| 1 | 早期干预促进早产儿智能发育显示文摘目的改善早产儿的预后,进行早期干预的协作研究。方法将103例早产儿随机分为干预组(52例)和常规育儿组(51例)。正常对照组(53例)。干预组从新生儿期开始接受智能早期干预,早产儿两组的围产期情况相似。结果纠正年龄1岁半、2岁时干预组精神发育指数(MDI)比常规育儿组平均高138和146(P<0.001),达正常组水平(P>0.05)。而常规育儿组MDI比正常对照组分别低115分和89分(P<0.05)。其中智力低下4例(7.8%)(4/51),干预组中无一例智力低下。结论早期干预可以促进早产儿智能发育,有利于防治其智力低下。 | Early Intervention of Premature Infant Coorperative Research Group, Beijing 100730 | 1998 | 中华儿科杂志1998,36,10: | 47 |

| 6 | Long-term metformin therapy and vitamin B12 deficiency: An association to bear in mind显示文摘To date,metformin remains the first-line oral glucose-lowering drug used for the treatment of type 2 diabetes thanks to its well-established long-term safety and efficacy profile.Indeed,metformin is the most widely used oral insulinsensitizing agent,being prescribed to more than 100 million people worldwide,including patients with prediabetes,insulin resistance,and polycystic ovary syndrome.However,over the last decades several observational studies and meta-analyses have reported a significant association between long-term metformin therapy and an increased prevalence of vitamin B12 deficiency.Of note,evidence suggests that long-term and high-dose metformin therapy impairs vitamin B12 status.Vitamin B12(also referred to as cobalamin)is a water-soluble vitamin that is mainly obtained from animal-sourced foods.At the cellular level,vitamin B12 acts as a cofactor for enzymes that play a critical role in DNA synthesis and neuroprotection.Thus,vitamin B12 deficiency can lead to a number of clinical consequences that include hematologic abnormalities(e.g.,megaloblastic anemia and formation of hypersegmented neutrophils),progressive axonal demyelination and peripheral neuropathy.Nevertheless,no definite guidelines are currently available for vitamin B12 deficiency screening in patients on metformin therapy,and vitamin B12 deficiency remains frequently unrecognized in such individuals.Therefore,in this“field of vision”article we propose a list of criteria for a cost-effective vitamin B12 deficiency screening in metformin-treated patients,which could serve as a practical guide for identifying individuals at high risk for this condition.Moreover,we discuss additional relevant topics related to this field,including:(1)The lack of consensus about the exact definition of vitamin B12 deficiency;(2)The definition of reliable biomarkers of vitamin B12 status;(3)Causes of vitamin B12 deficiency other than metformin therapy that should be identified promptly in metformin-treated patients for a proper differential diagnosis;and(4)Potential pathophysiological mechanisms underlying metformin-induced vitamin B12 deficiency.Finally,we briefly review basic concepts related to vitamin B12 supplementation for the treatment of vitamin B12 deficiency,particularly when this condition is induced by metformin. | Marco Infante Martina Leoni Massimiliano Caprio Andrea Fabbri | 2021 | World Journal of Diabetes2021,12,7: | 6 |

Gene alterations have been found to be associated with cardiovascular diseases, and epigenetic mechanisms are continuously being studied to find new useful strategies for the clinical management of afflicted patients. Numerous cardiovascular disorders are characterized by the abnormal methylation of Cp G islands and so specific drugs that could inhibit DNA methyltransferase directly or by reducing its gene expression(e.g., hydralazine and procainamide) are currently under investigation. The anti-proliferative and anti-inflammatory properties of histone deacetylase inhibitors and their cardio-protective effects have been confirmed in preclinical studies. Furthermore, the regulation of the expression of micro RNA targets through pharmacological tools is still under development. Indeed, large controlled trials are required to establish whether current possible candidate antisense micro RNAs could offer better therapeutic benefits in clinical practice. | 8 | Prevalence and dietetic management of mild gastrointestinal disorders in milk-fed infants显示文摘AIM: To assess the prevalence of mild gastrointestinal disorders in milk-fed infants in paediatric practice, and to evaluate the effectiveness and satisfaction with dietetic treatment.METHODS: A cross-sectional epidemiological study was first carried out. A total of 285 paediatricians included 3487 children seen during a period of one week. In a second phase an observational, prospective and multicentre study was conducted and 2069 milk-fed infants with mild gastrointestinal disorders (colic, constipation, regurgitation and diarrhoea) were included. There was a baseline visit (start of treatment) and a final visit four weeks later. The effectiveness of the various Novalac formulas, as well as the satisfaction of the parents/tutors and paediatricians with the dietetic treatment were assessed at the final visit.RESULTS: The prevalence of mild gastrointestinal disorders was 27.8% of all paediatrician consultations (9.2%, 7.8%, 6.1% and 4.6% in relation to colic, constipation, regurgitation and diarrhoea, respectively). The several Novalac adapted milk formulas resolved 88.4% of the mild gastrointestinal disorders. Depending on the type of disorder, differences in response rate were observed. The highest effectiveness was recorded with respect to diarrhoea (92.6%), followed by constipation (91.6%), colic (87.6%) and regurgitation (81%). Overall, 91% of the paediatricians and 88.8% of the parents/tutors were satisfied or very satisfied with the Novalac adapted milk formulas.CONCLUSION: Mild gastrointestinal disorders show a high prevalence in paediatric practice. | 11 | Portable adaptive optics for exoplanet imaging显示文摘The portable adaptive optics(PAO)device is a low-cost and compact system,designed for 4-meter class telescopes that have no adaptive optics(AO)system,because of the physical space limitation at the Nasmyth or Cassegrain focus and the historically high cost of conventional AO.The initial scientific observations of the PAO are focused on the direct imaging of exoplanets and sub-stellar companions.This paper discusses the concept of PAO and the associated high-contrast imaging performance in our recent observational runs.PAO deliver a Strehl ratio better than 60%in H band under median seeing conditions of 1'.Combined with our dedicated image rotation and subtraction(IRS)technique and the optimized IRS(O-IRS)algorithm,the averaged contrast ratio for a 5≤V_(mag)≤9 primary star is 1.3×10^(-5)and3.3×10^(-6)at angular distance of 0.36'with exposure time of 7 minutes and 2 hours,respectively.PAO has successfully revealed the known exoplanet ofκAnd b in our recent observation with the 3.5-meter ARC telescope at Apache Point Observatory.We have performed the associated astrometry and photometry analysis of the recoveredκAnd b planet,which gives a projected separation of 0.98'±0.05',a position angle of 51.1°±0.5°and a mass of 10.15_(-1.255)^(+2.19) MJup.These results demonstrate that PAO can be used for direct imaging of exoplanets with medium-sized telescopes. | Yong-Tian Zhu Jiang-Pei Dou Xi Zhang Gang Zhao Jing Guo Leopoldo Infante | 2021 | Research in Astronomy and Astrophysics2021,21,4: | 4 |
