| 1 | Interaction of obesity and inflammatory bowel disease显示文摘Inflammatory bowel disease(IBD) is a chronic inflammatory condition of unknown etiology that is thought to result from a combination of genetic, immunologic and environmental factors. The incidence of IBD has been increasing in recent decades, especially in developing and developed nations, and this is hypothesized to be in part related to the change in dietary and lifestyle factors associated with modernization. The prevalence of obesity has risen in parallel with the rise in IBD, suggesting a possible shared environmental link between these two conditions. Studies have shown that obesity impacts disease development and response to therapy in patients with IBD and other autoimmune conditions. The observation that adipose tissue produces pro-inflammatory adipokines provides a potential mechanism for the observed epidemiologic links between obesity and IBD, and this has developed into an active area of investigative inquiry. Additionally, emerging evidence highlights a role for the intestinal microbiota in the development of both obesity and IBD, representing another potential mechanistic connection between the two conditions. | 2 | Clinical outcomes following salvage Gamma Knife radiosurgery for recurrent glioblastoma显示文摘Glioblastoma multiforme(GBM) is the most common malignant primary brain tumor with a survival prognosis of 14-16 mo for the highest functioning patients. Despite aggressive, multimodal upfront therapies, the majority of GBMs will recur in approximately six months. Salvage therapy options for recurrent GBM(r GBM) are an area of intense research. This study compares recent survival and quality of life outcomes following Gamma Knife radiosurgery(GKRS) salvage therapy. Following a Pub Med search for studies usingGKRS as salvage therapy for malignant gliomas, nine articles from 2005 to July 2013 were identified which evaluated rG BM treatment. In this review, we compare overall survival following diagnosis, overall survival following salvage treatment, progression-free survival, time to recurrence, local tumor control, and adverse radiation effects. This report discusses results for rG BM patient populations alone, not for mixed populations with other tumor histology grades. All nine studies reported median overall survival rates(from diagnosis, range:16.7-33.2 mo; from salvage, range:9-17.9 mo). Three studies identified median progression-free survival(range:4.6-14.9 mo). Two showed median time to recurrence of GBM. Two discussed local tumor control. Six studies reported adverse radiation effects(range:0%-46% of patients). The greatest survival advantages were seen in patients who received GKRS salvage along with other treatments, like resection or bevacizumab, suggesting that appropriately tailored multimodal therapy should be considered with each rG BM patient. | 3 | Current state of endohepatology: Diagnosis and treatment of portal hypertension and its complications with endoscopic ultrasound显示文摘The diagnosis and management of cirrhosis and portal hypertension(PH)with its complications including variceal hemorrhage,ascites,and hepatic encephalopathy continues to evolve.Although there are established“standards of care”in liver biopsy and measurement of PH,gastric varices remain an area without a universally accepted therapeutic approach.The concept of“Endo Hepatology”has been used to describe of the applications of endoscopic ultrasound(EUS)to these challenges.EUS-liver biopsy(EUS-LB)offers an alternative to percutaneous and transjuglar liver biopsy without compromising safety or efficacy,and with added advantages including the potential to reduce sampling error by allowing biopsies in both hepatic lobes.Furthermore,EUS-LB can be performed during the same procedure as EUS-guided portal pressure gradient(PPG)measurements,allowing for the collection of valuable diagnostic and prognostic data.EUS-guided PPG measurements provide an appealing alternative to the transjugular approach,with proposed advantages including the ability to directly measure portal vein pressure.In addition,EUS-guided treatment of gastric varices(GV)offers several possible advantages to current therapies.EUS-guided treatment of GV allows detailed assessment of the vascular anatomy,similar efficacy and safety to current therapies,and allows the evaluation of treatment effect through doppler ultrasound visualization.The appropriate selection of patients for these procedures is paramount to ensuring generation of useful clinical data and patient safety. | Sean R Rudnick Jason D Conway Mark W Russo | 2021 | World Journal of Hepatology2021，13，8： | 4 |
| 4 | 机器人在头颈外科中的应用--香港中文大学经验显示文摘目的:机器人手术目前已在头颈部多个区域得到了应用,本文探讨香港中文大学头颈外科机器人手术的经验。方法:回顾性分析2015年1月以来在香港中文大学耳鼻咽喉头颈外科开展的所有机器人头颈外科手术病例资料。结果:24例患者采用达芬奇S或达芬奇Xi系统进行了手术,21例患者使用新型柔性机器人达芬奇SP系统行了手术。均无二次手术等严重不良事件。已经发表的达芬奇SP系统手术的7例患者早期结果表明,通过达芬奇SP系统可以到达鼻咽、口咽部、下咽部、喉部和咽后淋巴结完成手术,不需中途改变手术方式,没有发生与使用该系统有关的严重不良事件或不良事件。结论:通过我们在香港中文大学的经验,我们认为采用达芬奇机器人系统进行经口手术或经耳后入路颈部手术是安全有效的。 | 陈英权 王维杨 梁发雅 黄晓明 Jason Y K Chan Eddy W Y Wong | 2018 | 临床耳鼻咽喉头颈外科杂志2018，32，14： | 4 |

| 7 | Genetic and environmental control of fruit maturation,dry matter and firmness in apple(Malus×domestica Borkh.)显示文摘For any given genotype,the environment in which an apple is grown can influence the properties of the fruit considerably.While there has been extensive research on the mechanism of the genetic control of fruit quality traits,less effort has been made to investigate the way that these genetic mechanisms interact with the environment.To address this issue,we employed a large‘Royal Gala’בBraeburn’population of 572 seedlings replicated over sites in three climatically diverse apple-growing regions in New Zealand.Phenotyping for traits including fruit maturation timing,firmness and dry matter content was performed at each of these three sites for a single growing season(2011),and at two sites(Motueka and Hawke’s Bay)for two seasons(2009 and 2010).The phenotype data collected over 2 years at two sites enabled the detection of 190 quantitative trait loci(QTL)that controlled these traits regardless of year or growing location,as well as some chromosomal loci that influenced the traits in a single given environment or year.For those loci that were environmentally stable over three sites,there was an interdependency of fruit maturation date,dry matter content and storage potential within this population,with two regions on Linkage Groups(LGs)10 and 16 strongly contributing.If these loci were used in a marker-assisted selection programme to select for progeny bearing firmer fruit,this would have the unintentional consequence of selecting,high dry matter content,later maturing apples.In addition,a further 113 new QTLs with a smaller effect were identified,some of which were exhibited only in a single growing environment,demonstrating the underlying complexity of control of traits determining fruit quality,in addition to the need for being aware of environmental effects when developing new apple varieties. | David Chagne Daya Dayatilake Robert Diack Murray Oliver Hilary Ireland Amy Watson Susan E Gardiner Jason W Johnston Robert J Schaffer Stuart Tustin | 2014 | Horticulture Research2014，1，1： | 2 |
