| 1 | Influence of irritable bowel syndrome on treatment outcome in gastroesophageal reflux disease显示文摘AIM: To investigate the influence of irritable bowel syndrome (IBS)-like symptoms on treatment outcomes with pantoprazole in gastroesophageal reflux disease (GERD) in a real life setting. METHODS: For this prospective, open-label, multinational, multicentre study, 1888 patients assessed by the investigators as suffering from GERD were recruited. The patients were additionally classified as with or without IBS-like symptoms at baseline. They were treated with pantoprazole 40 mg once daily and completed the Reflux Questionnaire (ReQuest) short version daily. Response rates and symptom scores were compared after 4 and 8 wk of treatment for subgroups defined by the subclasses of GERD [erosive(ERD) and non-erosive reflux disease (NERD)] and the presence of IBS-like symptoms. RESULTS: IBS-like symptoms were more prevalent in NERD than in ERD (18.3% vs 12.7%, P = 0.0015). Response rates after 4 and/or 8 wk of treatment were lower in patients with IBS-like symptoms than in patients without IBS-like symptoms in both ERD (Week 4: P < 0.0001, Week 8: P < 0.0339) and NERD (Week 8: P = 0.0088). At baseline, ReQuest 'lower abdominal com- plaints' symptom scores were highest in NERD patients with IBS-like symptoms. Additionally, these patients had the strongest symptom improvement after treatment compared with all other subgroups. CONCLUSION: IBS-like symptoms influence treatment outcome and symptom burden in GERD and should be considered in management. | 3 | Microfluidic actuators based on temperature-responsive hydrogels显示文摘The concept of using stimuli-responsive hydrogels to actuate fluids in microfluidic devices is particularly attractive,but limitations,in terms of spatial resolution,speed,reliability and integration,have hindered its development during the past two decades.By patterning and grafting poly(N-isopropylacrylamide)PNIPAM hydrogel films on plane substrates with a 2μm horizontal resolution and closing the system afterward,we have succeeded in unblocking bottlenecks that thermo-sensitive hydrogel technology has been challenged with until now.In this paper,we demonstrate,for the first time with this technology,devices with up to 7800 actuated micro-cages that sequester and release solutes,along with valves actuated individually with closing and opening switching times of 0.6±0.1 and 0.25±0.15 s,respectively.Two applications of this technology are illustrated in the domain of single cell handling and the nuclear acid amplification test(NAAT)for the Human Synaptojanin 1 gene,which is suspected to be involved in several neurodegenerative diseases such as Parkinson’s disease.The performance of the temperature-responsive hydrogels we demonstrate here suggests that in association with their moderate costs,hydrogels may represent an alternative to the actuation or handling techniques currently used in microfluidics,that are,pressure actuated polydimethylsiloxane(PDMS)valves and droplets. | Loïc D'Eramo Benjamin Chollet Marie Leman Ekkachai Martwong Mengxing Li Hubert Geisler Jules Dupire Margaux Kerdraon Clémence Vergne Fabrice Monti Yvette Tran Patrick Tabeling | 2018 | Microsystems & Nanoengineering2018，4，1： | 3 |

| 17 | Successful emergency resection of a massive intra-abdominal hemophilic pseudotumor显示文摘An intra-abdominal pseudotumor is a rare complication of hemophilia. Surgical treatment is associated with high morbidity and mortality rates and reported cases are scarce. We present a 66-year-old Caucasian male suffering from severe hemophilia type A treated for 10 years with Factor Ⅷ. Major complications from the disease were chronic hepatitis B and C, cerebral hemorrhage and disabling arthropathy. Twenty-three years ago, retro-peritoneal bleeding led to the development of a large intra-abdominal pseudotumor, which was followed-up clinically due to the high surgical risk and the lack of clinical indication. The patient presented to the emergency department with severe sepsis and umbilical discharge that had appeared over the past two days. Abdominal computed tomography images were highly suggestive of a bowel fistula. The patient was taken to the operating room under continuous infusion of factor Ⅷ. Surgical exploration revealed a large infected pseudotumor with severe intra-abdominal adhesions and a left colonic fistula. The pseudotumor was partially resected en bloc with the left colon leaving the posterior wall intact. The postoperative period was complicated by septic shock and a small bowel fistula that required reoperation. He was discharged on the 73 rd hospital day and is well 8 mo after surgery.
