1Leptin in hepatocellular carcinoma显示文摘The risk factors for hepatocellular carcinoma (HCC) development have been established,and include chronic hepatitis B and C,heavy alcohol consumption,and aflatoxins.In fact,5%-30% of patients with HCC still lack a readily identifiable risk factor.It has been reported that the majority of 'cryptogenic' HCC may be attributed to nonalcoholic fatty liver disease,the hepatic presentation of the metabolic syndrome (MS).Obesity is associated with the development of the MS.Recently,adipose tissue has been considered as an endocrine organ because of its capacity to secrete a variety of cytokines,which are collectively known as the adipokines.Leptin,the product of the obese gene,is mainly produced by adipose tissue.Since leptin was first characterized in 1994,accumulated literature has demonstrated the involvement of this adipokine in several areas of human physiology.After binding to its receptor,leptin initiates a cascade of signaling events and subsequent cellular effects.In addition to being the regulatory mediator of energy homeostasis,several in vitro studies have demonstrated the fibrogenic role of leptin in the liver.Furthermore,the deregulated expression of leptin and its receptor have been demonstrated to be associated with a variety of metabolic disorders as well as human cancers.Most importantly,direct evidence supporting the inhibitory and/or activating role of leptin in the process of carcinogenesis and progression of human HCC has been accumulating rapidly.This review aims to provide important insights into the potential mechanisms of leptin in the development of HCC.Hopefully,further investigations will shed light on a new therapeutic target in HCC.Shen-Nien Wang King Teh Lee Chen-Guo Ker 2010World Journal of Gastroenterology2010,16,46:7

3Rhubarb extract partially improves mucosal integrity in chemotherapy-induced intestinal mucositis显示文摘AIM To investigate the effects of orally gavaged aqueous rhubarb extract(RE) on 5-fluorouracil(5-FU)-induced intestinal mucositis in rats. METHODS Female Dark Agouti rats(n = 8/group) were gavaged daily(1 mL) with water, high-dose RE(HDR; 200 mg/kg) or low-dose RE(LDR; 20mg/kg) for eight days. Intestinal mucositis was induced(day 5) with 5-FU(150 mg/kg) via intraperitoneal injection. Intestinal tissue samples were collected for myeloperoxidase(MPO) activity and histological examination. Xenopus oocytes expressing aquaporin 4 water channels were prepared to examine the effect of aqueous RE on cell volume, indicating a potential mechanism responsible for modulating net fluid absorption and secretion in the gastrointestinal tract. Statistical significance was assumed at P < 0.05 by one-way ANOVA. RESULTS B o d y w e i g h t w a s s i g n i f i c a n t l y r e d u c e d i n r a t s administered 5-FU compared to healthy controls(P < 0.01). Rats administered 5-FU significantly increased intestinal MPO levels(≥ 307%; P < 0.001), compared to healthy controls. However, LDR attenuated this effect in 5-FU treated rats, significantly decreasing ileal MPO activity(by 45%; P < 0.05), as compared to 5-FU controls. 5-FU significantly reduced intestinal mucosal thickness(by ≥ 29% P < 0.001) as compared to healthy controls. LDR significantly increased ileal mucosal thickness in 5-FU treated rats(19%; P < 0.05) relative to 5-FU controls. In xenopus oocytes expressing AQP4 water channels, RE selectively blocked water influx into the cell, induced by a decrease in external osmotic pressure. As water efflux was unaltered by the presence of extracellular RE, the directional flow of water across the epithelial barrier, in the presence of extracellular RE, indicated that RE may alleviate water loss across the epithelial barrier and promote intestinal health in chemotherapy-induced intestinal mucositis.CONCLUSION In summary, low dose RE improves selected parameters of mucosal integrity and reduces ileal inflammation, manifesting from 5-FU-induced intestinal mucositis.Juliana E Bajic Georgina L Eden Lorrinne S Lampton Ker Y Cheah Kerry A Lymn Jinxin V Pei Andrea J Yool Gordon S Howarth 2016World Journal of Gastroenterology2016,22,37:3

5Sphingomyelin synthase 2 facilitates M2-like macrophage polarization and tumor progression in a mouse model of triple-negative breast cancer显示文摘High infiltration of M2-polarized macrophages in the primary tumor indicates unfavorable prognosis and poor overall survival in the patients with triple-negative breast cancer(TNBC).Thus,reversing M2-polarized tumor-associated macrophages in the tumors has been considered as a potential therapeutic strategy for TNBC.Sphingomyelin synthase 2(SMS2)is the key enzyme for sphingomyelin production,which plays an important role in plasma membrane integrity and function.In this study we investigated whether SMS2 inhibitor or SMS2 gene knockout could reduce macrophages M2 polarization and tumor progression in a mouse model of TNBC.We showed that SMS2 mRNA expression was linked to immunosuppressive tumor microenvironment and poor prognosis in TNBC patients.The knockout of SMS2 or application of 15w(a specific SMS2 inhibitor)markedly decreased the generation of M2-type macrophages in vitro,and reduced the tumor weight and lung metastatic niche formation in a 4T1-TNBC mouse model.We further demonstrated that the in vivo antitumor efficacy of 15w was accompanied by a multifaceted remodeling of tumor immune environment reflecting not only the suppression of M2-type macrophages but also diminished levels of regulatory T cells and myeloid-derived suppressor cells leading to a dramatically improved infiltration of antitumor CD8+T lymphocytes.Collectively,our results reveal a novel and important role of SMS2 in the protumorigenic function and may offer a new strategy for macrophage-targeted anticancer therapy.Yan Deng Jia-chun Hu Shu-hua He Bin Lou Ting-bo Ding Jin-tong Yang Ming-guang Mo De-yong Ye Lu Zhou Xian-cheng Jiang Ker Yu Ji-bin Dong 2021Acta Pharmacologica Sinica2021,42,1:2

7Neurological heterotopic ossification:novel mechanisms,prognostic biomarkers and prophylactic therapies显示文摘Neurological heterotopic ossification(NHO)is a debilitating condition where bone forms in soft tissue,such as muscle surrounding the hip and knee,following an injury to the brain or spinal cord.This abnormal formation of bone can result in nerve impingement,pain,contractures and impaired movement.Patients are often diagnosed with NHO after the bone tissue has completely mineralised,leaving invasive surgical resection the only remaining treatment option.Surgical resection of NHO creates potential for added complications,particularly in patients with concomitant injury to the central nervous system(CNS).Although recent work has begun to shed light on the physiological mechanisms involved in NHO,there remains a significant knowledge gap related to the prognostic biomarkers and prophylactic treatments which are necessary to prevent NHO and optimise patient outcomes.This article reviews the current understanding pertaining to NHO epidemiology,pathobiology,biomarkers and treatment options.In particular,we focus on how concomitant CNS injury may drive ectopic bone formation and discuss considerations for treating polytrauma patients with NHO.We conclude that understanding of the pathogenesis of NHO is rapidly advancing,and as such,there is the strong potential for future research to unearth methods capable of identifying patients likely to develop NHO,and targeted treatments to prevent its manifestation.Ker Rui Wong Richelle Mychasiuk Terence J.O’Brien Sandy R.Shultz Stuart J.McDonald Rhys D.Brady 2020Bone Research2020,8,4:2

11Effect of Si addition on microstructure and wear properties of Mg-Sn as-cast alloys显示文摘Effect of Si addition on microstructure,hardness and wear resistance of as cast Mg-5Sn and Mg-10Sn alloys were investigated.For this purpose alloys with wt.-%0,0.5,1,1.5 Si were cast using an induction furnace.Addition of Si to both alloys caused an increasement in the amount of intermetallic phases at the grain boundaries.Also refined grain structure was reported with increasing Si content in the cast alloys.Both increased amount of intermetallics and decreased grain size caused an increase in the hardness of the alloys.Wear resistance of the binary alloys first increased with Si addition and then decreased.Increased amounts of Si in the alloys caused crack formation during wear tests and decreased wear resistance.Sliding speed also has a dramatical effect on the wear resistance of the alloys.Increased sliding speed resulted with higher wear resistance.Worn surfaces of the alloys also investigated under scanning electron microscope to understand the wear mechanisms operated during wear tests.Erdem Karakulak Yusuf Burak Küçüker 2018Journal of Magnesium and Alloys2018,6,4:2
12New flexible endoscopic controlled stapler technique for the treatment of Zenker's diverticulum:A case series显示文摘AIM To report about the combination and advantages of a tapler-assisted diverticulotomy performed by flexible endoscopy.METHODS From November 2014 till December 2015 17 patients(8 female, 9 male, average age 69.8 years) with a symptomatic Zenker diverticulum(mean size 3.5 cm) were treated by inserting a new 5 mm fully rotatable surgical stapler(Micro Cutter30 Xchange, Cardica Inc.) next to an ultrathin flexible endoscope through an overtube. The Patients were under conscious sedation with the head reclined in left position, the stapler placed centrally and pushed forward to the bottom of the diverticulum. The septum was divided by the staple rows under flexible endoscopic control.RESULTS In eleven patients(64.7%) the stapler successfully divided the septum completely. Mean procedure time was 21 min, medium size of the septum was 2.8 cm(range 1.5 cm to 4 cm). In four patients the septum was shorter than 3 cm, in seven longer than 3 cm. To divide the septum, averagely 1.3 stapler cartridges were used. Two minor bleedings occurred. Major adverse events like perforation or secondary haemorrhage did not occur. After an average time of two days patients were discharged from the hospital. In 6 patients(35.3%) the stapler failed due to a thick septum or insufficient reclination of the head. Follow up endoscopy was performed after an average of two months in 9 patients; 4 patients(44.4%) were free of symptoms, 5 patients(55.6%) stated an improvement. A relapse of symptoms did not occur. CONCLUSION Flexible endoscopic Zenker diverticulotomy by using a surgical stapler is a new, safe and efficient treatment modality.
