2Persistence and clearance of viral RNA in 2019 novel coronavirus disease rehabilitation patients显示文摘Background:A patient’s infectivity is determined by the presence of the virus in different body fluids,secretions,and excreta.The persistence and clearance of viral RNA from different specimens of patients with 2019 novel coronavirus disease(COVID-19)remain unclear.This study analyzed the clearance time and factors influencing 2019 novel coronavirus(2019-nCoV)RNA in different samples from patients with COVID-19,providing further evidence to improve the management of patients during convalescence.Methods:The clinical data and laboratory test results of convalescent patients with COVID-19 who were admitted to from January 20,2020 to February 10,2020 were collected retrospectively.The reverse transcription polymerase chain reaction(RT-PCR)results for patients’oropharyngeal swab,stool,urine,and serum samples were collected and analyzed.Convalescent patients refer to recovered non-febrile patients without respiratory symptoms who had two successive(minimum 24 h sampling interval)negative RT-PCR results for viral RNA from oropharyngeal swabs.The effects of cluster of differentiation 4(CD4)+T lymphocytes,inflammatory indicators,and glucocorticoid treatment on viral nucleic acid clearance were analyzed.Results:In the 292 confirmed cases,66 patients recovered after treatment and were included in our study.In total,28(42.4%)women and 38 men(57.6%)with a median age of 44.0(34.0-62.0)years were analyzed.After in-hospital treatment,patients’inflammatory indicators decreased with improved clinical condition.The median time from the onset of symptoms to first negative RT-PCR results for oropharyngeal swabs in convalescent patients was 9.5(6.0-11.0)days.By February 10,2020,11 convalescent patients(16.7%)still tested positive for viral RNA from stool specimens and the other 55 patients’stool specimens were negative for 2019-nCoV following a median duration of 11.0(9.0-16.0)days after symptom onset.Among these 55 patients,43 had a longer duration until stool specimens were negative for viral RNA than for throat swabs,with a median delay of 2.0(1.0-4.0)days.Results for only four(6.9%)urine samples were positive for viral nucleic acid out of 58 cases;viral RNA was still present in three patients’urine specimens after throat swabs were negative.Using a multiple linear regression model(F=2.669,P=0.044,and adjusted R2=0.122),the analysis showed that the CD4+T lymphocyte count may help predict the duration of viral RNA detection in patients’stools(t=-2.699,P=0.010).The duration of viral RNA detection from oropharyngeal swabs and fecal samples in the glucocorticoid treatment group was longer than that in the non-glucocorticoid treatment group(15 days vs.8.0 days,respectively;t=2.550,P=0.013)and the duration of viral RNA detection in fecal samples in the glucocorticoid treatment group was longer than that in the non-glucocorticoid treatment group(20 days vs.11 days,respectively;t=4.631,P<0.001).There was no statistically significant difference in inflammatory indicators between patients with positive fecal viral RNA test results and those with negative results(P>0.05).Conclusions:In brief,as the clearance of viral RNA in patients’stools was delayed compared to that in oropharyngeal swabs,it is important to identify viral RNA in feces during convalescence.Because of the delayed clearance of viral RNA in the glucocorticoid treatment group,glucocorticoids are not recommended in the treatment of COVID-19,especially for mild disease.The duration of RNA detection may relate to host cell immunity.Yun Ling Shui-Bao Xu Yi-Xiao Lin Di Tian Zhao-Qin Zhu Fa-Hui Dai Fan Wu Zhi-Gang Song Wei Huang Jun Chen Bi-Jie Hu Sheng Wang En-Qiang Mao Lei Zhu Wen-Hong Zhang Hong-Zhou Lu 2020Chinese Medical Journal2020,,9:58

4Field experiment on a robust hierarchical metropolitan quantum cryptography network显示文摘A hierarchical metropolitan quantum cryptography network upon the inner-city commercial telecom fiber cables is reported in this paper. The seven-user network contains a four-node backbone net with one node acting as the subnet gateway, a two-user subnet and a single-fiber access link, which is realized by the Faraday-Michelson interferometer set-ups. The techniques of the quantum router, optical switch and trusted relay are assembled here to guarantee the feasibility and expandability of the quantum cryptography network. Five nodes of the network are located in the government departments and the secure keys generated by the quantum key distribution network are utilized to encrypt the instant video, sound, text messages and confidential files transmitting between these bureaus. 5Spatial and temporal variation of particulate matter and gaseous pollutants in 26 cities in China显示文摘O3and PM2.5were introduced into the newly revised air quality standard system in February 2012, representing a milestone in the history of air pollution control, and China's urban air quality will be evaluated using six factors(SO2, NO2, O3, CO, PM2.5and PM10) from the beginning of 2013. To achieve the new air quality standard, it is extremely important to have a primary understanding of the current pollution status in various cities. The spatial and temporal variations of the air pollutants were investigated in 26 pilot cities in China from August 2011 to February 2012, just before the new standard was executed. Hourly averaged SO2, NO2and PM10were observed in 26 cities, and the pollutants O3, CO and PM2.5were measured in 15 of the 26 cities. The concentrations of SO2and CO were much higher in the cities in north China than those in the south. As for O3and NO2, however, there was no significant diference between northern and southern cities. Fine particles were found to account for a large proportion of airborne particles, with the ratio of PM2.5to PM10ranging from 55% to 77%. The concentrations of PM2.5(57.5 μg/m3) and PM10(91.2 μg/m3) were much higher than the values(PM2.5: 11.2 μg/m3; PM10 : 35.6 μg/m3) recommended by the World Health Organization. 6Crustal velocity structure in the Emeishan large igneous province and evidence of the Permian mantle plume activity显示文摘The Emeishan large igneous province(ELIP) in SW China is interpreted to be associated with an ancient mantle plume. Most of the constraints on the role of mantle plume in the generation of the Emeishan flood basalts were provided by geological and geochemical methods, but the geophysical investigation is very limited. In order to better understand the deep structure and features of ELIP, we have studied the crustal velocity structure using the data acquired from the Lijiang-Panzhihua-Qingzhen wide-angle seismic profile. This profile crosses the three sub-zones of the ELIP(the inner, intermediate, and outer zones), divided based on the differential erosion and uplift of the Maokou limestone. The results provided by the active source seismic experiment demonstrate:(1) The average depth of the crystalline basement along the profile is about 2 km.(2) The middle crust in the Inner Zone is characterized by high-velocity anomalies, with the average velocity of 6.2-6.6 km/s, which is about 0.1– 0.2 km/s higher than the normal one. The velocity of the lower crust in the inner zone is 6.9-7.2 km/s, higher than those observed in the intermediate and outer zones(6.7-7.0 km/s). Relatively low velocity anomalies appear in the upper, middle and lower crusts near the junction of the inner zone and intermediate zone, probably due to the effect of the Xiaojiang fault(XJF).(3) The average velocity of the crust is comparatively low on both sides of XJF, especially on the east side, and the average velocity of the consolidated continental crust is also low there. This may suggest that the XJF extends at least down to 40 km deep, even beyond through the crust.(4) The depth to the Moho discontinuity decrease gradually from 47-53 km in the inner zone, via 42-50 km in the intermediate zone to 38-42 km in the outer zone. In the inner zone, the Moho uplifts locally and the(consolidated) crust is characterized by high-velocity anomalies, which are likely related to intensive magma intrusion and underplating associated with melting of plume head. 7Linking atmospheric pollution to cryospheric change in the Third Pole region: current progress and future prospects显示文摘The Tibetan Plateau and its surroundings are known as the Third Pole(TP). This region is noted for its high rates of glacier melt and the associated hydrological shifts that affect water supplies in Asia. Atmospheric pollutants contribute to climatic and cryospheric changes through their effects on solar radiation and the albedos of snow and ice surfaces; moreover, the behavior and fates within the cryosphere and environmental impacts of environmental pollutants are topics of increasing concern. In this review, we introduce a coordinated monitoring and research framework and network to link atmospheric pollution and cryospheric changes(APCC) within the TP region. We then provide an up-to-date summary of progress and achievements related to the APCC research framework, including aspects of atmospheric pollution’s composition and concentration, spatial and temporal variations, trans-boundary transport pathways and mechanisms, and effects on the warming of atmosphere and changing in Indian monsoon, as well as melting of glacier and snow cover. We highlight that exogenous air pollutants can enter into the TP’s environments and cause great impacts on regional climatic and environmental changes. At last, we propose future research priorities and map out an extended program at the global scale. 8S-wave velocity and Poisson's ratio structure of crust in Yunnan and its implication显示文摘Receiver function of body wave under the 23 stations in Yunnan was extracted from 3-component broadband digital recording of teleseismic event. Thus, the S-wave velocity structure and distribution characteristics of Poisson's ratio in crust of Yunnan are obtained by inversion.The results show that the crustal thickness is gradually thinned from north to south. The crustal thickness in Zhongdian of northwest reaches as many as 62.0 km and the one in Jinghong of further south end is only 30.2 km. What should be especially noted is that there exists a Moho upheaval running in NS in the Chuxiong region and a Moho concave is generally parallel to it in Dongchuan. In addition, there exists an obvious transversal inhomogeneity for the S-wave veIocity structure in upper mantle and crust in the Yunnan region. The low velocity layer exists not only in 10.0-15.0 km in upper crust in some regions, but also in 30.0-40.0 km in lower crust.Generally, the Poisson's ratio is on the high side, however it has a better corresponding relation to the crustal velocity structure. An obvious block distribution feature is still shown on such a high background of Poisson's ratio. It is discovered by synthetically analyzing the velocity structure and Poisson's ratio distribution that there are high Poisson's ratio and complicated crust-mantle velocity structure feature in the Sichuan-Yunnan Diamond Block with Xiaojiang fault to be the east boundary and Yulong Snow Mountain fault to be the west boundary besides the frequent seismicity. 9Effects of large dose of dexamethasone on inflammatory mediators and pancreatic cell apoptosis of rats with severe acute pancreatitis显示文摘AIM: To investigate the influence of high dose of dexamethasone on inflammatory mediators and apoptosis of rats with severe acute pancreatitis (SAP). METHODS: SAP rats were randomly assigned to the model group and treatment group while the normal rats were assigned to the sham operation group. The mortality,ascite volumes,ascites/body weight ratio and pancreas pathological changes of all rats were observed at 3,6 and 12 h after operation. Their contents of amylase and endotoxin in plasma and contents of tumor necrosis factor (TNF-α),phospholipase A2 (PLA2) and IL-6 in serum were also determined. The microarray sections of their pancreatic tissues were prepared,terminal transferase dUTP nick end labeling (TUNEL) staining was performed and apoptotic indexes were calculated. RESULTS: There was no marked difference between treatment group and model group in survival. 12EFFECT OF ASPIRIN PLUS CLOPIDOGREL ON INFLAMMATORY MARKERS IN PATIENTS WITH NON-ST-SEGMENT ELEVATION ACUTE CORONARY SYNDROME显示文摘Background Aspirin can inhibit inflammatory reactions and platelet aggregation, but little is known about the effects of the combination of aspirin plus clopidogrel, a new antiplatelet agent, on inflammation. The purpose of this study was to determine whether aspirin plus clopidogrel can further suppress inflammation in patients with non-ST-segment elevation acute coronary syndrome (NSTEACS). Methods One hundred and fifteen patients with NSTEACS were randomized into two groups: group A (aspirin alone, n=58) and group B (aspirin plus clopidogrel, n=57). Patients in group A received a loading dose of 300 mg aspirin, then 100 mg per day. The patients in group B received a loading dose of 300 mg aspirin and 300 mg clopidogrel, then 100 mg aspirin and 75 mg clopidogrel per day. Serum high sensitivity C-reactive protein (hs-CRP) and tumor necrosis factor-α(TNF-α) were measured in all patients at baseline prior to any drug treatment after admission, and at 7 and 30 days after beginning drug treatment. Thirty healthy volunteers on no medications were enrolled as controls (group C). Results Baseline levels of hs-CRP and TNF-αin group A and group B were significantly higher than those in group C. Seven days after administration, the levels of hs-CRP in both group A and group B decreased significantly [Group A: (6.15 ± 1.39) mg/L vs (9.18 ± 1.62) mg/L, P <0.01; Group B:(4.99 ± 1.62) mg/L vs (10.29 ± 1.47) mg/L, P<0.01]. Similarly, levels of TNF-αin both groups decreased at 7 days compared to baseline [Group A: (90.99 ± 28.91) pg/ml vs (117.20 ± 37.13) pg/ml, P <0.01; Group B: (74.32± 21.83) pg/ml vs (115.27 ± 32.11) pg/ml, P <0.01]. Thirty days after administration, the levels of hs-CRP in both group A and group B decreased further to (3.49 ± 1.53) mg/L, and (2.40 ± 1.17) mg/L respectively (P <0.01 for both comparisons). Levels of TNF-αin groups A and B also decreased significantly between 7 and 30 days, to 63.28 ± 29.01 pg/ml (group A) and (43.95 ± 17.10) pg/ml (group B; P <0.01 for both comparisons). Significantly lower levels of hs-CRP and TNF-αwere observed in group B compared to Group A at thirty days after initiating drug treatment (P <0.05). Conclusions Aspirin plus clopidogrel treatment reduced levels of serum hs-CRP and TNF-α in patients with NSTEACS significantly more than aspirin alone. 15Ginkgo biloba extract improved cognitive and neurological functions of acute ischaemic stroke:a randomised controlled trial显示文摘Purpose To evaluate the efficacy and safety of Ginkgo biloba extract(GBE)in acute ischaemic stroke and its impact on the recurrence of vascular events.Methods We conducted a multicentre,prospective,randomised,open label,blinded,controlled clinical trial enrollingpatients with an onset of acute stroke within 7 days from five hospitals in China Jiangsu Province.Participants were assigned to the GBE group(450 mg GBE with 100 mg aspirin daily)or the control group(100 mg aspirin daily)for 6 months.The primary outcome was the decline in the Montreal Cognitive Assessment score at 6 months.Secondary outcomes were other neuropsychological tests of cognitive and neurological function,the the incidence of adverse events and vascular events.results 348 patients were enrolled:179 in the GBE group and 169 in the control group.With 18 patients lost to follow-up,the dropout rate was 5.17%.Admission data between two groups were similar,but in the GBE group there was a marked slow down in the decline in the Montreal Cognitive Assessment scores(−2.77±0.21 vs−1.99±0.23,P=0.0116(30 days);−3.34±0.24 vs−2.48±0.26,P=0.0165(90 days);−4.00±0.26 vs−2.71±0.26,P=0.0004(180 days))compared with controls.The National Institutes of Health Stroke Scale scores at 12 and 30 days,the modified Rankin Scale scores for independent rate at 30,90 and 180 days,and the Barthel Index scores at 30,90 and 180 days in the GBE group were significantly improved compared with controls.Improvements were also observedin GBE groups for Mini-Metal State Examination scores of 30,90 and 180 days,Webster's digit symbol test scores at 30 days and Executive Dysfunction Index scores at 30 and 180 days.No significant differences were seen in the incidence of adverse events or vascular events.conclusions We conclude that GBE in combination with aspirin treatment alleviated cognitive and neurological deficits after acute ischaemic stroke without increasing the incidence of vascular events.Shanshan Li Xinjiang Zhang Qi Fang Junshan Zhou Meijuan Zhang Hui Wang Yan Chen Biyun Xu Yanfeng Wu Lai Qian Yun Xu 2017Stroke & Vascular Neurology2017,2,4:25
16Synthesis and evaluation of an oil-soluble viscosity reducer for heavy oil显示文摘To reduce the viscosity of highly-viscous oil of the Tahe oilfield (Xinjiang,China),an oilsoluble polybasic copolymer viscosity reducer for heavy oil was synthesized using the orthogonal method.The optimum reaction conditions are obtained as follows:under the protection of nitrogen,a reaction time of 9 h,monomer mole ratio of reaction materials of 3:2:2 (The monomers are 2-propenoic acid,docosyl ester,maleic anhydride and styrene,respectively),initiator amount of 0.8% (mass percent of the sum of all the monomers) and reaction temperature of 80 oC.This synthesized viscosity reducer is more effective than commercial viscosity reducers.The rate of viscosity reduction reached 95.5% at 50 oC.Infrared spectra (IR) and interfacial tensions of heavy oil with and without viscosity reducer were investigated to understand the viscosity reduction mechanism.When viscosity reducer is added,the molecules of the viscosity reducer are inserted amongst the molecules of crude oil,altering the original intermolecular structure of crude oil and weakening its ability to form hydrogen bonds with hydroxyl or carboxyl groups,so the viscosity of crude oil is reduced.Field tests of the newly developed oil-soluble viscosity reducer was carried out in the Tahe Oilfield,and the results showed that 44.5% less light oil was needed to dilute the heavy oil to achieve the needed viscosity.Guo Jixiang Wang Heyi Chen Chaogang Chen Yun Xie Xiaohai 2010Petroleum Science2010,7,4:24

19Effects of precipitation and landuse on runoff during the past 50 years in a typical watershed in the Loess Plateau, China显示文摘In the past century, great progress has been made worldwide in our understanding of forest-water relationship. The successful forestation programs implemented in China-which have improved the ecological environmental conditions-have gained the attention of many researchers and highlighted the relationship between forestation and water yields. The arid and semi-arid Loess Plateau has received attention from water engineers and eco-hydrological researchers in China because of a shortage in water resources. We selected one of the oldest stations conducting soil and water conservation experiments, the Xifeng soil and water conservation station, and chose the Nanxiaohe catchment and its paired catchments (Yangjiagou catchment and Dongzhuanggou catchment) as our research areas. Trends in precipitation, air temperature, streamflow over the past 50 years, and the effect of changing land use on streamflow were analyzed. The Mann-Kendall test showed that precipitation had a negative trend (downward trend), whereas air temperature showed a positive trend (upward trend) from the past to present in the Nanxiaohe catchment. However, the trends seen in precipitation, air temperature did not contain any 'jumping points.' The paired catchment approach is used to detect the effects of land cover change on hydrology in the Yangjiagou and the contrast catchment, i.e., Dongzhuanggou catchment in our study. The results showed a large change in land use in the Yangjiagou catchment from 1954 to 2008. An increase in forested land (from 0% to 40.08% from 1954 to 2008) and a reduction of bare land (from 51.26% to 5.50% from 1954 to 2008) accounted for a large part of the change in land use. However, the land use changed little in the contrast catchment. 20Management of cytokine release syndrome related to CAR-T cell therapy显示文摘Chimeric antigen receptor T (CAR-T) cell therapy is a novel cellular immunotherapy that is widely used to treat hematological malignancies, including acute leukemia, lymphoma, and multiple myeloma. Despite its remarkable clinical effects, this therapy has side effects that cannot be underestimated. Cytokine release syndrome (CRS) is one of the most clinically important and potentially life-threatening toxicities. This syndrome is a systemic immune storm that involves the mass cytokines releasing by activated immune cells. This phenomenon causes multisystem damages and sometimes even death. In this study, we reported the management of a patient with recurrent and refractory multiple myeloma and three patients with acute lymphocytic leukemia who suffered CRS during CAR-T treatment. The early application of tocilizumab, an anti-IL-6 receptor antibody, according to toxicity grading and clinical manifestation is recommended especially for patients who suffer continuous hyperpyrexia, hypotensive shock, acute respiratory failure, and whose CRS toxicities deteriorated rapidly. Moreover, low doses of dexamethasone (5-10 mg/day) were used for refractory CRS not responding to tocilizumab.
