1Cystic echinococcosis of the liver: A primer for hepatologists显示文摘Cystic echinococcosis(CE) is a complex, chronic and neglected disease with a worldwide distribution. The liver is the most frequent location of parasitic cysts. In humans, its clinical spectrum ranges from asymptom-atic infection to severe, potentially fatal disease. Four approaches exist in the clinical management of CE: surgery, percutaneous techniques and drug treatment for active cysts, and the 'watch and wait' approach for inactive cysts. Allocation of patients to these treat-ments should be based on cyst stage, size and location, available clinical expertise, and comorbidities. However, clinical decision algorithms, efficacy, relapse rates, and costs have never been properly evaluated. 2Medical treatment for gastro-entero-pancreatic neuroendocrine tumours显示文摘Gastro-entero-pancreatic neuroendocrine neoplasms(GEPNENs) represents a various family of rare tumours. Surgery is the first choice in GEP-NENs patients with localized disease whilst in the metastatic setting many other treatment options are available. Somatostatin analogues are indicated for symptoms control in functioning tumours. Furthermore they may be effective to inhibit tumour progression. GEP-NENs pathogenesis has been extensively studied in the last years therefore several driver mutations pathway genes have been identified as crucial factors in their tumourigenesis. GEP-NENs can over-express vascular endothelial growth factor(VEGF), basic-fibroblastic growth factor, transforming growth factor(TGF-α and-β), platelet derived growth factor(PDGF), insulin-like growth factor-1(IGF-1) and their receptors PDGF receptor, IGF-1 receptor, epidermal growth factor receptor, VEGF receptor, and c-kit(stem cell factor receptor) that can be considered as potential targets. The availability of new targeted agents, such as everolimus and sunitinib that are effective in advanced and metastatic pancreatic neuroendocrine tumours, has provided new treatment opportunities. 5Boceprevir or telaprevir in hepatitis C virus chronic infection:The Italian real life experience显示文摘AIM: To check the safety and efficacy of boceprevir/telaprevir with peginterferon/ribavirin for hepatitis C virus(HCV) genotype 1 in the real-world settings. METHODS: This study was a non-randomized, observational, prospective, multicenter. This study involved 47 centers in Italy. A database was prepared for the homogenous collection of the data, was used by all of the centers for data collection, and was updated continuously. All of the patients enrolled in this study were older than 18 years of age and were diagnosed with chronic infection due to HCV genotype 1. The HCV RNA testing was performed using COBAS-Taq Man2.0(Roche, LLQ 25 IU/m L). RESULTS: All consecutively treated patients were included. Forty-seven centers enrolled 834 patients as follows: Male 64%; median age 57(range 18-78), of whom 18.3% were over 65; mean body mass index 25.6(range 16-39); genotype 1b(79.4%); diagnosis of cirrhosis(38.2%); and fibrosis F3/4(71.2%). The following drugs were used: Telaprevir(66.2%) and PEG-IFN-alpha2a(67.6%). Patients were na?ve(24.4%), relapsers(30.5%), partial responders(14.8%) and null responders(30.3%). Overall, adverse events(AEs) occurred in 617 patients(73.9%) during the treatment. Anemia was the most frequent AE(52.9% of cases), especially in cirrhotic. The therapy was stopped for 14.6% of the patients because of adverse events or virological failure(15%). Sustained virological response was achieved in 62.7% of the cases, but was 43.8% in cirrhotic patients over 65 years of age. 6Hemorrhagic cardiac tamponade after percutaneous laser ablation of a liver metastasis in segment II显示文摘Despite percutaneous laser thermal ablation(LTA)of liver tumors being regarded as a safe technique,major complications can occur.We report the first case of hemorrhagic cardiac tamponade after LTA of a colorectal metastasis in segment II of the liver.Unpredictable heat diffusion causing indirect thermal injury to the pericardium with resultant hemorrhagic reaction was hypothesized as the most likely cause of tamponade.A pericardial drain was emergently placed,200 mL of bright red blood were drained,and the patient showed rapid hemodynamic improvement.For lesions located in segment II of the liver and strictly close to the pericardium,a careful risk/benefit analysis should be made by the multidisciplinary team to identify the best treatment option,taking into account both effectiveness and complications of each available technique.Paola Tombesi Francesca Di Vece Silvia Rinaldi Matteo Bertini Sergio Sartori 2016Hepatoma Research2016,2,1:0
7Squamous cell carcinoma of the lung: clinical criteria for treatment strategy显示文摘Aim:Primary lung cancer is the leading cause of human cancer deaths worldwide,and squamous cell carcinoma(SCC)is one of the most frequent histologic subtypes.The aim of our study was to analyze clinical factors potentially affecting the overall outcome of advanced lung SCC patients.Methods:A series of 72 consecutive patients with advanced SCC undergoing chemotherapy at our institution between January 2007 and July 2013 were eligible for our analysis.Results:By univariate analysis,a better overall survival(OS)was related to response to first-line chemotherapy:median OS were 19.7 vs.7.17 months,respectively,for responders and nonresponders patients(P<0.0001).Eastern Cooperative Oncology Group performance status,gender,and surgery were other prognostic factors.No signifi cant relationship between OS and smoking status,age,body mass index,or type of treatment was found.In the third-line setting,a better OS was associated with objective response to second-line treatment(P=0.015).Conclusion:Our results suggest that differences in OS seem strictly associated with clinical response to previous treatments.These data should be considered in the therapeutic strategy and management of patients with SCC of the lung.Agnese Savini Rossana Berardi Paola Mazzanti Miriam Caramanti Matteo Santoni Mariagrazia De Lisa Francesca Morgese Silvia Rinaldi Mariangela Torniai Ilaria Fiordoliva Azzurra Onofri Stefano Cascinu 2015Journal of Cancer Metastasis and Treatment2015,1,1:0
8Medical therapy for advanced gastro-entero-pancreatic and bronchopulmonary neuroendocrine tumors显示文摘Neuroendocrine tumors(NETs)represent a spectrum of rare neoplasms arising in different organism sites.Depending on the site of onset,they also can be distinguished using lab exams(secreting vs.nonsecreting),clinical symptoms(functioning vs.nonfunctioning),behavioral,morphological characteristics(tumor cells’architectural growth patterns,mitotic and Ki-67 index,presence of necrosis),and grade of cellular differentiation.The aim of this review is to focus on the main signaling pathways targeted by medical treatments of advanced sporadic gastro-entero-pancreatic(GEP)and bronchopulmonary(BP)neuroendocrine neoplasms.The scientific literature regarding treatment of advanced GEP and BP-NETs has been extensively reviewed using MEDLINE and PubMed databases,selecting principal and more recent research articles,clinical trials,and updated guidelines.Somatostatin analogues represent a valid approach to control symptoms in functioning tumors and to inhibit tumor progression in certain categories on the basis of the typical somatostatin receptor expression observed in NETs.The pathogenesis of NETs has been the subject of increased interest in recent years.Many driver mutations pathway genes have been identified as important factors in the carcinogenesis process and,therefore,as potential targets for new anticancer therapies.Activating mutations have been shown in epidermal growth factor receptor,stem cell factor receptor,platelet-derived growth factor receptor,vascular endothelial growth factor,basic-fibroblastic growth factor,transforming growth factor,insulin-like growth factor-1,and their receptors.Effective M-Tor inhibition pathway modulation has led to the approval of drugs in this field such as everolimus.New drugs and several combination regimens with targeted and newer biological agents are being developed and tested in recently conducted and ongoing trials.Mariangela Torniai Silvia Rinaldi Francesca Morgese Giulia Ricci Azzurra Onofri Christian Grohé Rossana Berardi 2016Journal of Cancer Metastasis and Treatment2016,2,1:0
9Incidental vertebral compression fractures in imaging studies:Lessons not learned by radiologists显示文摘AIM:To assess radiologists reporting rates of incidental vertebral compression fractures in imaging studies. METHODS:We performed a review of the current literature on the prevalence and reporting rates of incidental vertebral compression fractures in radiologic examinations. RESULTS:The bibliographic search revealed 12 studies:7 studies using conventional radiology and 5 using multidetector computed tomography(MDCT).The lossof height cut-off to define a vertebral fracture varied from 15%to 25%.Fracture prevalence was high (mean 21.1%;range 9.5%-35%)in both radiographic and MDCT studies(mean 21.6%and 20.2%,respectively).Reporting rates were low with a mean value of 27.4%(range 0%-66.3%)and were significantly lower in MDCT than in radiographic studies(mean 8.1%vs 41.1%).Notably,recent studies showed lower reporting rates than older studies.
