| 2 | Approach to medical therapy in perianal Crohn’s disease显示文摘Perianal Crohn’s disease remains a challenging condition to treat and can have a substantial negative impact on quality of life.It often requires combined surgical and medical interventions.Anti-tumor necrosis factor(anti-TNF)therapy,including infliximab and adalimumab,remain preferred medical therapies for perianal Crohn’s disease.Infliximab has been shown to be efficacious in improving fistula closure rates in randomized controlled trials.Clinicians can be faced with a number of questions relating to the optimal use of anti-TNF therapy in perianal Crohn’s disease.Specific issues include evaluation for the presence of perianal sepsis,the treatment target of therapy,the ideal time to commence treatment,whether additional medical therapy should be used in conjunction with anti-TNF therapy,and the duration of treatment.This article will discuss key studies which can assist clinicians in addressing these matters when they are considering or have already commenced anti-TNF therapy for the treatment of perianal Crohn’s disease.It will also discuss current evidence regarding the use of vedolizumab and ustekinumab in patients who are failing to achieve a response to anti-TNF therapy for perianal Crohn’s disease.Lastly,new therapies such as local injection of mesenchymal stem cell therapy will be discussed. | Abhinav Vasudevan David H Bruining Edward V Loftus Jr William Faubion Eric C Ehman Laura Raffals | 2021 | World Journal of Gastroenterology2021，27，25： | 4 |

| 4 | Predictive factors for compliance with transanal irrigation for the treatment of defecation disorders显示文摘AIM To investigate compliance with transanal irrigation(TAI) one year after a training session and to identify predictive factors for compliance.METHODS The compliance of one hundred eight patients [87 women and 21 men; median age 55 years(range 18-83)] suffering from constipation or fecal incontinence(FI) was retrospectively assessed. The patients were trained in TAI over a four-year period at a single institution. They were classified as adopters if they continued using TAI for at least one year after beginning the treatment or as non-adopters if they stopped. Predictive factors of compliance with TAI were based on pretreatment assessments and trainingprogress. The outcomes of the entire cohort of patients who had been recruited for the TAI treatment were expressed in terms of intention-to-treat.RESULTS Forty-six of the 108(43%) trained patients continued to use TAI one year after their training session. The patients with FI had the best results, with 54.5% remaining compliant with TAI. Only one-third of the patients who complained of slow transit constipation or obstructed defecation syndrome continued TAI. There was an overall discontinuation rate of 57%. The most common reason for discontinuing TAI was the lack of efficacy(41%). However, 36% of the patients who discontinued TAI gave reasons independent of the efficacy of the treatment such as technical problems(catheter expulsion, rectal balloon bursting, instilled water leakage or retention, pain during irrigation, anal bleeding, anal fissure) while 23% said that there were too many constraints. | 6 | Combination drug regimens for metastatic clear cell renal cell carcinoma显示文摘Renal cell carcinomas(RCC)make up about 90%of kidney cancers,of which 80%are of the clear cell subtype.About 20%of patients are already metastatic at the time of diagnosis.Initial treatment is often cytoreductive nephrectomy,but systemic therapy is required for advanced RCC.Single agent targeted therapies are moderately toxic and only somewhat effective,leading to development of immunotherapies and combination therapies.This review identifies limitations of monotherapies for metastatic renal cell carcinoma,discusses recent advances in combination therapies,and highlights therapeutic options under development.The goal behind combining various modalities of systemic therapy is to potentiate a synergistic antitumor effect.However,combining targeted therapies may cause increased toxicity.The initial attempts to create therapeutic combinations based on inhibition of the vascular endothelial growth factor or mammalian target of rapamycin pathways were largely unsuccessful in achieving a profile of increased synergy without increased toxicity.To date,five combination therapies have been approved by the U.S.Food and Drug Administration,with the most recently approved therapies being a combination of checkpoint inhibition plus targeted therapy.Several other combination therapies are under development,including some in the phase 3 stage.The new wave of combination therapies for metastatic RCC has the potential to increase response rates and improve survival outcomes while maintaining tolerable side effect profiles. | Viraj V Khetani Daniella E Portal Mansi R Shah Tina Mayer Eric A Singer | 2020 | World Journal of Clinical Oncology2020，11，8： | 2 |
| 7 | Who is at risk in the French mountains? Profiles of the accident victims in outdoor sports and mountain recreation显示文摘The risk of both severe and frequent injury associated with active pursuits in mountain areas is acknowledged. This phenomenon is all the most relevant to investigate in France that there is a growing enthusiasm for outdoor sports and recreation. This paper focuses on the social-demographic and sporting profiles of accident victims. Our intent is to better understand who is specifically at risk, while practicing mountain recreation, in the French mountains. Such identification doubtlessly constitutes an important asset in order to enhance prevention. Three critical activities are notably tackled, among some others: hiking, a widespread activity that exposes many practitioners to hazardous settings; mountaineering and ski touring, supposedly entailing the most severe injuries.In order to align the information collected, then provide estimations based on the reasoned crossing of these secondary data, we consulted and summarised all the databases that we knew of on the French territory: rescue team intervention reports, aggregated statistics from the sports ministry, sporting federation accident declarations, avalanche experts statistics, mountain guides union accident/incident reports, healthcare facilities admissions, etc. Only the sources consideredto be the most reliable and representative were used, owing to the number of cases processed, the exhaustive nature of data collection, or the level of information detail available. Our method also included consultation of all the parties playing an active role in prevention, training and mountain rescue, thus ensuring a participative, co-constructive approach to the study with the people that have the best knowledge of victim profiles. Around fifty such parties were interviewed on the methods implemented, the benefits and limits of current data, and the main expectations in terms of accidentology progress, thus further enhancing the credibility of the study proposed in this paper. Our results show that the parent population profiles(in terms of age, gender, place of residence, etc.), weighted by the level of exposure(frequency of participation, level of engagement, etc.) appears to have an impact on the accident victim profiles. There is certainly a very marked gender bias and a serious age impact to be taken into account, as well as the influence of the level of experience, which counter certain preconceived ideas and which, at the very least, go against instinct. However, aside from a probable over-exposure of experienced or expert participants, much remains to be explained concerning the accident mechanisms and circumstances involved.
