| 1 | Appropriateness of indication and diagnostic yield of colonoscopy: First report based on the 2000 guidelines of the American Society for Gastrointestinal Endoscopy显示文摘AIM: To assess the appropriateness of referrals and to determine the diagnostic yield of colonoscopy according to the 2000 guidelines of the American Society for Gastrointestinal Endoscopy (ASGE).METHODS: A total of 736 consecutive patients (415males, 321 females; mean age 43.6±16.6 years)undergoing colonoscopy during October 2001-March2002 Were prospectively enrolled in the study. The 2000ASGE guidelines were used to assess the appropriateness of the indications for the procedure. Diagnostic yield was defined as the ratio between significant findings detected on colonoscopy and the total number of procedures performed for that indication.RESULTS: The large majority (64%) of patients had colonoscopy for an indication that was considered'generally indicated'; it was 'generally not indicated' for20%, and it was 'not listed' for 16% in the guidelines.The diagnostic yield of colonoscopy was highest for the 'generally indicated' (38%) followed by 'not listed'(13%) and 'generally not indicated' (5%) categories.In the multivariable analysis, the diagnostic yield was independently associated with the appropriateness of indication that was 'generally indicated' (odds ratio=12.3) and referrals by gastroenterologist (odds ratio = 1.9).CONCLUSION: There is a high likelihood of inappropriate referrals for colonoscopy in an open-access endoscopy system. The diagnostic yield of the procedure is dependent on the appropriateness of indication and referring physician's specialty. | 13 | Management of nonalcoholic fatty liver disease in the Middle East显示文摘The prevalence of nonalcoholic fatty liver disease(NAFLD)in the Middle East is increasing in parallel to an increase in the prevalence of associated risk factors such as obesity,metabolic syndrome,and type 2 diabetes mellitus.About 20%to 30%of the patients progress to develop nonalcoholic steatohepatitis(NASH),a histological subtype of NAFLD,with features of hepatocyte injury such as hepatocyte ballooning.NASH can progress to fibrosis,cirrhosis,and even hepatocellular carcinoma.NAFLD thus causes a substantial burden on healthcare systems and it is imperative that appropriate strategies are discussed at a regional level to facilitate effective management tailored to the needs of the region.To fulfil this unmet need,expert gastroenterologists,hepatologists,and endocrinologists from the region came together in three advisory board meetings that were conducted in Saudi Arabia,United Arab Emirates,and Kuwait,to discuss current local challenges in NAFLD screening and diagnosis,and the different available management options.The experts discussed the disease burden of NAFLD/NASH in the Middle East;screening,diagnosis,and referral patterns in NAFLD;and available treatment options for NAFLD and NASH.This paper summarizes the discussions and opinion of the expert panel on the management of NAFLD/NASH and also presents an extensive literature review on the topic. | Faisal M Sanai Faisal Abaalkhail Fuad Hasan Muhammad Hamed Farooqi Nawal Al Nahdi Zobair M Younossi | 2020 | World Journal of Gastroenterology2020,26,25: | 0 |
| 14 | Accelerating the elimination of hepatitis C in Kuwait: An expert opinion显示文摘The hepatitis C virus(HCV)is estimated to affect 71 million people worldwide.In 2016,the World Health Organization adopted the first global health sector strategy to eliminate viral hepatitis as a public health threat by 2030.In December 2018,the European Association for the Study of the Liver,International Liver Foundation convened an expert panel to address the elimination of HCV in Kuwait.Several steps have already been taken to eliminate HCV in Kuwait,including free HCV treatment for Kuwait’s citizens,high blood safety standards,and the implementation of screening and awareness programs.The expert panel made several recommendations aimed at accelerating the elimination of HCV in Kuwait:The development of a national strategy and action plan to guide all HCV elimination activities;the formation of a coordination mechanism to support collaboration between hepatitis working committees;the prioritization of microelimination at primary,secondary or tertiary facilities,in prisons and rehabilitation centers;and ensuring the involvement of multiple stakeholders–including relevant civil society groups–in all activities.Enhanced screening and linkage to care should be prioritized in Kuwait,with the expansion of the prescriber base to primary healthcare providers and nurse practitioners to be considered.Raising awareness and educating people about HCV infection also remain essential to achieve the goal of HCV elimination.Lastly,a national HCV registry should be developed to help monitor the implementation of viral hepatitis plans and progress towards achieving national and international targets. | Fuad Hasan Ahmad Alfadhli Abeer Al-Gharabally Mahmoud Alkhaldi Massimo Colombo Jeffrey V Lazarus | 2020 | World Journal of Gastroenterology2020,26,30: | 0 |
