| 1 | 代谢综合征:皇帝的新衣显示文摘本文作者提出:首先,往往有将“代谢综合征”的某些代谢因素相互合并和强行规定的倾向;第二,这些危险因素单独或任何组合都可能伴随有增加心血管病(CVD)与糖尿病(DM)的危险;第三,对“代谢综合征”本身也没有特定的治疗方法。关键问题是缺乏诊断的生物学基础,要提出或制定“代谢综合征”的诊断要求,势必要有资料能证明此改变的特异性生物学证据及(或)新的或修订的定义的敏感性、特异性及诊断的阳性预测价值。不幸,在任何关于“代谢综合征”的文章中都没有这种资料,而在上万篇这一题目的文献中却有许多误导的信息。例如认为5个危险因素中有3个就达到诊断要求的科学证据是什么?无疑有“综合征”者伴有较高的DM与CVD的危险性,但也没有资料表明“代谢综合征”复杂、抽象的诊断是比检测LDL—C更有利于直接的临床治疗的手段。最近有研究指出如同时从代谢综合征的五大组分中任取其一、其二甚至其三,显示对cVD的相对危险性基本上是相同的,所以另一论点表明作为“综合征”整体合并CVD的危险性!并不比“综合征”各单个组分更大,甚至指出如要想一次搞定,最简单、最经济和最有预测DM和心血管病危险的办法就是只测空腹血糖。文中还讨论了有关血压值、血糖值等的修改问题。作者最后强调某些CVD危险因素呈集簇存在的重要性,医生们必须按已有的临床指南同等关注和处理已确认的CVD与DM危险因素,最为重要的足超重或肥胖的严重程度, | Richard Kahn 《中国糖尿病杂志编辑部》(摘译) | 2006 | 中国糖尿病杂志2006,14,6: | 9 |
