| 1 | Role of inflammatory response in liver diseases: Therapeutic strategies显示文摘Inflammation and tumorigenesis are tightly linked pathways impacting cancer development. Inflammasomes are key signalling platforms that detect pathogenic microorganisms, including hepatitis C virus(HCV) infection, and sterile stressors(oxidative stress, insulin resistance, lipotoxicity) able to activate pro-inflammatory cytokines interleukin-1β and IL-18. Most of the inflammasome complexes that have been described to date contain a NOD-like receptor sensor molecule. Redox state and autophagy can regulate inflammasome complex and, depending on the conditions, can be either pro-or antiapoptotic. Acute and chronic liver diseases are cytokinedriven diseases as several proinflammatory cytokines(IL-1α, IL-1β, tumor necrosis factor-alpha, and IL-6) are critically involved in inflammation, steatosis, fibrosis, and cancer development. NLRP3 inflammasome gain of function aggravates liver disease, resulting in severe liver fibrosis and highlighting this pathway in the pathogenesis of non-alcoholic fatty liver disease. On the other hand, HCV infection is the primary catalyst for progressive liver disease and development of liver cancer. It is well established that HCV-induced IL-1β production by hepatic macrophages plays a critical and central process that promotes liver inflammation and disease. | 2 | Determining indoor air quality and identifying the origin of odour episodes in indoor environments显示文摘A methodology for identifying volatile organic compounds(VOCs) and determining air quality of indoor air has been developed.The air samples are collected using pump samplers by the inhabitants when they perceive odorous and/or discomfort episodes.Glass multi-sorbent tubes are connected to the pump samplers for the retention of VOC.The analysis is performed by automatic thermal desorption(ATD) coupled with gas chromatography-mass spectrometry(GC/MS).This methodology can be applied in cases of sick building syndrome(SBS) evaluation,in which building occupants experience a series of varied symptoms that appear to be linked to time spent in the building.Chemical pollutants concentrations(e.g.,VOC) have been described to contribute to SBS.To exemplify the methodology,a qualitative determination and an evaluation of existing VOC were performed in a dwelling where the occupants experienced the SBS symptoms.Higher total VOC(TVOC) levels were detected during episodes in indoor air(1.33 ± 1.53 mg/m3) compared to outdoor air(0.71 ± 0.46 mg/m3).The concentrations of individual VOCs,such as ethanol,acetone,isopropanol,1-butanol,acetic acid,acetonitrile and 1-methoxy-2-propanol,were also higher than the expected for a standard dwelling.The external source of VOC was found to be an undeclared activity of storage and manipulation of solvents located at the bottom of a contiguous building. | Eva Gallego Xavier Roca Jose Francisco Perales Xavier Guardino | 2009 | Journal of Environmental Sciences2009,21,3: | 14 |

| 5 | The Study of Carbamoyl Phosphate Synthetase 1 Deficiency Sheds Light on the Mechanism for Switching On/Off the Urea Cycle显示文摘Carbamoyl phosphate synthetase 1(CPS1) deficiency(CPS1D) is an inborn error of the urea cycle having autosomal(2q34) recessive inheritance that can cause hyperammonemia and neonatal death or mental retardation. We analyzed the effects on CPS1 activity, kinetic parameters and enzyme stability of missense mutations reported in patients with CPS1 deficiency that map in the 20-k Da C-terminal domain of the enzyme. This domain turns on or off the enzyme depending on whether the essential allosteric activator of CPS1, N-acetylL-glutamate(NAG), is bound or is not bound to it. To carry out the present studies, we exploited a novel system that allows the expression in vitro and the purification of human CPS1, thus permitting site-directed mutagenesis. These studies have clarified disease causation by individual mutations, identifying functionally important residues, and revealing that a number of mutations decrease the affinity of the enzyme for NAG. Patients with NAG affinity-decreasing mutations might benefit from NAG site saturation therapy with N-carbamyl-Lglutamate(a registered drug, the analog of NAG). Our results, together with additional present and prior site-directed mutagenesis data for other residues mapping in this domain, suggest an NAG-triggered conformational change in the b4-a4 loop of the C-terminal domain of this enzyme. This change might be an early event in the NAG activation process. | 6 | Management of primary achalasia: The role of endoscopy显示文摘Achalasia is an oesophageal motor disorder which leads to the functional obstruction of the lower oesophageal sphincter(LES) and is currently incurable. The main objective of all existing therapies is to achieve a reduction in the obstruction of the distal oesophagus in order to improve oesophageal transit, relieve the symptomatology, and prevent long-term complications. The most common treatments used are pneumatic dilation(PD) and laparoscopic Heller myotomy, which involves partial fundoplication with comparable shortterm success rates. The most economic non-surgical therapy is PD, with botulinum toxin injections reserved for patients with a higher surgical risk for whom the former treatment option is unsuitable. A new technology is peroral endoscopic myotomy, postulated as a possible non-invasive alternative to surgical myotomy. Other endoluminal treatments subject to research more recently include injecting ethanolamine into the LES and using a temporary self-expanding metallic stent. At present, there is not enough evidence permitting a routine recommendation of any of these three novel methods. Patients must undergo follow-up after treatment to guarantee that their symptoms are under control and to prevent complications. Most experts are in favour of some form of endoscopic follow-up, however no established guidelines exist in this respect.
