1Management of postoperative spinal infections显示文摘Postoperative surgical site infection(SSI) is a common complication after posterior lumbar spine surgery. This review details an approach to the prevention,diagnosis and treatment of SSIs. Factors contributing to the development of a SSI can be split into three categories:(1) microbiological factors;(2) factors related to the patient and their spinal pathology; and(3) factors relating to the surgical procedure. SSI is most commonly caused by Staphylococcus aureus. The virulence of the organism causing the SSI can affect its presentation. SSI can be prevented by careful adherence to aseptic technique,prophylactic antibiotics,avoiding myonecrosis by frequently releasing retractors and preoperatively optimizing modifiable patient factors. Increasing pain is commonly the only symptom of a SSI and can lead to a delay in diagnosis. C-reactive protein and magnetic resonance imaging can help establish the diagnosis. Treatment requires acquiring intra-operative cultures to guide future antibiotic therapy and surgical debridement of all necrotic tissue. A SSI can usually be adequately treated without removing spinalinstrumentation. A multidisciplinary approach to SSIs is important. It is useful to involve an infectious disease specialist and use minimum serial bactericidal titers to enhance the effectiveness of antibiotic therapy. 3High-flow nasal oxygen availability for sedation decreases the use of general anesthesia during endoscopic retrograde cholangiopancreatography and endoscopic ultrasound显示文摘AIM To examine whether high-flow nasal oxygen(HFNO) availability influences the use of general anesthesia(GA) in patients undergoing endoscopic retrograde cholangiopancreatography(ERCP) and endoscopic ultrasound(EUS) and associated outcomes.METHODS In this retrospective study, patients were stratified into 3 eras between October 1, 2013 and June 30, 2014 based on HFNO availability for deep sedation at the time of their endoscopy. During the first and last 3-mo eras(era 1 and 3), no HFNO was available, whereas it was an option during the second 3-mo era(era 2). The primary outcome was the percent utilization of GA vs deep sedation in each period. Secondary outcomes included oxygen saturation nadir during sedation between periods, as well as procedure duration, and anesthesia-only time between periods and for GA vs sedation cases respectively.RESULTS During the study period 238 ERCP or EUS cases were identified for analysis. Statistical testing was employed and a P < 0.050 was significant unless the Bonferroni correction for multiple comparisons was used. General anesthesia use was significantly lower in era 2 compared to era 1 with the same trend between era 2 and 3(P = 0.012 and 0.045 respectively). The oxygen saturation nadir during sedation was significantly higher in era 2 compared to era 3(P < 0.001) but not between eras 1 and 2(P = 0.028) or 1 and 3(P = 0.069). 4盐酸二甲双胍缓释片的溶胀机制与释放规律相关性研究显示文摘以羟丙甲纤维素(HPMC)为骨架材料,采用正交设计优化盐酸二甲双胍缓释片的处方。所得优化处方为:HPMCK100M-药物重量比为0.25∶1,聚乙烯吡咯烷酮(PVP)K90为黏合剂。测定了优化制品在不同pH介质(pH 1.0盐酸、pH 4.5乙酸盐缓冲液和pH 7.4磷酸盐缓冲液)中的溶胀因子和体外释放行为,并用不同模型拟合,继而考察药物释放与缓释片溶胀因子间的相关性。研究表明,所得制品的溶胀与体外释药况均符合Korsmeyer-Peppas模型;药物释放机制为Fick扩散。释药行为不依赖于介质pH的改变,且骨架片的溶胀对药物的释放起主导作用。本品中药物释放与骨架溶胀间具有良好的线性关系。朱芳芳 高慧升 KULKARNI Anand HEGDE Deepak 涂家生 2013中国医药工业杂志2013,44,4:6

6Successful endoscopic treatment of an intraductal papillary neoplasm of the bile duct显示文摘We present a case of a 76-year-old man with right upper quadrant abdominal pain and weight loss,who was found to have an intraductal papillary neoplasm of the bile duct(IPNB)of the pancreaticobiliary subtype,deemed curatively resectable.The patient declined surgery and opted for endoscopic therapy.He underwent two sessions of endoscopic retrograde cholangiopancreatography(ERCP)-guided radiofrequency ablation(RFA).Ten months later,no evidence of recurrence was identified on repeat ERCP.To our knowledge,this is the first reported case of successful use of RFA as a primary treatment modality for resectable IPNB.Nikola S Natov Laura C Horton Sanjay R Hegde 2017World Journal of Gastrointestinal Endoscopy2017,9,5:5
7Effect of TiO_2 nanoparticles on hydrogen evolution reaction activity of Ni coatings显示文摘The electrocatalytic activity of electrodeposited Ni and Ni–TiO_2 coatings with regard to the alkaline hydrogen evolution reaction(HER) was investigated. The Ni coatings were electrodeposited from an acid chloride bath at different current densities, and their HER activities were examined in a 1.0-mol·L^(-1) KOH medium. The variations in the HER activity of the Ni coatings with changes in surface morphology and composition were examined via the electrochemical dissolution and incorporation of nanoparticles. Electrochemical analysis methods were used to monitor the HER activity of the test electrodes; this activity was confirmed via the quantification of gases that evolved during the analysis. The obtained results demonstrated that the Ni–TiO_2 nanocomposite test electrode exhibited maximum activity toward the alkaline HER. The surface appearance, composition, and the phase structure of all developed coatings were analyzed using scanning electron microscopy(SEM), energy dispersive spectroscopy(EDS), and X-ray diffraction(XRD), respectively. 9Investigations on Heat Transfer Enhancement in Pool Boiling with Water-CuO Nano-Fluids显示文摘The main focus of the present work is to investigate Critical Heat Flux (CHF) enhancement using CuO nanofluid relative to CHF of pure water. To estimate the effect of nanoparticles on the CHF, pool boiling CHF values were measured for various volume concentrations of CuO nanofluid and compared with pure water. CHF enhancement of 130% was recorded at 0.2 % by volume of CuO nano-fluids. Surface roughness of the heater surface exposed to three measured heating cycles indicated surface modifications at different volume concentrations of nanofluid. 16Sulfonic acid-functionalized mesoporous silica catalyst with different morphology for biodiesel production显示文摘Sulfonic acid functionalized mesoporous silica based solid acid catalysts with different morphology were designed and fabricated.The synthesized materials were characterized by various physicochemical and spectroscopic techniques like scanning electron microscopeenergy dispersive X-ray spectroscopy,Fourier transform infrared spectroscopy,Brunauer–Emmett–Teller surface area,thermogravimetric analysis and n-butylamine acidity.The shape of catalysts particles plays an important role in its activity.The sulfonic acid functionalized mesoporous silica catalysts of spherical shape and the cube shape were assessed for catalytic activity in biodiesel production.The catalytic biodiesel production reaction over the catalysts were studied by esterification of free fatty acid,oleic acid with methanol.The effect of various reaction parameters such as catalyst concentration,acid/alcohol molar ratio,catalyst amount,reaction temperature and reaction time on catalytic activity were investigated to optimize the conditions for maximum conversion.It was sulfonated cubic shape mesoporous silica which exhibited better activity as compared to the spherical shape silica catalysts.Additionally,the catalyst was regenerated and reused up to three cycles without any significant loss in activity.The present catalysts exhibit superior performance in biodiesel production and it can be used for the several biodiesel feedstock’s that are rich in free fatty acids.Vinayak Hegde Parimal Pandit Pranita Rananaware Varsha PBrahmkhatri 2022Frontiers of Chemical Science and Engineering2022,16,8:2
