| 1 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 4 | A broadband achromatic metalens array for integral imaging in the visible显示文摘Integral imaging is a promising three-dimensional(3D)imaging technique that captures and reconstructs light field information.Microlens arrays are usually used for the reconstruction process to display 3D scenes to the viewer.However,the inherent chromatic aberration of the microlens array reduces the viewing quality,and thus,broadband achromatic imaging remains a challenge for integral imaging.Here,we realize a silicon nitride metalens array in the visible region that can be used to reconstruct 3D optical scenes in the achromatic integral imaging for white light.The metalens array contains 60×60 polarization-insensitive metalenses with nearly diffraction-limited focusing.The nanoposts in each high-efficiency(measured as 47%on average)metalens are delicately designed with zero effective material dispersion and an effective achromatic refractive index distribution from 430 to 780 nm.In addition,such an achromatic metalens array is composed of only a single silicon nitride layer with an ultrathin thickness of 400 nm,making the array suitable for on-chip hybrid-CMOS integration and the parallel manipulation of optoelectronic information.We expect these findings to provide possibilities for full-color and aberration-free integral imaging,and we envision that the proposed approach may be potentially applicable in the fields of high-power microlithography,high-precision wavefront sensors,virtual/augmented reality and 3D imaging. | Zhi-Bin Fan Hao-Yang Qiu Han-Le Zhang Xiao-Ning Pang Li-Dan Zhou Lin Liu Hui Ren Qiong-Hua Wang Jian-Wen Dong | 2019 | Light(Science & Applications)2019,8,1: | 21 |

| 6 | Transjugular intrahepatic portosystemic shunt vs endoscopic therapy in preventing variceal rebleeding显示文摘AIM:To compare early use of transjugular intrahepatic portosystemic shunt(TIPS) with endoscopic treatment(ET) for the prophylaxis of recurrent variceal bleeding.METHODS:In-patient data were collected from 190 patients between January 2007 and June 2010 who suffured from variceal bleeding.Patients who were older than 75 years;previously received surgical treatment or endoscopic therapy for variceal bleeding;and complicated with hepatic encephalopathy or hepatic cancer,were excluded from this research.Thirty-five cases lost to follow-up were also excluded.Retrospective analysis was done in 126 eligible cases.Among them,64 patients received TIPS(TIPS group) while 62 patients received endoscopic therapy(ET group).The relevant data were collected by patient review or telephone calls.The occurrence of rebleeding,hepatic encephalopathy or other complications,survival rateand cost of treatment were compared between the two groups.RESULTS:During the follow-up period(median,20.7 and 18.7 mo in TIPS and ET groups,respectively),rebleeding from any source occurred in 11 patients in the TIPS group as compared with 31 patients in the ET group(Kaplan-Meier analysis and log-rank test,P = 0.000).Rebleeding rates at any time point(6 wk,1 year and 2 year) in the TIPS group were lower than in the ET group(Bonferroni correction α' = α/3).Eight patients in the TIPS group and 16 in the ET group died with the cumulative survival rates of 80.6% and 64.9%(Kaplan-Meier analysis and log-rank test c2 = 4.864,P = 0.02),respectively.There was no significant difference between the two groups with respect to 6-wk survival rates(Bonferroni correction α' = α/3).However,significant differences were observed between the two groups in the 1-year survival rates(92% and 79%) and the 2-year survival rates(89% and 64.9%)(Bonferroni correction α' = α/3).No significant differences were observed between the two treatment groups in the occurrence of hepatic encephalopathy(12 patients in TIPS group and 5 in ET group,KaplanMeier analysis and log-rank test,c2 = 3.103,P = 0.08).The average total cost for the TIPS group was higher than for ET group(Wilcxon-Mann Whitney test,52 678 RMB vs 38 844 RMB,P < 0.05),but hospitalization frequency and hospital stay during follow-up period were lower(Wilcxon-Mann Whitney test,0.4 d vs 1.3 d,P = 0.01;5 d vs 19 d,P < 0.05).CONCLUSION:Early use of TIPS is more effective than endoscopic treatment in preventing variceal rebleeding and improving survival rate,and does not increase occurrence of hepatic encephalopathy. | Hui Xue Meng Zhang Jack XQ Pang Fei Yan Ying-Chao Li Liang-Shan Lv Jia Yuan Muna Palikhe Wei-Zhi Li Zhi-Lun Wang | 2012 | World Journal of Gastroenterology2012,18,48: | 17 |

| 13 | Prevalence and Risk Factors of Primary Drug-Resistant Tuberculosis in China显示文摘Objective To investigate the prevalence of primary drug-resistant tuberculosis(TB)and associated risk factors in China.We also explored factors contributing to the transmission of multidrug-resistant tuberculosis(MDR-TB).Methods A total of 2794 representative,Mycobacterium tuberculosis isolates from treatment-naive patients were subjected to drug susceptibility testing,and risk factors for drug-resistant TB were analyzed.We also analyzed MDR-TB strain sublineages,drug-resistance-conferring mutations,and risk factors associated with clustered primary MDR strains.Results Among 2794 Mycobacterium tuberculosis isolates from treatment-naive patients,the prevalence of any resistance to first-line drugs was 33.2%and the prevalence of MDR-TB was 5.7%.We did not find any risk factors significantly associated with resistance to first-line drugs.The 93 primary MDR-TB isolates were classified into six sublineages,of which,75(80.6%)isolates were the RD105-deleted Beijing lineage.The largest sublineage included 65(69.9%)isolates with concurrent deletions of RD105,RD207,and RD181.Twenty-nine(31.2%)primary MDR strains grouped in clusters;MDR isolates in clusters were more likely to have S531L rpoB mutation.Conclusion This study indicates that primary drug-resistant TB and MDR-TB strains are prevalent in China,and multiple measures should be taken to address drug-resistant TB. | WANG Sheng Fen ZHOU Yang PANG Yu ZHENG Hui Wen ZHAO Yan Lin | 2016 | Biomedical and Environmental Sciences2016,29,2: | 6 |
| 14 | Genomic landscape of gastric cancer: molecular classification and potential targets显示文摘Gastric cancer imposes a considerable health burden worldwide, and its mortality ranks as the second highest for all types of cancers. The limited knowledge of the molecular mechanisms underlying gastric cancer tumorigenesis hinders the development of therapeutic strategies. However, ongoing collaborative sequencing efforts facilitate molecular classification and unveil the genomic landscape of gastric cancer. Several new drivers and tumorigenic pathways in gastric cancer, including chromatin remodeling genes, Rho A-related pathways, TP53 dysregulation, activation of receptor tyrosine kinases, stem cell pathways and abnormal DNA methylation, have been revealed. These newly identified genomic alterations await translation into clinical diagnosis and targeted therapies. Considering that loss-of-function mutations are intractable, synthetic lethality could be employed when discussing feasible therapeutic strategies. | 15 | Compression-related EMIC waves drive relativistic electron precipitation显示文摘With coordinated observations of the NOAA 15 satellite and OUL magnetometer station in Finland, we report that the electromagnetic ion cyclotron(EMIC) waves which were stimulated by the compression of the magnetosphere drive relativistic electron precipitation in geoquiescence on 1 Jan 2007. After an enhancement of solar wind dynamic pressure(SWDP), a dayside Pc1 pulsation was observed by the OUL station. Such a Pc1 pulsation is caused by an EMIC wave which propagates from the generation source to lower altitudes. Simultaneously, the NOAA 15 satellite registered an enhancement of precipitating electron count rates with energies >3 Me V within the anisotropic zone of protons. This phenomenon is coincident with the quasi-linear theoretical calculation presented in this paper. | 16 | Nosocomial Infection Surveillance in a Tuberculosis Specialized Hospital in China显示文摘Nosocomial infections(NIs) are a critical issue affecting the quality of healthcare. In this study, we performed a retrospective study to explore the incidence rates, mortality rates, and microbial spectrum of NIs in Beijing Chest Hospital, a tuberculosis(TB) specialized hospital in China. Our data demonstrate that the overall incidence rate of inpatients with NIs slightly decreased from 2012 to 2016, which may be associated with the implementation of hand hygiene measures, while the mortality rates associated with NI did not significantly change. | 19 | Efficacy and safety of repetitive transcranial magnetic stimulation for generalised anxiety disorder: A metaanalysis显示文摘Background Pharmacological and conventional nonpharmacological treatments are only moderately effective in treatingeneralised anxiety disorder(GAD).Recently,repetitive transcranial magnetic stimulation(rTMS)has attracted interest because of its potential therapeutic value.Aim To investigate the efficacy and safety of rTMS treatment for GAD.Methods Literature studies published in English or Chinese were screened in 10 electronic databases up to 5 December 2018.The included studies'bias risk was assessed using Cochrane risk of bias assessment tool.Meta-analysis was performed to compute the standardised mean difference(SMD)and risk ratio(RR)along with its 95%Cis through using RevMan V.5.3.Heterogeneity was inspected by I2 and the χ2 test.We performed subgroup analysis and meta-regression to investigate heterogeneity.We used funnel plot to assess publication bias.We used the GRADE approach to assess the whole quality of evidence.Results Twenty-one studies,with a total sample size of 1481,were analysed.The risk of bias in most studies included is moderate,the majority of which are lacking of blinding methods of treatment allocation.The treatment had beneficial effects in the rTMS group compared with the control group in mean anxiety score(SMD=-0.68;95%Cl-0.89 to-0.46).None of the 21 studies included here reported severe adverse events.As for dropout rates,there are no statistically significant differences between the two groups(RR 1.14,95%Cl 0.72 to 1.82)or adverse events(RR 0.95,95%Cl 0.77 to 1.18).No particular influence on the heterogeneity of any variable was observed.The risk of publication bias was low.According to the GRADE approach,the evidence levels of primary outcome(treatment effects)and secondary outcomes(acceptability and safety)were rated as‘medium’.Conclusion The use of rTMS combined with medication treatment may have a significant positive anti-anxiety effect on patients with GAD.However,we should interpret the results cautiously due to the relatively high heterogeneity of the meta-analysis.Future high-quality clinical trials are needed to confirm our results. | Huiru Cui Lijuan Jiang Yanyan Wei Wei Li Hui Li Junjuan Zhu Jiaoyan Pang Jijun Wang Chunbo Li | 2019 | General Psychiatry2019,32,5: | 5 |
