2Epithelioid hemangioendothelioma of the liver as a rare indication for liver transplantation显示文摘AIM: To investigate the indications and outcomes of liver transplantation for hepatic epithelioid hemangioendothelioma(HEHE). METHODS: Between 1989 and August 2013, in the Department of General, Transplant, and Liver Surgery, Medical University of Warsaw, 1306 orthotopic liver transplantations(OLTx) were performed, including 72 retransplantations. Unresectable HEHE was an indication for OLTx in 10 patients(0.8% of primary OLTx), the mean age of the patients was 40.5 ± 13.3 years(range 23-65 years), and the male-to-female ratio was 2:8. Kaplan-Meier survival analysis in HEHE, hepatocellular carcinoma(HCC), and other OLTx recipients groups was performed. The differences in mortality were compared using the χ2 test. A P-value < 0.05 indicated statistical significance.RESULTS: No concomitant liver disease was found in any patient. There was no neoadjuvant chemotherapy or radiotherapy. Liver function test results were normal in most of the patients. The levels of alpha-fetoprotein, carcinoembryonic antigen, and carbohydrate antigen 19-9 were normal. In immunohistochemical staining, the neoplastic cells were positive for factor Ⅷ-related antigen, CD31, and CD34, which are endothelial cell markers, and negative for cytokeratin 19, cytokeratin 7, and HepPar-1. Nine patients were alive without tumor recurrence. One patient died 2 mo after OLTx due to septic complications. No morbidity was observed. Maximum follow-up was 11.4 years, with a minimum of 1 mo. The cumulative survival rate at the end of followup in HEHE patients was 87.5% compared with 54.3% in the HCC group and 76.3% in the other OLTx recipients group(χ2 test = 1.784, df = 2, P = 0.409).CONCLUSION: Unresectable HEHE, without extrahepatic metastases is an excellent indication for liver transplantation. 5Recreational potential as an indicator of accessibility control in protected mountain forest areas显示文摘The article presents research findings related to recreational use of forests located in protected mountainous areas with forestage of over 80%. The study was designed to identify recreational potential of the Carpathian national parks(Bieszczady National Park, Babia Góra National Park, Gorce National Park and Magura National Park; southern Poland) and to compare these findings with the actual number of visitors. The information received on the recreational potential of parks is important from the point of view of protection of natural resources and the financial situation of the parks. The calculated ratio may be an effective tool of management for park administration, that allows to reconcile statutory social and protective functions of national parks. The study determined the recreational potential of the forests with the use of recreational valorisation method designed for areas with varied terrain, and the evaluated factors included the stands of trees with their habitat and land relief. The permissible number of national park visitors, expressed as manhour/ha/year ranges from 19.31 in Bieszczady National Park(BG: 19o 35' E, 49o 35' N) to 32.06 in in Bieszczady National Park(B: 22o 40' E, 49o 10' N). In 3 out of 4 investigated parks, Magura National Park(M: 21°25' E, 49o 30' N), Gorce National Park(G: 20o10' E, 49o 35' N), B) recreation carrying capacity was not exceeded, whether or not the strictly protected area is taken into account. Only in BG was the recreation carrying capacity exceeded by nearly 24%,or by 85% if the strictly protected area isexcluded from tourism-related exploitation. The presented procedure for monitoring access to mountain forests in national parks, from the viewpoint of natural resources conservation, can be applied in other mountainous areas covered with forests and exposed to tourist and recreational traffic,and in forests facing particular risk of recreational damage, e.g. in urban and suburban forests growing in areas with varied orography.Tomasz DUDEK 2017Journal of Mountain Science2017,14,7:4

9Restoring the dampened expression of the core clock molecule BMAL1 protects against compression-induced intervertebral disc degeneration显示文摘The circadian clock participates in maintaining homeostasis in peripheral tissues,including intervertebral discs(IVDs).Abnormal mechanical loading is a known risk factor for intervertebral disc degeneration(IDD).Based on the rhythmic daily loading pattern of rest and activity,we hypothesized that abnormal mechanical loading could dampen the IVD clock,contributing to IDD.Here,we investigated the effects of abnormal loading on the IVD clock and aimed to inhibit compression-induced IDD by targeting the core clock molecule brain and muscle Arnt-like protein-1(BMAL1).In this study,we showed that BMAL1 KO mice exhibit radiographic features similar to those of human IDD and that BMAL1 expression was negatively correlated with IDD severity by systematic analysis based on 149 human IVD samples.The intrinsic circadian clock in the IVD was dampened by excessive loading,and BMAL1 overexpression by lentivirus attenuated compression-induced IDD.Inhibition of the RhoA/ROCK pathway by Y-27632 or melatonin attenuated the compression-induced decrease in BMAL1 expression.Finally,the two drugs partially restored BMAL1 expression and alleviated IDD in a diurnal compression model.Our results first show that excessive loading dampens the circadian clock of nucleus pulposus tissues via the RhoA/ROCK pathway,the inhibition of which potentially protects against compression-induced IDD by preserving BMAL1 expression.These findings underline the importance of the circadian clock for IVD homeostasis and provide a potentially effective therapeutic strategy for IDD.Dong Wang Pandi Peng Michal Dudek Xueyu Hu Xiaolong Xu Qiliang Shang Di Wang Haoruo Jia Han Wang Bo Gao Chao Zheng Jianxin Mao Chu Gao Xin He Pengzhen Cheng Huanbo Wang Jianmin Zheng Judith A.Hoyland Qing-Jun Meng Zhuojing Luo Liu Yang 2022Bone Research2022,10,2:2
