1Vascular endothelial dysfunction, cardiomyopathy a major mediator in diabetic cardiomyopathy显示文摘Diabetes mellitus is currently a major public health problem.A common complication of diabetes is cardiac dysfunction,which is recognized as a microvascular disease that leads to morbidity and mortality in diabetic patients.While ischemic events are commonly observed in diabetic patients,the risk for developing heart failure is also increased,independent of the severity of coronary artery disease and hypertension.This diabetes-associated clinical entity is considered a distinct disease process referred to as 'diabetic cardiomyopathy'.However,it is not clear how diabetes promotes cardiac dysfunction.Vascular endothelial dysfunction is thought to be one of the key risk factors.The impact of diabetes on the endothelium involves several alterations,including hyperglycemia,fatty acid oxidation,reduced nitric oxide (NO),oxidative stress,inflammatory activation,and altered barrier function. 2Role of Hox genes in stem cell differentiation显示文摘Hox genes are an evolutionary highly conserved gene family. They determine the anterior-posterior body axis in bilateral organisms and influence the developmental fate of cells. Embryonic stem cells are usually devoidof any Hox gene expression, but these transcription factors are activated in varying spatial and temporal patterns defining the development of various body regions. In the adult body, Hox genes are among others responsible for driving the differentiation of tissue stem cells towards their respective lineages in order to repair and maintain the correct function of tissues and organs. Due to their involvement in the embryonic and adult body, they have been suggested to be useable for improving stem cell differentiations in vitro and in vivo. In many studies Hox genes have been found as driving factors in stem cell differentiation towards adipogenesis, in lineages involved in bone and joint formation, mainly chondrogenesis and osteogenesis, in cardiovascular lineages including endothelial and smooth muscle cell differentiations, and in neurogenesis. As life expectancy is rising, the demand for tissue reconstruction continues to increase. Stem cells have become an increasingly popular choice for creating therapies in regenerative medicine due to their self-renewal and differentiation potential. Especially mesenchymal stem cells are used more and more frequently due to their easy handling and accessibility, combined with a low tumorgenicity and little ethical concerns. This review therefore intends to summarize to date known correlations between natural Hox gene expression patterns in body tissues and during the differentiation of various stem cells towards their respective lineages with a major focus on mesenchymal stem cell differentiations. 3High-flow nasal oxygen availability for sedation decreases the use of general anesthesia during endoscopic retrograde cholangiopancreatography and endoscopic ultrasound显示文摘AIM To examine whether high-flow nasal oxygen(HFNO) availability influences the use of general anesthesia(GA) in patients undergoing endoscopic retrograde cholangiopancreatography(ERCP) and endoscopic ultrasound(EUS) and associated outcomes.METHODS In this retrospective study, patients were stratified into 3 eras between October 1, 2013 and June 30, 2014 based on HFNO availability for deep sedation at the time of their endoscopy. During the first and last 3-mo eras(era 1 and 3), no HFNO was available, whereas it was an option during the second 3-mo era(era 2). The primary outcome was the percent utilization of GA vs deep sedation in each period. Secondary outcomes included oxygen saturation nadir during sedation between periods, as well as procedure duration, and anesthesia-only time between periods and for GA vs sedation cases respectively.RESULTS During the study period 238 ERCP or EUS cases were identified for analysis. Statistical testing was employed and a P < 0.050 was significant unless the Bonferroni correction for multiple comparisons was used. General anesthesia use was significantly lower in era 2 compared to era 1 with the same trend between era 2 and 3(P = 0.012 and 0.045 respectively). The oxygen saturation nadir during sedation was significantly higher in era 2 compared to era 3(P < 0.001) but not between eras 1 and 2(P = 0.028) or 1 and 3(P = 0.069). 4Proposed approach to the challenging management of progressive gastroesophageal reflux disease显示文摘The progression of gastroesophageal reflux disease(GERD) in patients who are taking proton pump inhibitors(PPIs) has been reported by several investigators,leading to concerns that PPI therapy does not address all aspects of the disease.Patients who are at risk of progression need to be identified early in the course of their disease in order to receive preventive treatment.A review of the literature on GERD progression to Barrett's esophagus and the associated physiological and pathological changes was performed and risk factors for progression were identified.In addition,a potential approach to the prevention of progression is discussed.Current evidence shows that GERD can progress;however,patients at risk of progression may not be identified early enough for it to be prevented.Biopsies of the squamocolumnar junction that show microscopic intestinalization of metaplastic cardiac mucosa in endoscopically normal patients are predictive of future visible Barrett's esophagus,and an indicator of GERD progression.Such changes can be identified only through biopsy,which is not currently recommended for endoscopically normal patients.GERD treatment should aim to prevent progression.We propose that endoscopically normal patients who partially respond or do not respond to PPI therapy undergo routine biopsies at the squamocolumnar junction to identify histological changes that may predict future progression.This will allow earlier intervention,aimed at preventing Barrett's esophagus.Joachim Labenz Parakrama T Chandrasoma Laura J Knapp Tom R DeMeester 2018World Journal of Gastrointestinal Endoscopy2018,10,9:7

9Evaluation of a gravel transport sensor for bed load measurements in natural flows显示文摘A recent acoustic instrument(Gravel Transport Sensor,GTS) was tested for predicting sediment transport rate(bed load rate) in gravel bed streams. The GTS operation is based on the particle collision theory of submerged obstacles in fluids. When particles collide with the GTS cylinder their momentum is recorded in the form of ping rates. The GTS is attractive for further consideration here because of its potential to provide continuous unattended local bed load measurements,especially in areas found in streams that access may be difficult under extreme conditions. Laboratory experiments coupled with numerical simulations for the same flow conditions were performed in order to determine the conditions under which particles of different size will hit the GTS cylinder and be able to register a ping rate. The GTS was able to detect the number of particles with diameter in the range of 15.9 to 25.4 mm,with reasonable accuracy,if the applied Shields effective stress τe=ττcr was in the range of 0.006 to 0.015. A drawback of the tested prototype GTS,however,was that it exerted increased resistance on the incoming particles. The added drag effects increased the overall resistance that was exerted by the flow on particles and thus increased the likelihood that particles will rest in the ambient region of the cylinder instead of hitting it. 16Probing thermally-induced structural evolution during the synthesis of layered Li-, Na-, or K-containing 3d transition-metal oxides显示文摘Layered alkali-containing 3d transition-metal oxides are of the utmost importance in the use of electrode materials for advanced energy storage applications such as Li-,Na-,or K-ion batteries.A significant challenge in the field of materials chemistry is understanding the dynamics of the chemical reactions between alkali-free precursors and alkali species during the synthesis of these compounds.In this study,in situ high-resolution synchrotron-based X-ray diffraction was applied to reveal the Li/Na/K-ion insertion-induced structural transformation mechanism during high-temperature solid-state reaction.The in situ diffraction results demonstrate that the chemical reaction pathway strongly depends on the alkali-free precursor type,which is a structural matrix enabling phase transi-tions.Quantitative phase analysis identifies for the first time the decomposition of lithium sources as the most critical factor for the formation of metastable intermediates or impurities during the entire process of Li-rich layered Li[Li_(0.2)Ni_(0.2)Mn_(0.6)]O_(2) formation.Since the alkali ions have different ionic radii,Na/K ions tend to be located on prismatic sites in the defective layered structure(Na_(2/3-x)[Ni_(0.25)Mn_(0.75)]O_(2) or K_(2/3-x)[Ni_(0.25)Mn_(0.75)]O_(2))during calcination,whereas the Li ions prefer to be localized on the tetrahedral and/or octahedral sites,forming O-type structures.Weibo Hua Xiaoxia Yang Nicola PMCasati Laijun Liu Suning Wang Volodymyr Baran Michael Knapp Helmut Ehrenberg Sylvio Indris 2022eScience2022,2,2:2
