2Approach to the endoscopic resection of duodenal lesions显示文摘Duodenal polyps or lesions are uncommonly found on upper endoscopy. Duodenal lesions can be categorized as subepithelial or mucosally-based, and the type of lesion often dictates the work-up and possible therapeutic options. Subepithelial lesions that can arise in the duodenum include lipomas, gastrointestinal stromal tumors, and carcinoids. Endoscopic ultrasonography with fine needle aspiration is useful in the characterization and diagnosis of subepithelial lesions. Duodenal gastrointestinal stromal tumors and large or multifocal carcinoids are best managed by surgical resection. Brunner's gland tumors, solitary Peutz-Jeghers polyps, and non-ampullary and ampullary adenomas are mucosally-based duodenal lesions, which can require removal and are typically amenable to endoscopic resection. Several anatomic characteristics of the duodenum make endoscopic resection of duodenal lesions challenging. However, advanced endoscopic techniques exist that enable the resection of large mucosally-based duodenal lesions. Endoscopic papillectomy is not without risk, but this procedure can effectively resect ampullary adenomas and allows patients to avoid surgery, which typically involves pancreaticoduodenectomy. Endoscopic mucosal resection and its variations(such as cap-assisted, cap-band-assisted, and underwater techniques) enable the safe and effective resection of most duodenal adenomas. 3Effect of dephytinization on bioavailability of iron,calcium and zinc from infant cereals assessed in the Caco-2 cell model显示文摘AIM:To test the effect of the dephytinization of three different commercial infant cereals on iron, calcium, and zinc bioavailability by estimating the uptake, retention, and transport by Caco-2 cells.METHODS:Both dephytinized (by adding an exogenous phytase) and non-dephytinized infant cereals were digested using an in vitro digestion protocol adapted to the gastrointestinal conditions of infants younger than 6 mo. Mineral cell retention, transport, and uptake from infant cereals were measured using the soluble fraction of the simulated digestion and the Caco-2 cells. RESULTS: Dephytinization of infant cereals significantly increased (P<0.05) the cell uptake efficiency (from 0.66%-6.05% to 3.93%-13%), retention (from 6.04%-16.68% to 14.75%-20.14%) and transport efficiency (from 0.14%-2.21% to 1.47%-6.02%), of iron, and the uptake efficiency (from 5.0%-35.4% to 7.3%-41.6%) and retention (from 4.05%-20.53% to 14.45%-61.3%) of zinc, whereas calcium only cell uptake showed a significant increase (P<0.05) after removing phytate from most of the samples analyzed. A positive relationship (P<0.05) between mineral solubility and the cell uptake and transport efficiencies was observed.CONCLUSION: Removing phytate from infant cereals had a beneficial effect on iron and zinc bioavailability when infant cereals were reconstituted with water. 9Metabolic surgery and intestinal gene expression:Digestive tract and diabetes evolution considerations显示文摘AIM: To investigate the effects of bariatric surgery on metabolic parameters, incretin hormone secretion, and duodenal and ileal mucosal gene expression.METHODS: Nine patients with type 2 diabetes mellitus(T2DM), chronic serum hyperglycemia for more than2 years, and a body mass index(BMI) of 30-35 kg/m2 underwent metabolic surgery sleeve gastrectomy with transit bipartition between May 2011 and December2011. Blood samples were collected pre and 3, 6 and12 mo postsurgery. Duodenal and ileal mucosa samples were collected pre- and 3 mo postsurgery. Pre- and postoperative blood samples were collected in the fasting state before ingestion of a standard meal(520kcal) and again 30, 60, 90, and 120 min after the mealto determine hemoglobin A1c(HbA1c) levels and the lipid profile, which consisted of triglyceride and total cholesterol levels. Intestinal gene expression of p53 and transforming growth factor(TGF)-β was analyzed using quantitative reverse-transcription PCR. Gastric inhibitory polypeptide(GIP) and glucagon-like peptide-1(GLP-1)were quantified using the enzyme-linked immunoassay method and analyzed pre- and postoperatively.Student's t test or repeated measurements analysis of variance with Bonferroni corrections were performed as appropriate.RESULTS: BMI values decreased by 15.7% within the initial 3 mo after surgery(31.29 ± 0.73 vs 26.398± 0.68, P < 0.05) and then stabilized at 22% at 6mo postoperative, resulting in similar values 12 mo postoperatively(20-25 kg/m2). All of the patients experienced improved T2 DM, with 7 patients(78%)achieving complete remission(HbA1c < 6.5%), and 2patients(22%) achieving improved diabetes(HbA1c< 7.0% with or without the use of oral hypoglycemic agents). At 3 mo postoperatively, fasting plasma glucose had also decreased(59%)(269.55 ±18.24 mg/dL vs 100.77 ± 3.13 mg/dL, P < 0.05)with no further significant changes at 6 or 12 mo postoperatively. In the first month postoperatively,there was a complete withdrawal of hypoglycemic medications in all patients, who were taking at least2 hypoglycemic drugs preoperatively. 10Folate targeted coated SPIONs as efficient tool for MRI显示文摘The development of more sensitive diagnostic tools allowing an early-stageand highly efficient medical imaging of tumors remains a challenge. Magneticnanoparticles seem to be the contrast agents with the highest potential, if properlyconstructed. Therefore, in this study, hybrid magnetic nanoarchitectures weredeveloped using a new amphiphilic inulin-based graft copolymer (INU-LA-PEG-FA) as coating material for 10-nm spinel iron oxide (magnetite, Fe304)superparamagnetic nanoparticles (SPION). Folic acid (FA) covalently linked tothe coating copolymer in order to be exposed onto the nanoparticle surface waschosen as the targeting agent because folate receptors are upregulated in manycancer types. Physicochemical characterization and in vitro biocompatibilitystudy was then performed on the prepared magnetic nanoparticles. 12Finite element model with imposed slip surfaces for earth mass safety evaluation显示文摘The study of earth masses requires numerical methods that provide the quantification of the safety factor without requiring detrimental assumptions. For that, equilibrium analysis can perform fast computations but require assumptions that limit its potentiality. Limit analysis does not require detrimental assumptions but are numerically demanding. This work provides a new approach that combines the advantage of both the equilibrium method and the limit analysis. The defined hybrid model allows probabilistic analysis and optimization approaches without the assumption of interslice forces. It is compared with a published case and used to perform probabilistic studies in both a homogeneous and a layered foundation. 13能否用协调变异性识别运动员的竞技表现和竞技水平?——以竞走为例(英文)显示文摘Background:Marginal changes in the execution of competitive sports movements can represent a significant change for performance success.However,such differences may emerge only at certain execution intensities and are not easily detectable through conventional biomechanical techniques.This study aimed to investigate if and how competition standard and progression speed affect race walking kinematics from both a conventional and a coordination variability perspective.Methods:Fifteen experienced athletes divided into three groups(elite,international,and national) were studied while race walking on a treadmill at two different speeds(12.0 and 15.5 km/h).Basic gait parameters,the angular displacement of the pelvis and lower limbs,and the variability in continuous relative phase between six different joint couplings were analyzed.Results:Most of the spatio-temporal,kinematic,and coordination variability measures proved sensitive to the change in speed.Conversely,non-linear dynamics measures highlighted differences between athletes of different competition standard when conventional analytical tools were not able to discriminate between different skill levels.Continuous relative phase variability was higher for national level athletes than international and elite in two couplings(pelvis obliquity—hip flex/extension and pelvis rotation—ankle dorsi/plantarflexion) and gait phases(early stance for the first coupling,propulsive phase for the second) that are deemed fundamental for correct technique and performance.Conclusion:Measures of coordination variability showed to be a more sensitive tool for the fine detection of skill-dependent factors in competitive race walking,and showed good potential for being integrated in the assessment and monitoring of sports motor abilities.Dario Cazzola Gaspare Pavei Ezio Preatoni 2016Journal of Sport and Health Science2016,5,1:2
