1Antimicrobial susceptibility testing for Helicobacter pylori in times of increasing antibiotic resistance显示文摘The gram-negative bacterium Helicobacter pylori(H.pylori)causes chronic gastritis,gastric and duodenal ulcers,gastric cancer and mucosa-associated lymphoid tissue lymphoma.Treatment is recommended in all symptomatic patients.The current treatment options for H.pylori infection are outlined in this review in light of the recent challenges in eradication success,largely due to the rapid emergence of antibiotic resistant strains of H.pylori.Antibiotic resistance is a constantly evolving process and numerous studies have shown that the prevalence of H.pylori antibiotic resistance varies significantly from country to country,and even between regions within the same country.In addition,recent data has shown that previous antibiotic use is associated with harbouring antibiotic resistant H.pylori.Local surveillance of antibiotic resistance is warranted to guide clinicians in their choice of therapy.Antimicrobial resistance is assessed by H.pylori culture and antimicrobial susceptibility testing.Recently developed molecular tests offer an attractive alternative to culture and allow for the rapid molecular genetic identification of H.pylori and resistance-associated mutations directly from biopsy samples or bacterial culture material.Accumulating evidence indicates that surveillance of antimicrobial resistance by susceptibility testing is feasible and necessary to inform clinicians in their choice of therapy for management of H.pylori infection.Sinéad M Smith Colm O’Morain Deirdre McNamara 2014World Journal of Gastroenterology2014,20,29:40

3Efficacy,predictability and safety of small incision lenticule extraction(SMILE)显示文摘Background:The purpose of this case series is to report the one-year outcomes of small incision lenticule extraction(SMILE)using the VisuMax®femtosecond laser.Methods:Two hundred and six patients were recruited for this retrospective,single center study at TRSC International LASIK Center in Bangkok,Thailand.Patients underwent SMILE,whereby an intrastromal lenticule was cut using a femtosecond laser and then manually extracted without the need for flap creation.Outcome measures included refraction,visual acuity and contrast sensitivity evaluation.Patients were treated and followed for one year.Results:SMILE for the correction of low to high myopia was performed on 347 eyes of 206 patients.The mean preoperative spherical equivalent was−4.96±1.88 diopters(D).On the first day following surgery,for eyes with a plano target refraction(99.14%of all eyes),uncorrected distance visual acuity(UDVA)was 20/20 or better in 90%of eyes.At the one week postoperative exam,the mean spherical equivalent was 0.01±0.36 D and UDVA was 20/20 or better in 84%of eyes.After one year follow-up,no eyes showed loss of 2 or more lines of visual acuity and 31%of eyes gained one or more lines.The photopic contrast sensitivity of SMILE treated eyes at 12 and 18 cycles per degree(cpd)improved from 1.59 and 0.94 preoperatively to 1.6 and 0.98,respectively,after one year.Conclusions:In this series,SMILE using the VisuMax®femtosecond laser demonstrated that after one year it is an effective,predictable and safe minimally invasive corneal refractive procedure.Ekktet Chansue Morakot Tanehsakdi Sukanda Swasdibutra Colm McAlinden 2015Eye and Vision2015,2,1:16

7Colorectal cancer screening显示文摘Colorectal cancer is a major public health burden worldwide. There is clear-cut evidence that screening will reduce colorectal cancer mortality and the only contentious issue is which screening tool to use. Mos evidence points towards screening with fecal occul blood testing. The immunochemical fecal occult blood tests have a higher sensitivity than the guaiac-based tests. 8Centration axis in refractive surgery显示文摘The human eye is an asymmetric optical system and the real cornea is not a rotationally symmetrical volume.Each optical element in the eye has its own optical and neural axes.Defining the optimum center for laser ablation is difficult with many available approaches.We explain the various centration approaches(based on these reference axes)in refractive surgery and review their clinical outcomes.The line-of-sight(LOS)(the line joining the entrance pupil center with the fixation point)is often the recommended reference axis for representing wavefront aberrations of the whole eye(derived from the definition of chief ray in geometrical optics);however pupil centration can be unstable and change with the pupil size.The corneal vertex(CV)represents a stable preferable morphologic reference which is the best approximate for alignment to the visual axis.However,the corneal light reflex can be considered as non-constant,but dependent on the direction of gaze of the eye with respect to the light source.A compromise between the pupil and CV centered ablations is seen in the form of an asymmetric offset where the manifest refraction is referenced to the CV while the higher order aberrations are referenced to the pupil center.There is a need for a flexible choice of centration in excimer laser systems to design customized and non-customized treatments optimally.Samuel Arba Mosquera Shwetabh Verma Colm McAlinden 2015Eye and Vision2015,2,1:7
9Molecular detection of Helicobacter pylori antibiotic resistance in stool vs biopsy samples显示文摘AIM To compare(1) demographics in urea breath test(UBT) vs endoscopy patients; and(2) the molecular detection of antibiotic resistance in stool vs biopsy samples.METHODS Six hundred and sixteen adult patients undergoing endoscopy or a UBT were prospectively recruited to the study. The Geno Type Helico DR assay was used to detect Helicobacter pylori(H. pylori) and antibiotic resistance using biopsy and/or stool samples from CLOpositive endoscopy patients and stool samples from UBT-positive patients. RESULTS Infection rates were significantly higher in patients referred for a UBT than endoscopy(overall rates: 33% vs 19%; treatment-na?ve patients: 33% vs 14.7%, respectively). H. pylori-infected UBT patients were younger than H. pylori-infected endoscopy patients(41.4 vs 48.4 years, respectively, P < 0.005), with a higher percentage of H. pylori-infected males in the endoscopy-compared to the UBT-cohort(52.6% vs 33.3%, P = 0.03). The Geno Type Helico DR assay was more accurate at detecting H. pylori infection using biopsy samples than stool samples [98.2%(n = 54/55) vs 80.3%(n =53/66), P < 0.005]. Subset analysis using stool and biopsy samples from CLO-positive endoscopy patients revealed a higher detection rate ofresistance-associated mutations using stool samples compared to biopsies. The concordance rates between stool and biopsy samples for the detection of H. pylori DNA, clarithromycin and fluoroquinolone resistance were just 85%, 53% and 35%, respectively. CONCLUSION Differences between endoscopy and UBT patients provide a rationale for non-invasive detection of H. pylori antibiotic resistance. 10Identifying more reliable parameters for the detection of change during the follow-up of mild to moderate keratoconus patients显示文摘Background:Reaching a consensus on which parameters are most reliable at detecting progressive keratoconus patients with serial topography imaging is not evident.The aim of the study was to isolate the parameters best positioned to detect keratoconus progression using the Pentacam HR^(®)measures based on the respective limits of repeatability and range of measurement.Method:Using the Pentacam HR^(®),a tolerance index was calculated on anterior segment parameters in healthy and keratoconic eyes.The tolerance index provides a scale from least to most affected parameters in terms of measurement noise relative to that observed in healthy eyes.Then,based on the“number of increments”from no disease to advanced disease,a relative utility(RU)score was also calculated.RU values close to 1 indicate parameters best positioned to detect a change in keratoconic eyes.Results:The tolerance index values indicated that 36%of ocular parameters for keratoconic eyes had repeatability limits which were wider than normative limits(worse),but 28%of the ocular parameters were narrower than normative limits(better).Considering only those parameters with a RU greater than 0.95,a small number of parameters were within this range,such as corneal curvature and asphericity indices.Conclusions:This study demonstrates that measurement error in keratoconic eyes is significantly greater than healthy eyes.Indices implemented here provide guidance on the levels of expected precision in keratoconic eyes relative to healthy eyes to aid clinicians in distinguishing real change from noise.Importantly maximal keratometry(Kmax),central corneal thickness(CCT)and thinnest corneal thickness(TCT)were highlighted as problematic indices for the follow-up of keratoconus in terms of repeatability.Ivo Guber Colm McAlinden François Majo Ciara Bergin 2017Eye and Vision2017,4,1:6

12Can bacterial virulence factors predict antibiotic resistant Helicobacter pylori infection?显示文摘AIM To evaluate the association between virulence factor status and antibiotic resistance in Helicobacter pylori(H. pylori)-infected patients in Ireland. METHODS DNA was extracted from antral and corpus biopsies obtained from 165 H. pylori-infected patients. Genotyping for clarithromycin and fluoroquinolone-mediating mutations was performed using the Genotype Helico DR assay. cag A and vac A genotypes were investigated using PCR. RESULTS Primary, secondary and overall resistance rates for clarithromycin were 50.5%(n = 53/105), 78.3%(n = 47/60) and 60.6%(n = 100/165), respectively. Primary, secondary and overall resistance rates for fluoroquinolones were 15.2%(n = 16/105) and 28.3%(n = 17/60) and 20%(n = 33/165), respectively. Resistance to both antibiotics was 12.4%(n = 13/105) in treatment-na?ve patients, 25%(n = 15/60) in those previously treated and 17%(n = 28/165) overall. A cag A-positive genotype was detected in 22.4%(n = 37/165) of patient samples. The dominant vac A genotype was S1/M2 at 44.8%(n = 74/165), followed by S2/M2 at 26.7%(n = 44/165), S1/M1 at 23.6%(n = 39/165) and S2/M1 at 4.8%(n = 8/165). Primary clarithromycin resistance was significantly lower in cag A-positive strains than in cag A-negative strains [32%(n = 8/25) vs 56.3%(n = 45/80); P = 0.03]. Similarly, in patients infected with more virulent H. pylori strains bearing the vac A s1 genotype, primary clarithromycin resistance was significantly lower than in those infected with less virulent strains bearing the vac A s2 genotype, [41%(n = 32/78) vs 77.8%(n = 21/27); P = 0.0001]. No statistically significant association was found between primary fluoroquinolone resistance and virulence factor status.CONCLUSION Genotypic H. pylori clarithromycin resistance is high and cag A-negative strains are dominant in our population. 13Safety and efficacy of VisuMax®circle patterns for flap creation and enhancement following small incision lenticule extraction显示文摘Background:The purpose of this case series is to evaluate the safety and efficacy of VisuMax®Circle patterns in eyes that have undergone small incision lenticule extraction,thus creating a flap to perform an enhancement procedure or residual lenticule extraction.Methods:This prospective,single center,case study series evaluated the use of a VisuMax®Circle pattern to create a corneal flap following small incision lenticule extraction.Patients were treated and followed at TRSC International LASIK Center(Bangkok,Thailand)for 3 months to assess the efficacy and safety of the procedure.Efficacy was determined by the surgeon’s ability to lift the created corneal flap.Results:The study enrolled 28 eyes.Twenty-seven underwent the VisuMax®Circle pattern procedure for refractive enhancement,and one for residual lenticule extraction.In 100%of cases(28 eyes)the lifting of the flap was possible,as planned.In all cases of refractive enhancement(27 eyes)by laser in situ keratomileusis(LASIK),the exposure of the stromal bed was sufficient for the necessary excimer laser ablation.No eyes lost two or more Snellen lines of corrected distance visual acuity(CDVA)and no procedure or flap-related complications or serious adverse events occurred.Conclusions:This initial case series demonstrates that VisuMax®Circle pattern is efficacious and a suitable method to create a corneal flap for enhancement,following small incision lenticule extraction.Ekktet Chansue Morakot Tanehsakdi Sukanda Swasdibutra Colm McAlinden 2015Eye and Vision2015,2,1:4

17Mismatch repair enzyme expression in primary and castrate resistant prostate cancer显示文摘Objective:Although the utility of immunohistochemistry(IHC)for assessing mismatch repair(MMR)protein expression has been demonstrated in solid tumors including primary prostate cancer(PCa),its utility has not been assessed in castration-resistant PCa(CRPC).Methods:Tissue microarrays were constructed from 127 radical prostatectomies and 155 CRPC metastases from 50 patients.MMR(MLH1,MSH2,MSH6,and PMS2)expression was assessed by IHC and gene expression arrays.Associations between MMR protein expression in PCa and CRPC and biochemical recurrence(BCR)or time from diagnosis to death respectively were determined.Results:There was no correlation between levels of MMR protein and BCR.Absence of MSH2 and MSH6 was the most pronounced at 15%and 22%in PCa and 17.8%and 16%in CRPC patients,respectively.MSH2 and MSH6 protein were absent in 9.4%and 8%of PCa and CRPC respectively.Absence of individual MMR proteins did not correlate with BCR or time from diagnosis to death.However absent MSH2/MSH6 in CRPC was associated with shorter time to death(pZ0.0006).Loss of MSH2 was verified at the gene expression level.This finding correlated with microsatellite instability previously reported in this CRPC cohort.Belinda Nghiem Xiaotun Zhang Hung-Ming Lam Lawrence DTrue Ilsa Coleman Celestia SHigano Peter SNelson Colin CPritchard Colm Morrissey 2016Asian Journal of Urology2016,3,4:2
