1Expenditure and financial burden for the diagnosis and treatment of colorectal cancer in China:a hospital.based,multicenter,cross-sectional survey显示文摘Background: The increasing prevalence of colorectal cancer(CRC) in China and the paucity of information about relevant expenditure highlight the necessity of better understanding the financial burden and effect of CRC diagnosis and treatment. We performed a survey to quantify the direct medical and non-medical expenditure as well as the resulting financial burden of CRC patients in China.Methods: We conducted a multicenter, cross-sectional survey in 37 tertiary hospitals in 13 provinces across China between 2012 and 2014. Each enrolled patient was interviewed using a structured questionnaire. All expenditure data were inflated to the 2014 Chinese Yuan(CNY; 1 CNY = 0.163 USD). We quantified the overall expenditure and financial burden and by subgroup(hospital type, age at diagnosis, sex, education, occupation, insurance type, household income, clinical stage, pathologic type, and therapeutic regimen). We then performed generalized linear modeling to determine the factors associated with overall expenditure.Results: A total of 2356 patients with a mean age of 57.4 years were included, 57.1 % of whom were men; 13.9% of patients had stage I cancer; and the average previous-year household income was 54,525 CNY.The overall average direct expenditure per patient was estimated to be 67,408 CNY, and the expenditures for stage Ⅰ, Ⅱ, Ⅲ, and Ⅳ disease were 56,099 CNY, 59,952 CNY, 67,292 CNY, and 82,729 CNY, respectively. Non-medical expenditure accounted for 8.3%of the overall expenditure. The 1-year out-of-pocket expenditure of a newly diagnosed patient was 32,649 CNY, which accounted for 59.9% of their previous-year household income and caused 75.0% of families to suffer an unmanageable financial burden. Univariate analysis showed that financial burden and overall expenditure differed in almost all subgroups(P < 0.05), except for sex. Multivariate analysis showed that patients who were treated in specialized hospitals and those who were diagnosed with adenocarcinoma or diagnosed at a later stage were likely to spend more,whereas those with a lower household income and those who underwent surgery spent less(all P < 0.05).Conclusions: For patients in China, direct expenditure for the diagnosis and treatment of CRC seemed catastrophic,and non-medical expenditure was non-ignorable. 3Evaluation of health benefit using Ben MAP-CE with an integrated scheme of model and monitor data during Guangzhou Asian Games显示文摘Guangzhou is the capital and largest city(land area:7287 km2)of Guangdong province in South China.The air quality in Guangzhou typically worsens in November due to unfavorable meteorological conditions for pollutant dispersion.During the Guangzhou Asian Games in November 2010,the Guangzhou government carried out a number of emission control measures that significantly improved the air quality.In this paper,we estimated the acute health outcome changes related to the air quality improvement during the 2010 Guangzhou Asian Games using a next-generation,fully-integrated assessment system for air quality and health benefits.This advanced system generates air quality data by fusing model and monitoring data instead of using monitoring data alone,which provides more reliable results.The air quality estimates retain the spatial distribution of model results while calibrating the value with observations.The results show that the mean PM_2.5concentration in November 2010 decreased by 3.5μg/m^3 compared to that in 2009 due to the emission control measures.From the analysis,we estimate that the air quality improvement avoided 106 premature deaths,1869 cases of hospital admission,and 20,026 cases of outpatient visits.The overall cost benefit of the improved air quality is estimated to be 165 million CNY,with the avoided premature death contributing 90%of this figure.The research demonstrates that Ben MAP-CE is capable of assessing the health and cost benefits of air pollution control for sound policy making.Dian Ding Yun Zhu Carey Jang Che-Jen Lin Shuxiao Wang Joshua Fu Jian Gao Shuang Deng Junping Xie Xuezhen Qiu 2016Journal of Environmental Sciences2016,28,4:13
4Design and demonstration of a next-generation air quality attainment assessment system for PM_(2.5)and O_3显示文摘Due to the increasingly stringent standards, it is important to assess whether the proposed emission reduction will result in ambient concentrations that meet the standards. The Software for Model Attainment Test—Community Edition(SMAT-CE) is developed for demonstrating attainment of air quality standards of O3 and PM2.5. SMAT-CE improves computational efficiency and provides a number of advanced visualization and analytical functionalities on an integrated GIS platform. SMAT-CE incorporates historical measurements of air quality parameters and simulated air pollutant concentrations under a number of emission inventory scenarios to project the level of compliance to air quality standards in a targeted future year. 7Dynamic Balance Control Based on an Adaptive Gain-scheduled Backstepping Scheme for Power-line Inspection Robots显示文摘This paper presents an adaptive gain-scheduled backstepping control(AGSBC) scheme for the balance control of an underactuated mechanical power-line inspection(PLI) robotic system with two degrees of freedom and a single control input.First, a nonlinear dynamic model of the balance adjustment process of the PLI robot is constructed, and then the model is linearized at a nominal equilibrium point to overcome the computational infeasibility of the conventional backstepping technique. Second, to solve generalized stabilization control issue for underactuated systems with multiple equilibrium points,an equilibrium manifold linearized model is developed using a scheduling variable, and then a gain-scheduled backstepping control(GSBC) scheme for expanding the operational area of the controlled system is constructed. Finally, an adaptive mechanism is proposed to counteract the impact of external disturbances. The robust stability of the closed-loop system is ensured by Lyapunov theorem. 11Appropriate choice of collision-induced dissociation energy for qualitative analysis of notoginsenosides based on liquid chromatography hybrid ion trap time-of-flight mass spectrometry显示文摘Liquid chromatography hybrid ion trap/time-of-flight mass spectrometry possesses both the MS^n ability of ion trap and the excellent resolution of a time-of-flight and has been widely used to identify drug metabolites and determine trace multi-components in natural products. Collision energy, one of the most important factors in acquiring MS^n information, could be set freely in the range of 10%–400%. Herein, notoginsenosides were chosen as model compounds to build a novel methodology for the collision energy optimization. Firstly, the fragmental patterns of the representatives for the authentic standards of protopanaxadiol-type and protopanaxatriol-type notoginsenosides were obtained based on accurate MS^2 and MS^3 measurements via liquid chromatography hybrid ion trap/time-of-flight mass spectrometry. The extracted ion chromatograms of characteristic product ions of notoginsenosides in Panax Notoginseng Extract were produced under a series of collision energies and compared to screen the optimum collision energies values for MS^2 and MS^3. The results demonstrated that the qualitative capability of liquid chromatography hybrid ion trap/time-of-flight mass spectrometry was greatly influenced by collision energies, and 50% of MS^2 collision energy was found to produce the highest collision-induced dissociation efficiency for notoginsenosides. 14Photonic Bandgap Properties of Atom-lattice Photonic Crystals in Polymer显示文摘The present paper covers the various photonic crystals(PhCs) structures mimicking real atom-lattice structures in electronic crystals by using the femtosecond laser-induced two-photon photopolymerization of SU-8 resin. The bandgap properties were investigated by varying the crystal orientations in <111>, <110> and <100> of diamond-lattice PhCs. The photonic stop gaps were present at λ=3.88 μm in <111> direction, λ=4.01 μm in <110> direction and λ=5.30 μm in <100> direction, respectively. In addition, defects were introduced in graphite-lattice PhCs and the strong localization of photons in this structure with defects at λ=5 μm was achieved.
