| 1 | I.31, a new combination of probiotics, improves irritable bowel syndrome-related quality of life显示文摘AIM:To determine the dose-related effects of a novel probiotic combination,I.31,on irritable bowel syndrome(IBS)-related quality of life(IBS-QoL).METHODS:A multicenter,randomized,double-blind,placebo-controlled intervention clinical trial with three parallel arms was designed.A total of 84 patients(53female,31 male;age range 20-70 years)with IBS and diarrhea according to Rome-Ⅲcriteria were randomly allocated to receive one capsule a day for 6 wk containing:(1)I.31 high dose(n=28);(2)I.31 low dose(n=27);and(3)placebo(n=29).At baseline,and 3and 6 wk of treatment,patients filled the IBSQoL,Visceral Sensitivity Index(VSI),and global symptom relief questionnaires.RESULTS:During treatment,IBS-QoL increased in all groups,but this increment was significantly larger in patients treated with I.31 than in those receiving placebo(P=0.008).After 6 wk of treatment,IBS-QoL increased by 18±3 and 22±4 points in the high and the low dose groups,respectively(P=0.041 and P=0.023vs placebo),but only 9±3 in the placebo group.Gutspecific anxiety,as measured with VSI,also showed a significantly greater improvement after 6 wk of treatment in patients treated with probiotics(by 10±2 and14±2 points,high and low dose respectively,P<0.05for both vs 7±1 score increment in placebo).Symptom relief showed no significant changes between groups.No adverse drug reactions were reported following the consumption of probiotic or placebo capsules.CONCLUSION:A new combination of three different probiotic bacteria was superior to placebo in improving IBS-related quality of life in patients with IBS and diarrhea. | Vicente Lorenzo-Zú?iga Elba Llop Cristina Suárez Beatriz álvarez Luis Abreu Jordi Espadaler Jordi Serra | 2014 | World Journal of Gastroenterology2014，20，26： | 13 |
| 2 | Hazardous elements and amorphous nanoparticles in historical estuary coal mining area显示文摘In Brazil, intense coal exploitation activities have led to environmental deterioration, including soil mortification, water contamination, loss of ecosystem, and atmospheric contamination. In addition,considerable quantities of sulfur-rich residues are left behind in the mining area; these residues pose grave environmental issues as they undergo sulfide oxidation reactions. When sulfur oxides come in contact with water, extreme acid leachate is produced with great proportions of sulfate, and hazardous elements(HEs), which are identified as coal drainage(CMD). CMD is an environmental pollution challenge, particularly in countries with historic or active coal mines. To prevent CMD formation or its migration, the source must be controlled; however, this may not be feasible at many locations. In such scenarios, the mine water should be collected, treated, and discharged. In this study, data from 2005 to2010 was gathered on the geochemistry of 11 CMD discharges from ten different mines. There are several concerns and questions on the formation of nanominerals in mine acid drainage and on their reactions and interfaces. The detailed mineralogical and geochemical data presented in this paper were derived from previous studies on the coal mine areas in Brazil. Oxyhydroxides, sulfates, and nanoparticles in these areas possibly go through structural transformations depending on their size and formation conditions. The geochemistry of Fe-precipitates(such as jarosite, goethite, and hematite) existent in the CMD-generating coal areas and those that could be considered as a potential source of hazardous elements(HEs)(e.g., Cr) were also studied because these precipitates are relatively stable in extremely low pH conditions. To simplify and improve poorly ordered iron, strontium, and aluminum phase characterization, field emission scanning electron microscopy(FE-SEM), high-resolution transmission electron microscopy(HR-TEM), micro-Raman spectroscopy, and X-ray diffraction(XRD) and sequential extraction(SE) studies were executed on a set CMD samples from the Brazilian mines. | 4 | Estimation of visceral fat is useful for the diagnosis of significant fibrosis in patients with non-alcoholic fatty liver disease显示文摘BACKGROUND Obesity is a risk factor for non-alcoholic fatty liver disease(NAFLD),although obese patients with NAFLD do not always develop significant fibrosis.The distribution of body fat could predict the risk of NAFLD progression.AIM To investigate the role of bioelectrical impedance-estimated visceral fat(VF)in assessing NAFLD severity.METHODS In this cross-sectional study,patients with biopsy-proven NAFLD were prospectively included.All patients underwent anthropometric evaluation,blood tests and bioelectrical impedance analysis.RESULTS Between 2017 and 2020,119 patients were included[66.4%male,56 years(SD 10.7),62.2%obese,61.3%with metabolic syndrome].Sixty of them(50.4%)showed significant fibrosis(≥F2)in liver biopsy.Age,VF and metabolic syndrome were associated with significant fibrosis(61 years vs 52 years,16.4 vs 13.1,73.3%vs 49.2%,respectively;P<0.001 for all).In the multivariate analysis,VF and age were independently associated with significant fibrosis(VF,OR:1.11,95%CI:1.02-1.22,P=0.02;age,OR:1.08,95%CI:1.03-1.12,P<0.01).A model including these variables showed and area under the receiver operating characteristic curve(AUROC)of 0.75,which was not inferior to transient elastography or NAFLD fibrosis score AUROCs.We developed a nomogram including age and VF for assessing significant fibrosis in routine practice.CONCLUSION VF is a surrogate marker of liver fibrosis in patients with NAFLD.Bioelectrical impedance analysis is an inexpensive and simple method that can be combined with age to guide patient referral when other resources may be unavailable. | Marta Hernández-Conde Elba Llop Carlos Fernández Carrillo Beatriz Tormo Javier Abad Luis Rodriguez Christie Perelló Marta López Gomez JoséLuis Martínez-Porras Natalia Fernández Puga Maria Trapero-Marugan Enrique Fraga Carlos Ferre Aracil JoséLuis Calleja Panero | 2020 | World Journal of Gastroenterology2020，26，42： | 2 |
