Imaging techniques available include contrast enhanced ultrasound, computed tomography and magnetic resonance imaging. This review discusses the application of several imaging techniques in the diagnosis and staging of both hepatocellular carcinoma and cholangiocarcinoma and outlines certain characteristics of benign liver lesions. | 11 | Stress and sleep quality in doctors working on-call shifts are associated with functional gastrointestinal disorders显示文摘AIM To investigate the role of sleep quality and psychosocial problems as predictors of functional gastrointestinal disorders(FGIDs) in doctors that work 24 hour-on-call shifts.METHODS In this cross-sectional observation study, using the Rome Ⅲ Questionnaire and Pittsburgh Sleep Quality Index(PSQI), we analyzed 170 doctors with 24 houron-call shifts. RESULTS Among the participants that had experienced a 24 hour-on-call shift within the last 6 mo, 48(28.2%) had FGIDs. Overall prevalence of irritable bowel syndrome(IBS) and functional dyspepsia(FD) were 16.5% and 17.1%, respectively, with 5.3% exhibiting both. Sleep scores(PSQI)(8.79 ± 2.71 vs 7.30 ± 3.43, P = 0.008), the presence of serious psychosocial alarm(83.3%vs 56.6%, P = 0.004), and the proportion of doctors who experienced over two months of recent on-call work(81.2% vs 68.9%, P = 0.044) were significantly different between individuals with or without FGIDs. Multivariate analysis revealed that presenting serious psychosocial alarm was an independent risk factor for prevalence of FD(OR = 5.47, 95%CI: 1.06-28.15, P = 0.042) and poor sleep quality(PSQI ≥ 6) was a predictor of IBS(OR = 4.17, 95%CI: 1.92-19.02, P = 0.016). | 15 | Cancer-associated fibroblast-derived secreted phosphoprotein 1 contributes to resistance of hepatocellular carcinoma to sorafenib and lenvatinib显示文摘Background:Cancer-associated fibroblasts(CAFs)play an important role in the induction of chemo-resistance.This study aimed to clarify the mechanism underlying CAF-mediated resistance to two tyrosine kinase inhibitors(TKIs),sorafenib and lenvatinib,and to identify a novel therapeutic target for overcoming TKI resistance in hepatocellular carcinoma(HCC).Methods:We performed a systematic integrative analysis of publicly available gene expression datasets and whole-transcriptome sequencing data from 9 pairs of CAFs and para-cancer fibroblasts isolated from human HCC and para-tumor tissues,respectively,to identify key molecules that might induce resistance to TKIs.We then performed in vitro and in vivo experiments to validate selected targets and related mechanisms.The associations of plasma secreted phosphoprotein 1(SPP1)expression levels before sorafenib/lenvatinib treatment with progression-free survival(PFS)and overall survival(OS)of 54 patients with advanced HCC were evaluated using Kaplan-Meier and Cox regression analysis.Results:Bioinformatic analysis identified CAF-derived SPP1 as a candidate molecule driving TKI resistance.SPP1 inhibitors reversed CAF-induced TKI resistance in vitro and in vivo.CAF-derived SPP1 activated rapidly accelerated fibrosarcoma(RAF)/mitogen-activated protein kinase(MAPK)and phosphatidylinositol 3-kinase(PI3K)/protein kinase B(AKT)/mammalian target of rapamycin(mTOR)through the integrin-protein kinase C-alpha(PKCα)signaling pathway and promoted epithelial-to-mesenchymal transition(EMT).A high plasma SPP1 level before TKI treatment was identified as an independent predictor of poor PFS(P=0.026)and OS(P=0.047)in patients with advanced HCC after TKI treatment.Conclusions:CAF-derived SPP1 enhances TKI resistance in HCC via bypass activation of oncogenic signals and EMT promotion.Its inhibition represents a promising therapeutic strategy against TKI resistance inHCC.Moreover,plasma SPP1 level before TKI treatment represents a potential biomarker for treatment response prediction. | JungWoo Eun Jung Hwan Yoon Hye Ri Ahn Seokhwi Kim Young Bae Kim Su Bin Lim Won Park TaeWook Kang Geum Ok Baek Moon Gyeong Yoon Ju A Son Ji HyangWeon Soon Sun Kim Hyo Jung Cho Jae Youn Cheong | 2023 | Cancer Communications2023，43，4： | 2 |

| 18 | Metabolic positron emission tomography imaging of cancer:Pairing lipid metabolism with glycolysis显示文摘The limitations of fluorine-18 fluorodeoxy-D-glucose(FDG)in detecting some cancers has prompted a longstandin search for other positron emission tomography(PET tracers to complement the imaging of glycolysis i oncology, with much attention paid to lipogenesis base on observations that the production of various lipid an lipid-containing compounds is increased in most cancers Radiolabeled analogs of choline and acetate have now been used as oncologic PET probes for over a decade showing convincingly improved detection sensitivit over FDG for certain cancers. However, neither cholin nor acetate have been thoroughly validated as lipogeni biomarkers, and while acetyl-Co A produced from acetat is used in de-novo lipogenesis to synthesize fatty acids acetate is also consumed by various other syntheti and metabolic pathways, with recent experimenta observations challenging the assumption that lipogenesi is its predominant role in all cancers. Since tumor detected by acetate PET are also frequently detected b choline PET, imaging of choline metabolism might serv as an alternative albeit indirect marker of lipogenesis particularly if the fatty acids produced in cancer cell are mainly destined for membrane synthesis throug incorporation into phosphatidylcholines. Aerobic glycolysi may or may not coincide with changes in lipid metabolism resulting in combinatorial metabolic phenotypes that ma have different prognostic or therapeutic implications Consequently, PET imaging using dual metabolic tracers in addition to being diagnostically superior to imagin with individual tracers, could eventually play a greate role in supporting precision medicine, as efforts t develop small-molecule metabolic pathway inhibitors ar coming to fruition.
