| 7 | Clear mortality gap caused by graft macrosteatosis in Chinese patients after cadaveric liver transplantation显示文摘Background:Liver transplantation(LT)is one of the most effective surgical treatment for patients with end-stage liver disease.Steatosis is a contributor for inferior graft quality.But its impact and safety on transplantation was less assessed in Chinese patients.Methods:Graft steatosis and related information involved in recipients,donors and surgical procedures were retrospectively collected from 239 patients.Results:Donor macrosteatosis(MaS)caused about 2.14 and 2.80 folds of increment on patient and graft mortality.Dose-response analysis revealed prominent risk of grafts on overall patient/organ mortality when MaS content exceeded 10%(P<0.05).Noteworthy,deaths were only observed in MaS group when concurrent with extremely higher post-transplant alanine aminotransferase(ALT,64%).However,microsteatosis(MiS)grafts didn’t affect outcomes after LT.In a cohort of Chinese patients,MaS had comprehensive effects on post-transplant outcomes with relatively lower safety threshold at 10%.Mortality gap caused by MaS grafts was observed in patients with severer ischemia reperfusion injury.Conclusions:Our study revealled the graft MaS affected the post-transplant outcomes in lower risk cutoff in Chinese patients.Further study is worthy to validate these results and investigate inner mechanism under the phenomenon. | Zhengtao Liu Wenchao Wang Li Zhuang Jingfeng Liu Shuping Que Dan Zhu Linfang Dong Jian Yu Lin Zhou Shusen Zheng | 2020 | Hepatobiliary Surgery and Nutrition2020,9,6: | 2 |

| 9 | Risk stratification in symptomatic intracranial atherosclerotic disease with conventional vascular risk factors and cerebral haemodynamics显示文摘Background and purpose Symptomatic intracranial atherosclerotic stenosis(sICAS)is associated with a considerable risk of recurrent stroke despite contemporarily optimal medical treatment.Severity of luminal stenosis in sICAS and its haemodynamic significance quantified with computational fluid dynamics(CFD)models were associated with the risk of stroke recurrence.We aimed to develop and compare stroke risk prediction nomograms in sICAS,based on vascular risk factors and these metrics.Methods Patients with 50%-99%sICAS confirmed in CT angiography(CTA)were enrolled.Conventional vascular risk factors were collected.Severity of luminal stenosis in sICAS was dichotomised as moderate(50%-69%)and severe(70%-99%).Translesional pressure ratio(PR)and wall shear stress ratio(WSSR)were quantified via CTA-based CFD modelling;the haemodynamic status of sICAS was classified as normal(normal PR&WSSR),intermediate(otherwise)and abnormal(abnormal PR&WSSR).All patients received guideline-recommended medical treatment.We developed and compared performance of nomograms composed of these variables and independent predictors identified in multivariate logistic regression,in predicting the primary outcome,recurrent ischaemic stroke in the same territory(SIT)within 1 year.Results Among 245 sICAS patients,20(8.2%)had SIT.The D2H2A nomogram,incorporating diabetes,dyslipidaemia,haemodynamic status of sICAS,hypertension and age≥50 years,showed good calibration(P for Hosmer-Lemeshow test=0.560)and discrimination(C-statistic 0.73,95%CI 0.60 to 0.85).It also had better performance in risk reclassification and provided larger net benefits in decision curve analysis,compared with nomograms composed of conventional vascular risk factors only,and plus the severity of luminal stenosis in sICAS.Sensitivity analysis in patients with anterior-circulation sICAS showed similar results.Conclusions The D2H2A nomogram,incorporating conventional vascular risk factors and the haemodynamic significance of sICAS as assessed in CFD models,could be a useful tool to stratify sICAS patients for the risk of recurrent stroke under contemporarily optimal medical treatment. | Xuan Tian Hui Fang Linfang Lan Hing Lung Ip Jill Abrigo Haipeng Liu Lina Zheng Florence S Y Fan Sze Ho Ma Bonaventure Ip Bo Song Yuming Xu Jingwei Li Bing Zhang Yun Xu Yannie O Y Soo Vincent Mok Ka Sing Wong Thomas W Leung Xinyi Leng | 2023 | Stroke & Vascular Neurology2023,8,1: | 2 |
| 10 | The E3 ubiquitin ligase NEDD4-1 protects against acetaminophen-induced liver injury by targeting VDAC1 for degradation显示文摘Acetaminophen(APAP)overdose is a major cause of liver injury.Neural precursor cell expressed developmentally downregulated 4—1(NEDD4-1)is an E3 ubiquitin ligase that has been implicated in the pathogenesis of numerous liver diseases;however,its role in APAP-induced liver injury(AILI)is unclear.Thus,this study aimed to investigate the role of NEDD4-1 in the pathogenesis of AILI.We found that NEDD4-1 was dramatically downregulated in response to APAP treatment in mouse livers and isolated mouse hepatocytes.Hepatocyte-specific NEDD4-1 knockout exacerbated APAP-induced mitochondrial damage and the resultant hepatocyte necrosis and liver injury,while hepatocyte-specific NEDD4-1 overexpression mitigated these pathological events both in vivo and in vitro.Additionally,hepatocyte NEDD4-1 deficiency led to marked accumulation of voltage-dependent anion channel 1(VDAC1)and increased VDAC1 oligomerization.Furthermore,VDAC1 knockdown alleviated AILI and weakened the exacerbation of AILI caused by hepatocyte NEDD4-1 deficiency.Mechanistically,NEDD4-1 was found to interact with the PPTY motif of VDAC1 through its WW domain and regulate K48-linked ubiquitination and degradation of VDAC1.Our present study indicates that NEDD4-1 is a suppressor of AILI and functions by regulating the degradation of VDAC1. | Yiwei Zhu Lin Lei Xinghui Wang Linfang Chen Wei Li Jinxia Li Chenchen Zhao Xiliang Du Yuxiang Song Wenwen Gao Guowen Liu Xinwei Li | 2023 | Acta Pharmaceutica Sinica B2023,13,4: | 1 |

The authors present these advantages in what they believe are a realistic scenario of a duel between the guerrilla force and the organized force. | 15 | Neuroprotective effect of shenfu injection after intra-arterial thrombolysis显示文摘BACKGROUND: Within 3 hours after ischemic stroke, thrombolysis can improve prognosis of patients; however, cerebral edema and cerebral hemorrhage induced by vascular recanalization at an early phase can influence on therapeutic effect of thrombolysis; therefore, thrombolysis is combined with neuroprotective agent for recently clinical treatment. OBJECTIVE: To evaluate the neuroprotective effect of shenfu injection on patients with ischemic stroke after arterial thrombolysis and compare with the controls. DESIGN: Case-controlled study. SETTING: Department of Medical Image, Second Hospital of Hebei Medical University. PARTICIPANTS: A total of 45 patients with acute stroke were selected from Emergency Department or Neurological Department of the Second Hospital of Hebei Medical University from August 2005 to May 2006. All patients were met the diagnosis criteria published by the Fourth National Stroke Conference and had middle cerebral arterial embolism with angiography. Attacked course was less than 6 hours. Patients did not have a history of stroke. Scores of the National Institutes of Health Stroke Scale (NIHSS) were not less than 4 points and patients were aged from 18 to 80 years. All subjects consented of the whole therapeutic process. METHODS: ① Intermittent perfusion was used to treat intra-arterial thrombolysis. Thirty patients with vascular recanalization or partially vascular recanalization (18 males and 12 females) were randomly divided into treatment group and control group with 15 in each group. There was no significant difference of baseline data between the two groups. After recanalization, patients in treatment group were given 20 mL shenfu injection, which consisted of ginseng saponin and aconitine (Ya’an Sanjiu Pharmaceutical Company, batch number: 041013, 10 mL/ampoule) and slowly injected into vein for longer than 5 minutes. And then, patients were intravenously dripped with 50 mL shenfu injection and 50 g/L 250 mL glucose solution once a day for 2 courses. Fourteen days were regarded as a course. Patients in control group were treated with the same volume of glucose solution (50 g/L), and other processes in this group were as the same as those in treatment group. ② NIHSS scores before thrombolysis and at 24 hours after thrombolysis: Patients were scored as the follows: level of consciousness, staring, sight, facial paralysis, upper limb movement, lower limb movement, incoordination, sense, language, dysarthria and neglect. Total scores ranged from 0 to 42, and the more the scores were, the more obvious the symptom was. ③ Patients were checked with head CT examination at 24 hours after thrombolysis. ④ Modified Rankin scores at 3 months after thrombolysis: Total scores ranged from 0 to 6. 0: symptomless; 6: death; 0-2: well; 3-6: poor. ⑤ Adverse events and side effects were checked after thrombolysis. MAIN OUTCOME MEASURES: ① Comparison of NIHSS scores between the two groups before and after thrombolysis; ② comparison of modified Rankin scores at 3 months after thrombolysis; ③ comparison of adverse events and side effects. RESULTS: Among 45 patients with ischemic stroke, 30 ones with vascular recanalization were involved in the final analysis. ① Comparison of NIHSS scores before and after thrombolysis: After thrombolysis, NIHSS scores were 5 in treatment group and 8 in control group, which were lower than those before thrombolysis (12, 13; U =2.054, 1.652; P < 0.05). At 24 hours after thrombolysis, NIHSS score was 5 in treatment group, which was lower than that in control group (8, U =2.157, P < 0.05). ② Comparison of modified Rankin scores at 3 months after thrombolysis: Rankin scores which were regarded as well accounted for 93% (14/15) in treatment group, which was higher than those in control group (53%, 8/15, P < 0.05). ③ Comparison of adverse events and side effects: Patients in treatment group did not have cerebral hemorrhage and whole-body hemorrhage with head CT examination; meanwhile, all patients were living at 24 hours after thrombolysis. Among 2 patients with cerebral hemorrhage, 1 case were dead in control group with head CT examination at 24 hours after thrombolysis. It is hypothesized that a cocktail therapy involving a combined application of neuroprotective agents is feasible and offers excellent therapeutic potential. OBJECTIVE: To evaluate the neuroprotective effects of a cocktail therapy of insulin-like growth factor 1 (IGF1) combined with anti-intercellular adhesion molecule 1 (ICAM1) antibody in the treatment of cerebral ischemia/reperfusion injury using medical imaging, pathology, and functional neurological deficit scoring techniques. DESIGN, TIME AND SETTING: This randomized, controlled, neuroimaging analysis of function and pathological observation was performed at the Laboratory of Molecular Imaging, Second Hospital, Hebei Medical University between September 2006 and December 2007. MATERIALS: Transient middle cerebral artery occlusion (MCAO) was induced in 24 healthy adult cats. Anti-ICAM1 antibody and IGF1 were sourced from the Shanghai Kangcheng Biological Product Co., Ltd., China. Stereotaxic apparatus was purchased from the Center for Medical Apparatus and Instruments, Shandong Liaocheng People’s Hospital, China. The in situ apoptosis kit was provided by the Beijing Zhongshan Biotechnique Co., Ltd., China. METHODS: Twenty-four cat models of MCAO were randomly divided into four groups (n = 6): control, IGF1, anti-ICAM1 antibody and cocktail therapy. Following a 2-hour ischemia and subsequent lateral cerebral ventricular puncture, 100 μg IGF1(cerebral ventricular), 100 μg anti-ICAM1 antibody (i.v.), 50 μg IGF1(lateral cerebral ventricular) + 50 μg anti-ICAM1 antibody (i.v.), and 100 μg physiological saline (i.v.) were administered to the IGF1, anti-ICAM1 antibody, cocktail therapy and control groups, respectively. On the following day, the same administration was performed again. MAIN OUTCOME MEASURES: Pathological observation of the cerebral dura mater tissue surrounding the MCAO target site was performed by electron microscopy. At days 3 and 7 following MCAO induction, the volume of the cerebral infarction was measured by Philips functional neurological deficit scoring and nuclear magnetic resonance imaging. RESULTS: Pathological observation revealed that the control group exhibited a great number of swollen neurons, glial cells and vascular endothelial cells, and showed severely injured mitochondria with an absence of the double membrane structure. The same phenomena were partially alleviated in the IGF1 and anti-ICAM1 antibody groups, and the most obvious alleviation of injuries was in the cocktail therapy group. At day 6 following MCAO establishment, the cocktail therapy group exhibited the smallest cerebral infarction volumes among the four groups (F = 71.322, P < 0.01). At days 3 and 7 following MCAO induction, the F value of Philips functional neurological deficit scoring was 10.398 and 14.430, respectively (P < 0.01); however, the best neurological functional recovery was in the cocktail therapy group.
