2Application of artificial intelligence in the diagnosis and treatment of hepatocellular carcinoma: A review显示文摘Although artificial intelligence(AI)was initially developed many years ago,it has experienced spectacular advances over the last 10 years for application in the field of medicine,and is now used for diagnostic,therapeutic and prognostic purposes in almost all fields.Its application in the area of hepatology is especially relevant for the study of hepatocellular carcinoma(HCC),as this is a very common tumor,with particular radiological characteristics that allow its diagnosis without the need for a histological study.However,the interpretation and analysis of the resulting images is not always easy,in addition to which the images vary during the course of the disease,and prognosis and treatment response can be conditioned by multiple factors.The vast amount of data available lend themselves to study and analysis by AI in its various branches,such as deeplearning(DL)and machine learning(ML),which play a fundamental role in decision-making as well as overcoming the constraints involved in human evaluation.ML is a form of AI based on automated learning from a set of previously provided data and training in algorithms to organize and recognize patterns.DL is a more extensive form of learning that attempts to simulate the working of the human brain,using a lot more data and more complex algorithms.This review specifies the type of AI used by the various authors.However,welldesigned prospective studies are needed in order to avoid as far as possible any bias that may later affect the interpretability of the images and thereby limit the acceptance and application of these models in clinical practice.In addition,professionals now need to understand the true usefulness of these techniques,as well as their associated strengths and limitations.Miguel Jimenez Perez Rocio Gonzalez Grande 2020World Journal of Gastroenterology2020,26,37:7
3Z-line examination by the PillCam^(TM) SB:Prospective comparison of three ingestion protocols显示文摘AIM:To evaluate the Z-line visualization by the PillCamTM SB2 using three different ingestion protocols. METHODS:Ninety consecutive patients undergoing small bowel capsule endoscopy(SBCE)between January and May 2008 were included in the study. They swallowed the capsule in the standing(Group A= 30),supine(Group B=30)and right supine positions (Group C=30).Baseline patient characteristics, difficulties in capsule ingestion,esophageal transit times(ETT)and Z-line visualization were noted. RESULTS:No significant differences were found between the groups with regard to baseline patient characteristics,ingestion difficulties and complete SB examinations(P>0.05).At least 1 frame of the Z-line was detected in 15.8%,46.7%and 90%of patients in groups A,B and C,respectively(P<0.001).The average number of Z-line images was 0.21±0.53,3.23 ±6.59 and 5.53±7.55 and the mean%of the Z-linedetected was 71.3,25.1 and 8.3,in groups A,B and C, respectively(both P<0.001).ETT times were longer in the supine group followed by the right supine and the standing groups(median of 237 s vs 64 s and 39 s, respectively;P<0.001). 4Assessment of potential impacts to surface and subsurface water bodies due to longwall mining显示文摘Ground movements due to longwall mining operations have the potential to damage the hydrological balance within as well as outside the mine permit area in the form of increased surface ponding and changes to hydrogeological properties. Recently, the Office of Surface Mining, Reclamation and Enforcement(OSMRE) in the USA, has completed a public comment period on a newly proposed rule for the protection of streams and groundwater from adverse impacts of surface and underground mining operations(80 FR 44435). With increased community and regulatory focus on mining operations and their potential to adversely affect streams and groundwater, now there is a greater need for better prediction of the possible effects mining has on both surface and subsurface bodies of water. With mining induced stress and strain within the overburden correlated to changes in the hydrogeological properties of rock and soil, this paper investigates the evaluation of the hydrogeological system within the vicinity of an underground mining operation based on strain values calculated through a surface deformation prediction model. 5Laparoscopic approach in gastrointestinal emergencies显示文摘This review focuses on the laparoscopic approach to gastrointestinal emergencies and its more recent indications. Laparoscopic surgery has a specific place in elective procedures, but that does not apply in emergency situations. In specific emergencies, there is a huge range of indications and different techniques to apply, and not all of them are equally settle. We consider that the most controversial points in minimally invasive procedures are indications in emergency situations due to technical difficulties. Some pathologies, such as oesophageal emergencies, obstruction due to colon cancer, abdominal hernias or incarcerated postsurgical hernias, are nearly always resolved by conventional surgery, that is, an open approach due to limited intraabdominal cavity space or due to the vulnerability of the bowel. These technical problems have been solved in many diseases, such as for perforated peptic ulcer or acute appendectomy for which a laparoscopic approach has become a wellknown and globally supported procedure. On the other hand, endoscopic procedures have acquired further indications, relegating surgical solutions to a second place; this happens in cholangitis or pancreatic abscess drainage. This endoluminal approach avoids the need for laparoscopic development in these diseases. Nevertheless, new instruments and new technologies could extend the laparoscopic approach to a broader array of potentials procedures. 12Targeted treatment of cancer with radiofrequency electromagnetic fields amplitude-modulated at tumor-specific frequencies显示文摘In the past century, there have been many attempts to treat cancer with low levels of electric and magnetic fields. We have developed noninvasive biofeedback examination devices and techniques and discovered that patients with the same tumor type exhibit biofeedback responses to the same, precise frequencies. Intrabuccal administration of 27.12 MHz radiofrequency(RF) electromagnetic fields(EMF), which are amplitude-modulated at tumor-specific frequencies, results in long-term objective responses in patients with cancer and is not associated with any significant adverse effects. Intrabuccal administration allows for therapeutic delivery of very low and safe levels of EMF throughout the body as exemplified by responses observed in the femur, liver, adrenal glands, and lungs. In vitro studies have demonstrated that tumor-specific frequencies identified in patients with various forms of cancer are capable of blocking the growth of tumor cells in a tissue- and tumor-specific fashion. Current experimental evidence suggests that tumor-specific modulation frequencies regulate the expression of genes involved in migration and invasion and disrupt the mitotic spindle. This novel targeted treatment approach is emerging as an appealing therapeutic option for patients with advanced cancer given its excellent tolerability. 13Erosion problem in tool steel using cold box core-making process显示文摘This paper presents the erosion results of the AISI H13 steel impinged by resin-bonded silica sand, using a testing rig that closely simulates the real blowing conditions during industrial core-making. Steel specimens were heat treated to obtain hardness of 294, 445 and 595 HV200 (29, 45 and 55 HRC). Erosion tests were carried out at impingement angles from 20° to 90° and air drag pressures of 1.38, 2.07 and 2.76 bar (20, 30 and 40 psi). The main results are summarized as follows:(i) The harder material, the lower erosion;(ii) the maximum erosion rate is at 30°;(iii) Little difference in erosion rate at impact angle of 60° and 90° for a constant pressure tested regardless of the hardness level;(iv) As the pressure increases, so does the erosion rate, being more sensitive for low impact angles. 14Plant micro-reserves in Valencia(E. Spain):A model to preserve threatened flora in China?显示文摘The Valencian Community(eastern Spain) was the pioneer territory establishing plant micro-reserves(PMRs). Its model to protect small sites for endemic and endangered plants has been exported to several countries around the globe. This paper highlights 1) the role of PMRs to complement the protection provided by large protected areas, 2) how the establishment of PMRs fosters the increase of floristic knowledge, and 3)the fact that continuous monitoring of PMRs also yields new records of endangered species found within the same PMRs. 16西班牙抗结核药物所致严重肝毒性反应显示文摘目的:判定抗结核药物引起的严重肝毒性反应的发生频率及发生急性肝功能衰竭或死亡的预报指标。方法﹕西班牙肺科学会成员对1997-2001年18家医院的资料进行回顾性研究。严重肝毒性反应界定为,无临床症状者转氨酶有10倍升高或胆汁郁滞参数3倍增加;有肝炎症状者则为肝功能参数增加或发生肝功能衰竭。采用logistic 回归分析计算比值比(OR)及其95%可信区间(CI)研究其预报因素。结果﹕3510 例中共有166 例发生肝毒性反应,其中90 例(2.56%)为活动性结核病治疗所致。11 例(10.3%)发生急性肝功能衰竭,其中3 例进行了肝移植。总体病死率为4.7%(5 例,其中3 例与饮酒或肝毒性药物有关)。预后不良的预报指标为总胆红素>2mg/dl(OR=9.4 , 95%CI : 1.0-85.5) 及血清肌肝>1.5mg/dl(OR=32.1,95%CI:2.4-424.6)。结论﹕抗结核药物引起的严重肝毒性反应死亡率较高。必须进行经常性的临床检查及临床实验室观察以预防其发生。J.
