| 1 | Recent advances in bone regeneration using adult stem cells显示文摘Bone is a highly vascularized tissue reliant on the close spatial and temporal association between bloodvessels and bone cells. Therefore, cells that participate in vasculogenesis and osteogenesis play a pivotal role in bone formation during prenatal and postnatal periods. Nevertheless, spontaneous healing of bone fracture is occasionally impaired due to insufficient blood and cellular supply to the site of injury. In these cases, bone regeneration process is interrupted, which might result in delayed union or even nonunion of the fracture. Nonunion fracture is difficult to treat and have a high financial impact. In the last decade, numerous technological advancements in bone tissue engineering and cell-therapy opened new horizon in the field of bone regeneration. This review starts with presentation of the biological processes involved in bone development, bone remodeling, fracture healing process and the microenvironment at bone healing sites. Then, we discuss the rationale for using adult stem cells and listed the characteristics of the available cells for bone regeneration. The mechanism of action and epigenetic regulations for osteogenic differentiation are also described. | 3 | Less common etiologies of exocrine pancreatic insufficiency显示文摘Exocrine pancreatic insufficiency(EPI), an important cause of maldigestion and malabsorption, results from primary pancreatic diseases or secondarily impaired exocrine pancreatic function. Besides cystic fibrosis and chronic pancreatitis, the most common etiologies of EPI, other causes of EPI include unresectable pancreatic cancer, metabolic diseases(diabetes); impaired hormonal stimulation of exocrine pancreatic secretion by cholecystokinin(CCK); celiac or inflammatory bowel disease(IBD) due to loss of intestinal brush border proteins; and gastrointestinal surgery(asynchrony between motor and secretory functions, impaired enteropancreatic feedback, and inadequate mixing of pancreatic secretions with food). This paper reviews such conditions that have less straightforward associations with EPI and examines the role of pancreatic enzyme replacement therapy(PERT). Relevant literature was identified by database searches. Most patients with inoperable pancreatic cancer develop EPI(66%-92%). EPI occurs in patients with type 1(26%-57%) or type 2 diabetes(20%-36%) and is typically mild to moderate; by definition, all patients with type 3 c(pancreatogenic) diabetes have EPI. EPI occurs in untreated celiac disease(4%-80%), but typically resolves on a gluten-free diet. EPI manifests in patients with IBD(14%-74%) and up to 100% of gastrointestinal surgery patients(47%-100%; dependent on surgical site). With the paucity of published studies on PERT use for these conditions, recommendations for or against PERT use remain ambiguous. | 4 | Mucin gene expression in bile of patients with and without gallstone disease,collected by endoscopic retrograde cholangiography显示文摘AIM:To investigate the pattern of mucin expression and concentration in bile obtained during endoscopic retrograde cholangiography(ERC) in relation to gallstone disease.METHODS:Bile samples obtained at ERC from 29 consecutive patients,17 with and 12 without gallstone disease were evaluated for mucin content by gel filtration on a Sepharose CL-4B column.Dot blot analysis for bile mucin apoproteins was performed with antibodies to Mucin 1(MUC1) ,MUC2,MUC3,MUC5AC,MUC5B and MUC6.Staining intensity score(0-3) was used as a measure of antigen expression.RESULTS:MUC1,MUC2,MUC3,MUC5AC,MUC5B and MUC6 were demonstrated in 34.4%,34.4%,51.7%,51.7%,55.1% and 27.5% of bile samples,respectively.The staining intensity scores were 0.62 ± 0.94,0.58 ± 0.90,0.79 ± 0.97,1.06 ± 1.22,1.20 ± 1.26 and 0.41 ± 0.73,respectively.Mean mucin concentration measured in bile by the Sepharose CL-4B method was 22.8 ± 24.0 mg/mL(range 3.4-89.0 mg/mL) .Mean protein concentration was 8.1 ± 4.8 mg/mL(range 1.7-23.2 mg/mL) .CONCLUSION:High levels of MUC3,MUC5AC and MUC5B are expressed in bile aspirated during ERC examination.A specific pattern of mucin gene expression or change in mucin concentration was not found in gallstone disease. | Alexander Vilkin Alex Geller Zohar Levi Yaron Niv | 2009 | World Journal of Gastroenterology2009，15，19： | 4 |
| 5 | Longitudinal evaluation of time related femoral neck narrowing after metal-on-metal hip resurfacing显示文摘AIM: To track the short-term neck narrowing changes in Birmingham metal-on-metal hip resurfacing(MOMHR) patients. METHODS: Since 2001, the Center for Hip and Knee Replacement started a registry to prospectively collect data on hip and knee replacement patients. From June 2006 to October 2008, 139 MOMHR were performed at our center by two participate surgeons using Birmingham MOMHR prosthesis(Smith Nephew, United States). It is standard of care for patients to obtain low, anteriorposterior(LAP) pelvis radiographs immediately after MOMHR procedure and then at 3 mo, 1 year and 2 year follow up office visits. Inclusion criteria for the present study included patients who came back for follow up office visit at above mentioned time points and got LAP radiographs. Exclusion criteria include patients who missed more than two follow up time points and those with poor-quality X-rays. Two orthopaedic residency trained research fellows reviewed the X-rays independently at 4 time points, i.e., immediate after surgery, 3 mo, 1 year and 2 year. Neck-to-prosthesis ratio(NPR) was used as main outcome measure. Twenty cases were used as subjects to identify the reliability between two observers. An intraclass correlation coefficient at 0.8 was considered as satisfied. A paired t-test was used to evaluate the significant difference between different time points with P < 0.05 considered to be statistically significant.RESULTS: The mean NPRs were 0.852 ± 0.056, 0.839 ± 0.052, 0.835 ± 0.051, 0.83 ± 0.04 immediately, 3 mo, 1 year and 2 years post-operatively respectively. At 3 mo, NPR was significantly different from immediate postoperative X-ray(P < 0.001). There was no difference between 3 mo and 1 year(P = 0.14) and 2 years(P = 0.53). Femoral neck narrowing(FNN) exceeding 10% of the diameter of the neck was observed in only 4 patients(5.6%) at two years follow up. None of these patients developed a femoral neck fracture(FNF). CONCLUSION: Femoral neck narrowing after MOMHR occurred as early as 3 mo postoperatively, and stabilized thereafter.
