1A study of the efficacy of bacterial biofilm cleanout for gastrointestinal endoscopes显示文摘AIM:To compare the influence and clearance effect of enzymatic and non-enzymatic detergents against Escherichia coli (E. coli) biofilm on the inner surface of gastroscopes.METHODS:Teflon tubes were incubated in a mixture of different detergents and E. coli culture (106 CFU/mL) for 72 h at 15℃,and biofilms on the inner surface of the teflon tubes were analyzed by bacterial count and scanning electron microscopy. To evaluate the clear-ance effect of detergents,after biofilms were formed on the inner surface of Teflon tubes by 72 h lavage with E. coli culture,tubes were lavaged by enzymatic and non-enzymatic detergents at a speed of 250 mL/min,then biofilms on the inner surface were analyzed by bacterial count and scanning electron microscopy.RESULTS:Non-enzymatic detergent had a better inhi-bition function on biofilm formation than enzymatic de-tergent as it reduced bacterial burden by 2.4 log compared with the control samples (P = 0.00). Inhibition function of enzymatic detergent was not significantly different to that of control samples and reduced bac-terial burden by 0.2 log on average (P > 0.05). After lavaging at 250 mL/min for 3 min,no living bacteria were left in the tubes. Scanning electron microscopy observation showed biofi lms became very loose by the high shear force effect. CONCLUSION:Non-enzymatic detergent has a better inhibition effect on biofilm formation at room temperature. 4Repurposing vitamin D for treatment of human malignancies via targeting tumor microenvironment显示文摘Tumor cells along with a small proportion of cancer stem cells exist in a stromal microenvironment consisting of vasculature, cancer-associated fibroblasts, immune cells and extracellular components.Recent epidemiological and clinical studies strongly support that vitamin D supplementation is associated with reduced cancer risk and favorable prognosis. Experimental results suggest that vitamin D not only suppresses cancer cells, but also regulates tumor microenvironment to facilitate tumor repression. In this review, we have outlined the current knowledge on epidemiological studies and clinical trials of vitamin D. Notably, wesummarized and discussed the anticancer action of vitamin D in cancer cells, cancer stem cells and stroma cells in tumor microenvironment, providing a better understanding of the role of vitamin D in cancer. 5QTL editing confers opposing yield performance in different rice varieties显示文摘Grain yield is one of the most important and complex trait for genetic improvement in crops; it is known to be controlled by a number of genes known as quantitative trait loci(QTLs). In the past decade, many yield-contributing QTLs have been identified in crops.However, it remains unclear whether those QTLs confer the same yield performance in different genetic backgrounds. 6Traditional Chinese medicinal herbs combined with epidermal growth factor receptor tyrosine kinase inhibitor for advanced non-small cell lung cancer: a systematic review and meta-analysis显示文摘BACKGROUND: Epidermal growth factor receptor tyrosine kinase inhibitor(EGFR-TKI) targeted treatment has been a standard therapy for advanced non-small cell lung cancer(NSCLC), but it is not tolerated well by all patients. In China, some studies have reported that traditional Chinese medicinal herbs(TCMHs) may increase effi cacy and reduce toxicity when combined with EGFR-TKI, but outside of China few studies of this kind have been attempted. OBJECTIVE: This study is intended to systematically review the existing clinical evidence on TCMHs combined with EGFR-TKI for treatment of advanced NSCLC. SEARCH STRATEGY: PubMed, the Cochrane Library, the Excerpta Medica Database(EMBASE), the China BioMedical Literature(CBM), and the China National Knowledge Infrastructure(CNKI) and web site of the American Society of Clinical Oncology(ASCO), the European Society for Medical Oncology(ESMO), the World Conference of Lung Cancer(WCLC) were searched; the search included all documents published in English or Chinese before October 2013. INCLUSION CRITERIA: We selected randomized controlled trials based on specifi c criteria, the most important of which was that a TCMH plus EGFR-TKI treatment group was compared with an EGFR-TKI control group in patients with advanced NSCLC. DATA EXTRACTION AND ANALYSIS: The modifi ed Jadad scale was used to assess the quality of studies. For each included study, patient characteristics, treatment details, therapeutic approach and clinical outcomes were collected on a standardized form. When disagreements on study inclusion or data extracted from a study emerged, the consensus of all coauthors provided the resolution. The clinical outcome metrics consisted of objective response rate(ORR; complete response + partial response divided by the total number of patients), disease control rate(DCR; complete response + partial response + no change divided by the total number of patients), survival rate, improved or stabilized Karnofsky performance status(KPS), and severe toxicity. RevMan 5.0 software was used for data syntheses and analyses. Risk ratio(RR) and 95% confidence interval(CI) were calculated; if the hypothesis of homogeneity was not rejected(P>0.1, I2<50%), the fi xed-effect model was used to calculate the summary RR and the 95% CI. Otherwise, a random-effect model was used.RESULTS: In this review, 19 studies were included based on the selection criteria. Of them, 13studies were of high quality and 6 studies were of low quality, according to the modifi ed Jadad scale. When the TCMH plus EGFR-TKI treatment groups were compared with the EGFR-TKI control groups the meta-analysis demonstrated a statistically signifi cant higher ORR(RR 1.34; 95% CI 1.15 to 1.57; P=0.000 2), DCR(RR 1.18; 95% CI 1.09 to 1.27; P<0.000 1), one-year survival rate(RR 1.21; 95% CI 1.01 to 1.44; P=0.04), 2-year survival rate(RR 1.91; 95% CI 1.26 to 2.89; P=0.002) and improved or stable KPS(RR 1.38; 95% CI 1.26 to 1.51; P<0.000 01). Severe toxicity for rash was decreased(RR 0.55; 95% CI 0.32 to 0.94; P=0.03), as were nausea and vomiting(RR 0.17; 95% CI 0.04 to 0.72; P=0.02) and diarrhea(RR 0.46; 95% CI 0.24 to 0.89; P=0.02). Sensitivity analysis indicated that fi ndings of the meta-analysis were robust to study quality. 8Histopathological classification and follow-up analysis of chronic atrophic gastritis显示文摘BACKGROUND The pathological diagnosis and follow-up analysis of gastric mucosal biopsy have been paid much attention,and some scholars have proposed the pathological diagnosis of 12 kinds of lesions and accompanying pathological diagnosis,which is of great significance for the treatment of precision gastric diseases,the improvement of the early diagnosis rate of gastric cancer,and the reduction of missed diagnosis rate and misdiagnosis rate.AIM To perform a histopathological classification and follow-up analysis of chronic atrophic gastritis(CAG).METHODS A total of 2248 CAG tissue samples were collected,and data of their clinical characteristics were also gathered.Based on these samples,the expression levels of Mucin 1(MUC1),MUC2,MUC5AC,and MUC6 in CAG tissue were tested by immunohistochemical assay.Moreover,we followed these patients for up to four years.The difference between different stages of gastroscopic biopsy was observed.RESULTS Through observation,it is believed that CAG should be divided into four types,simple type,hyperplasia type,intestinal metaplasia(IM)type,and intraepithelial neoplasia(IEN)type.Simple CAG accounted for 9.1%(205/2248),which was more common in elderly people over 60 years old.The main change was that the lamina propria glands were reduced in size and number.Hyperplastic CAG accounted for 29.1%(654/2248),mostly occurring between 40 and 60 years old.The main change was that the lamina propria glands were atrophy accompanied by glandular hyperplasia and slight expansion of the glands.IM CAG accounted for 50.4%(1132/2248),most of which increased with age,and were more common in those over 50 years.The atrophy of the lamina propria glands was accompanied by significant IM,and the mucus containing sialic acid or sulfate was distinguished according to the nature of the mucus.The IEN type CAG accounted for 11.4%(257/2248),which developed from the previous types,with severe gland atrophy and reduced mucus secretion,and is an important precancerous lesion.CONCLUSION The histological typing of CAG is convenient to understand the property of lesion,determine the follow-up time,and guide the clinical treatment.Yang-Kun Wang Lan Shen Tian Yun Bin-Feng Yang Chao-Ya Zhu Su-Nan Wang 2021World Journal of Clinical Cases2021,9,16:15
9Impact of mechanical circulatory support and immunomodulation therapy on outcome of patients with fulminant myocarditis: Chinese registry of fulminant myocarditis显示文摘Dear Editor,Patients presenting with acute myocarditis and sudden hemodynamic instability (termed fulminant myocarditis [FM]) still have a high mortality and need for heart transplantation, up to 28% at 60 days.1,2,3 Recent scientific statements and expert opinion consensus suggests early use of temporary mechanical circulatory supports (t-MCS).3,4 Specifically, Chinese scientific statement proposed an extensive use of t-MCS combined with immunoregulatory therapy (IT),4 although formal trials are lacking. We present a multicenter, retrospective study to compare the outcome of patients who were treated with t-MCS and IT vs. patients who didn’t receive these treatments. We included patients with the diagnosis of FM based on the presence of viral prodromal signs/symptoms followed by acute onset of severe heart failure (HF) without other relevant differential diagnosis or pre-existing cardiac disorders. 11Ionic liquid based dispersive liquid-liquid microextraction of aromatic amines in water samples显示文摘In this work,a new microextraction method termed ionic liquid based dispersive liquid-liquid microextraction(IL-DLLME) was demonstrated for the extraction of 2-methylaniline,4-chloroaniline,1-naphthylamine and 4-aminobiphenyl in aqueous matrices.After extraction the ionic liquid(IL)phase was injected directly into the high performance liquid chromatography(HPLC) system for determination.Some parameters that might affect the extraction efficiency were optimized.Under the optimum conditions,good linear relationship,sensitivity and reproducibility were obtained.The limits of detection(LOD,S/N=3)for the four analytes were in the range of 0.45-2.6μg L^(-1).The relative standard deviations(R.S.D.,n=6)were in the range of 6.2-9.8%. 12Trabecular-like Ti-6Al-4V scaffolds for orthopedic: fabrication by selective laser melting and in vitro biocompatibility显示文摘Porous metal scaffolds play an important role in the orthopedic field, due to their wide applications in prostheses implantation. Some previous studies showed that the scaffolds with trabecular bone structure reconstructed via computed tomography had satisfactory biocompatibility. However, the reverse modeling scaffolds were inflexible for customized design. Therefore, a top-down designing biomimetic bone scaffold with favorable mechanical performances and cytocompatibility is urgently demanded for orthopedic implants. An emerging additive manufacturing technique, selective laser melting, was employed to fabricate the trabecular-like porous Ti-6Al-4 V scaffolds with varying irregularities(0.05-0.5) and porosities(48.83%–74.28%) designed through a novel Voronoi-Tessellation based method. Micro-computed tomography and scanning electron microscopy were used to characterize the scaffolds’ morphology.Quasi-static compression tests were performed to evaluate the scaffolds’ mechanical properties. The MG63 cells culture in vitro experiments, including adhesion, proliferation, and differentiation, were conducted to study the cytocompatibility of scaffolds. Compressive tests of scaffolds revealed an apparent elastic modulus range of 1.93–5.24 GPa and an ultimate strength ranging within 44.9–237.5 MPa, which were influenced by irregularity and porosity, and improved by heat treatment. Furthermore, the in vitro assay suggested that the original surface of the SLM-fabricated scaffolds was favorable for osteoblasts adhesion and migration because of micro scale pores and ravines. The trabecular-like porous scaffolds with full irregularity and higher porosity exhibited enhanced cells proliferation and osteoblast differentiation at earlier time, due to their preferable combination of small and large pores with various shapes. 14Routine exposure of recurrent laryngeal nerve in thyroid surgery can prevent nerve injury显示文摘To determine the value of dissecting the recurrent laryngeal nerve during thyroid surgery with respect to preventing recurrent laryngeal nerve injury, we retrospectively analyzed clinical data from 5 344 patients undergoing thyroidectomy. Among these cases, 548 underwent dissection of the recurrent laryngeal nerve, while 4 796 did not. There were 12 cases of recurrent laryngeal nerve injury following recurrent laryngeal nerve dissection (injury rate of 2.2%) and 512 cases of recurrent laryngeal nerve injury in those not undergoing nerve dissection (injury rate of 10.7%). This difference remained statistically significant between the two groups in terms of type of thyroid disease, type of surgery, and number of surgeries. Among the 548 cases undergoing recurrent laryngeal nerve dissection, 128 developed anatomical variations of the recurrent laryngeal nerve (incidence rate of 23.4%), but no recurrent laryngeal nerve injury was found. In addition, the incidence of recurrent laryngeal nerve injury was significantly lower in patients with the inferior parathyroid gland and middle thyroid veins used as landmarks for locating the recurrent laryngeal nerve compared with those with the entry of the recurrent laryngeal nerve into the larynx as a landmark. 18Rapid generation of genetic diversity by multiplex CRISPR/Cas9 genome editing in rice显示文摘The clustered regularly interspaced short palindromic repeats(CRISPR)-associated endonuclease 9(CRISPR/Cas9) system has emerged as a promising technology for specific genome editing in many species. Here we constructed one vector targeting eight agronomic genes in rice using the CRISPR/Cas9 multiplex genome editing system. By subsequent genetic transformation and DNA sequencing, we found that the eight target genes have high mutation efficiencies in the T_0 generation. Both heterozygous and homozygous mutations of all editing genes were obtained in T_0 plants. In addition, homozygous sextuple, septuple, and octuple mutants were identified. As the abundant genotypes in T_0 transgenic plants, various phenotypes related to the editing genes were observed. 19ABNORMAL FLOWER AND GRAIN 1 encodes OsMADS6 and determines palea identity and affects rice grain yield and quality显示文摘The palea and lemma are floral organ structures unique to grasses;these structures form the hull and directly affect grain size and quality. However, the molecular mechanisms controlling the development of the hull are not well understood. In this study, we characterized the rice(Oryza sativa) abnormal flower and grain1(afg1) mutant, a new allele of OsMADS6. Similar to previously characterized osmads6 alleles, in the afg1 floret, the palea lost its marginal region and acquired the lemma identity. However, in contrast to other osmads6 alleles, the afg1 mutant showed altered grain size and grain quality, with decreased total starch and amylose contents, and increased protein and soluble sugar contents. The analysis of transcriptional activity suggested that AFG1 is a transcriptional activator and may affect grain size by regulating the expression levels of several genes related to cell expansion and proliferation in the afg1 mutant.
