| 2 | Liver transplantation in patients with incidental hepatocellular carcinoma/cholangiocarcinoma and intrahepatic cholangiocarcinoma: a single-center experience显示文摘BACKGROUND: Reports of liver transplantation(LT) in patients with mixed hepatocellular carcinoma/cholangiocarcinoma(HCC/CC) and intrahepatic cholangiocarcinoma(ICC) are modest and have been mostly retrospective after pathological categorization in the setting of presumed HCC. Some studies suggest that patients undergoing LT with small and unifocal ICC or mixed HCC/CC can achieve about 40%-60% 5-year post-transplant survival. The study aimed to report our experience in patients undergoing LT with explant pathology revealing HCC/CC and ICC.METHODS: From a prospectively maintained database, we performed cohort analysis. We identified 13 patients who underwent LT with explant pathology revealing HCC/CC or ICC. RESULTS: The observed recurrence rate post-LT was 31%(4/13) and overall survival was 85%, 51%, and 51% at 1, 3 and 5 years, respectively. Disease-free survival was 68%, 51%, and 41% at 1, 3 and 5 years, respectively. In our cohort, four patients would have qualified for exception points based on updated HCC Organ Procurement and Transplantation Network imaging guidelines. CONCLUSIONS: Lesions which lack complete imaging characteristics of HCC may warrant pre-LT biopsy to fully elucidate their pathology. Identified patients with early HCC/CC or ICC may benefit from LT if unresectable. | 5 | Endoscopic assessment and management of early esophageal adenocarcinoma显示文摘Esophageal carcinoma affects more than 450000people worldwide and the incidence is rapidly increasing.In the United States and Europe,esophageal adenocarcinoma has superseded esophageal squamous cell carcinoma in its incidence.Esophageal cancer has a high mortality rates secondary to the late presentation of most patients at advanced stages.Endoscopic screening is recommended for patients with multiple risk factors for cancer in Barrett’s esophagus.These risk factors include chronic gastroesophageal reflux disease,hiatal hernia,advanced age,male sex,white race,cigarette smoking,and obesity.The annual risk of esophageal cancer is approximately 0.25%for patients without dysplasia and 6%for patients with high-grade dysplasia.Twenty percent of all esophageal adenocarcinoma in the United States is early stage with disease confined to the mucosa or submucosa.The significant morbidity and mortality of esophagectomy make endoscopic treatment an attractive option.The American Gastroenterological Association recommends endoscopic eradication therapy for patients with high-grade dysplasia.Endoscopic modalities for treatment of early esophageal adenocarcinoma include endoscopic resection techniques and endoscopic ablative techniques such as radiofrequency ablation,photodynamic therapy and cryoablation.Endoscopic therapy should be precluded to patients with no evidence of lymphovascular invasion.Local tumor recurrence is low after endoscopic therapy and is predicted by poor differentiation of tumor,positive lymph node and submucosal invasion.Surgical resection should be offered to patients with deep submucosal invasion. | Ghassan M Hammoud Hazem Hammad Jamal A Ibdah | 2014 | World Journal of Gastrointestinal Oncology2014,6,8: | 4 |
| 6 | Colon mucosal neoplasia referred for endoscopic mucosal resection:Recurrence of adenomas and prediction of submucosal invasion显示文摘BACKGROUND Endoscopic mucosal resection(EMR)is an effective and minimally invasive alternative to surgery for large polyps and laterally spreading lesions.Gross morphology and surface characteristics may help predict submucosal invasion of the lesion(SMIL)during endoscopic evaluation.This is one of the largest singlecenter studies reporting endoscopic mucosal resection for larger(≥20 mm)colorectal lesions in the United States.AIM To determine the recurrence rate of adenomas and endoscopic features that may predict submucosal invasion of colonic mucosal neoplasia.METHODS This is a retrospective cohort study of all the patients referred for endoscopic mucosal resection for lesions≥20 mm,spanning a period from January 2013 to February 2017.The main outcome measure was identifying features that may predict submucosal invasion of mucosal lesions and predict recurrence of adenomas on follow-up surveillance colonoscopy performed at 4-6 mo.RESULTS A total of 480 patients with 500 lesions were included in the study.The median age was 68(Inter quantile range:14)with 52%males.The most common lesion location was ascending colon(161;32%).Paris classification 0-IIa(Flat elevation of mucosa-316;63.2%);Kudo Pit Pattern IIIs(192;38%)and Granular surface morphology(260;52%)were most prevalent.Submucosal invasion was present in 23(4.6%)out of 500 lesions.The independent risk factors for SMIL were Kudo Pit Pattern IIIL+IV and V(Odds ratio:4.5;P value<0.004)and Paris classification 0-IIc(Odds ratio:18.2;P value<0.01).Out of 500,354 post-endoscopic mucosal resection scars were examined at surveillance colonoscopy.Recurrence was noted in 21.8%(77 cases).CONCLUSION There was overall low prevalence of SMIL in our study.Kudo pit pattern(IIIL+IV and V)and Paris classification 0-IIc were the only factors identified as an independent risk factor for submucosal invasion.The independent risk factor for recurrence was adenoma size(>40 mm).Almost all recurrences(98.8%)were treated endoscopically. | Mamoon Ur Rashid Neelam Khetpal Hammad Zafar Saeed Ali Evgeny Idrisov Yuan Du Assaf Stein Deepanshu Jain Muhammad Khalid Hasan | 2020 | World Journal of Gastrointestinal Endoscopy2020,12,7: | 3 |
| 7 | Forest Cover Change Assessment in Conflict-affected Areas of Northwest Pakistan: the Case of Swat and Shangla Districts显示文摘This study examines the spatial and temporal forest cover changes in Swat and Shangla districts to understand the deforestation pattern in context of the recent security conflict in these districts. We used multi-resolution satellite images to assess the long term deforestation from 2001 to 2009 and also to identify episodic forest cutting areas appeared during the conflict period of Oct. 2007 - Oct. 2008. There are only 58 ha of deforestation identified during the conflict period while 1268 ha of gross annual deforestation were assessed during last eight years. Most of the deforestation patches persist around the administrative boundaries at sub-district levels (tehsils) which can be attributed to ambiguity in unclear jurisdiction between the forest official. The results highlight that the forest cutting appeared in Swat and Shangla during the conflict period is not as significant when compared with the long term deforestation pattern in the area. On the one side the results of the study are supportive to the picture that emerges from international studies which report high rate of deforestation in the country and on the other side it negates any relation between the security situation and the increasing deforestation in the north western Pakistan. | 9 | Predictions of future hydrological conditions and contribution of snow and ice melt in total discharge of Shigar River Basin in Central Karakoram,Pakistan显示文摘The high mountains of Hindu-Kush Karakoram and Himalaya(HKKH) contain a large volume of snow and ice, which are the primary sources of water for the entire mountainous population of HKKH. Thus, knowledge of these available resources is very important in relation to their sustainable use. A Modified Positive Degree Day Model was used to simulate daily discharge with the contribution of snow and ice melt from the Shigar River Basin, Central Karakoram, Pakistan. The basin covers an area of 6,921 km2 with an elevation range of 2,204 to 8,611 m a.s.l.. Forty percent of the total area is glaciated among which 20% is covered by debris and remaining 80% by clean ice and permanent snow. To simulate daily discharge, the entire basin was divided into 26 altitude belts. Remotely sensed land cover types are derived by classifying Landsat images of 2009. Daily temperature and precipitation from Skardu meteorological station is used to calibrate the glacio-hydrological model as an input variable after correlating data with the Shigar station data(r=0.88). Local temperature lapse rate of 0.0075 °C/m is used. 2 °C critical temperature is used to separate rain and snow from precipitation. The model is calibrated for 1988~1991 and validated for 1992~1997. The model shows a good Nash-Sutcliffe efficiency and volume difference in calibration(0.86% and 0.90%) and validation(0.78% and 6.85%). Contribution of snow and ice melt in discharge is 32.37% in calibration period and 33.01% is validation period. The model is also used to predict future hydrological regime up to 2099 by using CORDEX South Asia RCM considering RCP4.5 and RCP8.5 climate scenarios.Predicted future snow and ice melt contributions in both RCP4.5 and RCP8.5 are 36% and 37%, respectively. | 11 | Clinico-epidemiological features of dengue fever in Saudi Arabia显示文摘Objective:To highlight some clinical and epidemiological features of dengue fever.Methods: All patients who were admitted to hospitals in Holly Mecca City,Saudi Arabia and were confirmed as dengue fever(DF) or dengue hemorrhagic fever(DHF) were included in this study. The data were collected from patient files and through direct interview with patients or their relatives.Cases were followed through their hospital stay.Routine laboratory investigations were done and diagnosis was confirmed by PCR.Results:Most of cases admitted in stable condition(94.37%) and only one case(1.41%) died.Dengue-1 and 3 types were the prevalent dengue viruses and cases in age group 16-44 were the most frequent(70.40%).The most common symptoms was fever reported from all cases followed by headache(74.60%),myalgia and anorexia(67.60%),back pain(59.20%) and chills(54.90%).DF represented(60.57%) of the cases while DHF represented(39.43%).About half of cases had underground water tanks for human use,5.60%had over house roof water tanks and 43.70%had both types,16.90% of these tanks were uncovered.Approximately 70.00%of cases reported presence of small collection of water nearby houses and 46.80%reported the presence of mosquitoes within their houses.Conclusions:Most dengue fever cases might be endogenous in origin due to prevalence of mosquitoes and their breeding places within the houses and in nearby localities.Control of mosquitoes and their breeding places will contribute to prevention of dengue fever. | Abdel-Hady El-Gilany Abdelaziz Eldeib Sabri Hammad | 2010 | Asian Pacific Journal of Tropical Medicine2010,3,3: | 2 |
| 12 | Electrical Properties of Li-doped P-type ZnO Ceramics显示文摘Li-doped p-type ZnO ceramics were prepared by conventional methods according to the chemical formula Zn1-xLixO2 where x=0.5, 1.0, 1.5 and 2.0 mole fraction, respectively. The crystal structures of the prepared samples were studied by X-ray diffraction analysis. The dielectric properties (including dielectric constant ε and dielectric loss ε ) and dc-electrical conductivity [σ(Ω-1·cm-1)] were investigated. The dielectric constant ε was sharply decreased at the low frequency range and independent on frequency at high frequency range. Otherwise, the dielectric loss ε varied with frequency and showed absorption peak located from 200 Hz to 4 kHz and moved to higher frequency as the concentration of Li+ doped increased. It was found that dcelectrical conductivity logσ varied from -9 to ?5 and the energy gap width were calculated by using Arrhenius equation. The p-type conductivity of Li-doped ZnO may be attributed to the formation of a LiZn-Lii donor complex, which is limited by reducing the amount of Lii. | A.H. Salama F.F. | 16 | Microfacies and Depositional Environments of Miocene Isolated Carbonate Platforms from Central Luconia, Offshore Sarawak, Malaysia显示文摘The Luconia Province-offshore Sarawak-is a key geological unit for understanding the distribution of hydrocarbon resources in Malaysia. Nevertheless, little effort has been made to address the palaeoenvironmental characteristics of the Tertiary carbonates in the key sector of Central Luconia.We study the sedimentology and petrography of core samples from a well in Central Luconia, for which thirteen microfacies have been identified reflecting different depositional settings. This is the first microfacies scheme elaborated for Luconian carbonates. Lithofacies and microfacies distribution are compatible with deposition in a reef complex, originating around a framework reef, within the euphotic zone. Sediments were deposited in environments of backreef, reef crest, and forereef. The fair weather wave base is marked by the presence of coralline red algae, foraminifera, decreasing degree of bioclast fragmentation and other microfacies features. As a result, a depositional-environmental model is constructed, depicting a reef complex built around a framework reef developed on the margin of an isolated platform.
