1Molecular mechanisms of liver ischemia reperfusion injury:Insights from transgenic knockout models显示文摘Ischemia reperfusion injury is a major obstacle in liver resection and liver transplantation surgery.Understanding the mechanisms of liver ischemia reperfusion injury(IRI) and developing strategies to counteract this injury will therefore reduce acute complications in hepatic resection and transplantation,as well as expanding the potential pool of usable donor grafts.The initial liver injury is initiated by reactive oxygen species which cause direct cellular injury and also activate a cascade of molecular mediators leading to microvascular changes,increased apoptosis and acute inflammatory changes with increased hepatocyte necrosis.Some adaptive pathways are activated during reperfusion that reduce the reperfusion injury.IRI involves a complex interplay between neutrophils,natural killer T-cells cells,CD4+ T cell subtypes,cytokines,nitric oxide synthases,haem oxygenase-1,survival kinases such as the signal transducer and activator of transcription,Phosphatidylinositol 3-kinases/Akt and nuclear factor κβ pathways.Transgenic animals,particularly genetic knockout models,have become a powerful tool at elucidating mechanisms of liver ischaemia reperfusion injury and are complementary to pharmacological studies.Targeted disruption of the protein at the genetic level is more specific and maintained than pharmacological inhibitors or stimulants of the same protein.This article reviews the evidence from knockout models of liver IRI about the cellular and molecular mechanisms underlying liver IRI.Gourab Datta Barry J Fuller Brian R Davidson 2013World Journal of Gastroenterology2013,19,11:50
2Amino acids and mammary gland development:nutritional implications for milk production and neonatal growth显示文摘Milk is synthesized by mammary epithelial cel s of lactating mammals. The synthetic capacity of the mammary gland depends largely on the number and efficiency of functional mammary epithelial cel s. Structural development of the mammary gland occurs during fetal growth, prepubertal and post-pubertal periods, pregnancy, and lactation under the control of various hormones(particularly estrogen, growth hormone, insulin-like growth factor-I, progesterone, placental lactogen, and prolactin) in a species- and stage-dependent manner. Milk is essential for the growth, development, and health of neonates. Amino acids(AA), present in both free and peptide-bound forms, are the most abundant organic nutrients in the milk of farm animals. Uptake of AA from the arterial blood of the lactating dam is the ultimate source of proteins(primarily β-casein and α-lactalbumin) and bioactive nitrogenous metabolites in milk. Results of recent studies indicate extensive catabolism of branched-chain AA(leucine, isoleucine and valine) and arginine to synthesize glutamate,glutamine, alanine, aspartate, asparagine, proline, and polyamines. The formation of polypeptides from AA is regulated not only by hormones(e.g., prolactin, insulin and glucocorticoids) and the rate of blood flow across the lactating mammary gland, but also by concentrations of AA, lipids, glucose, vitamins and minerals in the maternal plasma, as well as the activation of the mechanistic(mammalian) target rapamycin signaling by certain AA(e.g.,arginine, branched-chain AA, and glutamine). 3我国基于团队协作的社区慢性病管理研究的分析显示文摘目的:了解我国慢性病管理实施现状,探索团队协作管理社区慢性病的改进方法。方法:利用关键词检索中国学术期刊网全文数据库、PubMed等常用中英文数据库,收集2008年1月至2013年12月公开发表的我国社区慢性病管理的研究报道,结合一定的纳入和排除标准,进行严格文献筛选;并基于慢性病团队协作管理模式对最后得出的文献进行归纳分析。结果:经系统检索后共得到698篇文献,进一步筛选得到33篇符合纳入标准的文献。在基于慢性病团队协作管理模式的几大要素中,全部文献涉及对慢性病患者的自我管理支持,但仅9篇提及团队成员之间的交流,11篇文献提及有明确的团队分工。结论:我国的社区慢性病管理工作存在不足,应建立一支真正意义上的全科服务团队,明确团队成员的角色与职责,提高团队成员的服务能力,同时应注意为患者提供多种形式的自我管理支持服务。李华艳 Jeffrey Fuller 孙玫 王勇 徐霜 冯辉 2014中南大学学报(医学版)2014,39,11:34

5湖南省基层医疗卫生机构医务人员慢性病管理能力现状调查显示文摘目的了解湖南省基层医疗卫生机构慢性病管理能力现状。方法采用多阶段分层整群随机抽样方法,对湖南省长沙、株洲、郴州、怀化、岳阳市的390名基层医疗卫生机构医务人员进行慢性病保健评估问卷调查。结果湖南省社区医务人员慢性病保健评估问卷(ACIC)总分为(7.14±1.84)分,各维度得分由低到高依次为自我管理支持(5.29±1.63)分、决策支持(6.92±2.25)分、组成部分整合(7.31±2.09)分、卫生信息系统(7.35±1.99)分、社区联系(7.36±2.29)分、服务系统的设计(7.49±2.12)分、卫生保健系统(7.91±2.07)分。长沙市社区医务人员在ACIC总分、卫生保健系统的组织、社区联系、自我管理支持、服务系统的设计得分均高于郴州和怀化市(P<0.05);初级职称社区医务人员在决策支持、服务系统的设计得分高于中级职称和副高级以上职称,同时在总分、社区联系、卫生信息系统得分高于副高及以上职称,而中级职称在社区联系、决策支持、服务系统的设计、卫生信息系统得分均高于副高及以上职称(P<0.05);不同所属地(农村/城市)、性别、学历、工作年限、职位社区医务人员ACIC量表得分差异无统计学意义(P>0.05)。结论湖南省基层医疗卫生机构医务人员在进行慢性病管理时自我管理支持能力较低,同时慢性病管理服务能力也存在地区差异。王勇 李华艳 孙玫 Jeffrey Fuller 胡平成 徐霜 冯辉 2016中国公共卫生2016,32,9:11
6HDL signaling and protection against coronary artery atherosclerosis in mice显示文摘Atherosclerosis is a leading underlying factor in cardiovascular disease and stroke,important causes of morbidity and mortality across the globe.Abundant epidemiological studies demonstrate that high levels of high density lipoprotein(HDL) are associated with reduced risk of atherosclerosis and preclinical,animal model studies demonstrate that this association is causative.Understanding the molecular mechanisms underlying the protective effects of HDL will allow more strategic approaches to development of HDL based therapeutics.Recent evidence suggests that an important aspect of the ability of HDL to protect against atherosclerosis is its ability to trigger signaling responses in a variety of target cells including endothelial cells and macrophages in the vessel wall.These signaling responses require the HDL receptor,scavenger receptor class B type 1(SR-B1),an adaptor protein(PDZK1) that binds to the cytosolic C terminus of SR-B1,Akt1 activation and(at least in endothelial cells) activation of endothelial NO synthase(eNOS).Mouse models of atherosclerosis,exemplified by apolipoprotein E or low density lipoprotein receptor gene inactivated mice(apoE or LDLR KO) develop atherosclerosis in their aortas but appear generally resistant to coronary artery atherosclerosis.On the other hand,inactivation of each of the components of HDL signaling(above)in either apoE or LDLR KO mice renders them susceptible to extensive coronary artery atherosclerosis suggesting that HDL signaling may play an important role in protection against coronary artery disease.Bernardo L Trigatti Mark Fuller 2016The Journal of Biomedical Research2016,30,2:8

8慢性病保健评估量表的汉化及条目评价显示文摘目的:汉化英文版的慢性病保健评估量表(assessment of chronic illness care,ACIC),并在湖南省试用进行文化调试,以形成适合中国国情的慢性病保健评估量表。方法:根据WHO规定的量表翻译程序汉化英文版的ACIC,并进行文化调试。对湖南省365名基层医疗卫生服务机构从事慢性病管理工作的人员进行调查,随机抽取183份有效问卷,采用离散趋势法、G-P分析法、相关系数法、逐步回归分析法、Cronbach’sα法、因子分析法6种方法进行条目筛选,用以评价量表条目。结果:6种方法评价C-ACIC量表条目显示:1)敏感性好,离散趋势法中每个条目的标准差均大于2;G-P分析法中高低分两组进行t检验,差异有统计学意义(均P<0.001);2)代表性好,相关系数法中各条目与量表总分、所属维度的总分的相关系数范围为0.588~0.916;3)内部一致性高,量表的总Cronbach’sα法为0.975,各维度的范围为0.854~0.936;4)独立性强,因子分析中各条目的因子载荷均大于0.40,因子载荷范围为0.487~0.798;5)重要性,逐步回归分析(α入=0.01,α出=0.05)中,34个条目均留在方程中。6种方法筛选条目结果保留了原英文版ACIC的7个维度34个条目。结论:中文版C-ACIC量表保留了原英文版的ACIC的34个条目,且条目具有较好的敏感性、代表性、内部一致性、独立性、重要性。崔虹艳 胡平成 徐霜 Jeffrey Fuller 孙玫 李华艳 冯辉 2015中南大学学报(医学版)2015,40,2:6
9慢性病保健评估问卷的汉化及信效度评价显示文摘目的:翻译、跨文化修订形成机构版慢性病保健评估问卷( assessment of chronic illness care,ACIC)简体中文版,并评价其信效度。方法引进 ACIC,进行翻译、回译及文化调试,最终修订为包含七个维度,34个条目的 ACIC 简体中文版。并于2013年11月-2014年2月,采用多阶段分层整群随机抽样法在湖南省抽取102家基层医疗卫生机构的400名医务人员进行问卷调查,对问卷进行内部一致性、重测信度、内容效度、因子分析等信效度分析。结果 ACIC 简体中文版可行性良好(得到有效问卷390份,有效回收率为89.0%)。问卷的 Cronbach's α系数为0.826,问卷各维度的 Cronbach's α系数为0.880~0.930;重测信度为0.722(P 〈0.01);七因子结构方程模型拟合良好,χ2/ df 为2.70,RMSEA 为0.066。结论 ACIC 简体中文版有较好的信度、效度,可作为特异性 ACIC,用于评价医疗机构慢性病保健服务能力及水平。孙玫 冯辉 李华艳 Jeffrey Fuller 徐霜 崔虹艳 王秀华 唐四元 2014中国全科医学2014,17,31:6
10Pregnancy recognition signaling mechanisms in ruminants and pigs显示文摘Maternal recognition of pregnancy refers to the requirement for the conceptus(embryo and its associated extraembryonic membranes) to produce a hormone that acts on the uterus and/or corpus luteum(CL) to ensure maintenance of a functional CL for production of progesterone;the hormone required for pregnancy in most mammals.The pregnancy recognition signal in primates is chorionic gonadotrophin which acts directly on the CL via luteinizing hormone receptors to ensure maintenance of functional CL during pregnancy.In ruminants,interferon tau(IFNT) is the pregnancy recognition signal.IFNT is secreted during the peri-implantation period of pregnancy and acts on uterine epithelia to silence expression of estrogen receptor alpha and oxytocin receptor which abrogates the oxytocin-dependent release of luteolytic pulses of prostaglandin F2-alpha(PGF) by uterine epithelia;therefore,the CL continues to produce progesterone required for pregnancy.Pig conceptuses secrete interferon delta and interferon gamma during the peri-implantation period of pregnancy,but there is no evidence that they are involved in pregnancy recognition signaling.Rather,pig conceptuses secrete abundant amounts of estrogens between Days 11 to 15 of pregnancy required for maternal recognition of pregnancy.Estrogen,likely in concert with prolactin,prevents secretion of PGF into the uterine venous drainage(endocrine secretion),but maintains secretion of PGF into the uterine lumen(exocrine secretion) where it is metabolized to a form that is not luteolytic.Since PGF is sequestered within the uterine lumen and unavailable to induce luteolysis,functional CL are maintained for production of progesterone.In addition to effects of chorionic gonadotrophin,IFNT and estrogens to signal pregnancy recognition,these hormones act on uterine epithelia to enhance expression of genes critical for growth and development of the conceptus.Fuller W Bazer 2013Journal of Animal Science and Biotechnology2013,4,3:6
11美国国家卒中协会短暂性脑缺血发作处理指南显示文摘目的:短暂性脑缺血发作(TIA)是常见和重要的卒中先兆。其处理方法多样,大多数发表的指南在近年来均未被更新。我们试图制定一个全面的、无偏倚的、循证的TIA处理指南。方法:根据一种客观标准(可以预测在该研究领域中执业医师对专家提名的文献测量学方法)挑选出15名专家组成员。通过系统回顾检索到过去发表的指南,由专家对其中的推荐意见的质量进行独立评价。选择出质量最高的推荐意见,然后让专家组采用改良Delphi法通过多次问卷调查的方式进行修订,从而对新的修改达成共识。给专家们提供近期临床研究的系统评价,要求专家根据新的证据判断措辞修改的合理性并且根据证据等级和质量对最终的推荐意见进行评定。不允许专家在预期有可能存在任何利益冲突的专题方面提出推荐意见。结果:通过系统回顾检索到257个指南,其中有13篇文献包括的137条推荐意见符合所有纳入标准。需要6次重复的问卷调查对53条最终推荐意见的措辞达成共识。最终的推荐意见涉及TIA的初步处理、评价、内科治疗、外科治疗和危险因素控制。结论:对TIA患者医疗诊治的最终推荐意见强调了紧急评价和治疗的重要性。这种用于制定本指南的新方法是可行的,考虑到了快速更新并能减少偏倚。S. 12Application of new biotechnologies for improvements in swine nutrition and pork production显示文摘Meeting the increasing demands for high-quality pork protein requires not only improved diets but also biotechnology-based breeding to generate swine with desired production traits.Biotechnology can be classified as the cloning of animals with identical genetic composition or genetic engineering(via recombinant DNA technology and gene editing)to produce genetically modified animals or microorganisms.Cloning helps to conserve species and breeds,particularly those with excellent biological and economical traits.Recombinant DNA technology combines genetic materials from multiple sources into single cells to generate proteins.Gene(genome)editing involves the deletion,insertion or silencing of genes to produce:(a)genetically modified pigs with important production traits;or(b)microorganisms without an ability to resist antimicrobial substances.Current gene-editing tools include the use of zinc finger nuclease(ZFN),transcription activator-like effector nuclease(TALEN),or clustered regularly interspaced short palindromic repeats-associated nuclease-9(CRISPR/Cas9)as editors.ZFN,TALEN,or CRISPR/Cas9 components are delivered into target cells through transfection(lipid-based agents,electroporation,nucleofection,or microinjection)or bacteriophages,depending on cell type and plasmid.Compared to the ZFN and TALEN,CRISPR/Cas9 offers greater ease of design and greater flexibility in genetic engineering,but has a higher frequency of off-target effects.To date,genetically modified pigs have been generated to express bovine growth hormone,bacterial phytase,fungal carbohydrases,plant and C.elagan fatty acid desaturases,and uncoupling protein-1;and to lack myostatin,α-1,3-galactosyltransferase,or CD163(a cellular receptor for the'blue ear disease'virus).Biotechnology holds promise in improving the efficiency of swine production and developing alternatives to antibiotics in the future.Guoyao Wu Fuller W.Bazer 2019Journal of Animal Science and Biotechnology2019,10,4:5

14Tumor Antigen Specific Activation of Primary Human T-Cells Expressing a Virally Encoded Chimeric T-Cell Receptor Specific for p185HER2显示文摘We have developed and tested chimeric T-cell receptors (TCR) specific for p185HER2. In these experiments, retroviral vectors expressing the N29γ or N29ζ receptors were constructed in pRET6. Amphotropic viral producer cells were established in the GALV-based PG13 packaging cell line. Ficoll purified human peripheral blood lymphocytes (PBL) were virally transduced using an optimized protocol incorporating activation with immobilized anti-CD3/anti-CD28 monoclonal anti- bodies, followed by viral infection in the presence of fibronectin fragment CH296. Transduced cells were co-cultured with human tumor cell lines that overexpress (SK-OV-3) or underexpress (MCF7) p185HER2 to assay for antigen specific im- mune responses. Both CM+ and CD8+ T-cells transduced with the N29γ or N29ζ chTCR demonstrated HER2-specific anti- gen responses, as determined by release of Th1 like cytokines, and cellular cytotoxicity assays. 15Osteopontin: a leading candidate adhesion molecule for implantation in pigs and sheep显示文摘Osteopontin(OPN; also known as Secreted Phosphoprotein 1, SPP1) is a secreted extra-cellular matrix(ECM) protein that binds to a variety of cell surface integrins to stimulate cell-cell and cell-ECM adhesion and communication. It is generally accepted that OPN interacts with apically expressed integrin receptors on the uterine luminal epithelium(LE)and conceptus trophectoderm to attach the conceptus to the uterus for implantation. Research conducted with pigs and sheep has significantly advanced understanding of the role(s) of OPN during implantation through exploitation of the prolonged peri-implantation period of pregnancy when elongating conceptuses are free within the uterine lumen requiring extensive paracrine signaling between conceptus and endometrium. This is followed by a protracted and incremental attachment cascade of trophectoderm to uterine LE during implantation, and development of a true epitheliochorial or synepitheliochorial placenta exhibited by pigs and sheep, respectively. In pigs, implanting conceptuses secrete estrogens which induce the synthesis and secretion of OPN in adjacent uterine LE. OPN then binds to αvβ6 integrin receptors on trophectoderm, and the αvβ3 integrin receptors on uterine LE to bridge conceptus attachment to uterine LE for implantation. In sheep, implanting conceptuses secrete interferon tau that prolongs the lifespan of CL. Progesterone released by CL then induces OPN synthesis and secretion from the endometrial GE into the uterine lumen where OPN binds integrins expressed on trophectoderm(αvβ3) and uterine LE(identity of specific integrins unknown) to adhere the conceptus to the uterus for implantation. OPN binding to the αvβ3 integrin receptor on ovine trophectoderm cells induces in vitro focal adhesion assembly, a prerequisite for adhesion and migration of trophectoderm, through activation of: 1) P70S6 K via crosstalk between FRAP1/MTOR and MAPK pathways; 2) MTOR,PI3 K, MAPK3/MAPK1(Erk1/2) and MAPK14(p38) signaling to stimulate trohectoderm cell migration; and 3) focal adhesion assembly and myosin II motor activity to induce migration of trophectoderm cells. 17Uterine biology in pigs and sheep显示文摘There is a dialogue between the developing conceptus (embryo-fetus and associated placental membranes) and maternal uterus which must be established during the peri-implantation period for pregnancy recognition signaling, implantation, regulation of gene expression by uterine epithelial and stromal cells, placentation and exchange of nutrients and gases. The uterus provide a microenvironment in which molecules secreted by uterine epithelia or transported into the uterine lumen represent histotroph required for growth and development of the conceptus and receptivity of the uterus to implantation. Pregnancy recognition signaling mechanisms sustain the functional lifespan of the corpora lutea (CL) which produce progesterone, the hormone of pregnancy essential for uterine functions that support implantation and placentation required for a successful outcome of pregnancy. It is within the peri-implantation period that most embryonic deaths occur due to deficiencies attributed to uterine functions or failure of the conceptus to develop appropriately, signal pregnancy recognition and/or undergo implantation and placentation. With proper placentation, the fetal fluids and fetal membranes each have unique functions to ensure hematotrophic and histotrophic nutrition in support of growth and development of the fetus. The endocrine status of the pregnant female and her nutritional status are critical for successful establishment and maintenance of pregnancy. 18Haemoxygenase modulates cytokine induced neutrophil chemoattractant in hepatic ischemia reperfusion injury显示文摘AIM To investigate the hepatic microcirculatory changes due to Haemoxygenase(HO),effect of HO inhibition on remote ischemic preconditioning(RIPC) and modulation of CINC.METHODS Eight groups of animals were studied- Sham,ischemia reperfusion injury(IRI) the animals were subjected to 45 min of hepatic ischemia followed by three hours of reperfusion,RIPC(remote ischemic preconditioning) + IRI group,remote ischemic preconditioning in sham(RIPC + Sham),PDTC + IR(Pyridodithiocarbamate,HO donor),Zn PP + RIPC + IRI(Zinc protoporphyrin prior to preconditioning),IR-24(45 min of ischemia followed by 24 h of reperfusion),RIPC+IR-24(preconditioning prior to. After 3 and 24 h of reperfusion the animals were killed by exsanguination and samples were taken. RESULTS Velocity of flow(160.83 ± 12.24 μm/s),sinusoidal flow(8.42 ± 1.19) and sinusoidal perfusion index(42.12 ± 7.28) in hepatic IR were lower(P < 0.05) in comparison to RIPC and PDTC(HO inducer). RIPC increased velocity of flow(328.04 ± 19.13 μm/s),sinusoidal flow(17.75 ± 2.59) and the sinusoidal perfusion index(67.28 ± 1.82)(P < 0.05). PDTC(HO induction) reproduced the effects of RIPC in hepatic IR. PDTC restored RBC velocity(300.88 ± 22.109 μm/s),sinusoidal flow(17.66 ± 3.71) and sinusoidal perfusion(82.33 ± 3.5) to near sham levels. Zn PP(HO inhibition) reduced velocity of flow of RBC in the RIPC group(170.74 ± 13.43 μm/s and sinusoidal flow in the RIPC group(9.46 ± 1.34). Zn PP in RIPC(60.29 ± 1.82) showed a fall in perfusion only at 180 min of reperfusion. Neutrophil adhesion in IR injury is seen in both postsinusoidal venules(769.05 ± 87.48) and sinusoids(97.4 ± 7.49). Neutrophil adhesion in RIPC + IR injury is reduced in both postsinusoidal venules(219.66 ± 93.79) and sinusoids(25.69 ± 9.08)(P < 0.05). PDTC reduced neutrophil adhesion in both postsinusoidal venules(89.58 ± 58.32) and sinusoids(17.98 ± 11.01)(P < 0.05) reproducing the effects of RIPC. Zn PP(HO inhibition) increased venular(589.04 ± 144.36) and sinusoidal neutrophil adhesion in preconditioned animals(121.39 ± 30.65)(P < 0.05). IR after 24 h of reperfusion increased venular and sinusoidal neutrophil adhesion in comparison to the early phase and was significantly reduced by RIPC. Hepatocellular cell death in IRI(80.83 ± 13.03),RIPC + IR(17.35 ± 2.47),and PTDC+IR(11.66 ± 1.17) Zn PP + RIPC + IR(41.33 ± 3.07) reduced hepatocellular death. Zn PP significantly increased hepatocellular death(P < 0.05 PTDC/RIPC vs Zn PP and IR). The CINC cytokine levels in sham(101.32 ± 6.42). RIPC + sham(412.18 ± 65.24) as compared to sham(P < 0.05). Hepatic IR(644.08 ± 181.24)(P < 0.05). RIPC CINC-1 levels in the early phase(401.62 ± 78.56). And PDTC(HO inducer) CINC-1 levels in hepatic IR(413.36 ± 63.06) were significantly lower. HO inhibition in preconditioned animals with Zinc protoporphyrin increased serum CINC levels(521.81 ± 74.9)(P < 0.05). The serum CINC levels were high in the late phase of hepatic IR(15306 ± 1222.04). 19In vivo anti-tumor activity of murine hematopoietic stem cells expressing a p185HER2-specific chimeric T-cell receptor gene显示文摘We have confirmed efficient anti-tumor activities of the peripheral lymphocytes transduced with a p185HER2-specific chimeric T-cell receptor gene both in murine and in human in our previous studies. To further test the feasibility of chimeric T-cell receptor in a bone marrow transplantation model, we first, made two murine tumor cell lines: MT901 and MCA-205, to express human p185HER2 by retroviral gene transduction. Murine bone marrow cells were retrovirally transduced to express the chimeric T-cell receptor and gene-modified bone marrow cells were transplanted into lethally irradiated mouse. Six months post transplantation, p185HER2-positive tumor cells:MT-901/HER2 or MCA-205/ HER2 was subcutaneously or intravenously injected to make mouse models simulating primary breast cancer or pulmonary metastasis. The in vivo anti-tumor effects were monitored by the size of the subcutaneous tumor or counting the tumor nodules in the lungs after India ink staining. The size of the subcutaneous tumor was significantly inhibited and the number of pulmonary nodules were significantly decreased in mouse recipients transplanted with chimeric T-cell receptor modified bone marrow cells compared with the control group.
