use of herbalists showed significantly negative associations with timely treatment.Respondents’use of herbalists was often motivated by the desire for quick recovery in order to continue with work and because herbalists were relatives and easily accessible.However,drinking unclean water was significantly associated with timely treatment and access to health services encouraged timely treatment(OR 8.5,p=0.012).Findings show that health system factors of access are responsible for non-compliance to treatment regimes.Conclusions:Findings highlight the importance of an integrated approach to BU control and management considering the social and economic features that influence delayed treatment and factors that encourage timely medical treatment.This approach should consider periodic screening for early case-detection,collaboration with private practitioners and traditional healers,use of mobile services to improve access,adherence and treatment outcomes. | Mercy M Ackumey Margaret Gyapong Matilda Pappoe Cynthia Kwakye Maclean Mitchell G Weiss | 2012 | Infectious Diseases of Poverty2012,1,1: | 3 |
| 4 | Role of dipeptidyl peptidase 4 inhibitors in the new era of antidiabetic treatment显示文摘The last few years important changes have occurred in the field of diabetes treatment.The priority in the therapy of patients with diabetes is not glycemic control per se rather an overall management of risk factors,while individualization of glycemic target is suggested.Furthermore,regulatory authorities now require evidence of cardiovascular(CV)safety in order to approve new antidiabetic agents.The most novel drug classes,i.e.,sodium-glucose transporter 2 inhibitors(SGLT2-i)and some glucagon-like peptide-1 receptor agonists(GLP-1 RA),have been demonstrated to reduce major adverse CV events and,thus,have a prominent position in the therapeutic algorithm of hyperglycemia.In this context,the role of previously used hypoglycemic agents,including dipeptidyl peptidase 4(DPP-4)inhibitors,has been modified.DPP-4 inhibitors have a favorable safety profile,do not cause hypoglycemia or weight gain and do not require dose uptitration.Furthermore,they can be administered in patients with chronic kidney disease after dose modification and elderly patients with diabetes.Still,though,they have been undermined to a third line therapeutic choice as they have not been shown to reduce CV events as is the case with SGLT2-i and GLP-1 RA.Overall,DPP-4 inhibitors appear to have a place in the management of patients with diabetes as a safe class of oral glucose lowering agents with great experience in their use. | Matilda Florentin Michael S Kostapanos Athanasia K Papazafiropoulou | 2022 | World Journal of Diabetes2022,13,2: | 3 |
| 5 | Colonoscopy preparation-induced disorders in renal function and electrolytes显示文摘Colonoscopy and flexible sigmoidoscopy are commonly used mainly for colon cancer screening and detection, but also in several other situations such as inflammatory bowel disease (for diagnosis and follow up) and gastrointestinal hemorrhage. Bowel cleansing preparations mainly include polyethylene glycol and oral sodium phosphate solutions, with the later being most frequently used due to better toleration from patients. Despite their favourable safety profile these agents have been associated with renal function deterioration and electrolyte disorders, some of which were serious or even fatal.
