| 1 | Endoscopic features of early-stage signet-ring-cell carcinoma of the stomach显示文摘AIM: To identify the features of early signet ring cell gastric carcinoma using magnification endoscopy with narrow band imaging(NBI).METHODS: A retrospective review was conducted of 12 cases of early signet ring cell gastric carcinoma who underwent treatment in a single institution between January 2009 and April 2013. All patients had magnification endoscopy with NBI and indigo carmine contrast to closely examine the mucosal architecture, including the microvasculature and arrangement of gastric pits. Histologic examination of the final endoscopic submucosal dissection or gastrectomy specimen was performed and compared with the endoscopic findings to identify patterns specific to signet ring cell carcinoma.RESULTS: Twelve patients with early signet ring cell gastric carcinoma were identified; 75% were male, and average age was 61 years. Most of the lesions were stage T1a(83%), while the remainder were T1b(17%). The mean lesion size was 1.4 cm2. On standard endoscopy, all 12 patients had a pale, flat lesion without any evidence of mucosal abnormality such as ulceration, elevation, or depression. On magnification endoscopywith NBI, all of the patients had irregularities in the glands and microvasculature consistent with early gastric cancer. In addition, all 12 patients exhibited the 'stretch sign', an elongation or expansion of the architectural structure. | 2 | Inflammatory response related scoring systems in assessing the prognosis of patients with pancreatic ductal adenocarcinoma:a systematic review显示文摘BACKGROUND: Various scoring systems based on assessment of the systemic inflammatory response help assessing the prognosis of patients with pancreatic ductal adenocarcinoma.In the present systematic review we evaluated the validity of four pre-intervention scoring systems: Glasgow prognostic score(GPS) and its modified version(mGPS), platelet lymphocyte ratio(PLR), neutrophil lymphocyte ratio(NLR), and prognostic nutrition index(PNI).DATA SOURCES: MOOSE guidelines were followed and EMBASE and MEDLINE databases were searched for all published studies until September 2013 using comprehensive text word and MeSH terms. All identified studies were analyzed, and relevant studies were included in the systematic review.RESULTS: Six studies were identified for GPS/mGPS with3 reporting statistical significance for GPS/mGPS on both univariate analysis(UVA) and multivariate analysis(MVA).Two studies suggested prognostic significance on UVA but not MVA, and in the final study UVA failed to show significance.Eleven studies evaluated the prognostic value of NLR. Six of them reported prognostic significance for NLR on UVA that persisted at MVA in 4 studies, and in the remaining 2 studies NLR was the only significant factor on UVA. In the remaining5 studies, all in patients undergoing resection, there was no significance on UVA. Seven studies evaluated PLR, with only one study demonstrated its prognostic significance on both UVAand MVA, the rest did not show the significance on UVA. Of the two studies identified for PNI, one demonstrated a statistically significant difference in survival on both UVA and MVA, and the other reported no significance for PNI on UVA.CONCLUSIONS: Both GPS/mGPS and NLR may be useful but further better-designed studies are required to confirm their value. PLR might be little useful, and there are at present inadequate data to assess the prognostic value of PNI. | 3 | 上海和北京城市创新空间结构的时空演化模式(英文)显示文摘In today's world, the innovation of science and technology has become the key support for improving comprehensive national strength and changing the mode of social production and lifestyle. The country that possesses world-class scientific and technological innovation cities maximizes the attraction of global innovation factors and wins a strategic initiative in international competition. Based on the urban zip code geodatabase, an evaluation system of urban innovation with the perspective of innovation outputs, and the spatial evolutionary mode, concerning the structure of innovation space of Shanghai and Beijing from 1991 to 2014, was developed. The results of the research indicated that the zip code geodatabase provided a new perspective for studying the evolving spatial structure of urban innovation. The resulting evaluation of the spatial structure of urban innovation using the urban zip code geodatabase established by connecting random edge points, was relatively effective. The study illustrates the value of this methodology. During the study period, the spatial structure of innovation of Shanghai and Beijing demonstrated many common features: with the increase in urban space units participating in innovation year by year, the overall gap of regional innovation outputs has narrowed, and the trend towards spatial agglomeration has strengthened. The evolving spatial structure of innovation of Shanghai and Beijing demonstrated differences between the common features during the 25 years as well: in the trend towards the suburbanization of innovation resources, the spatial structure of innovation of Shanghai evolved from a single-core to a multi-core structure. A radiation effect related to traffic arteries as spatial diffusion corridors was prominent. Accordingly, a spatial correlation effect of its innovation outputs also indicated a hollowness in the city center; the spatial structure of innovation of Beijing had a single-core oriented structure all the way. Together with the tendency for innovation resources to be agglomerated in the city center, the spatial correlation effect of innovation outputs reflected the characteristics of the evolutionary feature where 'rural area encircles cities'. The innovation spatial structure of Shanghai and Beijing have intrinsic consistency with the spatial structure of their respective regions(Yangtze River Delta urban agglomeration and Beijing-Tianjin-Hebei metropolitan region), which suggested that the principle of proportional and disproportional distribution of a city-scale pattern of technological and innovational activities is closely related to its regional innovation pattern. | 段德忠 杜德斌 刘承良 Seamus GRIMES | 2016 | Journal of Geographical Sciences2016，26，12： | 6 |
