| 1 | Thermogenic protein UCP1 and UCP3 expression in non- small cell lung cancer: relation with glycolysis and anaerobic metabolism显示文摘Uncoupling protein 1(UCP1)is a proton transporter/channel residing on the inner mitochondrial membrane and is involved in cellular heat production.Using immunohistochemistry,we investigated the expression of UCP1 and UCP3 in a series of 98 patients with non-small cell lung cancer(NSCLC)treated with surgery.Expression patterns were correlated with histopathological variables,prognosis,and the expression of enzymes/proteins related to cell metabolism.Bronchial epithelium did not express UCP1 or UCP3,while alveolar cells strongly expressed UCP1.In tumors,strong expression of UCP1 and UCP3 was recorded in43/98(43.8%)and 27/98(27.6%)cases,respectively.UCP1 was significantly associated with squamous cell histology(P=0.05),whilst UCP3 was more frequently overexpressed in large cell carcinomas(P=0.08),and was inversely related to necrosis(P=0.009).In linear regression analysis,UCP1 was directly related to markers of glycolysis[hexokinase(HXKII)and phosphofructokinase(PFK1)]and anaerobic glucose metabolism[pyruvate dehydrogenase kinase(PDK1)and lactate dehydrogenase(LDH5)].UCP3 was directly linked with a glucose transporter(GLUT2),monocarboxylate transporter(MCT2),glycolysis markers(PFK1 and aldolase),and with the phosphorylation of pyruvate dehydrogenase(p PDH).Kaplan-Meier survival analysis showed that UCP3 was significantly related to poor prognosis in squamous cell carcinomas(P=0.04).UCP1 and UCP3 are overexpressed in a large subgroup of non-small cell lung tumors and their expression coincides with increased glucose absorption,intensified glycolysis,and anaerobic glucose usage.Whether UCPs are targets for therapeutic interventions in lung cancer is a hypothesis that demands further investigation. | Alexandra Giatromanolaki Konstantina Balaska Dimitra Kalamida Christos Kakouratos Efthimios Sivridis Michael I. | 3 | Restless legs syndrome is contributing to fatigue and low quality of life levels in hemodialysis patients显示文摘AIM To examine whether hemodialysis(HD) patients with restless legs syndrome(RLS) are subjects of greater fatigue and impaired quality of life(QoL) compared to HD patients without RLS.METHODS Eighty five stable HD patients participated in this study. According to their RLS status, the patients were dividedinto the RLS group(n = 23) and the non-RLS group(n = 62). QoL, fatigue, sleep quality, daily sleepiness and depression symptoms were assessed by using various questionnaires. Finally, biochemical parameters including iron, ferritin, hemoglobin, hematocrit and parathormone were assessed. RESULTS The HD patients with RLS scored worse in all the questionnaires used in the study(P < 0.05). The patients with RLS were more likely to receive the HD therapy on the morning shift, whilst 43.5% of the RLS patients reported to experience the RLS symptoms also during HD. The severity of RLS was correlated with fatigue, depression score and sleep quality(P < 0.05). CONCLUSION HD patients with RLS are subject to lower QoL related parameters and greater fatigue compared to HD patients without RLS. | 4 | Novel approaches for the treatment of ventricular tachycardia显示文摘Ventricular tachycardia(VT) is a crucial cause of sudden cardiac death(SCD) and a primary cause of mortality and morbidity in patients with structural cardiac disease. VT includes clinical disorders varying from benign to lifethreatening. Most life-threatening episodes are correlated with coronary artery disease, but the risk of SCD varies in certain populations, with various underlying heart conditions, specific family history, and genetic variants. The targets of VT management are symptom alleviation, improved quality of life, reduced implantable cardioverter defibrillator shocks, prevention of reduction of left ventricular function, reduced risk of SCD, and improved overall survival. Antiarrhythmic drug therapy and endocardial catheter ablation remains the cornerstone of guideline-endorsed VT treatment strategies in patients with structural cardiac abnormalities. Novel strategies such as epicardial ablation, surgical cryoablation, transcoronary alcohol ablation, pre-procedural imaging, and stereotactic ablative radiotherapy are an appealing area of res-earch. | 19 | Minimally invasive outpatient management of iliopsoas muscle abscess in complicated spondylodiscitis显示文摘BACKGROUND Iliopsoas muscle abscess(IPA)and spondylodiscitis are two clinical conditions often related to atypical presentation and challenging management.They are both frequently related to underlying conditions,such as immunosuppression,and in many cases they are combined.IPA can be primary due to the hematogenous spread of a microorganism to the muscle or secondary from a direct expansion of an inflammatory process,including spondylodiscitis.Computed tomographyguided percutaneous drainage has been established in the current management of this condition.AIM To present a retrospective analysis of a series of 8 immunocompromised patients suffering from spondylodiscitis complicated with IPA and treated with percutaneous computed tomography-guided drainage and drain insertion in an outpatient setting.METHODS Patient demographics,clinical presentation,underlying conditions,isolated microorganisms,antibiotic regimes used,abscess size,days until the withdrawal of the catheter,and final treatment outcomes were recorded and analyzed.RESULTS All patients presented with night back pain and local stiffness with no fever.The laboratory tests revealed elevated inflammatory markers.Radiological findings of spondylodiscitis with unilateral or bilateral IPA were present in all cases.Staphylococcus aureus was isolated in 3 patients and Mycobacterium tuberculosis in 2 patients.Negative cultures were found in the remaining 3 patients.The treatment protocol included percutaneous computed tomographyguided abscess drainage and drain insertion along with a course of targeted or empiric antibiotic therapy.All procedures were done in an outpatient setting with no need for patient hospitalization.CONCLUSION The minimally invasive outpatient management of IPA is a safe and effective approach with a high success rate and low morbidity. | Vasiliki Fesatidou Evangelos Petsatodis Dimitrios Kitridis Panagiotis Givissis Efthimios Samoladas | 2022 | World Journal of Orthopedics2022，13，4： | 1 |
