1Phase 1 human trial of autologous bone marrow-hematopoietic stem cell transplantation in patients with decompensated cirrhosis显示文摘AIM: To evaluate safety and feasibility of autologous bone marrow-enriched CD34+ hematopoietic stem cell Tx through the hepatic artery in patients with decompensated cirrhosis.METHODS: Four patients with decompensated cirrhosis were included. Approximately 200 mL of the bone marrow of the patients was aspirated, and CD34+ stem cells were selected. Between 3 to 10 million CD34+ cells were isolated. The cells were slowly infused through the hepatic artery of the patients.RESULTS: Patient 1 showed marginal improvement in serum albumin and no significant changes in other test results. In patient 2 prothrombin time was decreased; however, her total bilirubin, serum creatinine, and Model of End-Stage Liver Disease (MELD) score worsened at the end of follow up. In patient 3 there was improvement in serum albumin, porthrombin time (PT), and MELD score. Patient 4 developed radiocontrast nephropathy after the procedure, and progressed to type 1 hepatorenal syndrome and died of liver failure a few days later. Because of the major side effects seen in the last patient, the trial was prematurely stopped.CONCLUSION: Infusion of CD34+ stem cells through the hepatic artery is not safe in decompensated cirrhosis. Radiocontrast nephropathy and hepatorenal syndrome could be major side effects. 2Hepatitis B and C virus-induced hepatitis: Apoptosis, autophagy, and unfolded protein response显示文摘AIM: To investigate the co-incidence of apoptosis, autophagy, and unfolded protein response(UPR) in hepatitis B(HBV) and C(HCV) infected hepatocytes.METHODS: We performed immunofluorescence confocal microscopy on 10 liver biopsies from HBV and HCV patients and tissue microarrays of HBV positive liver samples. We used specific antibodies for LC3β, cleaved caspase-3, BIP(GRP78), and XBP1 to detect autophagy, apoptosis and UPR, respectively. AntiHCV NS3 and anti-HBs antibodies were also used to confirm infection. We performed triple blind counting of events to determine the co-incidence of autophagy(LC3β punctuate), apoptosis(cleaved caspase-3), and unfolded protein response(GRP78) with HBV and HCV infection in hepatocytes. All statistical analyses were performed using SPSS software for Windows(Version 16 SPSS Inc, Chicago, IL, United States). P-values < 0.05 were considered statistically significant. Statistical analyses were performed with Mann-Whitney test to compare incidence rates for autophagy, apoptosis, and UPR in HBV- and HCV-infected cells and adjacent noninfected cells.RESULTS: Our results showed that infection of hepatocytes with either HBV and HCV induces significant increase(P < 0.001) in apoptosis(cleavage of caspase-3), autophagy(LC3β punctate), and UPR(increase in GRP78 expression) in the HCV- and HBVinfected cells, as compared to non-infected cells of the same biopsy sections. Our tissue microarray immunohistochemical expression analysis of LC3β in HBV^(Neg) and HBV^(Pos) revealed that majority of HBVinfected hepatocytes display strong positive stainingfor LC3β. Interestingly, although XBP splicing in HBVinfected cells was significantly higher(P < 0.05), our analyses show a slight increase of XBP splicing was in HCV-infected cells(P > 0.05). Furthermore, our evaluation of patients with HBV and HCV infection based on stage and grade of the liver diseases revealed no correlation between these pathological findings and induction of apoptosis, autophagy, and UPR.CONCLUSION: The results of this study indicate that HCV and HBV infection activates apoptosis, autophagy and UPR, but slightly differently by each virus. 4Predictive value and main determinants of abnormal features of intraoperative cholangiography during cholecystectomy显示文摘BACKGROUND:The major issue with intraoperative cholangiography (IOC) is whether its diagnostic accuracy for common bile duct (CBD) stones matches that of other diagnostic procedures,and thus,whether it will become a routine diagnostic procedure.The current study aimed to address the main determinants of CBD stone diagnosis in IOC among an Iranian population.METHODS:In a retrospective review database-based study conducted in Taleghani Hospital in Tehran between 2006 and 2008,baseline data and perioperative information of 2060 patients (male to female ratio 542:1518,mean age 53.7 years) who were candidates for cholecystectomy and underwent concomitant IOC for confirming CBD stones were reviewed.The predictive power of this procedure for diagnosis of abnormal biliary ducts with the focus on biliary stones was determined.RESULTS:Overall mortality and morbidity following cholecystectomy in the study population were 0.6% and 2.6%,respectively.Both early mortality and morbidity due to cholecystectomy were higher in male than female.The prevalence of CBD stones in IOC was 3.4% (5.2% in male and 2.8% in female,P=0.008).Among those without gallstones,8.7% had CBD stones and only 3.1% had concomitant gallstones and CBD stones.The main predictors of stone appearance as an abnormal feature of IOC during cholecystectomy were:advanced age (OR=1.022,P=0.001),male gender (OR=1.498,P=0.050),history of abdominal surgery (OR=1.543,P=0.040) and preoperative endoscopic retrograde cholangiopancreatography (OR=5.400,P<0.001).CONCLUSIONS:IOC is a safe and accurate method for the assessment of bile duct anatomy and stones.Therefore,the routine use of IOC within cholecystectomy seems reasonable and is recommended.Shahram Yousefpour Azary Heshmat Kalbasi Ali Setayesh Mirhadi Mousavi Asad Hashemi Mahsa Khodadoostan Mohammad Reza Zali Amir Houshang Mohammad Alizadeh 2011Hepatobiliary & Pancreatic Diseases International2011,10,3:11
5Microstructure and mechanical properties of Al/Cu/Mg laminated composite sheets produced by the ARB proces显示文摘In the present study, an Al/Cu/Mg multi-layered composite was produced by accumulative roll bonding(ARB) through seven passes, and its microstructure and mechanical properties were evaluated. The microstructure investigations show that plastic instability occurred in both the copper and magnesium reinforcements in the primary sandwich. In addition, a composite with a perfectly uniform distribution of copper and magnesium reinforcing layers was produced during the last pass. By increasing the number of ARB cycles, the microhardness of the layers including aluminum, copper, and magnesium was significantly increased. The ultimate tensile strength of the sandwich was enhanced continually and reached a maximum value of 355.5 MPa. This strength value was about 3.2, 2, and 2.1 times higher than the initial strength values for the aluminum, copper, and magnesium sheets, respectively. 7TIRAP rs8177374 gene polymorphism increased the risk of pulmonary tuberculosis in Zahedan,southeast Iran显示文摘Objective:To evaluate the possible association between Toll-interleukin 1 receptor(TIR) domain containing adaptor protein(TIRAP;also known as MAI.) rsl893352 and rs8l77374(S180L) gene polymorphisms and pulmonary tuberculosis(PTB) in a sample of Iranian population.Methods:This case-control study was performed on 174 PTB and 177 healthy subjects.Tetra amplification refractory mutation systetn-polymerase chain reaction(T-AKMS-PCR) was used to detect the polymorphisms.Results:Our finding showed that neither the overall Ckisqaare comparison of PTB and control subjects nor the logistic regression analysis indicated any association between rsl893352 polymorphism and PTB.Regarding rs8l77374 polymorphism,the CT genotype as well as CT+TT increased the risk of PTB in comparison with CC genotype(OR=4.73,95%CI=2.65-8.45,P<0.0001 and OR=6.47,95%C1=3.68-11.38,P<0.0001.respectively).The rs8177374 T allele increased the risk of PTB in comparison with C allele(OR=4.21.95%CI=2.43-7.26,P<0.0001).Conclusions:Our finding indicates that TIRAP rs8177374 polymorphism is associated with PTB in a sample of Iranian population.Mohammad Naderi Mohammad Hashemi Zahra Pourmontaseri Ebrahim Eskari-Nasab Gholamreza Bahari Mohsen Taheri 2014Asian Pacific Journal of Tropical Medicine2014,7,6:8
8An extended treatment protocol with pegylated interferon and ribavirin for hepatitis C recurrence after liver transplantation显示文摘AIM: To evaluate the efflicacy and tolerability of an extended treatment protocol and to determine the predictors of sustained virological response (SVR) after liver transplantation (LT).METHODS: Between August 2005 and November 2008, patients with recurrent hepatitis C virus (HCV) after LT were selected for treatment if liver biopsy showed at least grade 2 inflammation and/or stage 2 flibrosis. All patients were to receive pegylated interferon (PEG)/regimens combining ribavirin (RBV) for an additional 48 wk after HCV undetectability.RESULTS: Extended protocol treatment was initiated in thirty patients. Overall, 73% had end of treatmentresponse and 60% had SVR. Nineteen patients completed treatment per protocol, of them, sixteen (84%) had end of treatment response, and fourteen (74%) achieved SVR. Both early virological response and 24-week virological response were individually associated with SVR but this association was not signif icant on multivariate analysis. Eleven patients (37%) discontinued therapy due to adverse effects. Cytopenias were the most common and most severe adverse effect, and required frquent growth factor use, dose adjustments and treatment cessations. 11Synthesis and antibacterial activity of Schiff bases of 5-substituted isatins显示文摘Based on the existing reports on the bioactive isatin derivatives, a number of Schiff bases were synthesized by reacting 5-substituted isatins with bioactive amines/hydrazides and their structures were confirmed using spectroscopic methods such as NMR, IR and mass spectrometry. Furthermore, Nbenzylation of isatin followed by the Schiff base formation furnished a new series of compounds(11a–13c) which allowed the analysis of the effect of isatin N-substitution on the bioactivity of the resulting compounds. The antibacterial activity of the synthesized derivatives was evaluated using a microtiter plate method on a series of gram positive and gram negative bacterial strains. Compounds 2d, 3b, 5c and 6a were among the most potent derivatives against Pseudomonas aeruginosa(MIC = 6.25 μg/m L).Analysis of the structure–activity relationship showed that the incorporation of(thio)urea-based Schiff bases lead to more potent derivatives with a broader spectrum of antibacterial activity. In addition,highly lipophilic compounds such as 11a–12c did not show any measurable antibacterial activity, which implies that an optimal lipophilicity might be an important requirement for the antibacterial activity of the studied isatins. 12Myricetin protects hippocampal CA3 pyramidal neurons and improves learning and memory impairments in rats with Alzheimer's disease显示文摘There is currently no treatment for effectively slowing the progression of Alzheimer's disease, so early prevention is very important. Numerous studies have shown that flavonoids can improve memory impairment. The present study investigated the effects of myricetin, a member of the flavonoids, on intracerebroventricular streptozotocin induced neuronal loss and memory impairment in rat models of Alzheimer's disease. Myricetin at 5 or 10 mg/kg was intraperitoneally injected into rats over 21 days. Control rats were treated with 10 m L/kg saline. Behavioral test(the shuttle box test) was performed on day 22 to examine learning and memory in rats. Immediately after that, hematoxylin-eosin staining was performed to observe the morphological change in hippocampal CA3 pyramidal neurons. Myricetin greatly increased the number of hippocampal CA3 pyramidal neurons and improved learning and memory impairments in rats with Alzheimer's disease. 15Evaluation of integration methods for hybrid simulation of complex structural systems through collapse显示文摘This study examines the performance of integration methods for hybrid simulation of large and complex structural systems in the context of structural collapse due to seismic excitations.The target application is not necessarily for real-time testing,but rather for models that involve large-scale physical sub-structures and highly nonlinear numerical models.Four case studies are presented and discussed.In the first case study,the accuracy of integration schemes including two widely used methods,namely,modified version of the implicit Newmark with fixed-number of iteration(iterative)and the operator-splitting(non-iterative)is examined through pure numerical simulations.The second case study presents the results of 10 hybrid simulations repeated with the two aforementioned integration methods considering various time steps and fixed-number of iterations for the iterative integration method.The physical sub-structure in these tests consists of a single-degree-of-freedom(SDOF)cantilever column with replaceable steel coupons that provides repeatable highlynonlinear behavior including fracture-type strength and stiffness degradations.In case study three,the implicit Newmark with fixed-number of iterations is applied for hybrid simulations of a 1:2 scale steel moment frame that includes a relatively complex nonlinear numerical substructure.Lastly,a more complex numerical substructure is considered by constructing a nonlinear computational model of a moment frame coupled to a hybrid model of a 1:2 scale steel gravity frame.The last two case studies are conducted on the same porotype structure and the selection of time steps and fixed number of iterations are closely examined in pre-test simulations.The generated unbalance forces is used as an index to track the equilibrium error and predict the accuracy and stability of the simulations.Maikol Del Carpio R. 17Analysis of the formation conditions and characteristics of interphase and random vanadium precipitation in a low-carbon steel during isothermal heat treatment显示文摘The characteristics of nanosized precipitates in steels depend on the heat-treatment parameters. The effects of characteristics of vanadium precipitates formed during isothermal heat treatment on the hardness of the ferrite matrix in low-carbon vanadium-alloyed steel were investigated through analysis of transmission electron microscopy images and microhardness measurements. The results show that, during isothermal holding in the temperature range from 675 to 750℃, only interphase precipitation occurs, whereas only random precipitation occurs in the ferrite matrix during holding at 600℃. Furthermore, during isothermal heat treatment between 600 and 675℃, both random and interphase precipitates occurred in the ferrite. Nanoscale vanadium carbides with different atomic ratios of vanadium(V) and carbon(C) were the dominant precipitates in the random and interphase precipitates. The sizes of random precipitation carbides were smaller than those of interphase ones. 18Interactive effects of drought stress and chitosan application on physiological characteristics and essential oil yield of Thymus daenensis Celak显示文摘Thymus daenensis, a perennial herb, is often grown in areas that experience drought conditions during its growing period. Application of chitosan may compensate for the negative impact of drought stress on the yield of oil and secondary metabolites in Thymus.The interactive effects of foliar application of chitosan and drought stress on dry matter,essential oil yield, and selected physiological characteristics including photosynthetic pigments, osmotic adjustment, and lipid peroxidation of Thymus were investigated in a two-year study from 2014 to 2015. Treatments consisted of 0, 200, and 400(iL L'1 chitosan applied to plants grown under field capacity, mild drought stress(50% field capacity), and severe drought stress(25% field capacity). Dry matter yield decreased substantially as drought stress intensified. However, essential oil content increased under stress conditions,with the highest essential oil yield obtained from plants under mild drought stress. Foliar application of chitosan compensated to some extent for dry matter and oil yield reduction of plants grown under drought stress. The highest essential oil yield(1.52 g plant-1) was obtained by application of 400 \iL L_1 chitosan under the mild stress condition in 2015 when plants were mature. 19Implementation of multilayer perceptron(MLP)and radial basis function(RBF)neural networks to predict solution gas-oil ratio of crude oil systems显示文摘Exact determination of pressure-volume-temperature(PVT)properties of the reservoir oils is necessary for reservoir calculations,reservoir performance prediction,and the design of optimal production conditions.The objective of this study is to develop intelligent and reliable models based on multilayer perceptron(MLP)and radial basis function(RBF)neural networks for estimating the solution gas–oil ratio as a function of bubble point pressure,reservoir temperature,oil gravity(API),and gas specific gravity.These models were developed and tested using a total of 710 experimental data sets representing the samples of crude oil from various geographical locations around the world.Performance of the developed MLP and RBF models were evaluated and investigated against a number of well-known empirical correlations using statistical and graphical error analyses.The results indicated that the proposed models outperform the considered empirical correlations,providing a strong agreement between predicted and experimental values,However,the developed RBF exhibited higher accuracy and efficiency compared to the proposed MLP model.Aref Hashemi Fath Farshid Madanifar Masood Abbasia 2020Petroleum2020,6,1:3
20Development of an artificial neural network model for prediction of bubble point pressure of crude oils显示文摘Bubble point pressure is one of the most important pressureevolumeetemperature properties of crude oil,and it plays an important role in reservoir and production engineering calculations.It can be precisely determined experimentally.Although,experimental methods present valid and reliable results,they are expensive,time-consuming,and require much care when taking test samples.Some equations of state and empirical correlations can be used as alternative methods to estimate reservoir fluid properties(e.g.,bubble point pressure);however,these methods have a number of limitations.In the present study,a novel numerical model based on artificial neural network(ANN)is proposed for the prediction of bubble point pressure as a function of solution gaseoil ratio,reservoir temperature,oil gravity(API),and gas specific gravity in petroleum systems.The model was developed and evaluated using 760 experimental data sets gathered from oil fields around the world.An optimization process was performed on networks with different structures.Based on the obtained results,a network with one hidden layer and six neurons was observed to be associated with the highest efficiency for predicting bubble point pressure.The obtained ANN model was found to be reliable for the prediction of bubble point pressure of crude oils with solution gaseoil ratios in the range of 8.61e3298.66 SCF/STB,temperatures between 74 and 341.6F,oil gravity values of 6e56.8 API and gas gravity values between 0.521 and 3.444.The performance of the developed model was compared against those of several well-known predictive empirical correlations using statistical and graphical error analyses.The results showed that the proposed ANN model outperforms all of the studied empirical correlations significantly and provides predictions in acceptable agreement with experimental data.Aref Hashemi Fath Abdolrasoul Pouranfard Pouyan Foroughizadeh 2018Petroleum2018,4,3:3
