| 1 | Potential fly-ash utilization in agriculture:A global review显示文摘Though in last four decades various alternate energy sources have come into the limelight, the hyperbolic use of coal as a prime energy source cannot be counterbalanced. Disposal of high amount of fly-ash from thermal power plants absorbs huge amount of water, energy and land area by ash ponds. In order to meet the growing energy demand, various environmental, economic and social problems associated with the disposal of fly-ash would continue to increase. Therefore, fly-ash management would remain a great concern of the century. Fly-ash has great potentiality in agriculture due to its efficacy in modification of soil health and crop performance. The high concentration of elements (K, Na, Zn, Ca, Mg and Fe) in fly-ash increases the yield of many agricultural crops. But compared to other sectors, the use of fly-ash in agriculture is limited. An exhaustive review of numerous studies of last four decades took place in this paper, which systematically covers the importance, scope and apprehension regarding utilization of fly-ash in agriculture. The authors concluded that though studies have established some solutions to handle the problems of radioactivity and heavy metal content in fly-ash, long-term confirmatory research and demonstration are necessary. This paper also identified some areas, like proper handling of dry ash in plants as well as in fields, ash pond management (i.e., faster decantation, recycling of water, vertical expansion rather than horizontal), monitoring of soil health, crop quality, and fate of fly-ash in time domain, where research thrust is required. Agricultural lime application contributes to global warming as Intergovernmental Panel on Climate Change (IPCC) assumes that all the carbon in agricultural lime is finally released as CO2 to the atmosphere. | 2 | Characterization of hepatic progenitors from human fetal liver during second trimester显示文摘AIM: To enrich hepatic progenitors using epithelial cell adhesion molecule (EpCAM) as a marker from human fetal liver and investigate the expression of human leukocyte antigen (HLA) and their markers associated with hepatic progenitor cells. METHODS: EpCAM +ve cells were isolated using magnetic cell sorting (MACS) from human fetuses (n = 10) at 15-25 wk gestation. Expression of markers for hepatic progenitors such as albumin, alpha-fetoprotein (AFP), CD29 (integrin β1), CD49f (integrin α6) and CD90 (Thy 1) was studied by using flow cytometry, immunocytochemistry and RT-PCR; HLA class Ⅰ (A, B, C) and class Ⅱ (DR) expression was studied by flow cytometry only. RESULTS: FACS analysis indicated that EpCAM +ve cells were positive for CD29, CD49f, CD90, CD34, HLA class Ⅰ, albumin and AFP but negative for HLA class Ⅱ (DR) and CD45. RT PCR showed that EpCAM +ve cells expressed liver epithelial markers (CK18), biliary specific marker (CK19) and hepatic markers (albumin, AFP). On immunocytochemical staining, EpCAM +ve cells were shown positive signals for CK18 and albumin. Methods for the estimation of local scour depth based on dimensional analysis are also presented. The design methods based on empirical relations such as flow depth, flow intensity, sediment characteristics, abutments shape and size, alignment, and approach channel geometry are also discussed. Since clear-water scour is the maximum scour depth, the equation for clear-water scour is recommended for the computation of local scour depth around bridge abutments for uniformly and non- uniformly graded sediments. | Er. | 4 | Serum testosterone and prostate-specific antigen levels are major risk factors for prostatic volume increase among benign prostatic hyperplasia patients显示文摘Objective:Benign prostatic hyperplasia(BPH)is one of the most common diseases found among elderly men.Even though multiple risk factors of BPH have been identified in the past,the risk factors which have a direct impact on prostate volume have not been identified.In this study,we aim to determine the most significant contributing risk factors to prostate volume enlargement by analyzing possible associated risk factors previously studied.Methods:This is a quantitative study with an analytical observational design,performed using a retrospective cohort approach.Total sampling was performed on 83 patients who underwent transurethral resection of the prostate(TURP)in Sanglah General Hospital from January to February 2019.Bivariate analysis is performed to examine each variable's association with prostate volume followed by a multivariate analysis.All variables were reassessed with path analysis to measure the direct effects,indirect effects,and total effects on prostate volume.Results:Bivariate analysis shows that serum testosterone(R=0.208;p=0.059)and prostate-specific antigen(PSA)level(R=0.626;p=0.001)have a significant association with prostate volume.Multivariate analysis shows that serum PSA(B=1.4;p=0.001;95%confidence interval[95%CI]=1.039-1.770)and testosterone(B=0.024;p=0.005;95%CI=0.008-0.041)levels are significant among all the analyzed risk factors.There is a significant and strong effect of PSA to prostate volume(c=0.636;p=0.001)whereas testosterone has a significant albeit weak effect to prostate volume(c=0.246;p=0.009)based on the total effect of the path analysis.Conclusion:Serum testosterone and PSA levels are significantly associated with prostatic volume increase among BPH patients. | Gede Wirya Kusuma Duarsa Yudit Anastasia Sari Anak Agung Gde Oka Kadek Budi Santosa I Wayan Yudiana Pande Made Wisnu Tirtayasa Ida Bagus Putra Pramana Yudhistira Pradnyan Kloping | 2021 | Asian Journal of Urology2021,8,3: | 3 |
| 5 | Double-blind randomized sham controlled trial of intraperitoneal bupivacaine during emergency laparoscopic cholecystectomy显示文摘BACKGROUND: Intraperitoneal local anesthesia (IPLA) during elective laparoscopic cholecystectomy (el-LC) decreases post-operative pain. None of the studies have explored the efficacy of IPLA at emergency laparoscopic cholecystectomy (em-LC). A longer operative duration, the greater frequency of washing, and the inflammation associated with cholecystitis or pancreatitis are a few reasons why it cannot be assumed that a benefit in pain scores will be seen in em-LC with IPLA. This study was undertaken to assess the efficacy of IPLA in patients undergoing em-LC. METHODS: Double-blind randomized sham controlled trial was conducted of 41 consecutive subjects undergoing em-LC. IPLA was delivered by a combination of injection to the diaphragmatic and topical wash over the liver and gallbladder with bupivacaine or saline. The primary outcome was visual analogue scale pain scores until discharge. Secondary outcomes included pain scores in theatre recovery and analgesic consumption. RESULTS: One patient had a procedure converted to open and was excluded. There was no significant difference in pain scores in the ward or theatre recovery. Analgesic use, respiratory rate, oxygen saturation, duration to ambulation, eating, satisfaction scores, and time to discharge were comparable between the two groups. CONCLUSIONS: IPLA during em-LC does not influence postoperative pain. | 7 | Origin and geochemical characterization of the glauconites in the Upper Cretaceous Lameta Formation, Narmada Basin, central India显示文摘This study presents geochemical characteristics of glauconites in estuarine deposits within the Maastrichtian Lameta Formation in central India. Resting conformably over the Bagh Group, the Lameta Formation consists of ~4-5 m thick arenaceous, argillaceous and calcareous green sandstones underlying the Deccan Traps. The sandstone is friable, medium-to coarse-grained, well-sorted and thoroughly crossstratified, and contains marine fossils. Detailed petrography, spectroscopy and mineral chemistry indicates unique chemical composition of glauconite with high K_2O, MgO, Al_2O_3 and moderate TFe_2O_3. Glauconite is formed by the replacement of K-feldspars, initially as stringers in the cleavages and fractures of feldspars. Incipient glauconite subsequently evolves fully, appearing as pellets. Fully-evolved glauconite pellets often leave tiny relics of K-feldspar. XRD exhibits characteristic peak of 10A from basal(001)reflection of glauconite, indicating the 'evolved' character. The K_2O content of glauconites in the Lameta Formation varies from 5.51% to 8.29%, corroborating the 'evolved' to 'highly-evolved' maturation stage.The TFe_2O_3 content of glauconite varies from 12.56% to 18.90%. The PASS-normalized-REE patterns of glauconite exhibit a 'hat-shape' confirming the authigenic origin of glauconites. The slightly-negative to slightly-positive Ce anomaly value and the moderate TFe_2O_3 content of glauconite agree well with a suboxic,estuarine condition. The replacement of K-feldspar by the glauconite contributes towards the high K_2O content. | 9 | Para-aortic lymph node involvement should not be a contraindication to resection of pancreatic ductal adenocarcinoma显示文摘BACKGROUND Para-aortic lymph nodes(PALN)are found in the aortocaval groove and they are staged as metastatic disease if involved by pancreatic ductal adenocarcinoma(PDAC).The data in the literature is conflicting with some studies having associated PALN involvement with poor prognosis,while others not sharing the same results.PALN resection is not included in the standard lymphadenectomy during pancreatic resections as per the International Study Group for Pancreatic Surgery and there is no consensus on the management of these cases.AIM To investigate the prognostic significance of PALN metastases on the oncological outcomes after resection for PDAC.METHODS This is a retrospective cohort study of data retrieved from a prospectively maintained database on consecutive patients undergoing pancreatectomies for PDAC where PALN was sampled between 2011 and 2020.Statistical comparison of the data between PALN+and PALN-subgroups,survival analysis with the Kaplan-Meier method and risk analysis with univariable and multivariable time to event Cox regression analysis were performed,specifically assessing oncological outcomes such as median overall survival(OS)and disease-free survival(DFS).RESULTS 81 cases had PALN sampling and 17(21%)were positive.Pathological N stage was significantly different between PALN+and PALN-patients(P=0.005),while no difference was observed in any of the other characteristics.Preoperative imaging diagnosed PALN positivity in one case.OS and DFS were comparable between PALN+and PALN-patients with lymph node positive disease(OS:13.2 mo vs 18.8 mo,P=0.161;DFS:13 mo vs 16.4 mo,P=0.179).No difference in OS or DFS was identified between PALN positive and negative patients when they received chemotherapy either in the neoadjuvant or in the adjuvant setting(OS:23.4 mo vs 20.6 mo,P=0.192;DFS:23.9 mo vs 20.5 mo,P=0.718).On the contrary,when patients did not receive chemotherapy,PALN disease had substantially shorter OS(5.5 mo vs 14.2 mo;P=0.015)and DFS(4.4 mo vs 9.8 mo;P<0.001).PALN involvement was not identified as an independent predictor for OS after multivariable analysis,while it was for DFS doubling the risk of recurrence.CONCLUSION PALN involvement does not affect OS when patients complete the indicated treatment pathway for PDAC,surgery and chemotherapy,and should not be considered as a contraindication to resection. | Rupaly Pande Shafiq Chughtai Manish Ahuja Rachel Brown David C Bartlett Bobby V Dasari Ravi Marudanayagam Darius Mirza Keith Roberts John Isaac Robert P Sutcliffe Nikolaos A Chatzizacharias | 2022 | World Journal of Gastrointestinal Surgery2022,14,5: | 2 |
