| 1 | Endoscopic anti-reflux therapy for gastroesophageal reflux disease显示文摘Gastroesophageal reflux disease has an increasing incidence and prevalence worldwide.A significant proportion of patients have a suboptimal response to proton pump inhibitors or are unwilling to take lifelong medication due to concerns about long-term adverse effects.Endoscopic anti-reflux therapies offer a minimally invasive option for patients unwilling to undergo surgical treatment or take lifelong medication.The best candidates are those with a good response to proton pump inhibitors and without a significant sliding hiatal hernia.Transoral incisionless fundoplication and nonablative radiofrequency are the techniques with the largest body of evidence and that have been tested in several randomized clinical trials.Band-assisted ligation techniques,anti-reflux mucosectomy,antireflux mucosal ablation,and new plication devices have yielded promising results in recent noncontrolled studies.Nonetheless,the role of endoscopic procedures remains controversial due to limited long-term and comparative data,and no consensus exists in current clinical guidelines.This review provides an updated summary focused on the patient selection,technical details,clinical success,and safety of current and future endoscopic anti-reflux techniques. | Enrique Rodríguez de Santiago Eduardo Albéniz Fermin Estremera-Arevalo Carlos Teruel Sanchez-Vegazo Vicente Lorenzo-Zúñiga | 2021 | World Journal of Gastroenterology2021,27,39: | 9 |
| 2 | New Framework Based on Fusion Information from Multiple Landslide Data Sources and 3D Visualization显示文摘Recent monitoring techniques employ multiple sources of information for the characterization of the phenomenon to be studied, being the coupling and adjustment of multi-source data one of the first challenges to consider and solve. The authors propose a new framework of the multi-source and multi-temporal data-oriented fusion for the characterization of landslide events. The main objective is to generate 3D virtual models(in the form of dense point clouds) and feed them back with the characteristic of soil and subsoil information. The scheme consists of three main steps. The first one is on-site data collection(geological characterization, geophysical measurements, GPS measurements, and UAV/drone mapping). The second step is generation of a high-resolution 3D virtual model(~1-inch spatial resolution) from the frames acquired through the UAV using the structure of motion(SfM) processing;the developed virtual model is optimized with GPS measurements to minimize geolocation error and eliminate distortions. | 3 | Multiple myeloma mesenchymal stromal cells: Contribution to myeloma bone disease and therapeutics显示文摘Multiple myeloma is a hematological malignancy inwhich clonal plasma cells proliferate and accumulate within the bone marrow. The presence of osteolytic le-sions due to increased osteoclast(OC) activity and sup-pressed osteoblast(OB) function is characteristic of the disease. The bone marrow mesenchymal stromal cells(MSCs) play a critical role in multiple myeloma patho-physiology, greatly promoting the growth, survival, drug resistance and migration of myeloma cells. Here, we specifically discuss on the relative contribution of MSCs to the pathophysiology of osteolytic lesions in light of the current knowledge of the biology of my-eloma bone disease(MBD), together with the reported genomic, functional and gene expression differences between MSCs derived from myeloma patients(pMSCs) and their healthy counterparts(dMSCs). Being MSCs the progenitors of OBs, pMSCs primarily contribute to the pathogenesis of MBD because of their reduced osteogenic potential consequence of multiple OB inhibi-tory factors and direct interactions with myeloma cells in the bone marrow. Importantly, pMSCs also readily contribute to MBD by promoting OC formation and ac-tivity at various levels(i.e., increasing RANKL to OPG expression, augmenting secretion of activin A, uncou-pling ephrinB2-EphB4 signaling, and through augment-ed production of Wnt5a), thus further contributing to OB/OC uncoupling in osteolytic lesions.
