1Diagnostic value of glypican-3 in serum and liver for primary hepatocellular carcinoma显示文摘AIM:To evaluate the diagnostic value of glypican-3(GPC3) in serum and liver for primary hepatocellular carcinoma(HCC).METHODS:Serum levels of GPC3 and α-fetoprotein(AFP) were measured in 75 patients with primary HCC and 32 patients with liver cirrhosis.Expression of GPC3 and AFP in 58 HCC and 12 cirrhotic specimens was detected with immunohistochemical staining.RESULTS:When the cut-off value of serum GPC3 was set at 300 ng/L,its sensitivity and specificity for HCC were 47.0% and 93.5%,respectively.Among the 14 patients with HCC at stage according to the Barcelona Clinic Liver Cancer staging system,the serum GPC3 level was higher than 300 ng/L in 50%(7/14) patients,the serum AFP level was not ≥ 400 μg/L in any patient.Combined serum AFP and GPC3 significantly increased the sensitivity to the diagnosis of HCC.The GPC3 expression was detected in cytoplasm of HCC cells but not in hepatocytes and bile ducts of benign tumors.Among the 58 HCC patients,the GPC3 was expressed in 100%(28/28) patients with their serum AFP level ≥ 400 μg/L,and in 90%(27/30) patients with their AFP level < 400 μg/L,respectively.The GPC3 was weakly or negatively expressed in all paracarcinomatous and cirrhotic tissue samples.AFP positive HCC cells were only found in 1 out of the 58 HCC patients.CONCLUSION:GPC3 protein is a sensitive and specific serum marker for diagnosis of early HCC.Its expression in liver tissues can be used to discriminate tumor cells from benign hepatic cells.Hui Liu Li-Jie Zhang Peng Li Yun Zhai Yu-Fen Tan Ning Li Hui-Guo Ding Chun-Feng Qu 2010World Journal of Gastroenterology2010,16,35:57
2Service-aware 6G:An intelligent and open network based on the convergence of communication,computing and caching显示文摘With the proliferation of the Internet of Things(IoT),various services are emerging with totally different features and requirements,which cannot be supported by the current fifth generation of mobile cellular networks(5G).The future sixth generation of mobile cellular networks(6G)is expected to have the capability to support new and unknown services with changing requirements.Hence,in addition to enhancing its capability by 10–100 times compared with 5G,6G should also be intelligent and open to adapt to the ever-changing services in the IoT,which requires a convergence of Communication,Computing and Caching(3C).Based on the analysis of the requirements of new services for 6G,this paper identifies key enabling technologies for an intelligent and open 6G network,all featured with 3C convergence.These technologies cover fundamental and emerging topics,including 3C-based spectrum management,radio channel construction,delay-aware transmission,wireless distributed computing,and network self-evolution.From the detailed analysis of these 3C-based technologies presented in this paper,we can see that although they are promising to enable an intelligent and open 6G,more efforts are needed to realize the expected 6G network.Yiqing Zhou Ling Liu Lu Wang Ning Hui Xinyu Cui Jie Wu Yan Peng Yanli Qi Chengwen Xing 2020Digital Communications and Networks2020,6,3:57
3Soil Acidification of Alfisols as Influenced by Tea Cultivation in Eastern China显示文摘Soil acidification is an important process in land degradation around the world as well as in China.Acidification of Alfisols was investigated in the tea gardens with various years of tea cultivation in the eastern China.Cultivation of tea plants caused soil acidification and soil acidity increased with the increase of tea cultivation period.Soil pH of composite samples from cultivated layers decreased by 1.37,1.62 and 1.85,respectively,after 13,34 and 54 years of tea plantation,as compared to the surface soil obtained from the unused land.Soil acidification rates at early stages of tea cultivation were found to be higher than those at the later stages.The acidification rate for the period of 0-13 years was as high as 4.40 kmol H + ha ?1 year ?1 for the cultivated layer samples.Soil acidification induced the decrease of soil exchangeable base cations and base cation saturation and thus increased the soil exchangeable acidity.Soil acidification also caused the decrease of soil cation exchange capacity,especially for the 54-year-old tea garden.Soil acidification induced by tea plantation also led to the increase of soil exchangeable Al and soluble Al,which was responsible for the Al toxicity to plants.WANG Hui XU Ren-Kou WANG Ning LI Xing-Hui 2010Pedosphere2010,20,6:46
4A life support-based comprehensive treatment regimen dramatically lowers the in-hospital mortality of patients with fulminant myocarditis: a multiple center study显示文摘Fulminant myocarditis(FM) has unacceptable high mortality. This study aimed to evaluate the therapeutic efficacy of a life support-based comprehensive treatment regimen(LSBCTR), a completely novel treatment regimen, for FM. A total of 169 FM patients recruited from January 2008 to December 2018 were divided into two groups: patients receiving LSBCTR(81 cases),which includes(i) mechanical life support(positive pressure respiration, intra-aortic balloon pump with or without extracorporeal membrane oxygenation),(ii) immunomodulation therapy using sufficient doses of glucocorticoids and immunoglobulins, and(iii) application of neuraminidase inhibitors, and those receiving conventional treatment(88 cases). The endpoints were in-hospital death and heart-transplantation. Of all the population, 44 patients(26.0%) died in hospitals. Inhospital mortality was 3.7%(3/81) for LSBCTR group and 46.6%(41/88) for traditional treatment(P<0.001). Early application of LSBCTR, mechanical life support, neuraminidase inhibitors, and immunomodulation therapy significantly contributed to reduction in in-hospital mortality. This study describes a novel treatment regimen for FM patients that dramatically reduces inhospital mortality. 6Overexpression of brain-derived neurotrophic factor in the hippocampus protects against post-stroke depression显示文摘Post-stroke depression is associated with reduced expression of brain-derived neurotrophic factor(BDNF). In this study, we evaluated whether BDNF overexpression affects depression-like behavior in a rat model of post-stroke depression. The middle cerebral artery was occluded to produce a model of focal cerebral ischemia. These rats were then subjected to isolation-housing combined with chronic unpredictable mild stress to generate a model of post-stroke depression. A BDNF gene lentiviral vector was injected into the hippocampus. At 7 days after injection, western blot assay and real-time quantitative PCR revealed that BDNF expression in the hippocampus was increased in depressive rats injected with BDNF lentivirus compared with depressive rats injected with control vector. Furthermore, sucrose solution consumption was higher, and horizontal and vertical movement scores were increased in the open field test in these rats as well. 8Elevated serum alpha fetoprotein levels promote pathological progression of hepatocellular carcinoma显示文摘AIM:To investigate the biological role of alpha fetoprotein (AFP) and its clinical signif icance in carcinogenesis of hepatocellular carcinoma (HCC).METHODS:Clinical analysis of HCC patients and im-munohistochemical examination were conducted to evaluate the relationship between serum AFP level and patient mortality. Confocal microscopy,Western blotting, dimethylthiahzolyl-2,5-diphenyl-tetrazolium bromide,Cell Counting Kit-8 assays and flow cytometry were performed to explore the possible mechanism.RESULTS: Among the 160 HCC patients enrolled in this study,130 patients survived 2 years (81.25%),with a survival rate of 86.8% in AFP < 2 0 μg/L group,88.9% in AFP 20-250 μg/L group,and 69.6% in AFP > 250 μg/L group, demonstrating a higher mortality rate in HCC patients with higher AFP levels. Surgical treatment was benef icial only in patients with low AFP levels.The mortality rate of HCC patients with high AFP levels who were treated surgically was apparently higher than those treated with conservative management.The results of immunohistochemistry showed that AFP and AFP receptor were merely expressed in tissues of HCC patients with positive serum AFP.Consistently,in vitro analysis showed that AFP and AFPS were expressed in HepG2 but not in HLE cells. AFP showed a capability to promote cell growth,and this was more apparent in HepG2 cells,in which the proliferation was increased by 3.5 folds. 9The Prevalence of HIV Drug Resistance among Treatment-failure Individuals and Treatment-nave Individuals in China:A Meta-analysis显示文摘Objective To understand drug resistance prevalence among treatment-failure and treatment-nave HIV-positive individuals in China.Methods We searched five electronic databases(Wanfang,CNKI,CQVIP,SinoMed,and Pubmed) for studies of HIV drug resistance.Random-effects models were carried out to estimate the prevalence of drug resistance among treatment-failure and treatment-nave individuals,respectively.Results The estimated nationwide rates of HIV drug resistance to any-class drugs among treatment-failure and treatment-nave individuals were 57%(95% CI:49%-65%) and 3.23%(95% CI:2.47%-4.07%),respectively.Among the drug classes,the prevalence of resistance to PIs was low(1.45%;95% CI:0.73%-2.33%) in treatment-failure individuals,although high rates of resistance to NNRTIs(54%;95% CI:45%-63%) and NRTIs(40%;95% CI:32%-49%) were found.Resistance to any-class drugs,NNRTIs and NRTIs manifested regional differences,but resistance to PIs did not.Positive correlations were observed between resistance to NNRTIs and NRTIs among treatment-failure and treatment-nave individuals,respectively.Conclusion The prevalence of HIV drug resistance to NNRTIs and NRTIs among treatment-failure individuals was high.In contrast,the prevalence of drug resistance among treatment-nave individuals was low.The epidemics of drug resistance matched current treatment strategies and interventions in China.Surveillance for HIV drug resistance is necessary to assess the sustainability and durability of current treatment regimens.WU Jing NORRIS Jessie LIU Hui Xin LI Zheng SU Ying Ying ZHU Lin WANG Ning 2014Biomedical and Environmental Sciences2014,27,11:17
10Application of next-generation sequencing technology to precision medicine in cancer: joint consensus of the Tumor Biomarker Committee of the Chinese Society of Clinical Oncology显示文摘Next-generation sequencing(NGS) technology is capable of sequencing millions or billions of DNA molecules simultaneously.Therefore, it represents a promising tool for the analysis of molecular targets for the initial diagnosis of disease, monitoring of disease progression, and identifying the mechanism of drug resistance. On behalf of the Tumor Biomarker Committee of the Chinese Society of Clinical Oncology(CSCO) and the China Actionable Genome Consortium(CAGC), the present expert group hereby proposes advisory guidelines on clinical applications of NGS technology for the analysis of cancer driver genes for precision cancer therapy. This group comprises an assembly of laboratory cancer geneticists, clinical oncologists, bioinformaticians,pathologists, and other professionals. After multiple rounds of discussions and revisions, the expert group has reached a preliminary consensus on the need of NGS in clinical diagnosis, its regulation, and compliance standards in clinical sample collection. 12Recent advances on porous organic frameworks for the adsorptive removal of hazardous materials显示文摘Environmental pollution is one of the most serious problems facing mankind today,and has attracted widespread attention worldwide. The burgeoning class of crystalline porous organic framework materials, metal–organic frameworks and covalent organic frameworks present promising application potential in areas related to pollution control due to their interesting surface properties. In this review, the literature of the past five years on the adsorptive removal of various hazardous materials, mainly including heavy metal ions, harmful gases, organic dyes, pharmaceutical and personal care products, and radionuclides from the environment by using COFs and MOFs, is summarized. The adsorption mechanisms are also discussed to help understand their adsorption performance and selectivity. 14Giant submucosal lipoma located in the descending colon: A case report and review of the literature显示文摘Colonic lipoma is an uncommon tumor of the gastrointestinal tract.Most cases are asymptomatic,with a small tumor size,and do not need any special treatment.However,we encountered one patient with a giant submucosal lipoma,with a maximum diameter of 8.5 cm,which exhibited symptoms such as intermittent lower abdominal pain,changes in bowel habits with passage of fresh blood and mucus per rectum,abdominal distension,anorexia and weight loss.Unfortunately,the possibility of colonic malignancy could not be precluded and left hemicolectomy was planned.The exact diagnosis of this special case was accomplished by intraoperative pathology.In the end,local resection was performed instead of left hemicolectomy.To the best of our knowledge,colonic lipoma exceeding 8 cm in diameter has not been previously reported.We,therefore,present this case and discuss age and sex factors,clinical and histopathological findings,diagnostic methods and treatment by reviewing the available literature,to serve as a reminder that colonic lipoma can also exist in patients with significant symptoms.In addition,intraoperative pathology should be investigated in those doubtful cases,so as to guide the exact diagnosis and treatment plan.Li Jiang Li-Sheng Jiang Fu-Yu Li Hui Ye Ning Li Nan-Sheng Cheng Yong Zhou 2007World Journal of Gastroenterology2007,13,42:11
15Deep learning applied to two-dimensional color Doppler flow imaging ultrasound images significantly improves diagnostic performance in the classification of breast masses:a multicenter study显示文摘Background:The current deep learning diagnosis of breast masses is mainly reflected by the diagnosis of benign and malignant lesions.In China,breast masses are divided into four categories according to the treatment method:inflammatory masses,adenosis,benign tumors,and malignant tumors.These categorizations are important for guiding clinical treatment.In this study,we aimed to develop a convolutional neural network(CNN)for classification of these four breast mass types using ultrasound(US)images.Methods:Taking breast biopsy or pathological examinations as the reference standard,CNNs were used to establish models for the four-way classification of 3623 breast cancer patients from 13 centers.The patients were randomly divided into training and test groups(n=1810 vs.n=1813).Separate models were created for two-dimensional(2D)images only,2D and color Doppler flow imaging(2D-CDFI),and 2D-CDFI and pulsed wave Doppler(2D-CDFI-PW)images.The performance of these three models was compared using sensitivity,specificity,area under receiver operating characteristic curve(AUC),positive(PPV)and negative predictive values(NPV),positive(LR+)and negative likelihood ratios(LR-),and the performance of the 2D model was further compared between masses of different sizes with above statistical indicators,between images from different hospitals with AUC,and with the performance of 37 radiologists.Results:The accuracies of the 2D,2D-CDFI,and 2D-CDFI-PW models on the test set were 87.9%,89.2%,and 88.7%,respectively.The AUCs for classification of benign tumors,malignant tumors,inflammatory masses,and adenosis were 0.90,0.91,0.90,and 0.89,respectively(95%confidence intervals[CIs],0.87-0.91,0.89-0.92,0.87-0.91,and 0.86-0.90).The 2D-CDFI model showed better accuracy(89.2%)on the test set than the 2D(87.9%)and 2D-CDFI-PW(88.7%)models.The 2D model showed accuracy of 81.7%on breast masses≤1 cm and 82.3%on breast masses>1 cm;there was a significant difference between the two groups(P<0.001).The accuracy of the CNN classifications for the test set(89.2%)was significantly higher than that of all the radiologists(30%).Conclusions:The CNN may have high accuracy for classification of US images of breast masses and perform significantly better than human radiologists.Trial registration:Chictr.org,ChiCTR1900021375;http://gffzz62a03ee25e6f4a77hobpopkxv5kpu6wxk.ffgz.tsg.suse.edu.cn/showproj.aspx?proj=33139.Teng-Fei Yu Wen He Cong-Gui Gan Ming-Chang Zhao Qiang Zhu Wei Zhang Hui Wang Yu-Kun Luo Fang Nie Li-Jun Yuan Yong Wang Yan-Li Guo Jian-Jun Yuan Li-Tao Ruan Yi-Cheng Wang Rui-Fang Zhang Hong-Xia Zhang Bin Ning Hai-Man Song Shuai Zheng Yi Li Yang Guang 2021Chinese Medical Journal2021,,4:11

18Changes in the epidemiology of hepatitis A in three socio-economic regions of China,1990-2017显示文摘Background:Hepatitis A(HepA)vaccination and economic transitions can change the epidemiology of HepA.China's Gross Domestic Product(GDP)per capita was known to be inversely associated with the incidence of HepA,but a deeper understanding of the epidemiology of HepA in different socio-economic regions is lacking.We compare the changing epidemiology of HepA in three socioeconomic-geographic regions of China.Methods:We obtained data on all HepA cases reported through the National Notifiable Disease Reporting System and assessed trends and changes in age-specific incidence rates by age quartile and season.We categorized the country into three regions,the sequential years into five era,compared the incidence,quartile age;seasonal intensity and coverage of HepA of the three regions.Linear regression was peiformed to analyse trends in incidence of HepA and to analyse the association between coverage and incidence.Results:The annual mean incidences of HepA in the eastern,central,and western regions decreased from 63.52/100000,50.57/100000 and 46.39/100000 in 1990-1992 to 1.18/100000,1.05/100000 and 3.14/100000 in 2012-2017,respectively.Decreases in incidence were seen in all age groups in the three regions;the incidence was highest(9.3/100000)in the youngest age group(0-4 years)of the western region,while in the central region,the age group with the highest incidence changed from 0 to 9 years to adults>60 years old.In 2017,the median age of HepA cases was 43 years(Q-Q3:33-55),47 years(Q1-Q3:32-60)and 33 years(Q1-Q3:9-52)in the eastern,central,and western provinces,respectively.Seasonal peaks became smaller or were nearly elimination nationwide,but seasonality persisted in some provinces.After the Expanded Program on Immunization(EPI)included HepA vaccine into the routine schedule in 2007,HepA coverage increased to>80%in the three regions and was negatively association with the HepA incidence.Conclusion:The incidence of HepA decreased markedly between 1990 and 2017.A socioeconomic inequity in coverage of HepA vaccine was almost eliminated after HepA vaccine was introduced into China's EPI system,but inequity in incidence still existed in lower socio-economic developed region.Xiao-Jin Sun Guo-Min Zhang Rong-Jun Zhou Hui Zheng Ning Miao Zun-Dong Yin Fu-Zhen Wang 2019Infectious Diseases of Poverty2019,8,5:9
