| 1 | Outcome of laparoscopic cholecystectomy is not influenced by chronological age in the elderly显示文摘AIM:To evaluate the outcome of laparoscopic cholecystectomy(LC)in patients aged 80 years and older. METHODS:A total of 353 patients aged 65 to 79 years(group 1)and 35 patients aged 80 years and older(group 2)underwent LC.Patients were further classified into two other groups:those with uncomplicated gallbladder disease(group A)or those with complicated gallbladder disease(group B). RESULTS:There were no significant differences between the age groups(groups 1 and 2)with respect to clinical characteristics such as age,gender,comorbid disease,or disease presentation.Mean operative time,conversion rate,and the incidence of major postoperative complications were similar in groups 1 and 2.However,the percentage of high-risk patients was significantly higher in group 2 than in group 1 (20.0%vs 5.7%,P<0.01).Group A comprised 322 patients with a mean age of 71.0±5.3 years,and group B comprised 51 patients with a mean age of 69.9±4.8 years.In group B,mean operative time (78.4±49.3 min vs 58.3±35.8 min,P<0.01),mean postoperative hospital stay(7.9±6.5 d vs 5.0±3.7 d, P<0.01),and the incidence of major postoperative complications(9.8%vs 3.1%,P<0.05)were significantly greater than in group A.The conversion rate tended to be higher in group B,but this difference was not significant. | 3 | Hwangryunhaedoktang exerts anti-inflammation on LPS-induced NO production by suppressing MAPK and NF-κB activation in RAW264.7 macrophages显示文摘OBJECTIVE: This study aimed to evaluate whether Hwangryunhaedoktang(HHT), a herbal compound, has an inhibitory effect on lipopolysaccharide(LPS)-induced inflammation in RAW264.7 macrophages.METHODS: The effects of HHT were evaluated by confirming nitric oxide(NO) production and expression of inducible NO synthase(iN OS) and mitogen-activated protein kinases(MAPKs) in LPS-stimulated RAW264.7 macrophages via the Griess assay, Western blotting, and real-time reverse transcription quantitative polymerase chain reaction. Western blot analyses and luciferase assays were used to evaluate whether HHT has an effect on the phosphorylation and translocation of nuclear factor-κB(NF-κB). The secretion and expression of tumor necrosis factor-α(TNF-α) and interleukin-6(IL-6) were determined via enzyme-linked immunosorbent assay and Western blot analyses.RESULTS: HHT suppressed LPS-induced NO production and expression of iN OS in a dose-dependent manner. Additionally, MAPKs activation was also attenuated via inhibition of phosphorylation of extracellular signal-regulated kinases 1/2, c-Jun N-terminal kinase and p38 which were related to inflammatory pathway. Furthermore, HHT also effectively attenuated NF-κB activation and its translocation to the nucleus, a process that is closely linked to inflammation. LPS normally induced the expression of inflammatory cytokines such as TNF-α and IL-6, but the secretion and expression of TNF-α and IL-6 were significantly attenuated by pretreating the cells with HHT.CONCLUSION: HHT suppressed LPS-induced NO production by blocking the activation of NF-κB and MAPK signaling pathways in RAW264.7 macrophages. Furthermore, HHT may have an anti-inflammatory effect by suppressing the LPS-induced secretion of TNF-α and IL-6. | 5 | Characteristics of nonvariceal upper gastrointestinal hemorrhage in patients with chronic kidney disease显示文摘AIM:To evaluate the clinical characteristics of nonvariceal upper gastrointestinal hemorrhage(NGIH)in patients with chronic kidney disease(CKD).METHODS:From 2003 to 2010,a total of 72 CKD patients(male n=52,72.2%;female n=20,27.8%)who had undergone endoscopic treatments for NGIH were retrospectively identified.Clinical findings,endoscopic features,prognosis,rebleeding risk factors,and mortality-related factors were evaluated.The characteristics of the patients and rebleeding-related data were recorded for the following variables:gender,age,alcohol use and smoking history,past hemorrhage history,endoscopic findings(the cause,location,and size of the hemorrhage and the hemorrhagic state),therapeutic options for endoscopy,endoscopist experience,clinical outcomes,and mortality.RESULTS:The average size of the hemorrhagic site was 13.7±10.2 mm,and the most common hemorrhagic site in the stomach was the antrum(n=21,43.8%).The most frequent method of hemostasis was combination therapy(n=32,44.4%).The incidence of rebleeding was 37.5%(n=27),and 16.7%(n=12)of patients expired due to hemorrhage.In a multivariate analysis of the risk factors for rebleeding,alcoholism(OR=11.19,P=0.02),the experience of endoscopists(OR=0.56,P=0.03),and combination endoscopic therapy(OR=0.06,P=0.01)compared with monotherapy were significantly related to rebleeding after endoscopic therapy.In a risk analysis of mortality after endoscopic therapy,only rebleeding was related to mortality(OR=7.1,P=0.02).CONCLUSION:Intensive combined endoscopic treatments by experienced endoscopists are necessary for the treatment of NGIH in patients with CKD,especially when a patient is an alcoholic. | Chang Seok Bang Yong Seop Lee Yun Hyeong Lee Hotaik Sung Hong Jun Park Hyun Soo Kim Jin Bong Kim Gwang Ho Baik Yeon Soo Kim Jai Hoon Yoon Dong Joon Kim Ki Tae Suk | 2013 | World Journal of Gastroenterology2013，19，43： | 3 |
| 6 | Analysis on sports and recreation activity-related eye injuries presenting to the Emergency Department显示文摘AIM:To investigate the incidence and general characteristics of sports-related eye injuries in patients visiting the Emergency Department. METHODS: A cross-sectional,multi-center,observational study.Patients with an injured eye who visited the Emergency Department at one of nine hospitals in Korea were enrolled.All data were prospectively collected between March and September 2010 using a questionnaire.Eye injuries that occurred during risky sports were examined by gender and age.Additionally,the rate of open globe injuries that occurred with and without protective eyewear was examined for each activity.Continuous variables were compared using Student's t-test and categorical variables were compared using Chi-square test.RESULTS: A total of 446 patients had sports-related eye injuries.Teenagers(10-19 years old) and young adults(20-29 years old) had the most eye injuries.Eye injuries accounted for 0.2% of Emergency Department patients.Baseball was the most common cause of sports-related eye injuries,followed by soccer and hiking.Protective gear was worn by 9.4% of all patients.Patients that were 30-39 years of age had the highest rate of protective gear use,followed by patients that were 40-49 years of age.The proportion of sports-related eye injuries that were open-globe injuries was highest for soccer and hiking.CONCLUSION: Although injuries were most common in patients below the age of 10 years,these patients had the lowest rate of protective eyewear use.Injuries in adults over 40 years of age most commonly occurred during hiking,but the rate of protective eyewear use was low.Young athletes should be educated on and provided with protective eyewear and policies protective gear use should be established.For older adults,eye protection should be encouraged,especially during hiking. | Sungbae Moon Hyun Wook Ryoo Jae Yun Ahn Jung Bae Park Kang Suk Seo Sang Do Shin Kyoung Jun Song Kang Hyun Lee In Sool Yoo Jin Seong Cho Hyun Ho Ryu Tae Oh Jeong Seok Ran Yeom Young Taek Kim Sung Ok Hong | 2016 | International Journal of Ophthalmology(English edition)2016，9，10： | 3 |

| 8 | Ameliorating liver fibrosis in an animal model using the secretome released from miR-122-transfected adipose-derived stem cells显示文摘BACKGROUND Recently,the exclusive use of mesenchymal stem cell(MSC)-secreted molecules,called secretome,rather than cells,has been evaluated for overcoming the limitations of cell-based therapy,while maintaining its advantages.However,the use of na?ve secretome may not fully satisfy the specificity of each disease.Therefore,it appears to be more advantageous to use the functionally reinforced secretome through a series of processes involving physico-chemical adjustments or genetic manipulation rather than to the use na?ve secretome.AIM To determine the therapeutic potential of the secretome released from miR-122-transfected adipose-derived stromal cells(ASCs).METHODS We collected secretory materials released from ASCs that had been transfected with antifibrotic miR-122(MCM)and compared their antifibrotic effects with those of the na?ve secretome(CM).MCM and CM were intravenously administered to the mouse model of thioacetamide-induced liver fibrosis,and their therapeutic potentials were compared.RESULTS MCM infusion provided higher therapeutic potential in terms of:(A)Reducing collagen content in the liver;(B)Inhibiting proinflammatory cytokines;and(C)Reducing abnormally elevated liver enzymes than the infusion of the na?ve secretome.The proteomic analysis of MCM also indicated that the contents of antifibrotic proteins were significantly elevated compared to those in the na?ve secretome.CONCLUSION We could,thus,conclude that the secretome released from miR-122-transfected ASCs has higher antifibrotic and anti-inflammatory properties than the na?ve secretome.Because miR-122 transfection into ASCs provides a specific way of potentiating the antifibrotic properties of ASC secretome,it could be considered as an enhanced method for reinforcing secretome effectiveness. | Kee-Hwan Kim Jae Im Lee Ok-Hee Kim Ha-Eun Hong Bong Jun Kwak Ho Joong Choi Joseph Ahn Tae Yun Lee Sang Chul Lee Say-June Kim | 2019 | World Journal of Stem Cells2019，11，11： | 2 |
